Structural and Functional Changes of The Brain in The Patient with Schizophrenia, Paranoid type : Correlation among Brain MRI Findings, Neurocognitive Function and Psychiatric Symptoms (편집형 정신분열병 환자에서 뇌의 구조적 변화와 기능적 변화 : 뇌자기공명영상소견, 신경인지기능 및 정신증상간의 상관관계)

  • Objectives : The purpose of this study is to evaluate the role of structural and functional changes of the brain in the pathophysiology of schizophrenia. Methods : The authors measured the regions of interest on the magnetic resonance imaging of the brain in 20 patients with paranoid schizophrenia(15 men and 5 women) and 23 control subjects(15 men and 8 women). We also assessed the neurocognitive functions with the Wisconsin Card Sorting Test, the Benton Neuropsychological Assessment, and the Weschler IQ test-Korean version, soft neurologic signs, and psychiatric symptoms in the patient group. Results : In the patient group, all ventricles and basal ganglia including caudate nucleus and globus pallidus were significantly enlarged. Although there were no significant differences between the two groups in the values of right frontal lobe and left temporal lobe, there was a tendency of decrease in the values of right frontal lobe and left temporal lobe. There were significant positive correlations between the values of ventricles and the frequency of previous hospitalization. However, there were no significant correlations between other values of regions of interest and clinical data. The value of the right frontal lobe was significantly correlated with the score of soft neurologic signs, which is suggestive of the neurodevelopmental abnormalities. There were significant correlations between the value of frontal lobe and the scores of the various subscales of Benton Neuropsychiatric Inventory. In contrast, the value of left amygdala and putamen showed significant correlation with the score of verbal IQ on the Weschler IQ test. Structural changes of the temporal lobe areas were related with the positive and general symptom scores on PANSS, while those of the basal ganglia were related with the negative symptom scores. Conclusions : These results suggest that the structural changes of the brain in the patients with schizophrenia show the dual process, which is suggestive that the enlarged ventricle show the neurodegenerative process, while enlarged basal ganglia, and shrinked right frontal and left temporal lobe show the neurodevelopmental abnormalities. Among these changes, structural changes of the frontal lobe related with various neuropsychological deficits, while those of left temporal lobe related with language abnormality. Relative to the relation between structural changes and psychiatric symptoms, structural changes of the temporal lobe areas were related with the positive and general symptoms, while those of the basal ganglia were related with the negative symptoms.

  • Burning mouth syndrome(BMS) is a chronic oral pain and a symptom complex disorder usually unaccomplished by mucosal lesion or other clinical and laboratory signs of organic disease. BMS is characterized by a spontaneous burning sensation that mainly affects middle-aged and postmenopausal women. The etiology of BMS is poorly understood even though evidence for a possible neuropathic pathogenesis. BMS cause from various local or systemic factor, including nutritional deficiencies, hormonal change, local infection, dental procedure, dry mouth, medication and systemic disease including diabetes mellitus. Many studies suggest peripheral alteration in sensory of trigeminal nerve system. BMS patients with supertaster indicates pathologies of central and peripheral nerve system induced by an alteration in the taste system at the level of chorda tympani and glossopharyngeal nerve. The author discuss our current understanding of etiology and pathogenesis of BMS that refered chronic oral pain.

Symptoms and Cognitive Function in Chronic Schizophrenia: 6 Months Follow-up Study (만성 정신분열병 환자에서 증상과 인지기능:6개월 개월 추적연구)

  • Objectives: The purpose of this study was to investigate whether longitudinal changes in positive and negative symptoms affect cognitive functioning in chronic schizophrenia. Methods: Sixty-eight patients diagnosed with DSM-IV schizophrenia were examined on two occasions over 6 months for symptoms and cognitive changes. Symptoms were measured by PANSS. Cognitive functions were examined for sustained attention, executive function, concentration and attention, and verbal memory and learning using Degraded Stimulus Continuous Performance Test, Wisconsin Card Sorting Test, Digit Span, and Rey Auditory Verbal Learning Test, respectively. Twenty control subjects were assessed to compare the cognitive scores of remitted schizophrenic patients. Results: Patients showed significant improvement in symptoms and all cognitive tests after 6 months treatments. Significant improvements in positive and negative symptoms did not predict improvements in any aspect of cognitive functioning measured. Normal controls performed significantly better than remitted schizophrenic patients on all cognitive tests. The results show no relationship between change in symptoms and change in cognition in chronic schizophrenia. Conclusion: We suggest that symptomatic and cognitive impairment may be a distinct construct. These findings highlight the importance of treating cognitive impairment in addition to the clinical symptoms of schizophrenia.

Quality Characteristics of Sulgidduk added with Cirsium setidens Nakai (곤드레 첨가 설기떡의 품질특성)

  • The purpose of this study was to determine the desirable mixture ratio of Cirsium setidens Nakai powder to rice flour for the preparation of Sulgidduk. The moisture of samples raged from 36.51% to 40.67%. The addition of Cirsium setidens Nakai powder in the Sulgidduk tended to decrease the lightness (L) in Hunter color value, but to increase the redness (a) and yellowness (b). With increasing Cirsium setidens Nakai powder level, the hardness, springness and chewiness of the samples increased and their cohesiveness decreased. In sensory evaluation, the addition of 5% Cirsium setidens Nakai powder showed the best score in taste and overall preference. Over 3 days of storage, Sulgidduk showed a slightly high total bacterial count compared to control. Therefore, these results suggested that the addition of 5% Cirsium setidens Nakai powder could be applied for making Cirsium setidens Nakai powder Sulgidduk.

  • Gangjeong was prepared by adding Lentinus edodes to improve quality of the gangjeong and to give some functional properties. Contents of the crude protein and ash increased with increasing amount of L. edodes, while carbohydrate contents decreased. As the ratio of mushroom in gangjeong increased, lightness and yellowness in Hunter's color value tended to decrease. In terms of texture, 20% and 30% contents of L. edodes made no significant differences in gumminess, brittleness, cohesiveness, and springiness compared to control, whereas hardness showed no difference to control. Gangjeong with 30% L. edodes obtained the highest scores in quality and sensory characteristics.

  • Background: The measurement of nonspecific bronchial hyperreactivity is valuable for diagnosis and management of bronchial asthma. Methacholine or histamine is used for the pharmacologic provocation test. Usually a methacholine bronchial provocation test is performed by a dosing technique with counted number of breaths. A dosimeter is indispensable in the dosing technique. Recently a timed tidal breathing technique which dose not need an expensive dosimeter was introduced. We measured the degree of nonspecific bronchial hyperreactivity to histamine using a simple timed tidal breathing technique. Method: Forty two healthy volunteers, 12 patients with bronchial asthma(BA), 10 patients with rhinitis(RH) and 10 patients with upper respiratory infection(URI) participated in the study. The subject's nose was clipped and inhalation continued during tidal breathing for 2 minutes via a face mask. $FEV_1$ was measured at 30 seconds, 90 seconds after inhalation and inhalation of next solution was continued until there was a fall in $FEV_1$ of 20%. Histamine PC20 was defined as the concentration at 20% fall of $FEV_1$ and it was obtained from the log dose-response curve by linear interpolation. Results: Inhalation of serial dilution of histamine could be performed in all patients without significant side effects. The geometric mean${\pm}$standard deviation of histamine PC20 in healthy volunteers is $8.27{\pm}2.22mg/ml$, BA group $0.33{\pm}3.02mg/ml$, RH group $0.85{\pm}3.24mg/ml$, and URI group $1.47{\pm}1.98mg/ml$. Conclusion: Histamine bronchial provocation test using timed tidal breath method is a simple and suitable tool for management of patients with bronchial hyperreactivity.

  • The radioprotective effects of white ginseng and Entomopathogenic Fugi Extract on liver damage induced by X-ray were investigated. To one group of ICR male mice were given white ginseng(150 mg/kg/day for 7days, orally) and Entomopathogenic Fugi (200 mg/kg/day for 7days, orally) before X-ray irradiation. To another group were irradiated by 5 Gy(1.01 Gy/min) dose of X-ray. Contrast group were given with saline(0.1 ml). The levels of reduced(GSH) and oxidized(GSSG) glutathione in liver tissue were measured. The ratio of GSSG/total GSH was significantly decreased in the white ginseng and Entomopathogenic Fugi (200 mg/kg/day)(150 mg/kg/day) groups than irradiation group.

A Study on the Back Flow of Urine into the Ceca in Chicken (닭에서 맹장으로 역류하는 뇨(尿)에 관한 연구)

  • A cecostomy technique (surgery for inserted tube into ceca) was developed to quantify urine backflow into the ceca of fed and feed-deprived chickens. Two days post-surgery, cecostomised chickens were flushed with 20ml of warm saline solution every other day for 10 days. Excreta were collected daily from cecal tubing and cloaca by surgical attachment of polyethylene collection vessels to the chickens. Uric acid excretion was significantly increased in fed compared to feed-deprived chickens (P<0.05). Amount of determined uric acid from the ceca was 7.74% and 5.31% of total excretion for fed and ffeed-deprived chickens, respectively. Post-mortem examinations ascertained that the caeca were intact around the Latex tubing. The results of the study indicated that at least 5% of daily urine production flow retrograde into the ceca of roosters.

  • 수면단계는 수면감성을 평가하는데 있어서 중요한 생리지표로서 사용되어왔다. 그러나 수면다원검사를 이용한 전통적 수면단계 분류방법은 뇌전도, 안전도, 심전도, 근전도 등을 종합적으로 측정하므로 수면단계를 비교적 정확히 분류할 수 있지만 피험자에게 심한 구속감을 주는 문제가 있다. 본 연구에서는, 각성상태에서 교감신경계가 지배적인 반면에 수면 중에는 부교감신경계가 더 활동적인 점에 착안하여 수면단계를 간단히 분류할 수 있는 방법을 찾고자 수면단계에 따른 심박동변이도(heart rate variability: HRV)를 분석하였다. 단일채널 심전도를 이용하여 수면단계별로 HRV 의 교감신경계/부교감신경계 활성도의 비율을 분석한 결과, W(wakefulness) 단계가 NREN(non REM) 2 단계, 3 단계, 4 단계에 비하여 높게 나타났으며, NREM 4 단계는 REM(rapid eye movement) 단계와 NREM 1단계에 비하여 낮게 나타났다. 또한 교감신경계/부교감신경계 활성도 비율의 수면단계에 따라 변화하는 양상은 W, REM, NREM 1, 2, 3, 4 단계의 순으로 단조 감소하였다.

  • Catathrenia is a rare sleep disease characterized by monotonous groaning sounds that appear to be related with prolonged expiration, commonly experienced during rapid eye movement (REM) sleep. Catathrenia is also known as nocturnal groaning or sleep-related groaning and is currently categorized as a sleep-related breathing disorder. We present a rare case of a 19-year-old male with nocturnal groaning during non-REM sleep. We suggest that if catathrenia is suspected, polysomnography should be utilized to differentiate it from various sleep disorders such as snoring, central sleep apnea, sleep talking, parasomnia, and sleep-related movement disorders.
