Evaluation of Radiography of Ingested and Aspirated Foreign Bodies in Pediatric Patients (이물질 섭취 및 흡인 소아환자의 엑스선 영상 평가)

  • The purpose of this study was to introduce the radiography for the natural course and clinical diagnosis of foreign body ingestion and aspiration, to help diagnosis and treatment, to evaluate the accuracy of radiographic images of pediatric patients. A 2 to 7 year-old patient who ingested a foreign body was ingested and aspirated with foreign substances such as coin, cloth pin, earring, baduk stone, and hairpins, and chest and abdomen of the plain radiography. The pediatric patient who ingested and aspirated the foreign body of the coins, the clothespins, the earrings, the stones, and the hairpins were examined by chest and abdomen of the plain radiography and fluoroscopic images. The radiography examination can be combined to effectively cope with the treatment and the treatment of the foreign substance removal. It can be applied to the diagnosis of foreign body in pediatric patient's clinic and appropriate treatment and treatment direction.

Contrast Optimization using of Weight-based Injection Protocol in Pediatric Abdomen CT Examination (소아 복부 CT 검사에서 체중에 기반한 조영제 주입 프로토콜 적용에 따른 조영증강의 최적화)

  • The aim of this study was to achieve optimal portal phase while reducing contrast medium by applying weight-based dose protocol compared to standard fixed dose protocol to performing of pediatric abdominal CT examination. Discovery 750HD (General Electric Medical Systems, Milwaukee, USA) was used, and a total of 167 children consisting of 85 men and 82 women under the age of 18 were studied. The group in which the 300 mgI/ml(Xenetix, Guerbet, France) contrast medium was fixedly injected at twice body weight and the group injected with physiological saline while gradually decreasing the injection amount by 10% while applying the weight-based protocol were distinguished. Also, the CT number and SNR of abdominal organs were compared and evaluated while changing the scan delay time. Subjective image quality of enhancement and beam-hardening artifacts of around the heart was assessed with five-point criterion. The group adapted weight-based protocol with 20% reduction in contrast medium was most similar in contrast enhancement in the group with fixed injection at twice body weight. Furthermore, the group with a delay time of 20% had the highest contrast enhancement effect, and the difference in CT attenuation coefficient from the group scanned immediately after injection of the contrast media. Therefore, the appropriate delay time after injection of the contrast agent increased the contrast enhancement of the parenchymal organ. In addition, the weight-based injection protocol with normal saline reduced artifacts around the heart, and the effect of contrast enhancement could be maintained. In conclusion, it is possible to reduce dosage of contrast media through the application of weight-based injection protocols and appropriate latency, and to characterize optimal portal phase imaging on pediatric abdominal CT.

Evaluation of the Availability of the Wide Coverage Volume Axial Mode in Pediatric Cardiac CT (소아 심장 CT검사 시 Wide Coverage Volume Axial Mode의 유용성 평가)

  • We are trying to evaluate usefulness of Wide Coverage Volume Axial Mode by comparing and analyzing for exposure doses to patients and video quality extracted from two materials. First material is a computer tomography scanning of paediatric cardiac made by using High Pitch Mode, which is designed for diagnosing Congenital cardiac diseases. Second material is computer tomography scanning of paediatric cardiac made by using Wide Coverage Volume Axial Mode. When we did computer tomography scanning of paediatric cardiac, we used High Pitch Mode and Wide Coverage Volume Axial Mode to 50 patients of each, overall 100 patients. Also, we compared exposure doses to patients using videos got from each protocol. Then we compared video quality by calculating SNR and CNR by setting ROI of each. Not only exposure doses to patients were reduced by 13.07 %, but also SNR and CNR were improved when testing used Wide Coverage Volume Axial Mode rather than using High Pitch Mode. Wide Coverage Volume Axial Mode reduced testing time by using high-speed scanner. Furthermore, we can find out that Wide Coverage Volume Axial Mode is an useful method through improving video quality and reducing exposure doses to patients than using High Pitch Mode from ASiR-V, which is low-dose technology.

  • The incidence of childhood cancer is greatest in the first year of life. Early diagnosis and advances in medicine have significantly improved outcomes of treatment resulting in higher survival rate; however, this progress comes at the expense of a higher incidence of adverse side effects because of more aggressive antineoplastic treatment strategies. The oral cavity, a trauma-prone environment, is extremely sensitive to toxicities from antineoplastic agents. Oral health care specialists, including pediatric and hospital dentists can support the oncology team by providing basic oral care, implementing oral care protocols, delivering emergency dental treatment, and assisting and/or managing oral complications from cancer therapy. This article covers the considerations in the dental management of pediatric patients undergoing cancer treatment, specifically chemotherapy, radiotherapy, and hematopoietic stem cell transplantation.

Evaluation of Upper Airway Depth with Different Anteroposterior Skeletal Patterns in Children (소아에서 상하악골의 위치에 따른 상기도의 시상평면상 계측값의 연관성)

  • The respiratory function is relevant to the craniofacial growth and orthodontic diagnosis. The size of the pharyngeal airway was measured in lateral cephalometric view in children visited Kyung Hee University Hospital from January 2015 to August 2017. A total of 74 healthy children (36 boys and 38 girls) aged 7 - 11 years (mean, 8.5 years) with a normodivergent facial pattern were divided into three groups according to anteroposterior jaw relation measuring A point-Nasion-B point (ANB) angle. Lateral cephalometric data were used to measure the airway dimensions. The dimensions of the middle airway were significantly lower, reducing the upper airway space, in large ANB angle group than in other children, suggesting that children with large ANB angle have narrower airway space than others.

Effects of Maxillary Lateral Incisor Agenesis on Skeletodental Characteristics in Mixed Dentition (상악 측절치 결손이 어린이 안면골격과 치열궁 형태에 미치는 영향)

  • This study aimed to evaluate skeletodental characteristics of patient with maxillary lateral incisor agenesis (MLIA) in mixed dentition. It involved the children in early mixed dentition who visited Pusan National University dental hospital for orthodontic purposed and had intact primary canines. 38 children with MLIA and 38 controls with the same chronological age satisfying the inclusion criteria were selected. The craniofacial structures and dental arch dimensions of the MLIA were evaluated using model & cephalometric analysis and compared to controls. The rate of unilateral MLIA was high in male and the rate of bilateral MLIA was high in female. In model analysis, the width / length ratio of maxillary anterior portion of the MLIA group were higher and arch perimeter of the maxilla of the MLIA group were smaller than those of the control group (p = 0.003, 0.04). Cephalometric analysis showed that there were no significant differences in terms of skeletal, dental analysis. In soft tissue profile, nasolabial angle was larger in MLIAs than in controls (p = 0.039). Considering these skeletodental characteristics of MLIA, early diagnosis and proper management is highly recommended to minimize the possibility of functional defect.

Analysis of the Characteristics of First Permanent Molars with Delayed Eruption (제1대구치 맹출 지연의 유형에 따른 특징 분석)

  • The first permanent molars play a key role in maxillofacial development and occlusion. The purpose of this study is to investigate the characteristics and development stages of first molars with delayed eruption, and to evaluate their associations with congenital missing teeth. Eight-year-old patients who had delayed eruption in their first molars were classified into 75 patients with physical barriers and 77 patients without physical barriers. The development stages of the first and second molars in the delayed area were analyzed using Nolla method from the panoramic radiographs. The relationship between congenital missing teeth and delayed area was also investigated. Delayed eruption of first molars were more common in the maxilla alone. With the presence of physical barriers, male patients showed higher frequency in unilateral cases, while female patients had higher bilateral cases when there was no physical barrier. Delayed development of first molars were observed in delayed eruption area. In the absence of physical barriers, adjacent second molars were also developed slowly and the incidence of congenital missing teeth was high in delayed area. If first molars with delayed eruption are observed, clinical and radiographical follow-ups are necessary for the evaluation of their developmental stages and congenital missing teeth.

  • Diffuse panbronchiolitis(DPB) is a chronic inflammatory airway disease predominantly affecting respiratory bronchioles, with distinct clinicopathological profiles. It was first described in 1966 by Yamanaka et al. The etiology of DPB is not yet clear, and the natural history of the disease is respiratory failure leading to cor pulmonale and ultimately death. But the long-term use of low-dose macrolide has proven to be highly effective in treating patients with DPB. Usual age at diagnosis is over 40. A few cases of DPB have been reported in Korea since 1992 but there have been no reports in children. We experienced a case of DPB in a 12-year-old girl. Therefore, we report the case with a brief review of the related literature.

The Application of TW3 method for Prediction about Bone Age in Hand AP Image of Children (소아 Hand AP영상에서 골연령 예측을 위한 TW3법의 응용)

  • The study is to recognize the interactions with bone ages by measuring the length between the end of the bone and the growth plate on selected highest weight of regions of seven for bone maturity in TW3 method. The experiment is subjected on seventy-two children (36 males, 36 females) who have examined the growth plate test from March, 2014 to March, 2015 and implemented a regression analysis by measuring the length between the end of the bone and the growth plate in Hand AP image of the children. In result, each bone age has produced a mean value and a standard deviation corresponding to the specific range and as bone age increases the length between the end of the bone and the growth plate decreased. In addition, female children showed lower mean value in comparison to male and also the measurement of the length between the end of the bone and the growth plate and its bone age are shown to be statistically valid(p<0.001) according to the results of regression analysis using its result value. Therefore, the probability of prediction on the bone age read off through the applied TW3 method and regression equation in the Hand AP image of the children.

  • Computed tomography(CT) has become more widely available in recent years and is recognized as an important diagnostic tool for complex disease and condition of the orofacial structure in adults. In pediatric patients, especially in the mixed dentition period, the number of superimposing dental structures in the jaw bone is much greater than in the permanant dentition period. Therefore, in spite of radiation risk, computed tomography should sometimes be used in pediatric patients in order to obtain a precise and valid diagnosis. This case study presents clinical application of computed tomography on patients visiting department of pediatric dentistry in Seoul National University Dental Hospital. The results obtained through these cases were summarized as follow : 1. Localization of impacted teeth using CT allows for efficient treatment planning leading to reduced postoperative complications. 2. In orthodontic traction of impacted permanent teeth, utilization of CT made exact localization of window site and determination of prognosis possible. 3. Use of CT made possible definite diagnosis of lesions in the jaw that were only suspected on plain film radiographs. 4. Careful selection is necessary when applying CT on children due to increased radiation dose and high cost.
