Effects of Compassion Competence, Professional Self-concept, and Nursing Practice Environment on Caring Behaviors of Clinical Nurses in a Tertiary Hospital (일개 상급종합병원 간호사의 공감역량, 전문직 자아개념 및 간호근무환경이 돌봄 행위에 미치는 영향)

  • Objectives: This study aimed to examine the effects of compassion competence, professional self-concept and nursing practice environment on caring behaviors of clinical nurses. Methods: A convenience sample of 200 nurses was drawn from a tertiary hospital in G city. Structured self-report questionnaires were used to collect data, which were analyzed with descriptive statistics, t-test, ANOVA, Pearson's correlation coefficient, and stepwise multiple regression. Results: A significant difference in caring behaviors of nurses was found for age, educational level, and unit. There were statistically significant positive correlations between study variables. Sensitivity and communication of compassion competence, communication of professional self-concept, and nursing foundations for quality of care in nursing practice environment were significant predictors of caring behaviors, explaining 51.3% of total variance. Conclusions: To improve clinical nurses' caring behaviors, strategies must be developed and implemented for enhancing their compassion competence, professional self-concept, and nursing practice environment.

The Effects of Nursing Work Environment, Job Stress and Knowledge Sharing on Nursing Performance of Nurses in a Tertiary Hospital (일개 상급종합병원 간호사의 간호근무환경, 직무스트레스, 지식공유가 간호업무성과에 미치는 영향)

  • Purpose: The purpose of this study was to identify factors influencing nursing performance of clinical nurses. Methods: This was a descriptive study involving 172 nurses who worked for 6 months or longer at one general hospital in B City. The data analysis was conducted with t-test, ANOVA, Scheffé test, Pearson's correlation coefficients, and stepwise multiple regression. Results: Nursing performance of participants positively correlated with job stress (r=.16, p=.033). The factors affecting nursing performance were total clinical experience (β=.37 p<.001), salary (β=.19, p=.028), and job stress (β=.13, p=.045). These variables had a 27.2% variance to explain nursing performance. Conclusion: Based on these results, administrative and financial support are required for the development of a clinical career program, including the differentiation of salary systems of clinical nurses. There is also a need to develop and implement job stress management and coping programs.

Effects of Mindfulness, Resilience, and Organizational Commitment on Nursing Performance of Nurses in a Tertiary Hospital (상급종합병원 간호사의 마음챙김, 회복탄력성, 조직몰입이 간호업무성과에 미치는 영향)

  • Purpose: The study aim was to investigate factors affecting factors nursing performance. Methods: This descriptive correlation study examined degree of mindfulness, resilience, organizational commitment, and nursing performance in order to confirm the correlation between variables and their effects on nursing performance. Study participants were 193 nurses who had more than 1 year of work experience at a tertiary hospital. The data were collected from March to April 2022 using structured self-reported questionnaires. SPSS/WIN 25.0 program was used to analyze the data. The statistical significance of the mediating effects were verified by applying PROCESS Macro. Results: Resilience, health state, total clinical career years, and organizational commitment were identified as factors affecting nursing performance, with the model explaining 51.6% of outcomes. Resilience had a complete mediating effect on the relationship between mindfulness and nursing performance. Conclusion: To improve nursing performance, various education and training must be developed and implemented to strengthen resilience. Additionally, it will be helpful to apply a mindfulness-based program to improve resilience. Further, designing various measures to improve the work environment and increase organizational commitment is recommended.

A Study on the Correlation between Pressure Ulcer Assessment, Preventive Nursing Performance, and Related Factors of New Nurses at Tertiary Hospitals (상급종합병원 신규간호사의 욕창 사정 및 예방 간호 수행도와 관련 변수 간의 상관관계 연구)

  • Purpose: This study was conducted to describe the visual differentiation ability for pressure ulcer (PU), knowledge of the pressure injury classification system, attitudes, and ability to perform PU assessment and preventive nursing performance, and to examine the relationship between these variables. Methods: This was a descriptive, cross-sectional study. Data were collected from 154 new nurses working at tertiary hospitals in B metropolitan city. Results: Both visual differentiation ability for pressure ulcer and the performance of PU assessment and preventive nursing performance were positively correlated with knowledge of the pressure injury classification system and attitudes toward pressure ulcer prevention. Conclusion: The results of this study suggest that developing visual educational content, such as photographs and case studies, may improve visual differentiation ability for pressure ulcers. Additionally, the development of guidelines for implementing evidence-based nursing is recommended.

Factors Influencing the Empathy Competence and Moral Sensitivity of Nurses at Tertiary General Hospitals Performing End-of-life Care (상급종합병원 간호사의 공감 역량과 도덕적 민감성이 임종간호 수행에 미치는 영향)

  • Purpose: The goal of this study aimed to understand the factors influencing the empathy competence and moral sensitivity of nurses working in tertiary general hospitals who offered end-of-life care. Methods: Descriptive data were collected from self-reported questionnaires completed by 164 nurses with direct nursing care experience with end-of-life patients at two tertiary general hospitals located in Busan, Korea. The variables examined were empathy competence, moral sensitivity, and end-of-life care performance. Data were analyzed using SPSS version 27.0 software. Results: Participants scored 3.73±0.40 on empathy competence, 4.97±0.51 on moral sensitivity and 2.91±0.37 on end-of-life care performance. There was a significant positive correlation between empathy competence and end-of-life care performance, and between moral sensitivity and end-of-life care performance. The factors affecting nurses' end-of-life care performance, empathy competence and moral sensitivity, exhibited with an explanatory power of 31%. Conclusion: Programs to increase empathy competence and moral sensitivity should be developed to improve nurses' end-of-life care performance.

Factors Influencing Intensive Care Unit Nurses' Competency in Delirium Care in A Tertiary General Hospital (상급종합병원 중환자실 간호사의 섬망간호역량 영향요인)

  • Purpose : In this study, we aimed to identify the factors affecting nurses' competency in delirium care in an intensive care unit at a tertiary general hospital. Methods : The participants were 149 nurses in the intensive care unit. Collected data were analyzed using independent t-tests, ANOVA, Sheffé test, Pearson correlations coefficients, and multiple regressions using the SPSS 28.0 program. Results : The factors that significantly influenced the participants' competency in delirium care were delirium nursing confidence (𝛽=.44, p <.001) and intensive care unit clinical career (𝛽=.25, p =.002). These factors explained 32.5% of the total variance outcomes regarding the competency in delirium care (F=12.87, p <.001). Conclusion : The study results indicated that the higher the confidence in delirium care and the more extensive intensive care unit experience, the higher the competency in delirium care. Moreover, these results suggested that various policy strategies should be implemented to enable intensive care unit nurses to work for a long time and to develop educational programs to increase confidence in delirium care.

Impacts of death perceptions, terminal care stress, and life satisfaction on attitudes toward end-of-life care among nurses at a tertiary hospital (죽음인식, 임종간호 스트레스, 삶의 만족도가 상급종합병원 간호사의 임종간호태도에 미치는 영향)

  • Purpose: This study investigated the factors influencing attitudes toward end-of-life care among nurses at a tertiary hospital in Korea. Specifically, it examined the roles of nurses' characteristics, death perceptions, terminal care stress, and life satisfaction. Methods: The participants included 150 nurses working at a tertiary hospital, who were recruited between August 15 and September 19, 2023. Data were collected through an online survey and analyzed using descriptive statistics, the t-test, analysis of variance, Pearson correlation coefficients, and multiple regression analysis with SPSS/WIN 28.0. Results: Nurses' attitudes toward end-of-life care were positively correlated with death perceptions (β = .28, p < .001), 3 years or more of clinical experience (β = .25, p = .001), the experience of an acquaintance death (β = .22, p = .002) and life satisfaction (β = .20, p = .004). These variables explained 34.0% of the total variance in attitudes toward end-of-life care. Conclusion: It is essential to develop and implement individualized end-of-life nursing education programs, particularly utilizing simulations, for nurses with limited clinical experience and low levels of death perceptions. Further research should explore attitudes toward end-of-life care among various healthcare providers with a broader regional scope to improve the overall quality of end-of-life care.

Factors Influencing End-of-Life Care Stress among Nurses in a Superior General Hospital (상급종합병원 간호사의 임종간호 스트레스에 영향을 미치는 요인)

  • Purpose: This study aimed to identify the effects of end-of-life care competencies, death anxiety, and social support on stress among nurses at a superior general hospital. Methods: The participants were 198 nurses who had experience providing end-of-life care at a superior general hospital in city B. Data were collected between March 15, 2024, and April 14, 2024. Data were analyzed using the IBM SPSS Statistics 27 program for frequency, average, t-test, ANOVA, Pearson's correlation coefficient, and multiple regression analysis. Results: Factors influencing end-of-life care stress were death anxiety (β=.38, p<.001), age (21~25 years, β=-.23, p=.036), and religion (β=.23, p=.017), with an explanatory power of 20.1% (F=9.26, p<.001). Social support did not significantly affects end-of-life stress. Conclusion: Death anxiety, age, and religion significantly affected nurses' stress experiences during end-of-life care. The findings suggest that tailored education, emotional support, and organizational strategies addressing nurses' age and experience and balancing personal beliefs with professional duties are essential for reducing end-of-life stress. These results can inform strategies for improving the quality of end-of-life care and enhance nurses' well-being.

A Predictive Model of Turnover among Nurses in a Tertiary Hospital: Decision Tree Analysis (의사결정나무 분석기법을 이용한 상급종합병원 간호사의 이직 예측모형 구축)

  • Purpose: The purposes of this study were to develop a predictive model and evaluate this model of turnover in hospital nurses. Methods: Participants were 1,565 nurses from a tertiary hospital in South Korea. Descriptive statistics and a decision-tree analysis were performed using the SPSS WIN 23.0 program. Results: The turnover groups were presented in eleven different pathways by decision tree analysis. There were three high-risk groups with a higher turnover rate than the average, and eight low-risk groups with a lower turnover rate. Among them, two low-risk groups had a 0% turnover rate. The groups were classified according to general characteristics such as position, period of temporary position, clinical career at last working unit, total clinical career, and period of leave of absence. The accuracy of the model was 83.2%, sensitivity 63.7%, and specificity 98.1%. Conclusion: This predictive model of turnover may be used to screen the turnover risk groups and contribute for decreasing the turnover of hospital nurses in South Korea.

A Survey Study on Discharge Process of Critically Ill Patients: for Residents at a Tertiary Hospital in Korea (중증 환자 퇴원에 대한 설문연구: 한국의 일개 상급종합병원 전공의를 대상으로)

  • Purpose: The number of severely ill patients requiring post-acute care has been increasing. Careful discharge planning minimizes unplanned emergency room visits and readmissions. This study aimed to survey the knowledge, experience, confidence, and obstacles faced by medical residents and fellows regarding the discharge process of severely ill patients. Methods: A survey consisting of 27 questions was sent electronically to residents and fellows who had experience in discharging severely ill patients from a tertiary hospital in Korea. The survey was conducted over a two-week period from September 29, 2022. Results: A total of 98 residents and fellows responded to the survey. Of these, 94% experienced difficulties related to the discharge process. The main obstacle was changes in the patient's condition during discharge planning (92.3%). Although 95% of the respondents acknowledged the need for providing discharge information, only 53.1% of the residents and fellows practiced this. Only 42.9% of the respondents and 20.4% of residents and fellows explained local community healthcare and welfare resources to patients because of a lack of relevant knowledge (69.7%) and feeling no responsibility to explain (40.4%). Conclusion: This study revealed that residents and fellows experienced difficulties in devising discharge plans and providing post-acute care related information, despite recognizing the importance of these. These gaps result from the lack of a discharge planning curriculum regarding critically ill patients and appropriate training in the discharge process. This suggests that an integrated discharge planning curriculum should be developed and adopted in residents' training programs for the differentiated treatment of critically ill patients.
