  • 1% 가량 인간의 뇌세포를 보유한 쥐가 탄생했다는 기사가 있을 만큼 생명공학이 발달하고 있고, 의학이 발달함에 따라 난치병들도 그 치료방법을 찾아가고 있다는 반가운 소식이 종종 들린다. 그러나 그만큼 듣도 보도 못한 새로운 희귀병들이 속출하고 있고, 오랜 시간 인류에게 무서운 난치병, 암은 속시원한 치료법을 아직도 찾지 못한 채 여전히 그 위용을 떨치고 있다. 일상생활을 하며 너무나도 건강하게 살다가 그 징후가 느껴져 병원을 찾으면 이미 치료시기가 지나버리기도 하고, 정기검진을 받는 사람에게도 잦은 병으로 자리잡게 되었다. 또한 최근에는 어린 아이들에게도 소아암이라는 이름 아래 예외없이 발병률를 보이고 있다. 먹을거리의 오염과 바쁜 일상 속에서 그 누구에게나 찾아올 수 있는, 영원한 난치병 암백신의 개발은 현대인들에겐 영원히 풀어야 할 숙제인 것이다.

Childhood Cancer Survivor's Services Needs for the Better Quality of Life (소아암 완치자의 삶의 질 향상을 위한 서비스 욕구)

  • Purpose: The study aimed to identity specific needs for services and programs to help childhood cancer survivors adjust and adapt to life after treatment. Methods: In-depth interviews were conducted with 31 childhood cancer survivors, diagnosed with cancer before the age of 18 and currently between 15 and 39 years of age. Each survivor had completed his/her cancer treatment. Results: The participating cancer survivors reported needs for services related to psychological counseling, schooling and learning, social skills, mentorship, integrated health management, self support activities, families of survivors, and public recognition and awareness. Conclusion: The results of the study indicate a need to better understand childhood cancer survivors, provides a basis for developing various services and programs to improve the quality of life among childhood cancer patients, survivors, and their families, and supports the importance of psychosocial adjustment.

Trends in Research on Children with Cancer and Their Families in Korea (소아암 아동과 가족에 관한 국내 연구 동향)

  • Purpose: This study was designed to analyze recent trends in pediatric oncology research in Korea and suggest future research directions in this area. Methods: Studies (105) selected from http://www.riss4u.net for last 15 yr were used. They were analyzed by publication type, field and design of the study, study participants, main theme, and outcome variables used in intervention studies. Results: 1. Of the 105 studies, 67 were master and doctoral dissertations and 49.5% of studies were conducted in nursing. 2. There were 73 (69.5%) quantitative studies and 20 (19.1%) qualitative research studies and the most frequently used study design was that of a descriptive study. 3. Children with cancer undergoing treatment and their parents, mostly mothers, were the participants most frequently studied. 4. Most themes were psychological/spiritual problems, coping, and family function. 5. Most frequently measured outcome variables were nausea/vomiting, fear/anxiety and adjustment. Conclusion: The results indicate that future research should include more well-designed intervention studies to develop new intervention protocols and to confirm the effect of previous study findings. It is also necessary to use an interdisciplinary approach to deal with physical and psychosocial needs of these children and their families including siblings and fathers of children with cancer.

Bacteremia in Pediatric Cancer Patients : Causative Organisms and Antibiotic Sensitivities (소아 암 환자에서 발생한 균혈증 : 원인균과 항생제 감수성)

  • Purpose : Bacteremia in immunocompromised pediatric cancer patients can lead to high morbidity and mortality, if not treated early and properly. The incidence and antibiotic sensitivities to common pathogens of bacteremia in pediatric cancer patients are liable to change, according to region and time. We investigated the causative organisms and antibiotic sensitivities of bacteremia in pediatric cancer patients to assess the adequacy of empiric antimicrobial therapy. Methods : From September 1995 to August 2003, we retrospectively evaluated 58 episodes in 39 pediatric cancer patients with bacteremia treated at the Pediatric Department of Yeungnam University Hospital. We investigated and analyzed the causative organisms and the antibiotic sensitivity test results by reviewing the records of the microbiologically proven positive blood culture results. Results : The incidence of bacteremia in pediatric cancer patients in this study was 5.7 percent (58 episodes out of 1,022 occasions of blood cultures). Gram-positive organisms were isolated more often than gram-negative organisms (63.8 percent vs 36.2 percent) in the following order : Staphylococcus epidermidis (37.9 percent), Staphylococcus aureus (17.3 percent), Escherichia coli (12 percent), Streptococcus (8.6 percent), Enterobacter (6.9 percent), Klesiella (6.9 percent), Serratia (3.5 percent), Acinetobacter (3.5 percent), Proteus (1.7 percent) and Morganella morganii (1.7 percent). In antibiotic sensitivity tests, only six of 37 isolates (16 percent) of gram positive bacteria were sensitive to penicillin and 15 of 37 isolates (40 percent) were sensitive to oxacillin. All except one Staphylococcus aureus were sensitive to vancomycin and all except one Staphylococcus epidermidis were sensitive to teicoplanin among 37 isolates of gram positive bacteria. In the case of gram negative bacteria, two of 21 isolates (10 percent) and four of 21 isolates (19 percent) were sensitive to cefotaxime and ceftazidime, respectively. Only six of 21 isolates (29 percent) were sensitive to aminoglycoside, but all 21 isolates (100 percent) were sensitive to imipenem. All seven isolates tested after the year 2000 were sensitive to meropenem. Conclusion : In conclusion, we should choose the proper antimicrobials in treating pediatric cancer patients with suspected bacteremia, reflecting the increasing episodes of gram positive bacteremia and polymicrobial resistance of gram positive and negative organisms.

Effects of Granting Wish to Children with Life-threatening Conditions on Adjustment to Disease with a Focus on the Mediating Effects of Resilience and Stress Caused by Diseases (소원성취 프로그램이 소아암 및 난치병 환아들의 질병 적응에 미치는 영향: 레질리언스와 질병 스트레스의 매개효과를 중심으로)

  • Purpose: The purpose of this study is to examine how wish granting influences children with life-threatening medical conditions when it comes to their adaptation to disease with a focus on the mediating effect of resilience and stress caused by disease. Methods: From January 2, 2015 through January 12, 2015, a survey was conducted on 292 children with life-threatening diseases whose wishes were granted through Make-A-Wish Korea. The data were collected using the impact of a wish scale, the Children's Adjustment to Cancer Inventory, the Childhood Cancer Stressor Inventory, and the resilience scale in children with chronic illness. The data were analyzed using SPSS/WIN 20.0 and Amos 21.0. Results: Satisfaction with the wish granting program enhances resilience, and resilience affects stress caused by medical conditions as well as adaptation to disease. Also, stress caused by medical conditions influences adaptation to disease. Conclusion: Wish granting is effective in both facilitating chronically ill children to adjust to disease and reduce their stress from disease. Thus, children with life-threatening medical conditions could be assisted or motivated to adjust to disease by improving satisfaction achieved by wish granting.

Palliative Care Practitioners' Perception toward Pediatric Palliative Care in the Republic of Korea (소아완화의료에 대한 호스피스 완화의료 전문기관 종사자의 인식)

  • Purpose: This study was performed to investigate the current status of pediatric palliative care provision and how it is perceived by the palliative care experts. Methods: A descriptive study was conducted with 61 hospice institutions. From September through October 2017, a questionnaire was completed by experts from the participating institutions. Data were analyzed using SPSS 21.0. Results: Among 61 institutions, palliative care is currently provided for pediatric cancer patients by 11 institutions (18.0%), all of which are concentrated in Seoul, Incheon and Gyeonggi and Gyengsang provinces; 85.2% of all do not plan to provide specialized pediatric palliative care in the future. According to the experts, the main barriers in providing pediatric palliative care were the insufficient number of trained specialists regardless of the delivery type. Experts said that it was appropriate to intervene when children were diagnosed with cancer that was less likely to be cured (33.7%) and to move to palliative care institutions when their conditions worsened (38.2%); and it was necessary to establish a specialized pediatric palliative care system, independent from the existing institutions for adult patients (73.8%). Conclusion: It is necessary to develop an education program to establish a nationwide pediatric palliative care centers. Pediatric palliative care intervention should be provided upon diagnosis rather than at the point of death. Patients should be transferred to palliative care institutions after intervention by their existing pediatric palliative care team at the hospital is started.

Clinical Manifestations of Norovirus Infection in Korean Pediatric Cancer Patients (한국 소아 암환자에서 노로바이러스 감염증의 임상 양상)

  • Purpose : Norovirus infection, a common cause of community-acquired gastroenteritis, can also lead to severe illness in immunocompromised patients. We investigated clinical manifestations of norovirus infection in pediatric cancer patients. Methods : Stool specimens were collected from pediatric patients with gastrointestinal symptoms between November 2008 and September 2009 at Samsung Medical Center, Seoul, Korea. Norovirus infection was identified by reverse-transcription polymerase chain reaction (RT-PCR). A retrospective chart review was performed in pediatric cancer patients who were diagnosed with norovirus infection. Results : Ten patients were diagnosed with norovirus infection by RT-PCR in stool samples. The median age was 0.83 years (range 0.25-5.5 years) and the male to female ratio was 1.5:1 (6 males and 4 females). Underlying diseases were hematologic malignancies (4/10, 40%), neuroblastoma (4/10, 40%), and brain tumors (2/10, 20%). Three patients were infected before hematopoietic cell transplantation (HCT) and four patients after HCT. All patients had diarrhea (10/10, 100%), with a median frequency of diarrhea of 8.5 times/day (range 4-22 times/day). Median virus shedding duration was 72.5 days (range 19-299 days). Four patients with pneumatosis intestinalis were conservatively treated with bowel rest and total parenteral nutrition. One patient with severe diarrhea and bloody stool had concomitant chronic gut graft-versus-host disease (GVHD). Norovirus infection-related mortality was not observed. Conclusion : Norovirus infection can cause significant clinical manifestations with prolonged viral shedding in immunocompromised patients. Norovirus should be considered in pediatric cancer patients with severe gastrointestinal symptoms.

  • 병변의 양상에 따른 감별진단 재발성 이하선 종대를 보이는 경우 타액선조영술이 영상진단방법중 1차적인 선택이 된다. Stensen 씨관이나 중심선관(central glandular duct)의 확장을 보이는 경우 만성 타액선염으로 진단할 수 있다. 소아에서의 단일성 양성종괴의 감별진단은 임파절, 혈관종, 양성혼합종양, 저급점액상피암, 임파관종 등이다. 성인에서의 단일성, 양성으로 관찰되는 종괴는 양성혼합종양, Warthin씨 종양, 저급점액상피암, 선상낭성암, 소포상세포암 등이다. 여러개의 종괴를 보이는 경우 Warthin씨 종양, 소포상세포암, 임파종, 육아종, 전이암 등이며 단일성 낭성종괴의 경우는 branchial cleft cyst,, Warthin씨 종양, 상피낭포 등이다.

  • Pleuropulmonary blastoma (PPB) is a rare intrathoracic neoplasm, found solely in childhood. The usual symptoms are dyspnea, chest discomfort, recurrent respiratory infections, fever, dry cough, and chest pain. The progress of PPB is usually aggressive and its progress is generally poor. Lymphatic spread to the hilar and mediastinal nodes can occur Distant metastasis is found in brain, bones, and intra-abdominal organs. Surgical resection is the treatment of choice. When the disease Is too extensive for surgical resection, neoadjuvant chemotherapy can be used. We report 2 cases of pleuropulrnonary blastoma in children successfully treated with multimodal therapy.
