FRACTURE RESISTANCE OF CROWN-ROOT FRACTURED TEETH REPAIRED WITH DUAL-CURED COMPOSITE RESIN AND HORIZONTAL POSTS (수평 포스트와 이중중합 복합레진으로 수복된 치관-치근 복합파절 치아의 파절 저항성에 관한 연구)

  • The purpose of this study was to investigate the fracture resistance of crown-root fractured teeth repaired with dual-cured composite resin and horizontal posts. 48 extracted human premolars were assigned to control group and three experimental groups. Complete crown-root fractures were experimentally induced in all control and experimental teeth. In the control group. the teeth (n=12) were bonded with resin cement and endodontically treated. Thereafter, the access cavities were sealed with dual-cured composite resin. In composite resin core-post group (n=12), the teeth were endodontically treated and access cavities were sealed with dual-cured composite resin. In addition, the fractured segments in this group were fixed using horizontal posts. In composite resin core group (n=12), the teeth were endodontically treated and the access cavities were filled with dual-cured composite resin without horizontal posts. In bonded amalgam group (n = 12), the teeth were endodontically treated and the access cavities were sealed with bonded amalgam. Experimental complete crown-root fractures were induced again on repaired control and experimental teeth. The ratio of fracture resistance to original fracture resistance was analyzed with Kruskal-Wallis test. The results showed that teeth in control and composite resin core-post group showed significantly higher resistance to re-fracture than those in amalgam core group (p < 0.05). The resistance to refracture was high in the order of composite resin - post group, control group, composite resin group and bonded amalgam group. Within the scope of this study, the use of horizontal post could be beneficial in increasing the fracture resistance of previously fractured teeth.

Evaluation of static fracture resistances and patterns of pulpless tooth restored with poly-ether-ketone-ketone (PEKK) post (Poly-ether-ketone-ketone (PEKK) 포스트로 수복한 근관 치료 치아의 정적 파절 저항성 및 파절 형태에 관한 평가)

  • Purpose: The purpose of present study was to investigate fracture strength and mode of failure of endodontically treated teeth restored with metal cast post-core system, prefabricated fiber post system, and newly introduced polyetherketoneketone (PEKK) post-core system. Materials and methods: A total of 21 mandibular premolar were randomly grouped into 3 groups of 7 each according to the post material. Group A was for metal cast post core; Group B for prefabricated glass fiber post and resin core; and Group C for milled PEKK post cores. All specimens were restored with metal crown. The fracture strength of each specimen was measured by applying a static load of 135-degree to the tooth at 2 mm/min crosshead speed using a universal testing machine. After the fracture strength measurement, the mode of failure was observed. The results were analyzed using Kruscal-Wallis test and post hoc Mann-Whitney U test at confidence interval ${\alpha}=.05$. Results: Fracture resistance of PEKK post core was lower than those of cast metal post and fiber reinforced post with composite resin core. In the aspect of fracture mode most of the root fracture occurred in the metal post core, whereas the post detachment occurred mainly in the fiber reinforced post. In the case of PEKK post core, teeth and post were fractured together. Conclusion: It is necessary to select appropriate materials of post for extensively damaged teeth restoration and clinical application of the PEKK post seems to require more research on improvement of strength.

Implant assisted removable partial denture with implant surveyed crown: A 20-month follow-up case report (단일 임플란트 서베이드 크라운을 이용한 하악 임플란트 융합 국소의치: 20 개월 경과 관찰 증례 보고)

  • Traditional options for posterior edentulous treatment include removable partial dentures and implant fixed prostheses. Recently, the concept of implant assisted removable partial denture, in which two treatments are fused, has been introduced in consideration of systemic health and patient's needs, costs, residual alveolar bone status and so on. Implant assisted removable partial denture has the advantage of increasing the retention and stability of the denture and improving its esthetics in cases of large bone defects or biomechanical disadvantages. In addition, it is possible to strategically place the implants in a site where the alveolar bone is relatively sufficient, thereby overcome the limit of the conventional removable partial denture design as well as reducing the burden on a wide range of implant surgery. Cost reduction is also expected. In this case, the patient was treated by placing the implant in both premolar sites of the mandible and fabricating the distal extension removable partial denture with the implant fixed prosthesis as an abutment. After delivering the definitive prosthesis, the patient showed satisfaction with the masticatory function and esthetics. and has been regularly followed-up for more than one year. The following 20-months follow-up case report describes the design of an implant-assisted-removable partial denture (IARPD) in which two cementretained implant crowns used to provide support and stability.

Evaluation of mesiodistal tooth axis using a CBCT-generated panoramic view (CBCT-재구성 파노라마영상의 근원심 치축에 관한 연구)

  • Objective: The purpose of this study was to confirm the reliability of a cone beam computed tomography (CBCT)-generated panoramic view based on a CBCT 3D image and to find the most helpful 2D panoramic image compared with CBCT 3D image when examining the mesiodistal tooth axis. Methods: A test model was constructed according to cephalometric norms. The test model was repeatedly repositioned for CBCT and panoramic radiographic imaging. Panoramic radiographs were acquired at each of the following 3 occlusal plane positions: $-5^{\circ}$, $0^{\circ}$, and $+5^{\circ}$. Measurements of mesiodistal tooth axis in CBCT 3D image, CBCT-generated panoramic view, and panoramic radiographs were compared. Results: Compared with the CBCT-generated panoramic view, CBCT 3D image showed significant difference in the mesiodistal tooth axis in the premolars and no significant difference in the mesiodistal tooth axis in the incisors and canines. Mesiodistal tooth axis on the CBCT-generated panoramic view was significantly different from that on panoramic radiographs. Conclusions: CBCT-generated panoramic view can be a useful tool for evaluating mesiodistal tooth axis.

Impacts of Framing and the Interaction with Involvement on Responses toward the Campaign of Helping the Hungry Abroad (국제 기아 돕기 캠페인의 효과에 미치는 긍.부정 프레이밍과 관여도의 영향-상호작용을 중심으로)

  • This study investigates how framing and the interaction with involvement influences responses toward a help campaign. There could be two possible predictions about the results of framing. One is that, because helping is a risk avoidance behaviors, the positive framing would dominant or the other is that, because helping is mainly mediated by negative emotions such as sadness or guilty, the negative framing would be preferred. The present research was to solve the conflicting predictions and was conducted with a experimental design. The participants were exposed to a campaign emphasizing either positive aspects of the help or negative contexts which would be maintained without any help. The results were that the main effect of framing was not existent, but the significant interaction with involvement on responses was found. The interaction of framing and involvement, though, included unexpected inconsistency between the results of attitude and behavioral intention. The theoretical and practical meanings of the results were discussed.

  • Typical stratigraphic sequences of Paleozoic form the Baegunsan Syncline (equal to Hambaeg Syncline) and Taebaegsan Group is the basement in the southern part of Kangweon Province. Deformation pattern depends on their stratigraphic site and their internal properties. In the biotite schists and meta-sandstones of Pre-Cambrian sequences, flow folds and ptygmatic folds are developed, and high strain deformation is pervasive. Deformational patterns of Cambro-Ordovician are variable because of their various formations. Fracturing is pervasive in the Jangsan Quartzite and Daegi and Maggol Limestone. Welldeveloped slaty cleavage and angular folds of kink and chevron types are the prevailing structures of pelitic rocks. The most characteristic feature of limestone alternated with argillite is the "Compositional cleavage (Author's proposal)" known as the "Worm-eatern" structures. It was known that this structures have a sedimentary origin. But their preferred orienation of long axis of erosional lime holes, originated from tectonic deformation of folding. And this structures have the same character as the axial plane cleavages. Fracturing and long wave concentric folds are dominant characters of the Upper Paleozic sequences. In this area, two folds are superimposed. Field studying and analysis of structures show that the cross-folds of NS-direction are pervasive and were deformed by the EW-directional Baegunsan Syncline. The cross-folding is the major and penetrative deformation and prior to Baegunsan Syncline.

Characteristics of Spray Deposit Pattern and Flow Rate in the Air-Center Nozzle System (Air-center nozzle의 분무살포도(噴霧撒布度)와 분무량(噴霧量)의 특성(特性))

  • The air-center nozzles, being specially designed to supply air into the central part of water stream at seven levels of air volume, were tested for spray deposit pattern and flow-rate at each of the twelve pressure levels ranging from $0.35kg/cm^2$ (5 psi) to $6.33kg/cm^2$ (90 psi) in comparison with those of standard nozzles. The air-center nozzles produced comparatively more stable spray deposit patterns than the standard nozzles. The spray deposit patterns of the air-center nozzle were concentrated gradually in the central region with increase of air volume as a component of spray mixture. The degree of concentration of spray deposit on the central region from the air-center nozzle was higher than that of the standard nozzle, which suggested the possibility of obtaining farther travel distance spraying system at a given performance level. The flow rate of the air-center nozzle was not as much as that of the standard nozzle due to the air within certain limits. The rate of decrease of water flow became smaller with an increase in operating pressure although it changed rapidly in the beginning stage, ranging up to two or three percent of the air flow rate due to the compressive properties of air.

3-D Numerical Analysis for the Verification of Bearing Mechanism and Bearing Capacity Enhancement Effect on the Base Expansion Micropile (선단 확장형 마이크로파일의 3차원 수치해석을 통한 지지 메커니즘 및 지지력 증대효과 검증)

  • Micropiles are cast-in-place piles with small diameters. The advantage of micropile is low construction expense and simple procedures, so it is widely applied to existing buildings and structures for the reinforcement of foundation and seismic performances. The base expansion structure has been developed following the original mechanism of horizontal expansion steps under compressive loading. This kind of structure can be installed at the pile end to improve the bearing capacity by tip area enlargement and horizontal force increment to the pile surface area. However, 'Micropile with base expansion structure' cannot be put into practical use, because detailed verification for the developed technique has not been conducted so far. In this research, 3-D numerical analysis was conducted to figure out the bearing mechanism of base expansion micropile and to verify the bearing capacity improvement compared to the general micropiles. 3-D modelling of micropile with base expansion structure was carried out and input parameter was determined. Bearing mechanism induced by base expansion structure was analyzed by lab-scale modelling, and bearing capacity improvement was verified by field-scale analysis.

Fabrication of removable partial denture on scleroderma patient using 3-dimensional intraoral scanner (전신성 피부경화증 환자에서의 3차원 구강스캐너를 이용한 가철성 국소의치 제작 증례)

  • A three-dimensional (3D) intraoral scanner, which is one of the major developments in digital dentistry, is widely used in fixed prosthodontics. The application of intraoral scanner is now increasing in removable prosthodontics. Sclerotic change induced by scleroderma causes the limitation of mouth opening and multiple loss of the teeth. Conventional prosthodontic procedures are challenging for patients with this disease. This study showed a case of digital approach to the removable prosthodontic treatment of a patient who had the scleroderma and the consequent microstomia. At the provisional stage, the optical impression of patient's oral structures was digitally obtained. Using a 3D printer, the provisional dentures were fabricated. After extraction of hopeless tooth, the definitive digital impression was taken and the metal frameworks were fabricated, based on the data acquired from the impression. The definitive removable partial dentures were completed and delivered to the patient, who was satisfied with the prostheses.

  • For treatment of partially edentulous patients, a treatment using implant is widely used. Treatment method using implant are implant fixed prostheses and removable partial dentures, and for patients with severe bone resorption, removable implant overdenture with the effects of aesthetic and reducing cost can be used as treatment options. Specially, prosthesis with milled-bar and attachment has the effect of being splinted between implant fixtures, higher retention and stability than conventional removable partial denture. And it has the effect of improvement of aesthetic through lip support by denture base. In this case, the patient with severe alveolar bone resorption and partial edentulous maxilla and mandible was treated by implant-assisted removable partial denture using Milled-bar and ADD-TOC attachment. The esthetic was improved by removing the clasp because of effects of additional retention by using the attachment, and reducing palatal coverage of implant-assisted removable partial denture. The clinical results were satisfactory on the aspect of aesthetic and masticatory function.
