Study of the Antinociception Induced by Opioids and the Proenkephalin Gene Expression in Spontaneously Hypertensive Rats (선천성 고혈압쥐에서의 Opioid에 의한 진통작용과 Proenkephalin유전자 발현에 대한 연구)

  • The present studies were carried out to determine if antinociceptive action of morphine and ${\beta}-endorphin$ administered intraventricularly was changed in. pentobarbital anesthetized spontaneously hypertensive rats (SHR) and Wistar-Kyoto (WKY) rats. Antinociception was assessed by the tail-flick test. The $ED_{50}$ values of antinociception for morphine administered intraventricularly were 1.9 and 1.2 nmol for WKY and SHR rats, respectively. The $ED_{50}$ values of antinociception for ${\beta}-endorphin$ administered intraventricularly were 0.40 and 0.12 nmol for WKY and SHR rats, respectively. The $[Met^5]-enkephalin$ (ME) and proenkephalin mRNA levels in midbrain, pons and medulla, or lumbar section of the spinal cord in WKY and SHR rats were measured by the radioimmunoassay and Northern blot assay, respectively. There were no differences of ME and proenkephalin mRNA levels in these tissues between WKY and SHR rats. The results suggest that ${\beta}-endorphin$ but not morphine administered intraventricularly produces a greater antinociception in SHR rats. This increased antinociceptive effect of ${\beta}-endorphin$ in SHR rats may be not, at least, due to the alterations of ME And proenkephalin mRNA levels in the midbrain, pons and medulla, or spinal cord.

  • Smith-Magenis syndrome (SMS) is a clinically recognizable multiple congenital anomaly and mental retardation syndrome caused by an interstitial deletion of chromosome 17 p11.2. Physical features include short stature, characteristic facial appearance: flattened mid-face, down-turned mouth, prominent and often rosy cheeks; prominent jaw in older children and adults, chronic ear infections, hearing impairment, eye problems, including: strabismus (an eye which turns in or out) and myopia (nearsightedness), hoarse voice, short fingers and toes, heart defects or murmurs, problems related to the urinary system, scoliosis (curvature of the spine), an unusual gait (walking pattern), and decreased sensitivity to pain. Behavioral and developmental characteristics include speech delay and articulation problems, developmental delay, learning disability, mental retardation, hyperactivity, self-injury, including: head banging; hand biting; picking at skin, sores and nails; pulling off finger- and toenails; inserting foreign objects into ears, nose, or other body orifices, explosive outbursts, prolonged tantrums, destructive and aggressive behavior, excitability, arm hugging or hand squeezing when excited. This report is the case of a Korean 3-year-3-month old male with Smith-Magenis syndrome referred from local clinic for the treatment of dental caries. The patient was treated by physical restraint after prophylatic administration of antibiotic(Amoxacillin 50mg/kg).

Sevoflurane Insufflation Sedation for the Dental Treatment of a Patient with Pulmonary Arterial Hypertension : A Case Report (폐동맥 고혈압 환자의 치과치료에서 세보플루란 흡입 진정의 사용 : 증례보고)

  • Pulmonary arterial hypertension (PAH) is a common complication of Congenital heart defects (CHD) with left-to-right shunts, and PAH with increased pulmonary vascular resistance (PVR) is associated with considerable morbidity and mortality. General anesthesia (GA) can be life-threatening in patients with PAH, because the positive pressure ventilation during GA increases pulmonary arterial pressure and decreases pulmonary blood flow. This may also lead to hypoxia. Therefore, spontaneous ventilation may be safer than positive pressure ventilation in patients with PAH. A five-year-old male child, weighing 11 kg, with medical history showing a total correction of Tetralogy of Fallot (TOF) in 2009 and ongoing treatment with hypertension (HTN) medicine since 2007, visited the Dankook University Dental Hospital. He had multiple dental caries, and the treatment was completed under sevoflurane insufflation sedation via nasal cannula. The patient remained sedated throughout the operation while maintaining normal vital signs and spontaneous respiration. In conclusion, sevoflurane insufflation sedation may be a safer alternative to GA for the dental treatment of patients with PAH.

DENTAL TREATMENT OF A PEDIATRIC PATIENT WITH HOMOZYGOUS PROTEIN C DEFICIENCY: A CASE REPORT (동형접합성 단백 C 결핍 환아의 치과적 치험례)

  • Protein C deficiency increases the risk of thrombosis due to the lack of anticoagulant factor protein C. Among the numerous congenital protein C deficiencies, homozygous protein C deficiency has an especially low protein C activity level, that it is almost undetectable. It is a rare disease with a probability of 1:250000~500000. The signs and symptoms of homozygous protein C deficiency include purpuric, necrotic dermatosis, ecchymosis, blindness, and thrombosis in central nervous system. A 4-year-old girl was brought to the clinic with a chief complaint of extensive caries. The child was under warfarin medication in order to prevent possible complications during dental treatment. We consulted the pediatric department. Without warfarin intake, serious complications may occur due to thrombosis during dental treatment. Therefore, certain warfarin dosage (INR 3~5) and fresh frozen plasma as a backup for excessive hemorrhage were recommended. This child was a severely disabled child with the loss of vision, and it was difficult to manage her behavior effectively. Thus, dental treatment was carried out under general anesthesia, where bleeding control would be also easier to achieve.This report presents the case of a 4-year-old girl with protein C deficiency, who has received dental treatment for extensive caries under general anesthesia.

Effects of Fused Primary Teeth on the Permanent Dentition (유치열기의 융합치가 영구치열에 미치는 영향)

  • The purpose of the study was to investigate the distribution of primary fused teeth and identify the correlation between primary fused teeth and their effect on permanent dentition. 2575 children between the age of 4 and 6 in Kyungpook National University Hospital from January 2009 to August 2015 were investigated. A total of 84 children (46 boys and 38 girls) had fused teeth. 14 of these children had two fused teeth. Prevalence of caries involvement was in 65% of maxilla and 6% of mandible. Prevalence of permanent successors missing was 86.3% in the cases involving maxillary central and lateral incisor, 70% in mandibular lateral incisor and cuspid, 11.7% in mandibular central and lateral incisor. 27 of 84 children (32.1%) had supernumerary teeth. The highest prevalence rate is seen in the cases involving maxillary central and lateral incisor. Delayed permanent tooth eruption was only observed in the maxilla because of developing supernumerary tooth. Early diagnosis of fused tooth in the primary dentition can allow the dentist to make treatment plan at the appropriate time in accordance with the tooth arrangement and tooth development.

Retrospective Study of Infants who Visited Pediatric Dentistry (소아치과에 내원한 영아들에 대한 후향적 연구)

  • The first dental visit is recommended at the time of the eruption of the first tooth and no later than 12 months of age. However, even before the age of 1, children can visit the dental hospital for various reasons. The purpose of this study was to analyze the reasons for the dental visit of infant. From January 2006 to December 2015, medical records of infants who visited the Department of Pediatric Dentistry of Kyung Hee University were analyzed. The total number of patients was 419 (238 males and 181 females). The reasons for the dental visits were trauma (47.5%), natal/neonatal tooth (19.8%), dental caries (8.1%), teething problem (4.3%), abnormal frenum (3.6%), soft tissue swelling (3.6%), Bohn's nodule (3.3%), cleft lip and palate (2.9%), gingival neoplasm (1.9%), tongue ulceration (1.7%), oral examination (1.4%), enamel hypoplasia (1.2%) and abnormal temporomandibular joint sound (0.7%). According to this study, there were various oral diseases that could occur in infants. Since infants are usually cared by caregivers, pediatricians, and obstetricians, education of oral diseases of infants is needed to manage the oral symptoms properly.

Association between Developmental Anomalies of Permanent Lateral Incisors and Tooth Eruption Disturbances (영구 측절치 발육 이상과 치아 맹출 장애의 연관성)

  • The purpose of this study was to examine the prevalence of developmental anomalies of permanent lateral incisor and eruption disturbances and analyze the association between two components. Panoramic radiographs of 3984 patients (aged 5 to 13 years) who visited the Department of Pediatric Dentistry of Seoul National University Dental Hospital between November 2016 and October 2017 were screened. The prevalence of developmental anomalies of permanent lateral incisors was 10.2%. The most common developmental anomalies were congenitally missing teeth(66.1%), followed by peg lateralis(33.5%). The prevalence of eruption disturbances was 16.5%. Among the patient with developmental anomalies of permanent lateral incisors, associated eruption disturbances were appeared on 31.1% of patients. Peg lateralis(p < 0.001), underdeveloped lateral incisors(p < 0.001) and dens invaginatus(p = 0.004) were associated significantly with eruption disturbance of permanent teeth. Eruption disturbance in patients with peg lateralis and dens invaginatus was most prevalent in maxillary canine. For patients with underdeveloped lateral incisor, eruption disturbance of maxillary central incisor was most frequent. It is important for patients with developmental anomalies of lateral incisors to detect associated eruption disturbance early through regular checkup. Diagnosis and treatment plan in view of such relationships is important in order to treat appropriately at the optimal time.

A Study on the Refractive Error according to Subjects of Industrial High School Students in Deajeon Metropolitan City (대전광역시 공업고등학교 학생들의 학과별 굴절이상에 관한 연구)

  • The results of having studied the relationships between environmental factors and family history which affected distribution of ametropia according to departments are like these. The subjects of study were 920 persons who were the industrial high school students in Daejeon. First, When we searched ametropia degree and congenital factors, expecially in case of emmetropia, it showed that the students' parents who wore glasses amounted to 22.6% in ratio and the students' brothers and sisters wore glasses in 22.8%. But in case of the students of ametropia, their parents' rate of wearing glasses was 27.7%. The result showed that refractive error status of offsprings was related to their parents' eye condition. Second, We studied emmetropia and ametropia who used computer for over 3 hours in order to see relationships between refractive error and acquired factors. The emmetropias were 45.6%, while the ametropias were 70.1%. The result showed that it appeared 24.5% more in ametropia. In the end, refractive error can be greatly influenced by environmental factors. Third, The result of comparing distribution of ametropia with VDT using time according to each department showed the students of constructional information department had much more ametropia rate. They used computers than any other students in taking lessons and spent much more time than other department's students.

Effects of intramuscular injection of Taurine-FTM on anti-hepatotoxicity and innate immunity in olive flounder, paralichthys olivaceus (Taurine-FTM의 근육 주사가 넙치의 간 독성 완화와 선천성 면역에 미치는 효과)

  • In the study, we investigated the effect of Taurine-FTM, which is a commercially available fishery nutritional supplements complex, on anti-hepatotoxicity stressed with thioacetamide (TAA) and innate immune responses in olive flounder. To investigate the change in liver toxicity, firstly, TAA (30 ppm/100 g of fish) was intraperitoneally (i.p.) administered 12 hr after the intramuscular (i.m.) injection of Taurine-FTM (0.02 ml/100 g of fish)(Taurine/TAA). Secondly, Taurine-FTM was i.m. injected 24 hr after the administration of TAA (TAA/Taurine). Finally, TAA was administered simultaneously with Taurine-FTM (TAA+Taurine). All blood samples were collected 24 hr after injection. GOT level in group Taurine/TAA appeared similar to the control, whereas group TAA/Taurine and TAA+Taurine showed significantly increased (p<0.05) levels compared to the control. In GPT level, group Taurine/TAA and TAA/Taurine showed elevated levels compared to the control, whereas no significant difference was observed between group TAA+Taurine and the control. Serum ACH50 activity was significantly (p<0.05) augmented 24 hr after Taurine-FTM injection compared to the control group, whereas no significant increase was observed 48 hr after Taurine-FTM injection. On the other hand, serum lysozyme activity elevated in an acute stressed condition appeared significantly down-regulated 24 and 48 hr after Taurine-FTM injection compared to the control. In conclusion, i.m. injected Taurine-FTM augmented flounder serum complement activity and decreased a possible handling stress resulting in reducing a serum lysozyme activity and recovering hepatotoxicity. Thus, it is assumed that i.m. injection of Taurine-FTM mixed with antibiotics or available vaccines could be utilized as an anti-hepatotoxic recipe in fish culture industry.

THE EFFECTS OF CRANIOCERVICAL POSTURE AND THE POSITION OF TONGUE AND HYOID BONE ON CRANIOFACIAL MORPHOLOGY (두경부자세 및 혀, 설골의 위치가 두개안면헝태에 미치는 영향에 관한 연구)

  • The purpose of this study was to examine the associations of head posture the position of the tongue or the hyoid bone to craniofacial structure. Cephalograms taken in Natural head position(NHP) of 90 dental students (50 in male, 40 in female, 20 to 30 years in age) were traced and measured using the extracranial true horizontal and vertical lines. The obtained results were as follows; 1. There was no sex difference in head posture, but the hyoid bone was placed anteroinferiorly in male more than in female and anteroinferior inclination of the hyoid bone showed greatly in male. 2. The more inclined was the cervical column, the less prognathic was the face in natural head posture, and the larger cervical curvature, the more vertical pattern of the face. 3. The less small showed craniocervical angulation, the more anteriorly placed was the hyoid bone to the cranial base, and there was no significantly association between craniocervical angulation and the vertical position of the hyoid bone. 4. The more prognathic was the mandible, the more anteriorly placed was the hyoid bone, and there was slightly association between the craniofacial morphology and the vertical position of the hyoid bone.
