  • We report a case of an 85-year-old woman with tuberculous subdeltoid bursitis. Tubeculous bursitis without adjacent joint tuberculosis may occur any site but it is rare. It is common in bursae subjected to trauma like trochanteric, prepatellar, olecranon and so on. Because the clinical findings or physical examination of tuberculous subdeltoid bursitis are similar to rotator cuff disorder and the MRI or arthroscopic findings are similar to synovial diseases, careful differential diagnosis is necessary.

  • Multiple rice body formation is a complication of chronic bursitis frequently associated with seronegative rheumatoid arthritis or tuberculosis. It resembles synovial chondromatosis on imaging and clinically. We report on a pathologically diagnosed multiple rice body formation in subacromial and subdeltoid bursitis in a 44-year-old man who was treated by surgical removal and bursectomy. At 16 months after the removal, range of motion of affected shoulder was normal. No evidence of recurrence of rice body in plain X-ray and ultrasonography. Multiple rice body formed in chronic subacromial and subdeltoid bursitis could be treated with surgical removal and bursectomy successfully.

  • Numerous small fibrinous rice bodies are a common finding in joints afflicted with rheumatoid arthritis(RA) or seronegative arthropathy, Subacromial and subdeltoid bursitis of the shoulder associated with multiple rice body formation is a rare occurrence. To our knowledge, this is the first report to describe the arthroscopic treatment of massive rice bodies in both glenohumeral joint and subacromial-subdeltoid bursae combined with full thickness of rotator cuff in RA. Besides, the MR appearance of subacromial-subdeltoid bursal rice bodies have been previously described in only few recent reports. Therefore, we also describe the MR appearances subacromial-subdeltoid bursae associated with multiple rice bodies in RA.

  • Purpose: We introduce arthroscopically assisted mini-open rotator cuff repair using anterolateral approach. Operative Technique: Placing lateral decubitus position on general anesthesia, a standard arthroscopic glenohumeral examination is performed to evaluate lesions of shoulder joint through posterior and anterior portal. And then arthroscope is placed in the subacromial space and we evaluate the size of the torn tendon and perform arthroscopic acromioplasty through lateral portal. A 3 to 4 cm skin incision is performed from anterolateral edge of acromion to distal and dissected along to raphe between anterior and middle deltoid. A deltoid retractor is then placed, allowing direct visualization of the rotator cuff and humeral head. As torn tendon is tagged by traction suture, we try to anatomical reduction on the footprint and then perform single row or double row repair of the rotator cuff using suture anchors. To prevent avulsion of the deltoid from the acromion, additional sutures by bone tunnel with acromion and deltoid is performed. Conclusion: This technique is useful procedure to get direct approach to anterior portion of supraspinatus tendon and to need lesser deltoid retraction than portal extension approach due to dividing along to raphe between anterior and middle deltoid. Also it provide better visualization of the superior portion of subscapularis and infraspinatus.

  • This study is analyzing about application of taping by movement about shoulder joint's isometric contraction. M. college students take part in experiment. WEMG-8 and HUMAC system are used. Findings were as follows: EMG of muscle's MVIC at each angle before and after taping. 1. In flexion, EMG with taping is effective for supra spinatus at $0^{\circ}{\cdot}45^{\circ}{\cdot}90^{\circ}$, anterior deltoid at $0^{\circ}{\cdot}45^{\circ}$ and biceps brachii at $0^{\circ}{\cdot}90^{\circ}$. 2. In extension, EMG with taping is effective for triceps brachii at $0^{\circ}$, latissimus dorsi at $90^{\circ}$ and posterior deltoid at $45^{\circ}{\cdot}90^{\circ}$. 3. Each angle viewpoint, EMG of latissimus dorsi in extension can be arranged in its high order as $90^{\circ}<45^{\circ}$, $0^{\circ}$ without taping and $90^{\circ}<0^{\circ}$ respectively. EMG of posterior deltoid in extension is in its high order as $90^{\circ}<45^{\circ}<0^{\circ}$ without taping and $90^{\circ}<0^{\circ}$.

  • This study aimed to investigate the effect of hand grip force on the activity of shoulder muscles in 2 arm postures. The muscle activity of the upper trapezius, lower trapezius, infraspinatus, anterior deltoid, and posterior deltoid muscles in 22 healthy subjects (11 men and 11 women) were measured using surface electrodes during 4 hand gripping tasks (0%, 30%, 50%, and 70% of maximum voluntary contraction) in 2 shoulder positions (neutral position and $90^{\circ}$ elevation position). Among changing grip force significantly differenced infraspinatus, anterior deltoid, and posterior deltoid muscles' activity in the shoulder neutral position(p<0.05). In the shoulder $90^{\circ}$ elevation position, anterior deltoid, posterior deltoid muscles' activity was significantly differenced(p<0.05). Hand gripping was found to alter muscle activation. The hand grip task activated the infraspinatus muscle in the neutral position and inhibited the deltoid muscle in the $90^{\circ}$ elevation position. This finding may prove useful for the development preventative measures and rehabilitation strategies for shoulder injuries.

  • Diffuse-type giant cell tumor is relatively rare than localized giant cell tumor. Moreover, diffuse type giant cell tumor is common in intraarticular area, rarely occurs at intramuscular or subcutaneous layer. We experienced 1 case of giant cell tumor within the deltoid muscle. So we report this case with review of the literatures.

  • The purpose of this study was to determine the comparison of upper arm, trunk and core muscle activities according to different performance in high Plank exercise using weightless exercise equipment. Six males(age, 23.00±0.73 years; height, 172.95±2.05 cm; body mass, 66.83±2.75 kg; and BMI, 22.33±0.72 kg/m2) completed this study as the subjects. Four type's high Plank exercises using weightless exercise equipment were performed(high plank, HP; high plank with air walker, HPAW; high plank with surf board, HPSB; high plank with waist trainer, HPWT). For the EMG analysis, we measured the body muscle activities of right side on the deltoid(DT), triceps brachii(TB), latissimus dorsi(LD), and external oblique(EO). This research's results were as follows. DT, TB, LD, & EO muscle activities were greatest during HPSB(p=.000). Therefore, these results are expected to serve as basic data for high Plank exercise using weightless exercise equipment performance applications in effective exercise programs.

  • Purpose: A comparison of the radiographic and the clinical outcomes between two different surgical approaches-Deltoid splitting and Delto-pectoral interval-on the proximal humerus fractures treated by locking compressive plate (LCP), is done. Materials and Methods: Medical records and pre- and postoperative radiographs were reviewed retrospectively for 75 adult patients who underwent surgical fixations with locking compressive plates from May 2005 to December 2011. Patients were divided into two groups according to the surgical methods. Differences in the neck-shaft angle between immediate postoperative period and final follow-up were compared between the two groups. Differences in constant score and Korean shoulder score (KSS) between affected arms and contralateral arms at final follow-up were also compared. Results: The differences in the neck-shaft angle between immediate postoperative period and at final follow-up was 12.04 degrees on average in Deltoid splitting approach and 10.20 degrees in Delto-pectoral interval approach, which was not statistically significant. Differences in constant score/KSS between the affected arm and the contralateral arm were 13.78/22.74 points in deltoid-splitting approach on average and 19.41/31.13 points in Delto-pectoral interval approach, showing that deltoid-splitting approach is significantly superior. Conclusion: Deltoid-splitting approach showed better functional outcomes in the fracture reduction and internal fixation using LCP for the treatment of unstable proximal humerus fractures.
