Angle's Class II Division 2 Malocclusion Treated by Bioprogressive Mechanism: Report of a Case (Bioprogressive Mechanism에 의한 Angle씨 II급 2류 부정교합의 치험례)

  • A 25 year and 7 month old man patient who had Angle's classII division 2 malocclusion combined with anterior crowding of upper & lower part was treated by bioprogressive mechanism. After setting our objectives through the use of V.T.O., we programmed a sequence of mechanics. The possible objectives of treatment in the classII division 2 malocclusion can be listed as follows. 1) Relief of crowding & irregularities. 2) Relief of anterior gingival trauma & correction of interincisal inclination. 3) Correction of buccal segment relationship. We'd applied the classII intermaxillary elastics, Quad helix, utility arch wire and sectional arch wire in order to achieve anticipated objectives. As compared with pre & post treatment cephalogram, the result accomplished by this mechanics showed to us that interincisal angle was improved and favorable molar relationship was achieved.

A study of Korean Norm about tooth size and ratio in Korean adults with normal occlusion (한국성인 정상교합자의 치아크기와 비율에 관한 연구)

  • The purpose of this study was to measure the average tooth size of Korean with normal occlusion. According to the study, the average tooth ratios between the upper and lower teeth which could assure the proper ovebite, overjet and good interdigitation were calculated. The normal occluson sample of this study consisted of 43 Korean male adults and 51 Korean female adults. Among them, 22 Korean male adults and 51 Korean female adults were from KAO(Korean Association of Orthodontists), 21 Korean male adults were from Department of Orthodontics, College of Dentistry, Yonsei University. The results from this study were as follows : 1 The average tooth size of Korean Norm classified by male and female was measured. 2. The average tooth size of Korean male adults with normal occlusion was significantly larger than that of Korean female adults except upper and lower first molars. (p<0.05) 3. The tooth ratio which could Predict the proper overbite and overjet in anterior teeth and proper occlusion in posterior teeth was calculated. 1) Sum of inciosrs = 4:2.97 2) Neff's anterior coefficient = 1.22 3) Bolton's anterior ratio = 78.29%, overall ratio = 91.14% 4. A positive correlation was observed between the sum of lower anterior incisors and the sum of unilateral canine and premolars In each jaw. Based this correlation, the regression equation was made which could Predict the sum of unerupted unilateral canine and premolars in mired dentition. 1) Sum of unilateral unerupted upper canine and premolars' width = 10.435018 + 0.513346 ${\times}$ (sum or lower 4 incisors' width) 2) Sum of unilateral unerupted lower canine and premolars' width = 9.654002 + 0.502565 ${\times}$ (sum of lower 4 incisors' width)

Three dimensional structural analysis between dental arch and basal bone in normal occlusion (정상교합자의 치열궁과 기저골 형태에 대한 3차원적 연구)

  • Objective: The purpose of this study was to analyze the dental and basal arch forms in patients with normal occlusion using the computed tomography (CT) imaging method. Methods: CT images were taken from 27 normal occlusion subjects (male, 15; female, 12) and these images were reconstructed into three-dimensional models. A 3D-coordinate system was formed by setting the middle of the facial axis (FA) point of the maxillary central incisors as the origin. The morphology of the maxilla and mandibular dental and basal arches were analyzed by sectioning parallel to the maxillary occlusal plane. Results: There was no significant difference between A point and B point and between the maxillary 1st molars in both sides of the maxillary and mandibular basal bone. The dental arch was located more labially than the basal arch in the anterior portion. The bucco-lingual crossover of the dental arch and basal arch was formed at the molar region in the maxilla, and at the premolar region in the mandible. Conclusions: This study will help provide three-dimensional diagnostic information about the relationship between basal and dental arches in patients who have severe dental compensation and inappropriate jaw relationships.

  • 저자는 교정학의 진단 및 치료에 응용하기 위하여 한국인 10세 아동 남여 50명과 성인 남녀 106명의 두부측모 X-선사진을 사용하여 $Bj\ddot{o}k$, Downs, Steiner 씨 등의 씨 분석법에 의해 다음과 같은 계측치를 얻었다. (1) 정상교합자 10세 아동 남여 50명과 23세 성인 남녀 106의 표준편차표를 작성하였다. (2) Anterior Cranial Base에 대한 Mandibular Body의 비는 약 1:1.1로 나타났다. (3) T to Po의 비(Holdaway ratio)는 약 4.2:1로 나타났으며 Tweed Triangle은 각각 $32^{\circ},52^{\circ},96^{\circ}$로 나타났다. (4) 한국인은 백인에서 보다 하악신부의 후방위, 즉 하악골 후방위로 나타났으며 동시에 한국인은 백인보다 상하악 전치의 진측경사를 갖인 안모로 나타났다.

Analysis of Anterior Dentition for Identification of Bite-mark Evidence (교흔감정을 위한 상하악 전치부 치열상태에 관한 연구)

  • A human bite-mark shows special feature according to the suspect's dentition. The teeth which most frequently give useful bite-marks are six upper and lower anteriors, while the premolar teeth somtimes give marks it is often difficult to distinguish. The author tried to classify and to analize the anterior dentition which makes the bite-mark directly by means of 672 maxillary and 691 mandibular stone model taken from Korean adult aging from 17 to 40 years old. The results were as follows : 1. There was no particular correlation between the presence of six normal-shaped and correctly positioned upper and lower anteriors and the presence of rotation of teeth. 2. Inter central incisor, inter lateral incisor, inter canine width and angles of adjecent teeth were not identical eath other in studied models. 3. The results of this analysis supported the statement that any bite-mark had no same feature.

  • To evaluate the clinical usefulness of TENS theory, 12 dental studensts of KyungPook National University and 18 prosthodontic patients were applied by 3M DENTAL ELECTRONIC ANESTHESIA and the follwing results were obtained: 1. Using the TENS theory to 12students, and EPT test was conducted and 6 students among total 12 students showed the anesthetic effects to the pain(p<0.05). 2. 15 prosthetically preparated patients show the anesthetic effects to the pain among total 18 patients.

  • This study was performed to investigate the location of the ideal bracket positioning plane in lingual orthodontics using the three-dimensional finite element method. Displacement of the anterior teeth were evaluated according to the vertical and the angular movements of the bracket positioning plane. To achieve the ideal movement of anterior teeth in the lingual central plane, the location of the force application point and the amount of the moment applied to the four incisors were evaluated. As the bracket positioning plane was moved parallel toward the incisal edge, uncontrolled tipping and extrusion of the maxillary and the mandibular incisors were increased. But lingual tipping of the crown was decreased in the maxillary and the mandibular canines. As the bracket positioning plane was inclined toward the incisal edge, lingual tipping was increased in the 6 anterior teeth and extrusion of incisors and intrusion of the canine was also increased. As the retraction hook of the canine bracket was elongated, lingual tipping and extrusion of the central incisor and mesial movement and extrusion of the lateral incisor were increased. In the canine, mesial and labial movements of the crown were increased. When the moment was applied to the 4 incisors of the maxillary and the mandibular arch in the lingual central plane, 280 gf-mm in the maxillary central incisor, 500 gf-mm in the maxillary lateral incisor, 170 gf-mm in the mandibular central incisor and 370 gf-mm in the mandibular lateral incisor produced bodily movement of the individual tooth.

  • Patients with $Sj{\ddot{o}}gren^{\prime}s$ syndrome usually suffer from teeth discoloration and attrition due to xerostomia. If the anterior teeth are badly worn, problems such as loss of anterior guidance, occlusal disharmony, and limited space for restoration may occur. However, ideal occlusion is obtained in both centric and eccentric relation by regaining the disocclusion of the posterior teeth through the anterior and lateral guidance using twin-stage method. In this case, rehabilitation was performed for a $Sj{\ddot{o}}gren^{\prime}s$ syndrome patient with maxillary and mandibular incisor's severe attrition and teeth discoloration by using twin-stage method.

Effect of Developmental Disorders of Maxillary First Molars on Orofacial Morphology (악안면 형태에 대한 상악 제1대구치 발육장애의 영향)

  • This study was aimed to evaluate orofacial morphologies on the cases of developmental disorders of maxillary first molars. Panoramic radiographs, lateral cephalographs, and clinical photos of 2983 children who attended the Pediatric Dental Clinic of Pusan National University Dental Hospital from 2006 to August 2017 were assessed retrospectively. 34 patients were selected whose maxillary first molars were missed or developmentally delayed unilaterally or bilaterally. Demirjian's method was used for estimating dental age, then which was compared to chronologic age of children. Parameters expressing skeletal and dentoalveolar disharmony were checked and compared with control. Additionally, occlusion relationship was evaluated. Maxillary dental age was significantly delayed compared to chronologic age. Several parameters which show skeletal open-bite tendency and skeletal class III malocclusion with maxillary retrusion were statistically significant. Anterior crossbite and edge-bite were expected in most of these cases, but compensation by occlusion and soft tissue was also verified which might mask skeletal class III tendency. Congenital missed or developmentally delayed maxillary first molars might be related with declined growth of maxilla. If developmental disorders of maxillary first molars were verified during clinical examination, careful monitoring of orofacial growth was necessary during puberty and timed orthopedic and orthodontic intervention were considered.

Accuracy of interocclusal record established by different occlusal scans (교합스캔의 이용 방식에 따른 교합면간기록의 정확성)

  • Purpose: The purpose of this study was to investigate the accuracy of digital mounting with occlusal scans in the interocclusal record for a full-mouth fixed prosthetic treatment condition. Materials and Methods: The entire maxillary teeth were prepared in a dentiform model and attached to an articulator in maximum intercuspation. Temporary crowns were fabricated and occlusion was adjusted. After scanning the arch with tooth preparation, digital mounting was performed using an occlusion scan of the anterior or posterior teeth areas. The accuracy of the positioned jaw relation was evaluated three-dimensionally through positional deviation and angular error of the occlusal plane. Afterwards, the same procedure was repeated on the partially edentulous model to evaluate the accuracy of digital interocclusal record. Kruskal-Wallis test and Mann-Whitney U test were used for statistical analysis, and the significance level was set at 0.05. Results: There was no significant difference in the positional error at the center of the arch in the digitally established jaw relation depending on the scan area used and remaining tooth conditions (P = 0.53). The angular error of the occlusal plane was larger when the anterior teeth were used as the overlapping area, and a smaller error value was observed when both posterior teeth were used as the overlapping area (P < 0.001). Conclusion: The use of bilateral posterior occlusal scans is recommended when digitally mounting the jaw relation established with temporary teeth during full-mouth fixed prosthetic treatment.
