Supernumerary teeth are teeth added to the normal complement of teeth. They are most often found in the maxillary anterior region. Most supernumerary teeth in the maxillary anterior region have complications, such as impaction or malposition of permanent teeth, formation of diastema, cysts, and eruption into the nasal cavity or into the oral cavity. These complications have influence on deciding the treatment and its prognosis is depends on how these complications are treated. In clinical studies, it has been found that the removal of supernumerary teeth before the age of 5 years resulted in significant fewer eruption problems of the permanent tooth compared to removal at 7 years of age or later. However, such an early removal may leads to serious consequences of tooth development and behavioral management of patients. Therefore clinicians must consider complications of supernumerary tooth when deciding the time of removal. These cases report five-type of complication associated with supernumerary tooth such as diastema, delayed eruption, external root resorption, migration into nasal cavity and cyst formation.

Supernumerary teeth are frequently found in the anterior portion of the maxilla and develop as a result of abnormal proliferation of the dental lamina during tooth germ formation, caused by genetic or environmental factors. They may result in various complications, such as eruption interference, displacement, rotation of adjacent teeth, diastema, eruption into the nasal cavity, and development of dentigerous cyst. The optimal time for surgical extraction of supernumerary teeth has been a controversial issue. Someone prefer early surgical extraction because supernumerary teeth can cause eruption interference and displacement of adjacent teeth, eventually altering occlusion. Others prefer to delay surgical extraction until $8{\sim}10$ years of age in consideration of root maturation of the adjacent teeth and also patient's behavior. When surgical extraction of supernumerary teeth is postponed, there is possibility that impacted supernumerary teeth in the inverted or horizontal position move toward the nasal cavity, hard palate, or premolar area. When such intraosseous tooth migration is combined with the vertical growth of the maxilla, surgical approach becomes even harder. Therefore, possibility of intraosseous tooth migration should be considered as an important factor when deciding appropriate time for surgical extraction. We are presenting cases of mesiodens which showed intraosseous migration during $6{\sim}7$ years of follow-up period since the first diagnosis had been made at the $2{\sim}3$ years of age.

Supernumerary tooth occurs most frequently at premaxilla area. Followed by mandibular premolar area, mandibular fourth molar area, maxillary paramolar area. Mesiodens are mainly impacted in the palatal area and surgical approach is made at palatal side. The time of surgery remains controversial. In case of inverted or horizontal impacted supernumerary tooth, intraosseous tooth movement and vertical growth of premaxilla makes surgical extraction more difficult. And also the more quantity of removed bone is, the higher degree of difficulty is. Inverted mesiodens of these cases were impacted superior to apex level of adjacent permanent incisor. Although CT examination revealed exact location of impacted tooth, surgical procedure including ostectomy may take a long time more than expected. So, before surgical extraction, it's need to be considered several factors such as necessity of CT taking, degree of difficulty, direction of surgical approach, necessity of general anesthesia etc.

Supernumerary tooth is one of the abnormalities in tooth number. Supernumerary tooth result from excessive proliferation of dental lamina and incidence reports identify a range of $0.3\sim0.8%$ in primary dentition, $1.0\sim3.5%$ in permanent dentition with males being affected twice as frequently as females, maxilla nine times as frequently as mandible The incidence is more in anterior tooth region than in posterior region. The most common supernumerary tooth is the mesiodens, which located between maxillary central incisors. The occurrence is very rare in the incisor region of mandible and the reports on incidence is 2%. In this case, there were two supernumerary teeth in the mandibular region and we could acquire normal alignment of mandibular incisors by extraction and orthodontic treatment.

The prevalence of the supernumerary tooth is $1{\sim}4%$. It usually occurs in the permanent dentition. It frequently occurs in the maxillary mid palatal area. The presence of the supernumerary tooth causes the following problems ; (1) interfering the eruption of successional tooth, (2) displacing the neighboring teeth, (3) resulting large diastema, (4) forming cystic change. So the supernumerary tooth should be removed as soon as possible. To extract the supernumerary tooth, the exact position must be noticed first. Radiographic techniques that were used in the past are tube shift technique, right angle technique, sterioradiography, using radiopaque contrast media and conventional tomography. But these methods include the subjective opinion of the operator. So, a technique eliminating the operator's opinion and showing the position 3-dimentionally can be used. 3-dimentional computed tomography equipped with dental softwares can show the position of the impacted supernumerally tooth in 3-dimentional position. It has an advantage to eliminate the subjective opinion of the operator. With a fast computer this techenique is done in a relatively short period of time. The rationale of this methods is relatively simple. After tacking X-ray and reconstructing the data 3-dimentionally, sequential removal of the soft tissue and hard tissue(bone) CT number leaves the teeth alone in 3-dimentional position. The image can be seen from anywhere, so the operator can see the image in front, rear, upper, and lower positions. In both cases 1 and 2, the position of the impacted supernumerary tooth is viewed by the 3-dimensional computed tomography. And it made the operator easy to figureout the exact position.

Supernumerary tooth develops as a result of abnormal proliferation of the dental lamina during the initiation stage of dental development. It could be a sporadic occurrence or a hereditary transmission. Supernumerary tooth occurs with a frequency of 1 to 3%. Generally, there is a 2:1 preference for boys. It is usually found in the anterior portion of the maxilla and may be associated with complications such as impaction, malposition of permanent teeth, formation of diastema, cysts and eruption into nasal cavity, The position of supernumerary tooth found in the anterior portion of the maxilla is determined by the axis of the tooth. One third of supernumerary tooth in the anterior portion of the maxilla has no possibility of eruption due to its invertedly impacted position. However, as long as the coronal part of the follicle remains intact, migration of supernumerary tooth is possible. The migration may occur into the palate, the premolar region, the sinus or the nasal cavity. Also, growth of vertical dimension of maxilla could make surgical approach difficult as time goes by For this reason, we have found invertedly impacted mesiodens moved to the vault of the palate in the two cases, and extracted supernumerary tooth surgically.

The etiology of mesiodens is unknown but the most widely accepted theory is the hyperactivity of the dental lamina. Complications of mesiodens are delayed or prevented eruption of maxillary central incisors, displacement or rotation of permanent teeth, crowding of affected region, abnormal diastema or permanent space closure, dilaceration or abnormal root development of permanent teeth, primordial or follicular cyst formation, root resorption of adjacent teeth, eruption into nasal cavity. If mesiodens rotate the maxillary central incisors, space deficiency is not common and relapse is very common. So overcorrection is needed. To prevent the rotational relapse, early treatment, overcorrection, long retention period, properly formed proximal surface, use of coupled force, and surgical techniques have been suggested. The authors present two cases, whose chief complain were severely rotated maxillary incisors by mesiodens, treated by orthodontic and surgical technique and showed good results.

Supernumerary teeth are characterized by an excess number of teeth, which can be responsible for a variety of irregularities in the primary and transitional dentition. Supernumerary teeth, especially in the maxillary anterior region, may prevent the eruption of adjacent permanent teeth and cause their ectopic eruption, diastema, root resorption, or formation of dentigerous cyst. Therefore, early diagnosis of supernumerary teeth is important for prevention of such complications, and adequate treatment should be given according to their location, number, and morphologic features. In this case, four supernumerary teeth in the maxillary anterior region were disturbing the eruption of adjacent permanent incisors. Two of them were located in proximity to the central incisor tooth germs that their immediate removal may injure the permanent tooth germs. In order to minimize such complications, surgical extraction of the four supernumerary teeth was performed in two stages. At first, only two inverted conical supernumerary teeth were extracted. The other two tuberculous supernumerary teeth, close to the permanent tooth germs, were extracted later after their natural dislocation. In that way, we could minimize affects on the neighboring permanent tooth germs and also the amount of alveolar bone removed during surgery.

Impaction of maxillary incisor is rare than the third molar and canine, but its rate is higher than the other anterior teeth due to frequent mesiodens, trauma and variation of root formation (root dilaceration etc.). It is often observed in the dental age of about eight years and over. It will be occurred that the space loss, midline deviation and cyst formation due to the impaction of maxillary incisor. So it is important to evaluate the precise location of impacted tooth and to make appropriate treatment plan. Treatment would be surgical extraction or expectation for spontaneous eruption. If the impacted tooth has no pathologic change and development of the root is favorable, orthodontic traction is recommended for recovery of function and esthetics. In these cases, we performed orthodontic traction for the eruption of impacted maxillary incisors, and obtained satisfactory results.

Supernumerary teeth are dental abnormalies in the permanent dentition and the primary dentition. The etiology is unclear, but it may occur due to dichotomy of the tooth bud or hyperactivity of dental lamina. They occur more in the permanent dentition than in the primary dentition, with the most common site being the premaxillary area. Supernumerary teeth can be classified by morphology and position. Supplemental tooth refers to normal shape tooth. The treatment of supernumerary teeth depends on its shape, position, effect on dentition, and child's physiological condition. In this case, supernumerary primary tooth in the maxillary molar area was revealed by radiographical and clinical examination, but it was difficult to determine which is the supernumerary tooth. The tooth on the mesial side was extracted to induce the formation of adequate space and to prevent excessive space loss, and the result was favorable.
