AN ACOUSTIC ANALYSIS OF PRONUNCIATION IN CHILDREN WITH ANGLE'S CLASS II DIV. 1 MALOCCLUSION (Angle씨 II급 1류 부정교합아동의 발음에 관한 음향학적 연구)

  • The human speech organ consists of respiration system (lung, larynx), phonation system (vocal cord), articulation system (esophagus, pharynx, uvula, teeth, gingiva, palate, tongue, lip) and resonating system(oral cavity, nasal cavity, paranasal sinus). Because teeth are components of the articulation system, it has been reported that the persons with abnormally positioned teeth generally have abnormal occlusion and pronunciation. In this study, using /ㅅ(s)/, the most commonly mispronunced consonant in children with malocclusion, and the seven single vowels, /사(sa), 서($s\delta$), 소(so), 수(su), 스($s\omega$), 시(si), 세(se)/ and / ㅏ(a), ㅓ($\delta$), ㅗ(o), ㅜ(u), ㅡ($\omega$), 1(i), ㅔ(e)/ were recorded and analyzed using speech analysis program on computer by measuring formants and compared them for investigating the differences in pronunciation in children with Angle's class I occlusions and those with Angle's class II div.1 malocclusion. The result were as follows: 1. In the Angle's Class II div.1 group, there were no significant differences in F1 of all recorded sounds as compared with Angle's Class I group(p>0.05). 2. In the consonants, there were significant differences in F2 of /스($s\omega$)/ and F2/F1 ratio of /사(sa), 서($s\delta$), 시(si)/ between the two group(p<0.05). 3. In the vowels, there were significant differences F2/F1 ratio of /ㅓ($\delta$)/(p<0.05) and no significant differences in F2/F1 ratio between two group(p>0.05). 4. In the consonants, there were significant differences in F2 and F2/F1 ratio when succeeding vowels were high or low, and F2/F1 ratio when front in accordance with tongue position (p<0.05). 5. In the vowels, there were no significant differences in formant in accordance with tongue position(p>0.05)

A cephalometric study on the velopharyngeal changes after maxillary protraction (상악골 전방견인치료후 구개범인두 변화에 대한 단기간의 측모두부방사선 계측학적 연구)

  • The purpose of this study was to investigate cephalometrically the short term static velopharyngeal changes in 25 patients (10 boys and 15 girls, aged from 5 years 9 months to 12 years 10 months in the beginning of treatment) with skeletal Class III malocclusions who underwent nonsurgical maxillary protraction therapy with a facemask. The linear, angular and ratio measurements were made on lateral cephalograms. Only the change in hard palatal plane angle was negatively correlated with the change in maxillary depth or N-perp to A (p<0.01). The change in velar angle showed a statistically significant increase (p<0.001). This change was influenced more by the soft palatal plane angle than by the hard palatal plane angle (p<0.001). The changes in soft tissue nasopharyngeal depth and hard tissue nasopharyngeal depth showed statistically significant increases (p<0.001). Correlations between the changes in soft tissue (or hard tissue) nasopharyngeal depth and the change in soft palatal plane angle were significant (p<0.05). The increase in hard palate length was statistically significant (p<0.001). The change in hard palate length was negatively correlated with the change in soft tissue nasopharyngeal depth (p<0.05). The change in need ratio S (C) showed a statistically significant increase (p<0.001). But this difference was within the normal range reported by previous studies. These findings indicate that the velopharyngeal competence was maintained even if the anatomical condition of the static velopharyngeal area were changed after maxillary protraction.

  • 최근 몇 년간 유비쿼터스 서비스를 구현하기 위한 핵심 기술 중 하나인 무선 센서 네트워크에 대한 관심이 높아지고 있다. 무선 센서 네트워크 기술은 물류, 유통, 환경 감시, 홈오토메이션, 군사 분야 등 다양한 분야에 적용 될 수 있기 때문에 앞으로 관련 분야의 시장 또한 커질 것으로 예상되고 있다 [1]. 무선 센서 네트워크는 기존 유선 센서 네트워크나 무선 네트워크 환경과는 많은 차이를 가진다. 우선 극도로 제한된 시스템 자원만을 가질 수 있으며, 열악한 환경 속에서 무선매체를 통해 유기적으로 동작하여야 하는 특징을 가지고 있다. 적게는 수십 개에서 많게는 수백, 수천 개의 자율적인 하드웨어 노드들로 구성되는 무선 센서 네트워크에서 제한된 자원을 효과적으로 활용하기 위해서는 센서 노드에 적합한 운영체제가 필수적으로 요구된다. 지난 몇 년간 센서 노드하드웨어의 발달과 더불어 많은 센서 네트워크용 초소형 운영체제가 개발되어왔다. 현재 많이 알려져 있는 센서 네트워크용 OS로는, 가장 활발한 참여를 보이고 있는 TinyOS[3]부터 SOS[4], MANTIS[5], Contiki[6], T-kernel[7] 등이 있으며, 국내 기술로 개발된 Nano-Qplus[8] 등이 존재한다. 본고에서는 우선 센서 네트워크에 대한 배경 지식과 플랫폼 등에 대한 내용을 간단히 다루고, 본론에서 센서 네트워크용 운영체제가 가져야 할 조건과 현재 개발되어 있는 센서 네트워크 OS들의 특징에 대해 간략히 살펴보도록 하겠다. 또한 센서 네트워크 OS와 밀접한 연관성을 가지는 분야에 대해 간단히 살펴보고, 마지막으로 앞으로의 방향에 대해 알아본다.고려해 볼 때 atlas의 장축의 시계방향 회전은 액티베이터의 사용 효과로 생각되며, 이는 차후 II급 부정교합자에서 액티베이터 치료 효과를 판단하는 또 하나의 지표가 될 수 있다고 생각한다.인해 한반도를 포함한 동아시아 대륙이 태평양판 쪽으로 밀려감으로써 섭입하던 태평양판의 각도가 급해져 동아시아 연변에 강력한 흡입력이 발생하였으며, 이 때문에 태평양판의 운동 방향이 북북서에서 서북서방향으로 회전되었을 가능성이 있다. 따라서 약 51 Ma부터 한반도 동남부에는 지판 경계의 강력한 흡입력으로 동서 내지 서북서-동남동의 인장력이 작동되어 B그룹 암맥군이 관입한 것으로 해석된다.Ledge는 세 군 모두에서 나타나지 않았다. 4. 파일 binding 횟수는 MC군이 가장 적고 PT군이 가장 많았다 (p < 0.05). 이상의 결과를 볼 때, Mtwo 전동 파일을 crown-down technique으로 사용하는 것이 single length technique과 유사한 성형 효율을 보이면서도 더 안전할 것으로 추정된다.고 1명(3%)에서 원격전이를 보였다. 치료 중 급성 합병증으로 11명(37%)에서 RTOG grade 1-2의 장염을 보였으며 1명은 대장의 천공이 발생하여 수술로 치유되었다. 12명(40%)에서 RTOG grade 1-2의 급성 방광염을 보였다. 3명(10%)에서 RTOG grade 1-2의 백혈구 감소증이 보였으며 1명에서 심한 백혈구 감소증(RTOG grade 4)이 나타났으나 회복되어 치료를 완료하였다. 만성 합병증으로 5명(15%)에서 RTOG grade 1-2의 만성 장염을 보였으며 별다른 치료 없이 지내고 있으며

THE CEPHALOMETRIC ANALYSIS OF THE SOFT TISSUE CHANGES AT THE MIDDLE FACE IN THE SKELETAL CLASS III PATIENTS WITH THE ORTHOGNATHIC 2-JAW SURGERY (골격성 제3급 부정교합자의 양악 수술 후 중안면부 연조직 변화에 관한 두부 계측 방사선학적 연구)

  • Considering the skeletal class III malocclusion that complains of mandibular prognathism, there have been some studies of the mandibular change for comparing the changes of pre operative with post operative state. Nowadays it is common to do the orthognathic 2-jaw surgery for the correction of the maxillary deficiency, the post operative stability and the esthetics. We compare and analyze the changes of soft tissue around the nose and the lip with the changes in the direction and the amount of maxilla. Patients who were diagnosed as maxillofacial deformity and received orthognathic surgery of both jaws at Yongdong Severance hospital from 2001 through 2003 were included in this study. Their lateral cephalograms were analyzed, and the post operative change of hard tissue and soft tissue were studied. Upon analyzing the preoperative cephalograms and 6 month post operative cephalograms, there were significant in the vertical change of Labialis superius(Ls) and Stomion(Stm) in soft tissue in relation to the vertical change of skeletal landmarks (Anterior Nasal Spine, Subspinale, Prosthion, Incision Superious). In addition, there were no significance in horizontal movement of the skeletal landmarks among groups. In terms of hard tissue landmarks, group 3(maxillary posterior impaction and advancement surgery group) showed significantly greater change in the vertical movement of Anterior Nasal Spine(ANS), Subspinale(A), Prosthion(Pr), and Incision Superious(Is) compared with other groups. In terms of soft tissue change, group 3 showed more significant change in the vertical movement of Ls and Stm. This study calculated the changes of the skeletal and soft tissue landmarks in order to act as a guide in planning and performing the surgery and as a reference in predicting the postoperative change of facial appearance.

DISPLACEMENT OF MAXILLARY LATERAL INCISOR CAUSED BY IDIOPATHIC GINGIVAL FIBROMATOSIS (특발성 치은 섬유종증에 의한 상악 측절치의 변위)

  • Idiopathic gingival fibromatosisrarely occurs, but frequently recurred after surgical removal. It usually occurs in generalized symmetrical pattern but sometimes in localized unilateral pattern. The localized pattern usually affects the maxillary molar and tuberosity area. This disease usually causes tooth migration, malocclusion, and problems in eating, speech, and esthetics. A boy showed dense gingival fibromatosis localized at primary maxillary right lateral incisor area at the age of 5 years, and his maxillary right lateral incisor become severely displaced at the age of 9 years. He had no medical and hereditary factors relevant to the gingival fibromatosis. However, the dense fibrous tissue was dominant in his labial gingiva of maxillary right incisors. In order to realign the displaced incisors by orthodontic treatment, the dense fibrous tissue covered the defect space between the central incisor and the displaced lateral incisor was surgically removed. The removed specimen was examined by simple immunohistochemical(IHC) array method. IHC array showed increased expression of CTGF, HSP-70, MMP-1, PCNA, CMG2, and TNF-${\alpha}$ in keratinocytes, fibroblasts, endothelial cells, and macrophages of gingival fibromatosis tissue. Therefore, it was suggested that the gingival fibromatosis be caused by the concomitant overexpression of CTGF, HSP-70, MMP-1, PCNA, CMG2, and TNF-${\alpha}$, and resulted in the fibroepithelial proliferation and the inflammatory reaction of gingival tissue.

PHYSIOLOGIC DRIFT OF THE MANDIBULAR DENTITION FOLLOWING THE EXTRACTION OF FOUR FIRST PREMOLARS (상, 하악 제1소구치 발치 후 하악치열의 생리적 치아이동에 관한 연구)

  • Retrostpective study of two groups of patients was conducted to evaluate the physiologic drift of the mandibular teeth following the extraction of four first premolars. The concept of physiologic drift, commonly referred to as 'driftodontics', following first premolar extractions has been gaining acceptance in the orthodontic community, the exact nature and amount of drift has not been adequately documented. There were also no guide lines as to when drift should be allowed to now. The purpose of this study was to quantify physiologic drift of the untreated mandibular dentition following extraction of the four first premolars during the early permanent and late permanent dentition stages. The early permanent dentition extraction sample(Group 1) included 26 Patients and the mean age at pretreatment was approximately 13.5 years. The observation period following extraction was approximately 6.96 months. The late permanent dentition extraction sample(Group 2) included 31 patients. The mean age at pretreatment was 21.3 years, followed by a observation period of 7.26 months. During the observation period, except for the extractions, no other mandibular therapy was rendered. Pre-and post-treatment lateral cephalograms and dental casts were analyzed. The obtained results were as follows 1. Group 2 showed marked changes in movements of the mandibular incisors and canines but minimal changes in molars. 2. The amount of changes in movements of the mandibular incisors and canines were significantly greater in Group 1 than in Group 2. The results showed no differences in rates of molar movements between groups. 3. Physiologic drift of the dentition produced desirable changes such as decreased Incisor Irregularity.

A RADIOGRAPHIC STUDY ON THE MANDIBULAR CONDYLE POSITION IN KOREAN MALOCCLUSION (부정교합자의 하악과두 위치에 관한 방사선적 연구)

  • Temporomandibular joint is a major structure to play an important role in the function & stability of the occlusion as well as the stomatognathic system. Therefore, the TMJ is the structure that requires the complete analysis for diagnosing and planning treatment of pathologic changes by TMJ dysfunction and malocclusion. So, in this study, to evaluate TMJ situation in Korean malocclusion, based on the previous accomplishments, students of the dental college of Won-Kwang Univ. are surveyed and selected in terms of Angle's classification of malocclusion, whose TMJ radiographs were taken in the centric occlusion and centric relation. In each maiocclusion groups, the mean and standard deviation of anterior, posterior and superior joint space of the right, left and both side in CO & CR are evaluated and also those of the fossa height and the articular eminence angle of the right, left and both sides are evaluated. The obtained results were as follows: 1. In the correlation coefficient between the malocclusion groups, no other items except the posterior joint space of the right side in CR between in class I and class III are significant. 2. In the correlation coefficient between the right and left side, the each joint space in class I malocclusion group and class II malocclusion group are significant. 3. In the change of each joint space during the transmit from CO to CR, there is a tendency of increasing anterior joint space and decreasing posterior, superior joint spaces in class I, II malocclusion and increasing superior joint space and decreasing anterior, posterior joint space in class III malocclusion, which is significant in the correlation coefficient, but not significant in the T-test. 4. In each malocclusion group, the correlation coefficient between the posterior joint space and the superior joint space in C.R is highly significant. 5. The fossa height of class II malocclusion group is lesser than that of class I or class III, which is not significant in T-test. 6. In the correlation coefficient between Rt. and Lt. side in the fossa height, it is not significant in class I and class III group, but significant in class II malocclusion group. 7. The articular eminence angle of class II malocclusion group is larger then that of class I or class III groups, which is fairly significant. 8. In the correlation coefficient between Rt. and Lt. side in the articular eminence angle, it is significant in each malocclusion group.

THE CHANGES OF MANDIBULAR MOVEMENT AND MUSCLE ACTIVITY FOLLOWING ORTHOGNATHIC SURGERY IN PATIENTS WITH MANDIBULAR PROGNATHISM (골격성 III급 부정 교합을 동반한 악교정수술 환자에서 수술 전후의 하악기능 변화에 관한 실험적 연구)

  • The purpose of this study was to investigate the changes of mandibular movement and muscle activity following orthognathic surgery in patients with mandibular prognathism. Lateral cephalogram, M.K.G., E.M.G. recordings were obtained immediately before surgery, and 2 months and 8 months after surgery. Among the patients who received orthognathic surgery, 19(13 men, 6 women) were selected for this study. Statistical analysis for each time interval differences were performed with the SPSS package. The results were as follows: 1. Compared with the pre-operative group (opening 349.7mm/sec, closing 313.1mm/sec), the mean values of the maximum opening and closing velocity in the skeletal Class III surgery group were significantly decreased in the 2 months post-operative (opening 232.9mm/sec, closing 206.9mm/sec), but the values tended to increase in the 8 months post-operative group (opening 280.9 mm/sec, closing 319.1mm/sec). 2. Compared with the pre-operative group (61.7 mm/sec), the maximum velocity of the terminal tooth contact increased in the 2 months (72mm/sec) and 8 months (105.7mm/sec) postoperative groups. 3. In the mean value of vertical freeway space, there was significant difference between the normal group and the pre-operative group, but not between the normal group and the 8 months post-operative group. 4. In the mean values of the maximum opening, the maximum anterio-posterior movement from centric-occlusion, and the lateral deviation from centric occlusion, there was no significant difference between the normal group and the surgery group (the pre-operative and the 8 months post-operative groups). 5. The mature swallowing pattern was 58% in the pre-operative group, but 90% in the 2 months post-operative group, and 63% in the 8 months post-operative group. 6. In the comparison of muscle activity, there was no significant difference between the normal group and the surgery group during the rest position. However, during cotton roll clenching, there was significant difference between the normal group and the pre-operative group, but not between the normal group and the 8 months post-operative group.

TRIPLE TOOTH IN MAXILLARY PRIMARY INCISORS : CASE REPORT (상악 유전치부에 발생한 삼중치 : 증례보고)

  • The term 'triple tooth' is used to describe a rare dental abnormality in which three teeth appear to be joined. The literature contains many reports of joined primary teeth; most cases have involved, however, the joining of two teeth, and only rarely three teeth. Triple tooth has clinical problems such as dental caries, esthetic problems, malocclusions, and periodontal problems. Therefore, it may require multi-disciplinary approach. The present study describes rare case of triple tooth between maxillary primary central and lateral incisors and a supernumerary tooth. An one-year, eleven-month old girl was seen for evaluation of swelling in the labial gingiva above a right maxillary triple tooth. She experienced traumatic dental injury in that area three weeks ago. Intraoral examination revealed an abscess and fistula in the region of the triple tooth. A radiographic examination showed that right lateral incisor was missing. Endodontic treatment and composite resin restoration was performed on the triple tooth. After follow-ups of 7 months period, there were no marked complications.

APERT SYNDROME : A CASE REPORT (Apert syndrome 환자의 제증상에 관한 증례보고)

  • Apert syndrome is a kind of congenital-acrocephalosyndactyly syndrome which was first reported by Apert in 1906 and characterized by its acrocephaly and syndactyly. Clinical characteristic features are cone-shaped skull morphology due to early fusion of coronal suture, fusion of fingers of hands and toes of feet. It is an autosomal dominant-heritable syndrome. Due to hypo-development of midface region, Apert syndrome patients have a tendency to have ocular proptosis, hypertelorism, maxillary deficiency. High palate and soft palate cleft are common findings in these patients. In general, mandibular growth pattern is normal, but relative maxillary deficiency exaggerates mandibular forward position, so relative mandibular prognathism is inevitable. Narrow maxillary and mandibular dental arch worsen teeth alignment and crowding. Skeletal malocclusion and open bite are also common. This is a case report of a Korean 3 year 1 month male Apert syndrome child referred by department of plastic surgeon for the possibility of orthodontic treatment. General features of Apert syndrome, patient's medical history, radiographic evaluation, clinical examination, orthodontic and surgical treatment planning are discussed in this report.
