  • The PFAPA syndrome is characterized by periodic fever, malaise, aphthous stomatitis, pharyngitis and cervical lymphadenitis without long-term sequelae. It has been known as one of the cause of periodic fever in children. Because there is no specific laboratory findings, the diagnosis of PFAPA syndrome is not only made by exclusion of diseases which are related to the other causes of periodic fever, but also made by a careful history and characteristic clinical manifestations. The dramatic response to the single oral dose of corticosteroid is unique to this syndrome. The PFAPA syndrome tends to be resolved without any long-term adverse sequelae. We have reported a 3 year old boy who was diagnosed as having PFAPA syndrome by episodes of periodic fever for 3 months and by excluding other febrile diseases. He was treated with oral prednisolone and his symptoms had been improved dramatically.

Increased Allergen-specific IgE Values and Lymphocyte Proliferation Response to House Dust Mites in Dogs with Atopic Dermatitis (개의 아토피성 피부염에 대한 집먼지 진드기 항원 특이적 IgE와 림프구 증식 반응의 증가)

  • We examined the responses of PBMCs to house dust mite (HDM) allergen in atopic and healthy, non-atopic dogs to identify differences in lymphocyte reactivity that might reflect the immunologic status of atopic dermatitis. Thirteen of 20 (65%) atopic dogs showed a positive lymphocyte proliferative response to HDM allergen. The rate of response was significantly higher in the atopic dogs than that in healthy, non-atopic dogs insensitive to the allergen (P = 0.007). The proliferative responses were positively correlated with the level of HDM-specific IgE in serum (P = 0.035), and were thereby confirmed to reflect the activity of lymphocytes competent to promote IgE production. These results suggest that HDM-specific lymphocytes were present in peripheral blood and played a role in the pathogenesis of canine atopic dermatitis.

Decreased Nocturnal Blood Pressure Dipping in Patients with Periodic Limb Movements in Sleep (수면중 주기성 사지 운동에서 나타나는 야간 혈압 강하의 감소)

  • Objectives: Previous studies have shown that periodic limb movements in sleep (PLMS) could be one of risk factors for cardiovascular morbidity. The purpose of this study was to investigate the association between PLMS and blood pressure changes during sleep. Methods: We analyzed data from 358 adults (176 men and 182 women) aged 18 years and older who were free from sleep apnea syndrome (Respiratory Disturbance Index < 5) and sleep disorders such as REM sleep behavior disorder or narcolepsy. Demographic characteristics, polysomnography records, and clinical variable data including blood pressure, body mass index, alcohol, smoking, and current medications were collected. In addition, self-report questionnaires including the Beck Depression Index, Epworth Sleepiness Scale and Pittsburgh Sleep Quality Index were completed. Blood pressure change from bedtime to awakening was compared between the two periodic limb movement index (PLMI) groups [low PLMI ($PLMI{\leq}15$) and high PLMI (PLMI > 15)]. Blood pressure change patterns were compared using repeated measures analysis of variance. Results: Systolic blood pressure in the high PLMI group was lower than that in the low PLMI group (p = 0.036). These results were also significant when adjusted for gender and age, but were not statistically significant when adjusted for BMI, alcohol, smoking, anti-hypertension medication use and sleep efficiency (p = 0.098). Systolic blood pressure dropped by 9.7 mm Hg in the low PLMI group, and systolic blood pressure in the high PLMI group dropped by 2.9 mm Hg. There was a significant difference in delta systolic blood pressure after sleep between the two groups in women when adjusted for age, BMI, alcohol, smoking, antihypertensive medication use and sleep efficiency (p = 0.023). Conclusion: PLMS was significantly associated with a decreasing pattern in nocturnal BP during sleep, and this association remained significant in women when adjusted for age, BMI, alcohol, smoking, antihypertension medication use and sleep efficiency related to blood pressure. We suggest that PLMS may be associated with cardiovascular morbidity.

Development of a Cardiovascular Index That Estimates Cardiovascular Health Degree by Survey and Noninvasive Examination (설문과 비침습적 검사를 통하여 심혈관계 건강 정도를 알려주는 심혈관지수의 개발)

  • An index was developed that estimates cardiovascular health degree with easily available physiological information such as survey and noninvasive measurement. The survey score was calculated by utilizing questions related to personal disease history, self-feeling, and management state. The measurement score was calculated using physiological parameters such as blood pressure, accelerated plethysmograph(APG), and heart rate variability(HRV), and augmentation index(AI). In order to evaluate effectiveness of the cardiovascular index and modify weighting factors used in each item, a clinical trial was done in a general hospital. The cardiovascular index showed a clear correlation of 0.685 with the doctor's score on the cardiovascular health degree. The correlation between the self-estimated score and doctor's score was as low as 0.217. The large gap between these two scores demonstrated necessity of more objective tools like the cardiovascular index. The cardiovascular score showed a significant difference between normal persons and patients suffering hypertension or diabetes. (p=0.000).

Association of Sleep Perception With Objective and Subjective Sleep Variables in Insomnia Patients (불면증 환자의 수면인식과 객관적, 주관적 수면변수의 연관성에 대한 연구)

  • Objectives: Our study aims to investigate the clinical and polysomnographic variables associated with subjective sleep perception. Methods: Among the patients who underwent nocturnal polysomnography (PSG) at the Center for Sleep and Chronobiology of Seoul National University Hospital from May 2018 to July 2019, 109 diagnosed with insomnia disorder based on DSM-5 were recruited for the study, and their medical records were retrospectively analyzed. Self-report questionnaires about clinical characteristics including Pittsburgh sleep quality index (PSQI), Beck depression inventory (BDI), and Epworth sleepiness scale (ESS) were completed. Subjective sleep quality was measured using variables of subjective total sleep time (subjective TST), subjective sleep onset latency (subjective SOL), subjective number of awakenings, morning feeling after awakening, and sleep discrepancy (subjective TST-objective TST) the morning after PSG. Pearson and Spearman correlation analyses were used to determine the factors associated with subjective sleep perception. Results: In patients with insomnia, subjective TST was negatively correlated with Wake After Sleep Onset (WASO) (p = 0.001) and N1 sleep (p = 0.039) parameters on polysomnography. Also, it was negatively correlated with PSQI (p < 0.001) and BDI (p = 0.014) scores. Sleep discrepancy was negatively correlated with PSQI score (p = 0.018). Morning feeling was negatively correlated with PSQI (p = 0.019) and BDI (p < 0.001) scores. Conclusion: Our results demonstrated that subjective sleep perception is associated with PSG variables (WASO and N1 sleep) and with PSQI and BDI scores. In clinical practice, it is helpful to assess and manage insomnia patients in consideration of objective sleep variables, subjective sleep quality, and depressed mood, which can influence subjective sleep perception.

  • Acromioclavicular (AC) joint cyst is an uncommon presentation around the shoulder joint that is usually associated with chronic rotator cuff pathology and degenerative AC joint arthritis. Understanding the underlying pathophysiology of the shoulder joint is essential for appropriate interpretation of the AC joint cyst, assistance in determining the therapeutic strategy, and prevention of complications after treatment. We present a case of AC joint cyst with an aseptic fistula which was complicated by simple excision for recurred AC joint cyst after multiple aspirations. This case revealed a massive rotator cuff tear involving supraspinatus, infraspinatus, and subscapularis tendon on ultrasonography. We also reviewed the clinical approach and treatment options to be considered for its pathogenesis in the literature.

Quality Characteristics of Cream Soup Added with Pomegranate Cortex Powder (석류 외피 분말을 첨가한 크림수프의 품질 특성)

  • This study examines the quality characteristics of cream soup added with pomegranate cortex powder(0~0.8%). As for the proximate composition, the control group had significantly(p<0.05) higher moisture content than the PGCP samples; however, crude ash decreased with increasing PGCP content The PGCP samples presented significantly(p<0.05) higher viscosity of the control group. As the PGCP content increased, pH decreased. As for the color, Hunter L value increased with dark color while a value decreased with the PGCP content increased. b value decreased in the group added with 0.8% of PGCP content In terms of sensory evaluation, color, flavor decreased with the PGCP content increased. Viscosity was high in the group added with 0.4% of PGCP content Overall acceptability was the highest in the group added with 0,2% of PGCP content while bitterness, astringency, bitterness, and off-flavor increased. There were no significant(p>0.05) differences in creamness of the cream soup among the samples. In conclusion, the results indicate that the addition of 0.2% PGCP to cream soup is desirable for quality and provides good physiological properties to the products.

  • The purpose of this study is to evaluate correlation of adolescent eating attitude and psychological factors which might influence the eating attitude such as depression, aggression and impulsiveness. The author also tried to identify relationship between adolescent eating attitude and biological factors such as serum cholesterol level and triglyceride level. Two hundred and forty one female high school students without physical disorder were selected as study subjects. For the study, Eating Attitude Test-26KA (EAT -26KA), Beck Depression Inventory (BDI) -Korean version, Barrat Impulsiveness scale (BIS) -Korean version, and Buss-DurKee Hostility Inventory (BDHI) -Korean version were used. Serum cholesterol level and triglyceride level were also investigated. For the purpose of identifying factors which could influence eating attitude of adolescence, stepwise multiple linear regression analysis was done. Total score of BDHI could be most significant factor to explain eating attitude, and BDI, serum cholesterol/triglyceride level be second, third position respectively. But, BIS was not significant predicting parameter. The contributing portion of these factors to explain eating behavior was 25.9%.

Sensory Characteristics of Doenjang Supplemented with Sage Powder as Assessed by Response Surface Methodology (반응표면분석법을 이용한 세이지분말 첨가 된장의 관능적 특성)

  • We examined sensory characteristics to obtain the optimal conditions for doenjang preparation, using response surface methodology (RSM) to evaluate addition of sage (Salvia officinalis L.) powder to, and the salinity of, doenjang. We aimed to develop a new variety of doenjang linking traditional Korean doenjang preparation with the physiological functionalities of sage. Color values were 1.20-2.70, flavor values 1.60-3.20, taste values 1.40-3.50, texture scores 1.60-3.50, and overall preference values 1.60-3.05, with the differences depending on experimental variations in preparation. Analysis of a reaction surface formed by a quadratic regression equation found that the R-squared values for overall preference, texture, taste, flavor,and color were 0.11-0.41, thus relatively low and insignificant, being less than 5%. In sensory tests, the color value was 2.91 when the salinity was 21.50%, and sage powder was added to a concentration of 3.10% (all w/w). The flavor score was 3.21 when the salinity was 22.52% and sage powder concentration was 3.68%. The taste value was 2.87 when the salinity was 8.62% and sage powder concentration was 4.46%. The texture score was 2.88 when the salinity was 8.00% and sage powder concentration was 6.06%. The overall preference score was 2.74 when the salinity was 20.40% and sage powder concentration was 2.66%. Although this preparation method is new to Koreans, doenjang with added sage was associated with higher sensory scores than traditional doenjang, confirming the possibility of development of a novel functional doenjang.

A Study on the Usefulness of Subjective Lumbar Instability Factor for Respiratory Pattern Change and Abdominal Mobility in Peoples with CLBP (만성허리통증자의 호흡 패턴과 배부 운동성 변화에 대한 주관적 허리부위 불안정성 요소의 유용성에 관한 연구)

  • This study examined the correlation between the respiratory pattern change (RPC) and abdominal mobility (AM) according to the positive result of the subjective lumbar instability factor (SLIF) in people with chronic low back pain (CLBP). Thirty-six adults with CLBP participated in this study. Twenty-eight items of the SLIFs were examined, and the subjects were divided into three groups according to the positive response numbers (PRN). After the change lists were scored, three RPC scores [costo-diaphragmatic RPC (CDRPC), breath hold change (BHC), and total RPC (TRPC)] were obtained. The abdominal mobility (AM) was measured between the maximal inspiration and exhalation at the xiphoid (AM1) and the 10th rib (AM2) level of the trunk. The results showed that the RPC score and AM were compared according to the positive response number of SLIF, and the relationship between them was analyzed. A positive correlation was observed between the SLIF positive response number and CDRPC score, BHC score, and total RPC score, and a negative correlation was observed between the SLIFs positive response number and AM1 and AM2. Based on the results of this study, the combination of SLIF positive responses can be a predictor of non-physiological respiratory pattern changes in people with CLBP. Clinically, this prediction is expected to help save time for screening and improve the efficiency of therapy.
