The experimental study for the effect of tooth-brushing on the laser irradiated dentin surface (ND-YAG 레이저가 조사된 상아질 표면에 칫솔에 의한 기계적 마모가 미치는 영향에 대한 실험적 연구)

  • 치근부 민감성 (hypersensitivity)은 부분적으로 치근면에서의 개방된 상아세관이 존재하는 것에 기인한다고 생각되며 이러한 치근부의 개방된 상아세관은 치경부 병소 (cervical lesion)에 주로 존재하는데 이는 칫솔질에 의한 마모(toothbrush abrasion), 화학적 침식 (chemical erosion), 또는 abfraction 등의 결과로 나타난다고 한다. 이미 Nd-YAG 레이저를 이용한 실험에서 레이저를 조사한 상아질 표면의 상아 세관 구경이 감소되고 상아세관의 폐쇄가 많이 증가되는 양상을 관찰한 바 있다. 이 실험의 목적은 고출력레이저인 Nd-YAC 레이저를 이용한 상아질 표면처치의 임상사용가능성을 좀 더 상세히 평가하기 위해 상아질에 레이저를 처리한 후 기계적으로 마모시킨 경우 상아질 표면의 변화를 관찰하는 것이다. 50개의 발치된 치아의 상아질을 노출시켜 표면을 연마한 후 대조군에서는 37% 인산으로 산부식하여 상아 세관을 노출시킨 후 레이저를 조사하였고, 실험군에서는 대조군과 같은 조건으로 산과 레이저로 처리된 상아질 표면을 15, 45, 90 그리고 180분 동안 전동 칫솔로 기계적으로 마모시켜 그 표면을 주사전자현미경으로 관찰한 결과, 대조군, 칫솔질을 15, 45분간 시행한 실험군에서는 상아 세관 입구가 10% 이내에서 노출되었고 50 그리고 180분간 칫솔질을 시행한 실험군에서는 45 그리고 48%의 상아세관 입구의 노출이 관찰되었다. 그러므로 Nd-YAC레이저의 조사는 상아질 표면에서 축적 시간이 45분 이상에서 90분 이하인 기계적 마모에 의한 상아 세관 입구의 노출을 억제할 수 있을 것이라 사료된다.

Occlusion Effect of Dentinal Tubules of the Desensitizing Dentifrices Marketed in Korea (국내 시판중인 지각과민 완화 치약의 상아세관 폐쇄효과)

  • The aim of this study was to compare the effects of the short-term use of the desensitizing dentifrices marketed in Korea in vitro. Fifty human dentine specimens were wet ground with silicone carbide paper and etched with 6% citric acid for 90 seconds to allow complete opening of the dentinal tubule. Ten specimens from each group were brushed for 50 and 150 strokes with a V8 Cross Brushing Machine(Sabri Co., U. S. A). All the specimens were evaluated by SEM(${\times}3000$). The degree of occlusion of the dentinal tubules was quantified using an image analyzer. The results were analyzed by one-way ANOVA and Tukey's multiple comparisons using Window SPSS. The dentifrices containing nano-carbonate apatite, potassium nitrate and hydroxyapatite showed significantly higher occlusion effects than the other dentifrices after toothbrushing for 50 strokes(p<0.05). The Sensodyne freshmint$^{(R)}$dentifrice showed 34% fewer open tubular areas compared with the Sensodyne original$^{(R)}$dentifrice for 50 strokes. According to the short-term use of desensitizing dentifrices, the dentifrices containing nano-carbonate apatite, potassium nitrate and hydroxyapatite were most effective in occluding the dentinal tubules.

THE EFFECT OF CANAL FILLING WITH GUTTA-PERCHA OR RESILON ON ENTEROCOCCUS FAECALIS IN BOVINE DENTINAL TUBULES (Gutta-percha 또는 Resilon을 사용한 근관충전이 상아세관 내의 Enterococcus faecalis에 미치는 영향)

  • The purpose of this study was to observe the effect of canal filling on the bacteria left in the dentinal tubules and to compare the sealing ability between Gutta-percha and Resilon. The bovine dentin block models were prepared E. faecalis was inoculated to dentin blocks and incubated. The dentin blocks were divided into 5 groups. Group 1 was the negative control. Group 2 was the positive control. Group 3 was filled with ZOE based sealer and Gutta-percha, Group 4 with resin based sealer and Gutta-percha, and Group 5 with resin based sealer and Resilon. After 24 hour, the blocks were incubated at $37^{\circ}C$ for 1, 2, 3 and 4 weeks on BHI agar plates. The internal dentin portion of the blocks was removed using ISO 027, 029, 031, 035 round burs and the dentin chips were incubated at $37^{\circ}C$ for 24 hour Following incubation, the optical density of the medium was measured. The data were statistically analysed using repeated measures ANOVA and one-way ANOVA. The results were as follows, 1. There. was statistically significant reduction in the number of E. faecalis of the group where dentinal tubules were completely sealed with nail varnish in comparison with the groups obturated with gutta-percha or resilon (p < 0.05). 2. In group 5, the number of E. faecalis in the dentinal tubules decreased significantly with time (p < 0.05). whereas in Group 3 and 4, there was no reduction in its number (p > 0.05). 3. Under the conditions of this experiment, E. faecalis survived up to 4 weeks after obturation with gutta-percha or resilon (p > 0.05).

Real-time measurement of dentinal fluid flow during desensitizing agent application (상아질 지각과민 처치제의 적용 중에 일어나는 상아세관액 유동의 실시간 측정)

  • Objectives: The aim of this study was to examine changes in the dentinal fluid flow (DFF) during desensitizing agent application and to compare permeability after application among the agents. Materials and Methods: A Class 5 cavity was prepared to exposure cervical dentin on an extracted human premolar which was connected to a sub-nanoliter fluid flow measuring device (NFMD) under 20 cm water pressure. DFF was measured from before application of desensitizing agent (Seal&Protect, SP; SuperSeal, SS; BisBlock, BB; Gluma desensitizer, GL; Bi-Fluoride 12, BF) through application procedure to 5 min after application. Results: DFF rate after each desensitizing agent application was significantly reduced when compared to initial DFF rate before application (p < 0.05). SP showed a greater reduction in DFF rate than GL and BF did (p < 0.05). SS and BB showed a greater reduction in DFF rate than BF did (p < 0.05). Conclusions: Characteristic DFF aspect of each desensitizing agent was shown in NFMD during the application procedure.

EFFECT OF ACID-TREATMENT ON DENTIN BONDING (산 처리가 상아질 접착에 미치는 영향)

  • The purpose of this study was to evaluate the effect of various acid treatments on dentin bonding. Freshly extracted human teeth were uprightly embedded in self curing acrylic resin, and their occlusal surfaces were grinded to expose flat dentin surfaces. The specimens were divided into 4 groups. Specimens of one group were not treated so as to be a control and those of the other three groups were threated with 10% polyacrylic acid, 10% phosphoric acid, and 10-3 solution(10% citric acid/3% ferric chloride) respectively. Primer, bonding resin and composite resin were applied over the treated dentin surfaces sequentially. All specimens were stored in $37^{\circ}C$ distilled water for 24 hours, then the tensile bond strength was measured and the treated dentin surfaces and fracured dentin surfaces were examined under a scanning electron microscope. The results were as follows: Bond strengths of acid-treated groups were higher than those of the untreated group. In the acid-treated groups, bond strength was found to be the highest in the 10-3 solution group followed by the 10% phosphoric acid group and the 10% polyacrylic acid group(P<0.01). On SEM examination of dentin surfaces, the untreated dentin surface showed a remaining smear layer and closed dentinal tubules. Dentin surfaces treated with 10 % polyacrylic acid showed a clean dentin surface without the smear layer, but showed remaining smear plugs in dentinal tubules. A dentin surface treated with 10% phosphoric acid or 10-3 solution showed open dentinal tubules without the smear layer or smear plugs. On SEM observation of the fractured dentin-resin interface, the untreated group showed that failure occurred in the smear layer. The group treated with 10% polyacrylic acid showed no resin tag remained in the dentinal tubules, but resin tags in the dentinal tubules were observed in the group treated with the 10% phosphoric acid or the 10-3 solution. On the failure mode examination, the higher the bond strength of the group, the higher the frequency of cohesive failure. The coefficient between bond strength and cohesive failure rate was 0.71.

Occluding Effect of the Application of Fluoride Compounds and Desensitizers on Dentinal Tubules (불소도포 및 지각과민처치제 적용 후 상아세관 폐쇄효과와 지속력 평가)

  • This study compared and analyzed the occluding effects of fluoride compounds and desensitizers, which are commonly used in dental clinics, on dentinal tubules. This study also evaluated the persistence of the active ingredients over time by performing toothbrushing with an electric toothbrush. Thirty-five molar teeth, which had been extracted within the past 3 months from healthy people without tooth decays, amalgam fillings, or dental crowns, were divided into 4 pieces each. Of these, 135 teeth pieces were used as study specimens. These specimens were divided into a control group, an untreated group, and 5 experimental groups (acidulated fluoride gel, fluoride varnish, Gluma, Super Seal, and SE-Bond). The specimens were then subjected to toothbrushing equivalent to 1 week (140 times), 2 weeks (280 times), and 4 weeks (560 times), and the occluding effects on dentinal tubules in 3 regions of each specimen were examined under a scanning electron microscope. The fluoride varnish treated group showed the highest degree of dentinal tubule occlusion effects during the first, second, and fourth weeks of toothbrushing, with the SE-Bond treated group showing the second highest degree and the Gluma treated group showing the lowest degree. After 4 weeks of toothbrushing, the Gluma treated group and the Super Seal treated group showed the lowest degrees of dentinal tubule occlusion effects. In summary, the fluoride varnish treated group and the SE-Bond treated group displayed higher occlusion effects even after 4 weeks of treatment than did the other experimental groups. Therefore, it is the authors' belief that fluoride varnish and SE-Bond are effective for treating dentinal hyperesthesia.

EFFECTS OF ER:YAG LASER ABLATION ON THE DENTIN (Er:YAG 레이저를 이용한 치아삭제 효과)

  • The purpose of this study was to evaluate the effects of Er:YAG laser on cutting efficacy and its histologic findings. Dentin specimens of human premolars and molars were used and irradiated by Er:YAG laser with noncontact handpiece type delivery system under different treatment condition of irradiation time. Cavity pattern and volume were evaluated to determine the cutting efficacy and following results were obtained. 1. Cutting volume of sound dentin was getting larger with time immersed in water increase 2. With the condition of irradiation (150mJ, 10Hz, 30sec), surface irregularity was more increased in sound dentin comparing to carious dentin. For the light microscopic examination, dentinal tubules were opened and ash flecks and cracks were noted with inconsistence of dentinal tubules. 3. In case of 30 sec. irradiation in carious dentin, dark zone was limited to small focus whereas 1 min. irradiation, more wider, and cracks were noted in the perpendicular to direction of dentinal tubules. For the 2 min. irradiation, cavity was the widest and more cracks were found.

DEVELOPMENT OF NANO-FLUID MOVEMENT MEASURING DEVICE AND ITS APPLICATION TO HYDRODYNAMIC ANALYSIS OF DENTINAL FLUID (미세 물 흐름 측정장치의 개발과 상아세관액의 수력학에의 응용)

  • This study was aimed to develop an instrument for real-time measurement of fluid conductance and to investigate the hydrodynamics of dentinal fluid. The instrument consisted of three parts; (1) a glass capillary and a photo sensor for detection of fluid movement, (2) a servo-motor, a lead screw and a ball nut for tracking of fluid movement, (3) a rotary encoder and software for data processing. To observe the blocking effect of dentinal fluid movement, oxalate gel and self-etch adhesive agent were used. BisBlock (Bisco) and Clearfil SE Bond (Kuraray) were applied to the occlusal dentin surface of extracted human teeth. Using this new device, the fluid movement was measured and compared between before and after each agent was applied. The instrument was able to measure dentinal fluid movement with a high resolution (0.196 nL) and the flow occurred with a rate of 0.84 to 15.2 nL/s before treatment. After BisBlock or Clearfil SE Bond was used, the fluid movement was decreased by 39.8 to 89.6%.
