The purpose of this study is to define the characteristics of the skeleton and soft tissues of severe adult class III malocclusion. The materials selected for this study were lateral cephalograms of 112 adult class III malocclusion patients with ANB difference below -2 degrees. and the mean age was 22.9 years old. The normal control sampler consisted of lateral cephalograms of 50 adults in normal occlusion and the mean age was 22.1 years old. The Horizontal reference line was FH line and the vertical reference line was nasion perpendicular to FH line. The skeletal and soft tissue characteristics of Class III malocclusion are as follows : 1. In the skeletal profile evaluated by vertical reference line (Nasion perpendicular to FH), the forehead and maxilla was similar to normal, but the mandible was protruded significantly. 2. The soft tissue profile is concave. The thickness of soft tissue covering forehead area and nose is within normal range. but the upper lip is thicker and the nasolabial angle is smaller than normal. The lower lip and inferior labial sulcus is thinner than normal. The degree of eversion of lower lip is lesser than normal. 3. The cranial base of class III malocclusion is shorter and saddle angle is smaller than normal. 4. The location of midface evaluated in relations to cranial base is within normal range but, the length of midface is shorter than normal when compared from the deep portion of the facial skeleton. 5. The location of maxilla in reference to cranial base is within normal range but the length of maxilla was shorter in class III malocclusion. 6. The mandible was protruded, ramus height and body length, gonial angle were greater than normal, and the chin angle was smaller. 7. Upper incisor was proclined, lower incisor was retroclined.
Three Maltese dogs, 5 to 7 months old, were admitted to the Veterinary Medical Teaching Hospital, Chonnam National University with malocclusion including Class IV mesiodistocclusion. In the first case, the treatment was performed by moving the mandibular canine teeth caudally with orthodontic buttons and Masel chains. The second patient was treated for rostroverted mandibular canines using buttons and chains. When distal movement of the mandibular canine teeth was completed, a maxillary arch wire with finger springs was applied to push the incisor teeth forward. In the third case, the treatment began by moving the mandibular canine teeth caudally with buttons and chains. An arch wire with finger springs was applied at maxilla to move the maxillary incisor teeth labially. Additionally, the mandibular incisor teeth were moved lingually by an elastic band attached to the buttons cemented to mandibular canine teeth. As a result, all patients successfully regained a normal occlusion.
Treatment of adult patients with Class III malocclusion frequently requires a combined orthodontic and surgical approach. However, if for various reasons, nonsurgical orthodontic treatment is chosen, a stable outcome requires careful consideration of the patient's biologic limitation. This case presents the orthodontic treatment of an adult with a Class III malocclusion, which was treated nonsurgically using indirect skeletal anchorage.

Anterior open bite is one in which the teeth in the anterior portion of the maxilla and mandible are vertically apart and lack the overlapping necessary for the incisive function when the mandible is in closed position. Anterior open bite is a result of the interaction of many different etiologic factors including thumb and finger sucking, lip and tongue habits, airway obstruction, skeletal growth abnormalities and its tendency may appear with any type of skeletal patterns, such as Class I, II or III malocclusion types. Though the treatment methods for anterior open bite are various, the conventional FR-4, designed by Rolf Fr$\"{a}$nkel, is known to be effective in treating open bite cases with Class I or II skeletal patterns. It is due to that an incidence of skeletal Class II is high in the Occidentals, and open bite is accompanied by these malocclusion type in many cases. However, an incidence of skeletal Class III is high in the Orientals, and open bite is sometimes accompanied by skeletal Class III in many cases. Although the use of the conventional FR-4 was effective in the treatment of open bite, skeletal Class III would be worsened. So, a modified FR-4(placing the labial bow in the lower, the labial pads in the upper) was designed for the treatment of patients showing skeletal Class III and open bite.
In proper diagnosis of skeletal Class III malocclusion, it was important to know the pattern of three dimensional skeletal & facial disharmony. The purpose of this study was to obtain P-A cephalometric characteristics in skeletal Class III malocclusion comparing with normal occlusion. The samples were consisted of 120 subjects, divided into four groups : Male normal occlusion, Female normal occlusion, Male skeletal Class III malocclusion, Female skeletal Class III malocclusion. Posteroanterior and lateral cephalogram were taken from the subjects with a x-ray apparatus (ASHAI CX90SP, Japan) and traced on acetate paper with routine manner. The transverse and vertical values from posteroanterior cephalometry, the sagittal values from lateral cephalometry and their ratio were obtained. The results were as follows: 1. The anteroposterior discrepancy in skeletal Class III group was not due to short maxillary length(Cd-A), but to longer mandibular length(Cd-Gn) than normal occlusion group. 2. The faces of skeletal Class III group were longer than normal occlusion group. It was not due to increase of upper face height(Cg-ANS) but to increase of the lower face height(ANS-Me) especially mandibular height(Cd-Me). 3. There was no difference in the facial width values between normal occlusion group and skeletal Class III group, except upper molar width(U6-U6), lower molar width(L6-L6) and mandibular width(Ag-Ag) of female skeletal Class III group which were larger than normal occlusion group. 4. The increase of mandibular length of skeletal Class III group was reflected in the increase of lower facial height but did not have an effect on the mandibular width.
Multiple features of occlusion have been combined by epidemiologists in attempts to objectively to determine malocclusion severity and treatment priority. Jenny et al were developed DAI(Dental Aesthetic Index), which is an orthodontic index based on socially defined aesthetic standards. This study aimed to evaluate association between 10 occlusal components of DAI and individual measures of skeletal morphology by use of univariate and multivariate analysis. In addition the relationship between a Dental Aesthetic Index score and skeletal morphology was evaluated. The data for this study were obtained from cephalometric radiographs and dental casts of 182 Korean patients(79 men, 103 women) with permanent dentition who had been accepted for the orthodontic treatment at Department of Orthodontics, Chonbuk National University. The results are as follows: 1. Several occlusal components of DAI(missing, crowding and spacing in the incisal segment, diastema, largest anterior irregularity on upper and lower arch) were not associated with individual measures of craniofacial morphology. 2. Sagittal occlusal components of DAI were associated with anteroposterior skeletal measures. 3. The relation between vertical occlusal components of DAI and vertical skeletal measures was low magnitude. 4. A Dental Aesthetic Index, which is based on several aspects of occlusion, did not identify craniofacial morphology. We can conclude that combination of certain occlusal components of the DAI may be related with specific skeletal morphology; but, this concept could not be accepted generally.

The habit of finger sucking is a reflex occurring in the oral stage, due to nutritive and psychological desire. The habit of finger sucking is considered to be normal till 3 years of age. Dento-skeletal effect on maxillo-mandibular complex including occlusion is naturally correction, when habit stopped before 3 years. If finger sucking continues till $3{\sim}4$ years, Finger sucking leads to severe malocclusion and remarkable discrepancy maxillo-mandibular complex, which is difficult in expectation of natural correction. It is necessary to positive treatment. Treatment of malocclusion, as related to finger sucking is classified two methods. (psychological approach and orthodontic appliance) To stop a habit and to correct severe skeletal discrepancy and malocclusion, $fr\ddot{a}nkel$ appliance is very effective device. This study is to report two cases of treatment of malocclusion, as related to finger sucking. 2 years 10 months old girl with severe overjet, maxillo-mandibular skeletal discrepancy and severe convex facial profile was treated with a FR-II appliance. Finger sucking habit stopped immediately After 16 months, severe overjet, maxillo-mandibular skeletal discrepancy and severe convex facial profile was corrected. 4 years 2 months old girl with midline deviation, mandibular right shift, collateral posterior crossbite and facial asymmetry was treated with a FR-III appliance. Finger sucking habit stopped immediately. After 10 month, Midline deviation, mandibular right shift, collateral posterior crossbite and facial asymmetry were corrected. FR-appliance is a recommendable appliance for a habit breaker and correction of skeletal discrepancy.
In non-growing Class III malocclusion, the critical aspects which determine the need of orthognatic surgery are the severity of skeletal discrepancy, incisor inclination, overbile and soft tissue profile. Two-jaw surgery is more effective in correcting severe sagittal, vertical, transverse skeletal discrepancies and facial asymmetry. And more esthetic and stable profile can be achieved by two-jaw surgery Some midfacial deficiency Patients can be treated by Pyramidal Le Fort II osteotomy to maintain infraorbital rim and malar complex and to advance nasomaxillary complex. Others who require advancement of infraorbital rim and malar complex can be treated by quadrangular Le Fort II osteotomy. On the following cases, patients who had represented midfacial deficiency and mandibular prognathism were treated with combined orthodontic-surgical therapy by Le Fort II osteotomy and BSSRO.
Orthognathic surgery of skeletal Class III malocclusion improves oral function and facial appearance. The greater amount of skeletal discrepancy, the greater amount of teeth movement required for decompensation, and this often causes pathological changes in periodontal tissue especially in lower anterior dentition. We made a Top-Down treatment plan with personalized analysis using Face Hunter, Plane System and ARCUS Digma II, in order to resolve severe mobility and cross-bite of lower anterior teeth for 49-year-old female patient who had undergone orthognathic surgery 20 years ago due to skeletal Class III malocclusion and mandibular prognathism. Lower anterior teeth were extracted and alveoloplasty was done. After healing of the wound, immediate loading was conducted immediately after implant placement. Final restorations were fabricated Zirconia using CAD/CAM, and inserted intraorally screw-retained type. During 6-month follow-up, no abnormal episodes of restorations were observed, and obtained satisfactorily both of functional and esthetic outcomes.
