A Study About N.P.C, Heterophoria and Near Convergence and Divergence by Amount of the Refractive Errors (굴절이상도에 따른 폭주근점과 근거리 수평사위, 폭주 및 개산 여력의 연구)

  • Purpose: To study the relationship of N.P.C, heterophoria and near convergence and divergence by amount of the refractive error. Methods: All subjects have no ocular disease and their mean age is 22.7. All 39 subjects examined about refractive error, near point of convergence (NPC), heterophoria, near convergence and divergence. Results: Classified by low refractive state, middle refractive state, and high refractive state compared with the expected value of Morgan. NPC had been measured within low and middle refractive state eight all the 8cm. However, high refractive state measured 9.64 cm. Low and middle refractive state for the classification by near phoria. Near esophoria groups was smallest by near divergence and exophoria groups was smallest by near convergence. Conclusions: Near convergence were largest with esophoria while near divergence were largest with exophoria.

A Study on The Measurement and Tendency of Horizontal Heterophoria Using Von Graefe Method (Von graefe법을 이용한 사위의 측정과 경향에 관한 연구)

  • We researched distant & near horizontal heterophoria of myope using von graefe method on full correction condition for young people 49 subjects(98eyes) with myopia after testing the objective refraction. Among the whole subjects, we got the results as Orthophoria 51.00%, Exophoria 38.75%, Esophoria 10.25% and Heterophoria 49.00% at a long distant. At a short distant, they showed Orthophoria 22.45%, Exophoria 67.35, Esophoria 10.20%, and Heterophoria 77.55%, Orthophoria 22.45. At a long distant they showed Orthophoria 4.08%, Exophoria 8.16%, Esophoria 0% on high myopia, Orthophoria 18.37%, Exophoria 12.25%, Esophoria 0% on middle myopia and Orthophoria 28.57%, Exophoria 18.37%, Esophoria 10.20% on low myopia.

The Effect of Vision Training on Exophoria and Intermittent Extropia Using MYSTERY CIRCLE System (MYSTERY CIRCLE 시스템을 이용한 폭주부족형 사위 및 간헐성 사시 환자의 시기능 훈련 효과 연구)

  • Purpose: The purpose of this study was to investigate the effect of vision training using MYSTERY CIRCLE system on patients with exophoria and intermittent extropia. Mehtods: The vision training observation group consists of 26 clients who had exophoria (n=18) and intermittent extropia (n=8) without opthalmic diseases or vertical phoria. Our clients took each vision training once a week for eight weeks and were investigated for the symptoms changes of binocular vision. Results: According to the results, MISTERY CIRCLE vision training methods improved the binocular vision function and sensation symptoms. For Worth 4 Dot test, Titmus test and red lenses showed fusion rate changes. Conclusions: The study proved that MISTERY CIRCLE vision training system for patients with binocular vision was effective for the improvement of the binocular vision symptoms of these patients.

Correlations between Exophthalmetry and Visual Functions (안구돌출도와 시기능의 상관관계 분석)

  • Purpose: This study was to find the correlation between exophthalmetry and visual functions. Methods: Exophthalmetry and visual functions of 200 adults without ophthalmophathy and general disease were examined. Exophthalmetry were measured with Hertel's exophthalmometer. Visual functions (myopia, distance exophoria, near exophoria, near point of convergence, near point of accommodation) were measured for three times. Results: The exophthalmetry was $13.59{\pm}1.10mm$. Myopia, distance exophoria, near exophoria, near point convergence, and near point accommodation were measured as $-2.48{\pm}1.91D$, $3.51{\pm}1.78{\Delta}$, $5.73{\pm}2.44{\Delta}$, $9.63{\pm}2.82cm$, and $21.73{\pm}10.28cm$. With increasing exophthalmetry, the unaided visual acuity, myopia and astigmatism were decreased. On the other hand, distance exophoria, distance esophoria, near exophoria, near point of convergence and near point of accommodation were significantly increased, while exophthalmetry was increased. Conclusions: The relationship between visual functions and exophthalmetry were found to be statistically significant. It could be used as the clinical criteria for prescription of binocular inspections.

Relationship Between Subjective Symptoms with Near Work and Binocular Function (근업시 자각 증상과 양안시 기능의 관계)

  • The aim of this work was to study the relationship between subjective symptoms and binocular function during near work in forty one(12-17yr) schoolchildren. CISS symptom survey, amplitude of accommodation, accommodative facility, neagative relative accommodation(NRA)/positive relative accommodation(PRA) near point of convergence(NPC), heterophoria, AC/A ratios were measured. The results of measurement were compared with normal expected value of each test and classified into normal and abnormal group. Accommodative insufficiency was more general than convergence insufficiency and esophoria was greater than exophoria. NRA/PRA showed very high and AC/A ratios were very low compare with normal expected value. The results of CISS was as follows; most frequent symptom was tired, mean score was $16.63{\pm}7.49$, mean number of symptoms were $10.07{\pm}3.04$, and mean number of serious symptoms were $1.98{\pm}2.13$. After classified into 4 groups according to score of symptoms, number of symptoms, and degree of symptoms and then analyze the correlation compare with binocular vision test results. NRA was most correlated with group comparison, accommodative facility was most correlated with score of symptoms, NRA was most correlated with number of symptoms, and near horizontal phorias were most correlated with serious symptoms. NRA showed most correlative relation with near symptoms.

Graphic Method of Visual Analysis and Norms Determination for the Far Distance Phoria (원거리사위별 그래프 분석과 표준값 결정)

  • We investigated the visual analysis of the far distance phoria for 83 subjects who have emmetropia or myopia eyes, using the graphic method, and determined new norms. We got the following results from these experiments. 1. The measured norm of far distance phoria is $1.12{\Delta}$, exophoria, the that of near distance phoria is $0.50{\Delta}$, exophoria, the that of negative relative convergence at far distance(N.R.C.) is ${\times}$/11/5, the that of positive relative convergence at far distance(P.R.C.) is 10/20/8, the that negative relative convergence at near distance(N.R.C.) is 11/21/5, the that of positive relative convergence at near(P.R.C.) is 10/20/8, the that of negative relative accommodation(N.R.A.) is +2.54D, and the that of positive relative accommodation(P.R.A.) is -2.60D, respectively. We know from theses results that the measured norms of far distancet phoria is higher than Morgan's norms, while those of near distance phoria are lower than Morgan's norms. 2. We also know from these experiments that Morgan's norms are closely related to A, B, and C groups and Morgan's criterion can more usefully apply to the visual analysis. 3. In Sheard's criterion and Percival's criterion, the prism prescription is occurred when the exophoria of the far-point phoria is over $7{\sim}9{\Delta}$, and the esophoria of that is over $4{\sim}6{\Delta}$.

The factors influencing the quantity of Phoria: I. A study of effects to the quantity of Phoria by the refractive error and the conformity to the optical center and the pupillary center (사위량에 영향을 미치는 요인: I. 굴절이상, 동공중심과 광학중심의 일치에 따른 연구)

  • The purpose of this study was to evaluate the outcome and Factors that influence the quantity of Phoria in the low correction and perfect correction. Also the coincidence with the opticenter and the pupillary distance was a principal factor that influence the quantity of Phoria. Thereupon, this study is attributed to promote the perfect Phoria test. The subjects for this study were 120 persons(240 eye) in myopic refractive errors. ISP/WIN program was used for the data analysis. The collected data was analyzed by descriptive statistics and Spearman's correlation coefficient. The results of this study were as follows: 1. The prism pattern was difference between low correction and perfect correction in the Phoria test. The proportion of orthophoria was changed from 10.0% to 12.5%, exophoria was changed from 67.5% to 62.5%, and esophoria was changed from 22.0% to 32.5%, respectively. 2. The average of optical center distance and pupillary distance were 31.70mm and 31.49 mm, respectively. 3. Among the 120 myopic glasses wearers, the distance between optical centers was coincided with the pupillary distance in 37.5%, and discrepant in 62.5%. 4. For the patients who were coincided with the pupillary distance, the proportion of exophoria decreased 53.33%, esophoria increased 20.0%. 5. For the changing of the quantity of Phoria in the low correction, the degree of exophoria decreased 1.11 prism diopters in the perfect correction, esophoria increased 0.39 prism diopters.

Influence of Different Diagnostic Criteria on Frequency of Convergence Insufficiency (진단기준 차이가 폭주부족의 빈도에 미치는 영향)

  • Purpose: This study was to investigate whether the application of different diagnostic criteria affected the frequency of convergence insufficiency (CI). Methods: Eighty one subjects with mean age of 22.54 years (20 to 27 years) were evaluated. Binocularity tests after refraction were performed as the following tests: near point of convergence (NPC) with an accommodative target, phoria using von Graefe method, positive fusional vergence (PFV) with a phoropter. Subjects with CI were diagnosed when exophoria (exo) was greater at near than at distance ($exo{\geq}4{\Delta}$, or >$6{\Delta}$), fusional vergence was $PFV{\leq}11{\Delta}$ for blur, $PFV{\leq}15{\Delta}$ for break, Sheard's or Percival's criterion, and NPC was $NPC{\geq}6cm$, ${\geq}7.5cm$ or >10 cm. Results: Frequency of CI with one diagnostic criterion was ranged from 6.2% to 77.8%, and was overestimated or underestimated according to criteria. It was reduced to the range of 6.2% to 43.2% with diagnostic criteria more than two, especially to the range of 24.7% to 28.4% with lower variability in diagnostic criteria including phoria and Sheard's criterion. There were high relationship between total score of signs and phoria score (r = 0.772, p<0.001), and measured phoria and Sheard's criterion (r = -0.654, p<0.001), but NPC had a high variability and a weak or no significant relationship with other diagnostic criteria. Results suggested $exo{\geq}4{\Delta}$, Sheard's criterion and $NPC{\geq}7.5cm$ for diagnostic criteria of signs and sequence for CI. Conclusions: Frequency of CI is likely to be over- and underestimated with diagnostic criteria. Cutoff values and procedures for phoria, Sheard's criterion and NPC as clinical signs should be suggested definitely in diagnosis associated with CI.

Research on The Utility of Acquisition of Oblique Views of Bilateral Orbit During the Dacryoscintigraphy (눈물길 조영검사 시 양측 안 와 사위 상 획득의 유용성에 대한 연구)

  • Purpose: Diversity and the lachrymal duct deformities and the passage inside the nasal cavity except for anterior image such as epiphora happens during the test were able to express more precisely during the dacryoscintigraphy. Also, we thought about the necessity of a method to classify the passage into the naso-lachrymal duct from epiphora. Therefore, we are to find the validity of the method to obtain both oblique views except for anterior views. Materials and Methods: The targets of this research are 78 patients with epiphora due to the blockage at the lachrymal duct from January 2013 to August 2013. Average age was $56.96{\pm}13.36$. By using a micropipette, we dropped 1-2 drops of $^{99m}TcO4^-$ of 3.7 MBq (0.1 mCi) with $10{\mu}L$ of each drop into the inferior conjunctival fold, then we performed dynamic check for 20 minutes with 20 frames of each minute. In case of we checked the passage from both eyes to nasal cavity immediately after the dynamic check, we obtained oblique view immediately. If we didn't see the passage in either side of the orbit, we obtained oblique views of the orbit after checking the frontal film in 40 minutes. The instrument we used was Pin-hole Collimator with Gamma Camera(Siemens Orbiter, Hoffman Estates, IL, USA). Results: Among the 78 patients with dacryoscintigraphy, 35 patients were confirmed with passage into the nasal cavity from the anterior view. Among those 35 patients, 15 patients were confirmed with passage into the nasal cavity on both eyes, and it was able to observe better passage patterns through oblique view with a result of 8 on both eyes, 2 on left eye, and 1 on right eye. 20 patients had passage in left eye or right eye, among those patients 10 patients showed clear passage compared to the anterior view. 13 patients had possible passage, and 30 patients had no proof of motion of the tracer. To sum up, 21 patients (60%) among 35 patients showed clear pattern of passage with additional oblique views compared to anterior view. People responded obtaining oblique views though 5 points scale about the utility of passage identification helps make diagnoses the passage, passage delayed, and blockage of naso-lachrymal duct by showing the well-seen portions from anterior view. Also, when classifying passage to naso-lachrymal duct and flow to the skin, oblique views has higher chance of classification in case of epiphora (anterior:$4.14{\pm}0.3$, oblique:$4.55{\pm}0.4$). Conclusion: It is considered that if you obtain oblique views of the bilateral orbits in addition to anterior view during the dacryoscintigraphy, the ability of diagnose for reading will become higher because you will be able to see the areas that you could not observe from the anterior view so that you can see if it emitted after the naso-lachrymal duct and the flow of epiphora on the skin.

Possibility about evaluation test of Substitution of NPC Alteration for AC/A Ratio (폭주근점 변화의 AC/A비 대체 평가지표에 대한 가능성)

  • This study aimed to evaluate the possibility for assessment of binocular visual function about alteration for near point of convergence (NPC). Study subjects were total 30 (emmetropia 16, myopia 14), who had no eye disease except for the phoria and monocular corrected vision 1.0 and over. Near point of accommodation (NPA), NPC and phoria test were measured both at ordinary and stimulation of +1D. Ordinary NPC was 1.77cm shorter than NPC stimulated +1D and the NPC of emmetropia was shorter than that of myopia. The difference between NPC stimulated +1D and ordinary were increased in the emmetropia. As the NPC increased, the AC/A ratio was elevated and the feature of near exophoria appeared. The results suggested that the alteration of NPC might substitute AC/A ratio for reference variable of binocular vision. If converged and analyzed comparatively with another binocular visual evaluation test, the alteration of NPC could be developed for substitution of evaluation test.
