목적 : 고감도 형광판과 필름을 이용하여 실시간으로 선량을 측정하여 IMRT 선량분포를 검증하는데 사용하는 가능성을 알아보고자 하였다. 대상 및 방법 : 본 연구에서 개발한 물팬텀은 지름 25cm 아크릴 원통과 원통의 중앙부분에 삽입되는 고감도 형광판으로 구성되어 있다. 이를 사용하여 dose linearity correction factor를 구하기 위해 dmax 지점에서 6MV x-ray를 고감도형광판에 조사하여 blurring correction factor를 구하였다. CCD를 이용하여 고감도 형광판에서 나오는 영상을 수집하였다. 고감도 형광판에서 수집한 영상의 x축 profile은 RTP에서 얻은 profile과 비교하였고, 이온전리함으로 scanning한 데이터를 이용하여 고감도 형광판과 물에서 빛에 의한 산란선 때문에 발생하는 blurring effect를 교정하였다. 여기서 계산된 blurring effect factor를 고감도 형광판에서 수집된 영상에 적용하였다. 결과 : CCD 카메라는 형광판의 전 영역을 감지할 수 있고, 조사시간은 형광판의 중첩된 영상의 선량에 비례하였다. 물팬텀에서 형광판의 blurring effect 는 가우시안 분포로 표현할 수 있었다. 또한 Deconvolution kernel은 원통 팬텀에서 지름 $\pm$5cm 이내의 범위에 위치하였고, 따라서 형광판 영상으로부터의 실제 선량분 포를 뽑아낼 수 있었다. RTP 에서 계산된 선량분포와 blurring correction factor로 교정한 후 중첩시켜 얻은 고감도 형광판 영상의 선량분포는 일치하였다. 결론 : 정기적인 IMRT 선량 검증에 대한 실시간 선량측정 방법이 개발되었다. 고감도 형광판 영상과 CCD 카메라를 사용한 물팬텀으로, IMRT 치료계획에 대한 선량분포를 검증할 수 있는 가능성을 보였다.비의 회전에 의한 오차 보정, 필름의 광학적 밀도에 관한 보정 등 여러 가지 계통적 오차들에 대한 보정들이 선량분포 확인과정의 이해와 그 기준마련에 도움이 되겠지만 우리가 다룬 원점 불일치에 비해서 상대적으로 무시할 수 있었다. 마지막으로 선량분포 확인의 최종목표인 3 차원 선량분포 확인의 실제 적용을 위한 연구가 최적화 알고리듬을 이용하여 실험 중에 있다.\times$5cm, 10$\times$10cm, 15$\times$l5cm, 20$\times$20cm인 경우, 측정하여 얻은 PSF가 0.8%, 0.2%, 0.4%, 0.2%로 약간 높지만, 두 값은 매우 유사한 것으로 나타났다. 그리고, 기존의 BSF를 이용해 구한 TAR과 BJR 25에서 권고하는 PSF를 이용해 구한 TAR을 비교한 결과 field size 에 따라 약 1%-1.5% 정도로 BSF를 이용하여 구한 TAR보다 PSF를 이용하여 구한 TAR이 1.3% 정도 높게 나타났지만, 이것은 두 값의 절대적인 차이일 뿐, 실제로는 PSF를 이용하여 구한 TAR이 측정해서 구한 TAR과는 매우 유사한 값을 보여주고 있다. 결론 : 기존의 BSF를 이용해 구한 TAR과 PSF를 이용해 구한 TAR을 비교하였을 때, 약 1.3% 정도 높게 내고 있지만, 기존의 TAR보다는 PSF를 이용해 구한 TAR이 BJR 25와 잘 일치하고 있으므로 Co-60 원격치료용 방사선 조사장치를 사용할 경우 BSF보다는 PSF를 사용하는 것이 타당한 것으로 사료된다.tokines의 변화는 비록 통계학적인 차이는 없지만 비타민 C를 사용한 환자의 cytokines이 모두 사용하지 않은 환자에 비해 감소하였음을 보였다. 비타민 C는 부작용이 거의 없는 안전한 약으로서 말기 암 환자에서 비타민 C사용은 임상 증상을 호전시키는 데 도움
Some modern accerelators provide a dual energy for photon beam treatment. The main advantages of dual energy in the treatment of rectosigmoid or rectal cancer are as fellows. 1. Dose in the critical organ such as small intestine, bladder and genital organ are reduced. 2. Presacral and perineal area is fully covered. Dose distribution analysis such as calculation of dose in a target volume, isocenter, $D_{nax}$ and dose spectrum in any region of interest are possible. Examples of plan are given and results are discussed.
Methodology to evaluate the effective doses to adults undergoing various diagnostic x-ray examinations were established by Monte Carlo simulation of the x-ray examinations. Anthropomorphic mathematical phantoms, the MIRD5 male phantom and the ORNL female phantom, were used as the target body and x-ray spectra were produced by the x-ray spectrum generation code SPEC78. The computational procedure was validated by comparing the resulting doses to the results of NRPB studies for the same diagnostic procedures. The effective doses as well as the organ doses due to chest, abdomen, head and spine examinations were calculated for x-rays incident from AP, PA, LLAT and RLAT directions. For instance, the effective doses from the most common procedures, chest PA and abdomen AP, were 0.029 mSv and 0.44 mSv, respectively. The fact that the effective dose from PA chest x-ray is far lower than the traditional value of 0.3 mSv(or 30 mrem), which results partly from the advances of technology in diagnostic radiology and partly from the differences in the dose concept employed, emphasizes necessities of intensive assessment of the patient doses in wide ranges of medical exposures. The methodology and tools established in this study can easily be applied to dose assessments for other radiology procedures; dose from CT examinations, dose to the fetus due to examinations of pregnant women, dose from pediatric radiology.

Purpose : To setup procedures of quality assurance (OA) for implementing intensity modulated radiation therapy (IMRT) clinically, report OA procedures peformed for one patient with prostate cancer. Materials and methods : $P^3IMRT$ (ADAC) and linear accelerator (Siemens) with multileaf collimator are used to implement IMRT. At first, the positional accuracy, reproducibility of MLC, and leaf transmission factor were evaluated. RTP commissioning was peformed again to consider small field effect. After RTP recommissioning, a test plan of a C-shaped PTV was made using 9 intensity modulated beams, and the calculated isocenter dose was compared with the measured one in solid water phantom. As a patient-specific IMRT QA, one patient with prostate cancer was planned using 6 beams of total 74 segmented fields. The same beams were used to recalculate dose in a solid water phantom. Dose of these beams were measured with a 0.015 cc micro-ionization chamber, a diode detector, films, and an array detector and compared with calculated one. Results : The positioning accuracy of MLC was about 1 mm, and the reproducibility was around 0.5 mm. For leaf transmission factor for 10 MV photon beams, interleaf leakage was measured $1.9\%$ and midleaf leakage $0.9\%$ relative to $10\times\;cm^2$ open filed. Penumbra measured with film, diode detector, microionization chamber, and conventional 0.125 cc chamber showed that $80\~20\%$ penumbra width measured with a 0.125 cc chamber was 2 mm larger than that of film, which means a 0.125 cc ionization chamber was unacceptable for measuring small field such like 0.5 cm beamlet. After RTP recommissioning, the discrepancy between the measured and calculated dose profile for a small field of $1\times1\;cm^2$ size was less than $2\%$. The isocenter dose of the test plan of C-shaped PTV was measured two times with micro-ionization chamber in solid phantom showed that the errors upto $12\%$ for individual beam, but total dose delivered were agreed with the calculated within $2\%$. The transverse dose distribution measured with EC-L film was agreed with the calculated one in general. The isocenter dose for the patient measured in solid phantom was agreed within $1.5\%$. On-axis dose profiles of each individual beam at the position of the central leaf measured with film and array detector were found that at out-of-the-field region, the calculated dose underestimates about $2\%$, at inside-the-field the measured one was agreed within $3\%$, except some position. Conclusion : It is necessary more tight quality control of MLC for IMRT relative to conventional large field treatment and to develop QA procedures to check intensity pattern more efficiently. At the conclusion, we did setup an appropriate QA procedures for IMRT by a series of verifications including the measurement of absolute dose at the isocenter with a micro-ionization chamber, film dosimetry for verifying intensity pattern, and another measurement with an array detector for comparing off-axis dose profile.

Purpose: In radiation therapy, precise calculation of dose toward malignant tumors or normal tissue would be a critical factor in determining whether the treatment would be successful. The Radiation Treatment Planning (RTP) system is one of most effective methods to make it effective to the correction of dose due to CT number through converting linear attenuation coefficient to density of the inhomogeneous tissue by means of CT based reconstruction. Materials and Methods: In this study, we carried out the measurement of CT number and calculation of mass density by using RTP system and the homemade inhomogeneous tissue Phantom and the values were obtained with reference to water. Moreover, we intended to investigate the effectiveness and accuracy for the correction of inhomogeneous tissue by the CT number through comparing the measured dose (nC) and calculated dose (Percentage Depth Dose, PDD) used CT image during radiation exposure with RTP. Results: The difference in mass density between the calculated tissue equivalent material and the true value was ranged from $0.005g/cm^3\;to\;0.069g/cm^3$. A relative error between PDD of RTP and calculated dose obtained by radiation therapy of machine ranged from -2.8 to +1.06%(effective range within 3%). Conclusion: In conclusion, we confirmed the effectiveness of correction for the inhomogeneous tissues through CT images. These results would be one of good information on the basic outline of Quality Assurance (QA) in RTP system.

The hot cell facility for research activities related to the lithium reduction of spent fuel, which is designed to permit safe handling of source materials with radioactivity levels up to 1,385 TBq, is planned to be built. To meet this goal, the facility is designed to keep gamma and neutron radiation lower than the recommended dose-rate in normally occupied areas. The calculations peformed with QAD-CGGP and MCNP-4C are used to evaluate the proposed engineering design concepts that would provide acceptable dose-rates during a normal operation in hot cell facility. The maximum effective gamma dose-rates on the surfaces of the facility at operation area and at service area calculated by QAD-CGGP are estimated to be $2.10{\times}10^{-3}, 2.97{\times}10^{-3} and 1.01{\times}10{-1}$ mSv/h, respectively. And those calculated by MCNP-4C are $1.60{\times}10^{-3}, 2.99{\times}10^{-3} and 7.88{\times}10^{-2}$ mSv/h, respectively, The dose-rates contributed by neutrons are one order of magnitude less than that of gamma sources. Therefore, it is confirmed that the radiological design for hot cell facility satisfies the Korean criterion of 0.01 mSv/h for the operation area and 0.15 mSv/h for the service (maintenance) area.

The roof tiles excavated from the mountain fortress at Namsan-ri Seocheon-gun in Chungnam province were dated by using SAR protocol, Sample preparation was done with the quartz exclusion method. The paleodoses from 6 kinds of roof tiles were obtained from OSL measurements and the does rates of the soil around the excavated roof tiles and the tile themselves were measured from ICP/MS and ICP/AES with the conversion table. From the evaluated paleodose and the dose rate, the age of the roof tiles were determined to be from(733 ± 50)AD to (1160 ± 50)AD and the error was less than 10%.
High dose rate (HDR) brachytherapy for treating a cervix carcinoma has become popular, because it eliminates many of the problems associated with conventional brachytherapy. In order to improve the clinical effectiveness with HDR brachytherapy, a dose calculation algorithm, optimization procedures, and image registrations need to be verified by comparing the dose distributions from a planning computer and those from a phantom. In this study, the phantom was fabricated in order to verify the absolute doses and the relative dose distributions. The measured doses from the phantom were then compared with the treatment planning system for the dose verification. The phantom needs to be designed such that the dose distributions can be quantitatively evaluated by utilizing the dosimeters with a high spatial resolution. Therefore, the small size of the thermoluminescent dosimeter (TLD) chips with a dimension of <1/8"and film dosimetry with a spatial resolution of <1mm used to measure the radiation dosages in the phantom. The phantom called a pelvic phantom was made from water and the tissue-equivalent acrylic plates. In order to firmly hold the HDR applicators in the water phantom, the applicators were inserted into the grooves of the applicator holder. The dose distributions around the applicators, such as Point A and B, were measured by placing a series of TLD chips (TLD-to-TLD distance: 5mm) in the three TLD holders, and placing three verification films in the orthogonal planes. This study used a Nucletron Plato treatment planning system and a Microselectron Ir-192 source unit. The results showed good agreement between the treatment plan and measurement. The comparisons of the absolute dose showed agreement within $\pm$4.0 % of the dose at point A and B, and the bladder and rectum point. In addition, the relative dose distributions by film dosimetry and those calculated by the planning computer show good agreement. This pelvic phantom could be a useful to verify the dose calculation algorithm and the accuracy of the image localization algorithm in the high dose rate (HDR) planning computer. The dose verification with film dosimetry and TLD as quality assurance (QA) tools are currently being undertaken in the Catholic University, Seoul, Korea.
Purpose: To compare the dose distributions between three-dimensional (3D) and four-dimensional (4D) radiation treatment plans calculated by Ray-tracing or the Monte Carlo algorithm, and to highlight the difference of dose calculation between two algorithms for lung heterogeneity correction in lung cancers. Materials and Methods: Prospectively gated 4D CTs in seven patients were obtained with a Brilliance CT64-Channel scanner along with a respiratory bellows gating device. After 4D treatment planning with the Ray Tracing algorithm in Multiplan 3.5.1, a CyberKnife stereotactic radiotherapy planning system, 3D Ray Tracing, 3D and 4D Monte Carlo dose calculations were performed under the same beam conditions (same number, directions, monitor units of beams). The 3D plan was performed in a primary CT image setting corresponding to middle phase expiration (50%). Relative dose coverage, D95 of gross tumor volume and planning target volume, maximum doses of tumor, and the spinal cord were compared for each plan, taking into consideration the tumor location. Results: According to the Monte Carlo calculations, mean tumor volume coverage of the 4D plans was 4.4% higher than the 3D plans when tumors were located in the lower lobes of the lung, but were 4.6% lower when tumors were located in the upper lobes of the lung. Similarly, the D95 of 4D plans was 4.8% higher than 3D plans when tumors were located in the lower lobes of lung, but was 1.7% lower when tumors were located in the upper lobes of lung. This tendency was also observed at the maximum dose of the spinal cord. Lastly, a 30% reduction in the PTV volume coverage was observed for the Monte Carlo calculation compared with the Ray-tracing calculation. Conclusion: 3D and 4D robotic radiotherapy treatment plans for lung cancers were compared according to a dosimetric viewpoint for a tumor and the spinal cord. The difference of tumor dose distributions between 3D and 4D treatment plans was only significant when large tumor movement and deformation was suspected. Therefore, 4D treatment planning is only necessary for large tumor motion and deformation. However, a Monte Carlo calculation is always necessary, independent of tumor motion in the lung.
