Objectives. The purpose of this study is to analyze the social-environment factors by region for cause of death elderly people in Korea and to study the factors of longevity. Methods. The study included 16 regions with a total of 177,585 elderly peoples. The data in this study was collected from The National Statistical Office, Republic of Korea. Results. Those regions the highest cerebrovascular disease were Incheon County in that order. The correlation of social-environment by cause of death factors were divorce (+0.832), air pollution of Pb ; lead (+0.879), smoking (+0.895), fatness (+0.666), local tax revenue (+0.756), air pollution of SO2 (+0.602) and dirt road (+0.863). Conclusions. We should learn to live long and healthily from residence harmonized with natural environment. Longevity of elderly peoples is to be fostered for the promotion of health by control the social-environment factors.
In a situation of crisis, an ambulance is dispatched according to the given emergency management information system(EMIS). However, the problem of past EMIS is that they did not concentrate on the reliable communication between the ambulance and medical specialists, thereby causing some needless casualties. This paper presents HEMIS, a highly-reliable emergency management information system, to solve such flaws. The effects showed that HEMIS was superior in data packet size, data safety and quality of service(QoS) to that of typical EMIS. Therefore, HEMIS is expected to increase the quality of medical service in ambulances, thereby decreasing casualties of patients carried in ambulances.
Background: Descending necrotizing mediastinitis(DNM) is a serious complication originating in odontogenic or oropharyngeal infection with previously reported mortality rates of 25% to 40%. We retrospectively reviewed the 4 years of our surgical drainage and debridement in DNM. Material and Method: We studied 7 cases diagnosed as DNM from 1997 through 2000. Primary oropharyngeal infection lead to DNM in four cases(57%) and odontogenic abscess in three cases(43%). All patients were received emergent cervicotomy and thoracotomy or sternotomy for debridement of necrotic tissue and mediastinal or pleural drainage. Result: Five cases were evolved well and were discharged after a mean of 42 days. Two patients(28.6%) died. Three patients required reoperation due to local surgical complication; empyema(two) and impending cardiac tamponade. One of these patients died on 12 post-reoperative day due to great vessel erosion, renal and respiratory insufficiency. The other patient died of broncho- esophageal fistula and asphyxia on 10 postoperative day without reoperation. Conclusion: On the basis of experience accrued in treating these patients, early diagnosis by cervicothoracic computed tomographic scan of neck and thorax aids in rapid indication of a surgical approach of DNM. We emphasize that performing early surgical drainage and debridement of necrotic tissues with intensive postoperative care can significantly reduce the mortality rate.
Background: The simultaneous bilateral spontaneous pneumothorax is a rare clinical event. Contrary to the unilateral pneumothorax, the patients with simultaneous bilateral spontaneous pneumothorax sometimes complains of severe respiratory distress, cyanosis and chest pain without tention pneumothorax. It is often dangerous; therefore, the chest drain should be inserted immediately. Material and Method: Between March 1994 and February 2004, 802 patients were treated in our department for spontaneous pneumothorax. Among these, the simultaneous bilateral spontaneous pneumothorax developed in 14 patients (1.7%). Result: Out of fourteen patients, two females and twelve males presented with simultaneous bilateral spontaneous pneumothorax. The patient age ranged between 0 and 79 years with mean age of 31.2 years. In eleven patients, this was the first episode of pneumothorax. One patient had combined hemopneumothorax and two patients had combined pyopneurnothorax. Six patients had smoking history (42.8%, average 17.3 p-y). Five patients had pulmonary tuberculosis history and among these, two patients had active pulmonary tuberculosis. Three patients were died due to meconium-aspiration pneumonia (1 patient) and ARDS (Acute Respiratory Distress Syndrome) with pneumonia (2 patients). We treated these patients with nasal oxygen inhalation, chest drain insertion, thoracotomy, VATS (Video-Assisted Thoracoscopic Surgery) and chemical pleurodesis. Conclusion: The simultaneous bilateral spontaneous pneumothorax developed in 14 patients (1.7%) among 802 patients. Prompt insertion of chest drain is needed for a relief of severe symptoms, and to reduce the risk of recurrence, early thoracotomy or VATS should be performed rather than chest drain insertion only.
The increased risk of cancer with exposure to low-dose radiation was estimated through longitudinal study for radiation workers at the nuclear power plants in Korea. The radiation dose data were collected from the Radiation Safety Management System(RSMS) of the Korea Hydro & Nuclear Power Co., Ltd(KHNP). The cancer risks with radiation exposure were evaluated in terms of relative mortality ratios(RMR) and relative incidence ratios(RIR) to the unexposed employees at the nuclear power plants, and of the standardized mortality ratios(SMR) and standardized incidence ratios(SIR). There were no significant increases of canters of all sites in the exposed group either in RIR[1.08, 95% confidence interval(CI) 0.74-1.58] or in RMR[1.21, CI 0.70-2.08]. Neither SIR[0.81, CI 0.28-0.96] nor SMR[0.86, CI 0.66-1.10] significantly deviated from 1.0 for cancers of all sites. The trend analysis did not identify evident dose-response relationship due to insufficient numbers of the cases. Consequently, it is concluded that increases in cancer risks in the radiation worker group exposed to low doses at the nuclear power plants in Korea are not identified at this time.

Reducing the impact of collisions of cars is a major issue for reducing the injury and death of passengers. According to the statistical data of the Road Traffic Authority, the deaths from side collision accidents caused by the collision of passenger cars is greater than the deaths from head-on collision accidents. To accommodate this, vehicle designers have added a reinforcing material called the impact frame and impact beam on the inside of the door. Many experiments are needed to develop the door impact beam. These reinforcements to develop a collision experiment is essential. Collision experiments are costly and time consuming. This study used a drop Impactor to obtain the impulse and a strain experimental device was developed for this purpose. The economic costs were reduced and the ideal experiment device configuration was determined. A comparison of the experimental results with numerical value analysis revealed $3.5{\tiimes}10-3sec$ strain ranging from $3.49{\tiimes}10-3$ to $3.99{\tiimes}10-3$.
The study is to estimate the social costs of premature deaths due to nuclear power plant(NPP) accidents, by resorting to the contingent valuation method(CVM) which is used to estimate the value of a statistical life(VSL). The VSL estimate is about 3.55 billion won, which is multiplied by some 1.8 million premature deaths due to the accidents in world history of NPP, to get a maximum social cost of 1,952 trillion won. This estimate is equivalent to the 2022 real GDP of Korea. The annual average number of premature deaths and the resulting average social cost is 26,000 and 28 trillion won, respectively. The social cost of premature deaths due not only to accidents, but also the air pollutants from fired power plants(FPP) during 1987~2021 is estimated to be 26,919 trillion won. This is equivalent to 2021 US GDP, and is about 3,000 times higher than that for NPP of 9 trillion won. In 2021, the estimated social costs of FPP and NPP are 1,075 trillion won and 292 billion won, respectively. For South Korea, the study suggests to adapt an energy mix of increased share of electricity production for NPP relative to FPP, given that the 2050 carbon neutrality strategy of Korea is expected to lead to an increased share of renewable energy in electricity production. The study emphasizes accumulating the number of CVM-based VSL studies to ensure efficient energy policies.

Longitudinal data often occur in prospective follow-up studies. Joint model for longitudinal data and failure time has been applied on several works. In this paper, we extend it to the case where longitudinal data involve informative observation time process as well as competing risks survival times. We use a likelihood approach and derive an EM algorithm to obtain maximum likelihood estimate of parameters. A suggested joint model allows us to make inferences for three components: longitudinal outcome, observation time process and competing risk failure time. In addition, we can test the association among these components. In this paper, liver cirrhosis patients' data is analyzed. The relationship between prothrombin times measured at irregular visiting times and drop outs is investigated with a joint model.
