Primary Fibrosarcoma of the Lung with Spontaneous Hemothorax (혈흉을 동반한 원발성 폐섬유육종)

  • 폐에 발생하는 원발성육종은 모든 연령측에서 매우 드물고 종야잉 꽤 커질 때 까지 증상이 없는 경우가 있다. 갑작스런 흉통을 주소로 내원한 50세 여자는 자발성 혈흉을 동반한 폐종양이 발견되어 수술을 시행하였다. 수술은 우상엽절제술과 임파절곽청술을 시행하였고 술후 병리조직학적 검사에서 섬유육종으로 진단되었다.

  • Myxofibrosarcoma is one of the most common soft tissue tumors in elderly patients, mostly arising in the extremities, and rarely arising in the chest wall. A 53-year-old women presented with a painful chest wall mass in the manubrium. We excised the mass. The mass was located subdermally, but had infiltrated the underlying muscle layer, and was histologically diagnosed as an intermediate grade myxofibrosarcoma showing myxoid changes and hypercellularity. Here we report a rare case of chest wall myxofibrosarcoma and present a review of the literature.

  • Malignant degeneration of fibrous dysplasia is rare and involves transformation into osteosarcoma, fibrosarcoma and chondrosarcoma. The most frequent sites involved in malignant transformation were craniofacial bones, proximal femur, humerus, pelvis, tibia and scapula in a decreasing order of frequency. An 41-year-old man with a history of polyostotic fibrous dysplasia presented with increasing left arm pain. Plain radiograph showed expansile destructive lesion along the humeral shaft. As initial biopsy report was low grade chondrosarcoma, he underwent marginal resection. However, he developed local recurrence 7 month later and subsequent pathologic finding was chondroblastic osteosarcoma. We report one case of secondary chondroblastic osteosarcoma from polyostotic fibrous dysplasia initially misdiagnosed as low grade chondrosarcoma that caused fallacy in treatment strategy.

Pathological Study of Tumors Occurring in Dog (견종양(犬腫瘍)의 병리학적(病理學的) 검색(檢索))

  • The following tumors occurring naturally in the dog were studied pathologically and discussed briefly. Tumors of the skin and subcutis: Fibroma, Lipoma, Epidermal cyst, Melanosarcoma, Sweat gland adenoma, Mastocytoma (2 cases), Mastosarcoma, and Sebaceous gland carcinoma. Tumors of the spleen and lymph node: Fibrosarcoma of the capsule of spleen, Leiomysarcoma of the spleen, and Lymphosarcoma of the lymph node (2 cases). Tumors of the lung: Bronchogenic carcinoma (3 cases), Adenocarcinoma type, Squamous carcinoma type, and Undifferentiated (round cell) carcinoma type respectively. Tumors of the alimentary tract and liver: Fibroma of the stomach, Hemangioma of the liver, Bile duct carcinoma, Liver cell carcinoma, and Myelogenous leukemia manifested in the liver. Tumor of the peritoneum: Fibrosarcoma. Tumors of the urogenital system: Fibroma of the uterus, Fibroma of the prepuce, Follicular cyst of the ovary, Transmissible venereal tumor of the vagina (6 cases), Carcinoma of the kidney, Adenoma of the prostate (2 cases), and Seminoma of the testis. Tumors of the mammary gland: Mixed tumor (2 cases), and Myoepithelioma. Tumor of the nervous system: Neurofibrosarcoma of the thigh.

Low grade fibromyxoid sarcoma in shoulder -One case report- (견갑부에서 저등급 섬유점액성 육종 - 1례 보고-)

  • Low-grade fibromyxoid sarcoma (LGFMS) is very rarely seen, and it commonly arises from the deep soft tissues of the lower extremities. Histologically, it is characterized by the presence of bland spindle cells with mainly storiform pattern of growth, set in alternating areas with a myxoid or fibrous stroma. The immunohistochemical reaction of this tumor permits a positive diagnosis of low grade fibromyxoid sarcoma and allows its distinction from a number of other benign and malignant soft tissue tumor. We report this rare case together with the review of the literature.

Anti-tumor Effect of Combined Betacarotene with X-irradiation in the Mouse Fibrosarcoma : Cytotoxicity and Tumor Growth Delay (쥐 섬유육종에서 베타카로틴과 방사선조사 병용의 항종양 효과: 세포독성 및 종양성장 지연에 미치는 영향)

  • Purpose :To investigate whether combined beta-carotene with X-Irradiation has more enhanced radition response than X-irradiation or not, we peformed a experiment about in vitro cytotoxlcity of beta-carotene and/or X-irradiation in the fibrosarcoma cells, tumor growth delay of combined beta-caroten with/or X-irradiation in the mouse fibrosarcoma. Materials and Methods : 2$\%$ emulsion of beta-carotene was serially diluted and used. X-Irradiation was given by 6 MeV linear accelerator. The cytotoxicity of beta-carotene in vitro was evaluated from clonogenic assay. To compare the cytotoxiclty between combined beta-carotene with X-irradiation and X-irradiation group, 2 mg/ml of beta-carotene was contacted to fibrosarcoma (FSall) cells for 1 hour before X-irradiation. For the tumor growth delay, single 20 Gy was given to FSall tumor hearing C3H/N mice whic was classified as beta-crotene with X-irradiation group (n=5) and X-irradiation alone group (n=5). 0.2 ml of 20 mg/kg of beta-carotene were i.p. injected to mice 30 minute before X-irradiation in the beta-crotene with X-irradiation group. The tumor growth delay defined as the time which reach to 1,000 mm$^{3}$ of tumor volume. Results : (1) Cytotoxicity in vitro: 1) survival fraction at beta-carotene concentration of 0.002,0.02,0.2 and 2 mg/ml were 0.69$\pm$0.07, 0.59$\pm$0.08, 0.08$\pm$0.008 and 0.02$\pm$0.006, respectively. 2) each survival fraction at 2, 4, 6 and 8 Gy in the 2 mg/ml of beta-carotene + X-irradiation group were 0.13$\pm$0.05, 0.03$\pm$0.005, 0.01 $\pm$0.002 and 0.009$\pm$0.0008, respectively. But each survival fraction at same irradiation dose in the X-irradiation group were 0.66$\pm$0.05, 0.40$\pm$0.04, 0.11$\pm$0.01 and 0.03$\pm$0.006, respectively(p<0.05). (2) The time which reach to 1,000 mm$^{3}$ of tumor volume of beta-carotene + X-irradiation group and X-irradiation alone group were 18, 19 days, respectively(p>0.05) Conclusion : The contact of beta-caroten to Fsall cells showed mild cytotoxicity which 띤as increased according to concentration. The cytotoxicity of combined beta-carotene with X-irradiation more increased than that of X-irradiation, additionally, And there was significant difference of cytotoxicity between two groups. But there were no significant difference of the growth delay of fibrosarcoma between two groups.

  • Low grade fibromyxoid sarcoma is a recently recognized, uncommon soft tissue neoplasm. It has a tendency to develop in deep soft tissue of young adults and a possibility of local recurrence or distant metastasis. Diagnostic criteria have not been well defined and this tumor has not been accepted as a distinct entity. Histologically, it is characterized by the presence of bland spindle cells with mainly whorled pattern of growth, set in alternating areas with a myxoid or fibrous stroma. Careful consideration of the morphological and immunohistochemical features of this tumor permit a positive diagnosis of low grade fibromyxoid sarcoma and allow its distinction from a number of other benign and malignant soft tissue neoplasms. We experienced a low grade fibromyxoid sarcoma in chest wall and report this case with a review of the literature.

Surgical Treatment of Dermatofibrosarcoma Protuberans of the Chest Wall (흉벽의 융기피부섬유육종의 수술적 치료)

  • A 77-year-old man presented with a huge protruding mass on the left anterior chest wall. The tumor was resected and diagnosed as dermatofibrosarcoma protuberans (DFSP). DFSP is an uncommon, intermediate-grade mesenchymal cutaneous tumor which extends deep into subcutaneous tissue and may invade through the fascial planes and into muscle but rarely metastasize. Histologically, DFSP is composed of spindle cells arranged in an irregularly whorled or storiform pattern. The histological diagnosis can be confirmed with immunohistochemical staining for CD34. We report a case of DFSP. The tumor was completely excised and the chest wall was reconstructed using latissimus dorsi muscle flap and skin graft.
