  • We propose an EMG-based input interface technology for helping the tetraplegic to utilize mouse, keyboard and power wheelchair. Among possible actions for the tetraplegic utilizing these devices, teeth-clenching is chosen as an input action. By clenching left, right or both teeth, and controlling the clenching duration, several input commands for utilizing the devices can be conducted. EMG signals generated by teeth-clenching are acquired around one's left and right temples and they are used as control sources for utilizing the devices. We develop signal acquisition devices, signal processing algorithms, and prototype systems such as power wheelchair control, mouse control, and game control. Our experimental results with the tetraplegic show that the proposed method is useful for utilizing the devices.

  • In this paper, we analyze the interaction features using eye-tracker and the characteristics of attention concentration of children with intellectual disabilities, and present gamification elements that can be used when designing digital contents for them. For children with quadriplegic cerebral palsy, eye-trackers will increase the accessibility of digital contents. Foreground gamification can be used for selective attention concentration, background gamification and intrinsic motivation to sustain attention concentration, and foreground gamification and extrinsic motivation for mobility.

  • Non-traumatic acute myelopathy caused by cervical disc herniation is rare. To date, no case has been reported to be caused by extrusion cervical disc herniation, unrelated to patient posture during surgery. Here, we report the case of a 65-year-old male patient with cervical myelopathy who underwent subsequent arthroscopic rotator cuff surgery under general anesthesia; non-cervical spine surgery. Ed. Notes: I am unable to understand the insertion of the highlighted phrase. Please delete if not required, or revise the sentence appropriately. Patient showed acute postoperative tetraplegia in spite of optimal anesthetic management. He showed no limitation of neck movement at pre-operative airway evaluation, and had no history of trauma to the cervical spine. During surgery, there had been no overextension or twisting of the neck, including at the time of anesthetic induction by tracheal intubation. However, cervical disc herniation causing spinal canal cord compression was detected in the postoperative magnetic resonance imaging, which probably resulted in tetraplegia of the patient. Motor and sensory functions were recovered after 21 days of conservative treatment, including steroid pulse intravenous therapy without any surgical intervention. In this report, the disease is described after reviewing other reported cases; furthermore, we also discuss the pathophysiology of the disease. Based on our report, we propose that under general anesthesia, clinicians should pay attention to the possibility of pre-existing cervical disease, even in non-cervical spine surgeries of geriatric patients.

  • A 3-year-old, intact female English cocker spaniel dog was evaluated for an acute onset of tetraplegia. Utilizing magnetic resonance (MR) imaging, the cause of the neurologic deficits was determined to be an peracute hemorrhage of spinal cord. The MR study additionally demonstrated parenchymal hyperintensity on T2weighted images and similarly located hypointensity on T1weighted images. Hemorrhagic myelomalcia was suspected based on these MR characteristics, which was subsequently confirmed histopathologically.

  • Purpose: The purpose of this cross-sectional study was to assess the nutritional states and investigate the energy intake of tube-fed patients with severe neurodevelopmental disability. Methods: Eighty six tube-fed patients with severe neurodevelopmental disability were studied. Heights were measured by flexible scale segmentally. Nutritional parameters of weight, triceps skinfold thickness and mid-arm circumference were also measured. Total caloric intakes through the tube were calculated. Results: The anthropometric results showed that nutritional states of tube-fed patients with severe neurodevelopmental disability were poor. The mean daily caloric intake was much less than daily energy requirement (mean=45.2% of requirement). The caloric intake was 7.2 kcal/height (cm), 57.0 kcal/weight (kg). Height was more related with caloric intake (r=0.476) than weight (r=0.263). Conclusion: These results provide that tube-fed patients with severe neurodevelopmental disability were growth retarded and their energy intakes were much less than daily energy requirements. The preliminary evidence was that they need adequate nutritional supply.

  • The purpose of this study was to determine the effect of custom seating system on upper extremity access to mouse for a child with cerebral palsy. A single-subject ABAB design was used with one 13years old participant. The measurements were response time and accuracy of mouse click in the participant's typical position and in an intervention position. The intervention position was taken by using current clinical guideline and research on promoting upper extremity movement. The intervention position was achieved through hands-on postural support to the participant's typical seating. The results of this study were as follow: first, response time was decreased in the intervention position compared with the typical seating. Second, accuracy of mouse click was highly in every phases. So further analyses were not conducted on these data. The results of this study provide empirical evidence of the positive effects of functional seating on access to mouse for children with cerebral palsy. Further research is required to confirm the positive effect of the custom seating system across other disabilities type and more subjects.

  • A 2-year-old, 2 kg Maltese dog was evaluated for progressive tetraparesis and neck pain. The dog had been treated with steroids for the preceding 2 weeks after unknown trauma but was deteriorating progressively and had become tetraparetic. The dog was presented with a non-ambulatory tetraparesis. Radiographic and computed tomographic examinations revealed a transverse C2 fracture with subluxation of the atlantoaxial joint and C2-C3. In addition, hydrocephalus was observed on magnetic resonance imaging. Stabilization of C1-C3 using screws, wires, and polymethyl methacrylate (PMMA) was performed. Application of ventral screws, wires, and PMMA resulted in improvement of the clinical signs after 4 weeks, and the dog could walk as before the tetraparesis 6 weeks after the operation. This stabilization method is an effective surgical treatment for management of cervical instability.
