Treatment Outcomes of Septoplasty with Turbinate Surgery in Septal Deviation with Chronic Hypertrophic Rhinitis (비중격 만곡증 및 만성 비후성 비염 환자에서 비중격 성형술고 하비갑개 수술의 치료효과)

  • Background: Septoplasty with turbinate surgery is common surgical treatment in patients with septal deviation and chronic hypertrophic rhinitis. The aim of this study was to evaluation objective outcomes of septoplasty with turbinate surgery by analysis of subjective symptom score with objective acoustic rhinometric test before and after surgery, prospectively. Materials and Methods: We reviewed 45 adult patients which were done septoplasty with bilateral turbinectomy or turbinoplasty and followed up at least 3 months by one rhinologist from November 1999 to April 2000, prospectively. We analyzed subjective symptom score, minimal cross-sectional area (MCA), C-notch cross-sectional area, and total volume of both nasal cavity before and after surgery. Correlation test was studied between symptom improvement and acoustic rhinometric results. Results: Twenty nine cases were male and sixteen cases female. The average age was 26.9 year-old (range: 17 to 57 years). There was significantly improvement of symptom score in postoperative 3 months (p<0.05). There was significantly increased C-notch cross-sectional area and total volume in postoperative 3 months. Symptoms improvement were associated with acoustic rhinometric profiles, but, there was not significantly correlation. Conclusion: Septoplasty with turbinate surgery is considered to be effective for nasal obstruction in patients with septal deviation and turbinate hypertrophy. Acoustic rhinometric test is favorable objective test for evaluation of symptom improvement after septal surgery.

  • Background: Chronic paranasal sinusitis is one of the most common disease in the otorhinolaryngologic field. Endoscopic sinus surgery is treatment of choice in chronic paranasal sinusitis. The aim of this study was to evaluate objective and subjective treatment outcomes of endoscopic sinus surgery in adult chronic paranasal sinusitis with or without polyp. Materials and Methods: We reviewed 84 adult patients underwent endoscopic sinus surgery by one surgeon from June 1999 to June 2000, prospectively. We analyzed preoperative and postoperative subjective symptom scores and acoustic rhinometric results. Results: Fifty cases were male and thirty four cases female. The average age was of 33 year-old (range: 17 to 66 years). There was significantly improvement of symptom scores in postoperative 3 months and 6 months compared with preoperative symptom scores. There was significantly increased postoperative total volume of nasal cavity. When we compared high score group with low score group, there was statistically significant improvement of symptom scores between preoperative stage and postoperative 3 months in radiologic grading group. Conclusions: Endoscopic sinus surgery is considered to be effective for the treatment of chronic paranasal sinusitis. It seems to be helpful to employ subjective symptom score system and objective total volume change of nasal cavity through acoustic rhinometric test to analyze effectiveness of endoscopic sinus surgery. In this study, the most important preoperative factor of sinus surgery outcomes is radiologic grading system.

CANCER OF THE NASAL FOSSAE (비강 및 부비동암)

  • Nasal fossae 의 암이란 비강과 부비동들에서 발생하는 것으로서 환자를 가장 괴롭히며 또한 가장 믿을수 없는 악성종양들중의 하나이다. 비록 본 예수병원 암환자 등록부에 의하면 전 암환자의 2.2% 발생빈도로서 주요한 발생빈도를 보이지는 않지만, 이 부위의 암을 치유하는데는 세심하고 철저한 모든 진단적 검사와 과감한 외과 및 치료방사선의 병합치료가 요구된다. 저자들은 지난 22 년간 비강 및 부비동 (Nasal fossae) 에서 발생한 원발성 악성종양중 치유목적의 근치수술을 시행한 68 예를 임상고찰 하였다. 근치수술을 시행했던 68 예중 91% 에서 제 3 병기 또는 4 병기의 진행된 경우 이었다. 외과적 수술은 한예의 사골동 (篩骨洞) 종양적출술 및 부분상악동(上顎洞)절제술 한 예를 제외한 66 예 모두에서 전상악동(全上顎洞)절제술 (total maxillectomy) 또는 확장 전상악동(全上顎洞)절제술 (extended total maxillectomy) 을 시 하였다. 저자들은 역학적(疫學的), 병리학적(病理學的), 병기(病期) 및 치료, 재발율과 생존율들을 분석 고찰하였으며 3가지 치료형태를 서로 비교하였다. 즉 수술만 시행한 군, 수술전 방사선 치료 및 수술병합군, 수술과 수술후 방사선치료 병합 군으로 나눴다. 저자들의 예비적 (preliminary) 관찰 결과는 2 년간 무병생존율 (disease-free 2-year survival) 만을 볼때, 수술만 시행한 군에서 40%로써 통계학적으로는 가장 좋았으나 실제는 수술만 시행한 군에서는 단지 40%만이 제 4 병기(病期)의 진행된 경우였으나 수술전 방사선치료 또는 수술후 방사선치료등의 병합치료에서는 제 4 병기(病期)의 진행된 상태가 무려 60 %나 되었다. 전체적인 재발율 (Overall recurrence rate) 은 68.2%로써 무서울 정도로 높았으며 전체적인 2 년 무병율은 23.7%였다. 저자들은 이 분야에서 실패의 원인분석과 치료방법의 선택등에 대한 지침을 제시하고저 한다.

Outcome in Impingement Syndrome of the Shoulder According to Presence of Stiffness (견관절 충돌 증후군 환자에서 강직 여부에 따른 치료 결과)

  • Purpose: The purpose of this study is to compare the outcome of operative results in the impingement syndrome of the shoulder with and without the stiffness. Material and Method: Seventy-six patients who had the impingement syndrome without stiffness were evaluated, and treated with the subacromial decompression and 24 patients who had the impingement syndrome with stiffness, were treated with the subacromial decompression and the manipulation. The average follow-up period was 32 months. Result: The impingement syndrome of the shoulder with stiffness was more severe in the preoperative pain and worse in ASES score than without stiffness. The postoperative pain and ASES score improved in the both group. The satisfactory groups were 67% in the group with stiffness and 80% without stiffness. The satisfactory rate was 83% in the group with stiffness and 93% without stiffness. The satisfactory groups with diabetes were 47% in the group with stiffness and 81% without stiffness. Forward elevation, exeternal rotation at the side and internal rotation improved in both groups postoperatively and there were no statistically significant differences postoperatively External rotation was restricted statistically in the group with stiffness. Conclusion: Although patients may not regain the full range of motion, the technique of manipulation followed by arthroscopic subacromial decompression offers good pain relief and satisfactory functional recovery for the impingement syndrome with stiffness. However preoperative counseling is necessary for the impingement syndrome combined with diabetes and stiffness due to poor out come.

  • Children constitute a large majority of patients with foreign bodies in the nose. Adults with foreign bodies in the nose are usually mentally disturbed, and may be the victims of penetrating injuries or of operation on the nose. Foreign bodies enter the nose through the anterior naris, posterior naris, penetrating wound, $.$$.$$.$$.$ etc. Authors experienced a case of the broken pencil, 3.5cm in length, which entered the nose through the cheek and stayed there for 4 days.

  • In addition to respiration, the nose performs three other major physiological functions-air-conditioning, filtering, and smelling. On the basis of our experience in experimental investigations of nasal airflows in normal and abnormal nasal cavity models, airflows in the normal model and three artificially deformed models, which simulate the results of surgical treatments (inferior turbinectomy), are investigated by PIV and CFD. The left cavities of all three models are normal, and the right cavities are modified as follows: (1) excision of the head of the inferior turbinate, (2) resection of the lower fifth of the inferior turbinate, and (3) resection of almost the entire inferior turbinate. The use of high-resolution CT data and careful surface rendering of three-dimensional computer models with the help of an ENT doctor provide more sophisticated nasal cavity models. Nasal airflows for both normal and deformed cases are also compared.

  • Obstructive sleep apnea syndrome(OSAS) is a common disease in the field of otorhinolaryngology and is characterized by repeated upper airway occlusions occurring during sleep. OSAS can occur due to various etiologies of the nasal, oral, pharyngeal and laryngeal airway in adults. Nasal obstruction can be caused by septal deviation, nasal polyps, concha bullosa, choanal atresia, neoplasms, foreign body, postoperative/post-traumatic synechiae, various rhinitis and so on. There are various kinds of surgical treatment of OSAS including nasal surgery, LAUP, UPPP, surgery of tongue base, tracheostomy and so on, but the effect of nasal surgery on snoring and OSAS is controversial. The authors report the case of a patient who had experienced nasal obstruction, moderate snoring and OSAS and who improved after septoplasty and turbinoplasty.

Adjuvant Metronomic Chemotherapy in a Dog with Nasal Transitional Carcinoma (개의 비강 이행암종에서 메트로노믹항암 요법)

  • A four-year-old spayed female Maltese dog weighing 3 kg was referred with reverse sneezing and fourmonth history of unilateral nasal discharge. She was diagnosed as nasal transitional carcinoma which in bilateral nasal cavity and extending into the frontal sinus by biopsy with rhinoscopy. Metronomic chemotherapy with cyclophosphamide ($12.5mg/m^2$) and toceranib phosphate (2.5 mg/kg) was initiated following after surgical treatment of nasal mass debulking. Clinical response was good and had no side effects during the chemotherapy period (11 months after diagnosis). This is a case report describing adjuvant metronomic chemotherapy in nasal transitional carcinoma in Korea.

Two Cases of the Inverted Papilloma of the Nose and Sinuses (비강 및 부비동의 반전성유두종 2례)

  • Inverted papilloma of the nose and paranasal sinuses has been classified as a true neoplasm by most authors. Although these tumors are histologically benign, they tend to be act clinically in a malignant fashion. They are certainly characterized by the high recurrence after inadequate removal. The authors report 2 cases of the inverted papilloma which had been diagnosed as a nasal polyp for which an intranasal operation was performed. The polyp recurred and the total mass was removed at our E.N.T. department and proved to be an inverted papillcma by histologic examination.

Secondary Intracranial Tumor Associated with Nasal Adenocarcinoma in a Dog: Clinical, Computed Tomography, Magnetic Resonance Imaging, Histopathologic Findings (개에서 발생한 비강샘암종으로부터 유래한 이차성 두개 내 종양 증례)

  • A 12-year-old castrated male Miniature Schnauzer dog was presented with an acute seizure. On computed tomography, a mass was observed in the left nasal cavity and the lysis of the left cribriform plate was identified. Post-contrast magnetic resonance imaging showed a hyperintense mass in the left frontal lobe and hyperintense lesions in the left frontal sinus. The mass was tentatively diagnosed with a brain tumor secondary to a nasal tumor. Histopathology revealed that the mass was a nasal adenocarcinoma with invasion into the brain. This case report describes clinical, computed tomography, magnetic resonance imaging and histopathological findings of secondary intracranial tumor caused by extension of nasal adenocarcinoma in a dog. These findings may contribute to enhance the knowledge on secondary intracranial tumors in dogs.
