Treatment planning of lung cancer with density corrected Computed tomography. Eighty-seven patients with lung cnacer who had radiation therapy in Yeungnam University Medical Center between, April 1 1990 and Aug. 30 1993 were retrospectively evaluated total tumor dose, dose distribution, field correction, and loading change, compared with contour or CT image planning and density corrected CT planning. In dose distribution, higher dose was calculated in compare with density corrected CT planning less than 5% difference were found in 45 patient(52%), 5-10% in 25 patients (29%), 10-15% in 15 patients (17%) and over 15% in 2 patients (2%). Correction of treatment field was performed in 18 patients (21%) and changing of dose loading was given in 15 patients (17%). In conclusion, we emphasize that density corrected CT planning is the very important factor which contribute to increase therapeutic gain by exact selection of target volume, target dose, normal tissue dose and dose of critical organ.

The purpose of this paper is to develop an efficient method for the quick determination of multiple isocenters plans to provide optimal dose distribution in sterotactic radiosurgery. A Spherical dose model was developed through the use of fit to the exact dose data calculated in a 18cm diameter of spherical head phantom. It computes dose quickly for each spherical part and is useful to estimate dose distribution for multiple isocenters. An automatic computer search algorithm was developed using the relationship between the isocenter move and the change of dose shape, and adapted with a spherical dose model to determine isocenter separation and cellimator sizes quickly and automatically. A spheric81 dose model shows a comparable isodose distribution with exact dose data and permits rapid calculations of 3-D isodoses. the computer search can provide reasonable isocenter settings more quickly than trial and error types of plans, while producing steep dose gradient around target boundary. A spherical dose model can be used for the quick determination of the multiple isocenter plans with 3 computer automatic search. Our guideline is useful to determine the initial multiple isocenter plans.
This study tries to evaluate the usefulness CSI treatment. Compare the standard technique and simple technique, using the volume region of a high dose of Field joints (hot spot) or low dose regions (cold Spot). In patients who agreed to this study, obtain CT image using CT simulator skull to pelvis region. Standard Technique were performed on the movement of the joint radiation field range and simple technique has set a treatment plan to secure the radiation field range and analyzed treatment planning. Under analysis standard technique occurred the area of the high dose(Hot Spot) for the area overlapping the field and simple technique showing a uniform doses. CI indices of standard technique and simple technique was 1.6~3, 1.6~1.87, CN indices was 0.32~0.53, 0.46~0.51 and HI indices was 0.11~0.33, 0.2~0.26. Therefore, adjacent to part of the dose distribution junction more equally than simple technique compared to the Standard Technique. Compare the dose distribution patterns using CI, CN, HI indices, showed a uniform dose distribution in the simple technique. so, simple technique was determined appropriate treatment the CSI.

Purpose : To compare the radiation doses of prostate cancer patients according to changes in abdominal body shape and energy during Volumetric modulated arc therapy(VMAT). Materials and Methods : Seven patients with prostate cancer were enrolled in this study. VMAT treatment plan was established at 6, 10, and 15 MV while changing from -2.0 cm to 2 cm by 0.5 cm. Conformal index(CI), homogeneous index(HI), $D_{max}$, $D_{95%}$, $D_{50%}$ and $D_{2%}$ of PTV were examined in order to evaluate the change of dose in the target organ according to body shape change. Normal organ of the femoral head, rectum and bladder was analyzed to evaluate dose changes. Results : The dose of $D_{max}$ 6 MV in PTV increased to 107.2 % in 1.0 cm body shape reduction, and 10 MV and 15 MV dose increased to 107.1 % and 107.0 % in 1.5 cm body reduction, respectively. The dose of $D_{50%}$ 6 MV in PTV decreased to 99.64 % in 1.0 cm body shape increase, and in 10 MV and 15 MV dose decreased to 99.79 % and 99.97 % in 1.5 cm body increase, respectively. In 2.0 cm body type increase, the dose was decreased to 99.30 % and 99.52 %, respectively. Doses for rectum and bladder gradually increased with decreasing weight, and dose decreased with decreasing weight. 6 MV, and $V_{70Gy}$ at 10 MV increased from 11.50 % to 12.76 % when the external shape decreased by 2.0 cm. The bladder $V_{70Gy}$ also increased from 14.0 % to 15.2 %. It was also shown that the dose increased as the body weight decreased in the femoral head. Conclusion : In the treatment of VMAT, dose distribution can be changed according to the change of abdominal shape. SSD and CBCT were used to decrease the body shape by more than 1cm or more than 1.0 cm at 6 MV and the body shape by more than 1.5 cm or more than 1.5 cm at 10 MV or 15 MV. It is considered that a new treatment plan should be established through re-simulation.
Wedge shaped isodoses are desired in a number of clinical situations. Hard wedge filters have provided nominal angled isodoses with dosimetric consequences of beam hardening, increased peripheral dosing, nonidealized gradients at deep depths along with the practical consequendes of filter handling and placement problems. Dynamic wedging uses a combination of a moving collimator and changing monitor dose to achieve angled isodoses. The segmented treatment tables(STT) that monitor unit setting by every distance of moving collimator, was induced by numerical formular. The characteristics of dynamic wedge by STT compared with real dosimetry. Methods and Materials : The accelerator CLINAC 2100C/D at Yonsei Cancer Center has two photon energies (6MV and 10MV), currently with dynamic wedge angles of 15$^{\circ}$, 30$^{\circ}$, 45$^{\circ}$ and 60$^{\circ}$. The segmented treatment tables(STT) that drive the collimator in concert with a changing monitor unit are unique for field sizes ranging from 4.0cm to 20.0cm in 0.5cm steps. Transmission wedge factors were measured for each STT with an standard ion chamber. Isodose profiles, isodose curves, percentage depth dose for dynamic wedge filters were measured with film dosimetry. Dynamic wedge angle by STT was well coincident with film dosimetry. Percent depth doses were found to be closer to open field but more shallow than hard wedge filter. The wedge transmission factor were decreased by increased the wedge angle and more higher than hard wedge filters. Dynamic wedging probided more consistent gradients across the field compared with hard wedge filters. Dynamic wedging has practical and dosimetric advantages over hard filters for rapid setup and keeping from table collisions. Dynamic wedge filters are positive replacement for hard filters and introduction of dynamic conformal radiotherapy and intensity modulation radiotherapy in a future.

1. 목적 : head and neck cancer 환자의, C-T 영상을 이용한 방사선치료계획시 치과 보철물에 의해 발생하는 artifact가 선량 계산에 미치는 영향을 분석하고자 한다. 2. 재료 및 방법:두 경부와 유사한 크기의 Polystyrenes Phantom ($20{\times}20{\times}25cm^3$) 을 제작하고, 팬톰내에 금으로 인공보철물을 제작하여 보철물 부착 전.후를 C-T Scan (High Speed Advantage, GE, US) 하였다. artifact에 의한 영향을 쉽게 분석하기위해 팬톰내에 다른 구조물은 만들지 않았으며 두가지 방법으로 얻어진 영상을 이용하여 조사면의 크기와 조사 방향을 변화 시켜 가며 1문 조사(SSD 100 cm)에 의한 치료 계획(3D RTP system, Prowess, US)을 수립하여 기준점(5,10 cm depth)에서의 선량 변화를 비교 분석하였다. 아울러 3회 반복 scan하여 artifact에 발생 유형과 CTNo을 이용한 density을 분석하였다. 3. 결과: C-T Scan으로 얻어진 image 상에 나타난 Artifact는 CT no $-1000{\sim}+2775$(기준 $-1000{\sim}+3700$)까지의 다양한 값을 가지며 보철물을 기준으로 방사형태로 분포하였다. artifact가 선량 계산에 미치는 영향을 분석한 결과 보철물 사용시 5cm깊이의 기준점에서 절대선량은 평균 $+1.5{\pm}2.8\%$, 10 cm 깊이에서는 $+1.8{\pm}3.5\%$의 오차를 보였다. 조사방향에 의한 오차는 artifact에 대해 측면 조사한(gantry $270^{\circ}$)경우에서 높게 관찰되었다. 4. 결론: 두 경부 종양의 방사선 치료시 치과 보철물에 의한 artifact는 흔히 관찰가능하며 본 실험을 통해 다양한 형태와 다양한 density을 가짐을 알수있었다. 영상에 나타난 정도에 비해 선량계산에 미치는 평균 오차는 낮게 평가되었지만 조사 방향과 보철물의 위치에 따라 변동이 크게 나타날 수 있어 치료 계획시 가능한 artifact의 영향을 적게 받는 빔의 선택이 정확한 선량 계산에 도움을 줄 것으로 사료된다.
Purpose : The purpose of this study is to investigate fundamental aspects of the dose response of fluorescent screen-based electronic portal imaging devices (EPIDS). Materials and Methods : We acquired scanned signal across portal planes as we varied the radiation that entered the EPID by changing the thickness and anatomy of the phantom as well as the air gap between the phantom and the EPID. In addition, we simulated the relative contribution of the scintillation light signal in the EPID system. Results : We have shown that the dose profile across portal planes is a function of the air gap and phantom thickness. We have also found that depending on the density change within the phantom geometry, errors associated with dose response based on the EPID scan can be as high as $7\%$. We also found that scintillation light scattering within the EPID system is an important source of error. Conclusion : This study revealed and demonstrated fundamental characteristics of dose response of EPID, as relative to that of ion chambers. This study showed that EPID based on fluorescent screen cannot be an accurate dosimetry system.
The quasi-conformation therapy was performed to get a homogeneous dose distributions for irregeular shaped tumor lesion by using the arc moving beam and beam modifying filter which was made by cerrobend alloy($\rho$=9.4 g/cc) metal. In our dose calcuation programme, it was fundmentally based on Clarkson's method to calcuate the irregular multi-step block field in rotation therapy. In this study, the expected relative depth doses under multipartial attenuator agree well with measured data at same plane. The results of comparison the dose computation with that of TLD measurement are very closed within ${\pm}5\%$ uncertainties in the irradiation to phantom with quasi-comformation method. And it has shown that irregular typed multi-step filter can be applied to quasi-conformation therapy in high energy radiation plannings.

Wedge filter could use to increase the dose distribution at the hot dose regions. We evaluated dose discrepancy at surface and build region in the infield and outfield that Metal Wedge (MW) and Enhance Dynamic Wedge (EDW) were interact with photon. In this paper, we used Gafchromic EBT3 film that had excellent spatial resolution, composed the water equivalent materials and changed the optical density without development. The set up conditions of linear accelerator were fixed 6 MV photon, 100 cm SSD, $10{\times}10cm^2$ field size and were irradiated 400 cGy at Dmax. The dose distribution and absorbed dose were evaluated when we compared the open field with $15^{\circ}$, $30^{\circ}$, $45^{\circ}$ metal wedge and enhanced dynamic wedge. A $15^{\circ}$ metal wedge could increase the surface and build up region dose than using a $15^{\circ}$ enhanced dynamic wedge. A $30^{\circ}$ metal wedge could decrease the surface and build up region dose than using a $30^{\circ}$ enhanced dynamic wedge. A $45^{\circ}$ metal wedge could decrease by large deviation the surface and build up region dose than using a $15^{\circ}$ enhanced dynamic wedge. The dose of penumbra region at outfield were increased on the thick side but were decreased on the thin side. It could be decrease the surface dose and build up region dose, if the metal wedge filters were properly used to make a good dose distribution and not closed the distance of surface.
Aquaplast Thermoplastic (AT) is a tissue-equivalent oral compensator that has been developed to improve dose uniformity at the common boundary and around the treated area during radiotherapy in patients with head and neck cancer. In order to assess the usefulness of AT, the degree of improvement in dose distribution and physical properties were compared to those of oral compensators made using paraffin, alginate, and putty, which are materials conventionally used in dental imprinting. To assess the physical properties, strength evaluations (compression and drop evaluations) and natural deformation evaluations (volume change over time) were performed; a Gafchromic EBT2 film and a glass dosimeter inserted into a developed phantom for dose verification were used to measure the common boundary dose and the beam profile to assess the dose delivery. When the natural deformation of the oral compensators was assessed over a two-month period, alginate exhibited a maximum of 80% change in volume from moisture evaporation, while the remaining tissue-equivalent properties, including those of AT, showed a change in volume that was less than 3%. In a free-fall test at a height of 1.5 m (repeated 5 times as a strength evaluation), paraffin was easily damaged by the impact, but AT exhibited no damage from the fall. In compressive strength testing, AT was not destroyed even at 8 times the force needed for paraffin. In dose verification using a glass dosimeter, the results showed that in a single test, the tissue-equivalent (about 80 Hounsfield Units [HU]) AT delivered about 4.9% lower surface dose in terms of delivery of an output coefficient (monitor unit), which was 4% lower than putty and exhibited a value of about 1,000 HU or higher during a dose delivery of the same formulation. In addition, when the incident direction of the beam was used as a reference, the uniformity of the dose, as assessed from the beam profile at the boundary after passing through the oral compensators, was 11.41, 3.98, and 4.30 for air, AT, and putty, respectively. The AT oral compensator had a higher strength and lower probability of material transformation than the oral compensators conventionally used as a tissue-equivalent material, and a uniform dose distribution was successfully formed at the boundary and surrounding area including the mouth. It was also possible to deliver a uniformly formulated dose and reduce the skin dose delivery.
