A Review of the Domestic Study Trends on Adenomyosis Treated with Korean Medicine (자궁샘근증의 한의학적 치료에 대한 국내 연구 동향 고찰)

  • Objectives: The purpose of this study is to examines the domestic study trends on adenomyosis treated with Korean medicine. Methods: We investigated the studies on Korean medicine treatment for adenomyosis via searching 8 online databases. Results: As a result, 12 records (14 cases) were found to be analyzed according to their modes of diagnosis, symptoms, interventions, treatment period, follow up period and relevant outcomes. In diagnosis of adenomyosis, the most used method was ultrasonography. The most common complaint for patients with adenomyosis was dysmenorrhea. The most applied intervention for adenomyosis was herbal medication. Acupuncture was the second most used. Conclusions: Analyzing the domestic study trends would be helpful to establish a basic knowledge of the future study. It is anticipated that this review benefits the future in-depth study to confirm the efficacy of Korean medicine treatments for adenomyosis.

Development of a Guideline for the Application of a Diagnostic Tool for Menopausal Syndromes Based on the Use of Systemic Review and Delphi Method (체계적 문헌고찰과 델파이 기법을 활용한 갱년기장애 변증(辨證)진단 도구 개발을 위한 기초 연구)

  • Objectives: This study was conducted towards developing a screening tool for syndrome differentiation in the diagnosis of menopause in menopausal and perimenopausal women. Methods: We conducted a literature review of studies on menopausal diagnosis based on syndrome differentiation, and examined well-founded differentiated syndromes and their respective clinical symptoms. Based on the findings, we created a questionnaire through consultations with Oriental medicine experts in physiology, pathology, and diagnostics. Finally, the research team conducted an expert Delphi study on differentiated syndromes and the associated clinical symptoms. Results: Seven differentiated syndromes were selected, including Liver Depression (肝鬱), Kidney Yin Deficiency (腎陰虛), Kidney Yang Deficiency (腎陽虛), Liver and Kidney Yin Deficiency (肝腎陰虛), Kidney Yin and Yang Deficiency (腎陰陽兩虛), Heart-Kidney Noninteraction (心腎不交), and Dual Deficiency of Heart and Spleen (心脾兩虛); 4 disease locations, including liver (肝), heart (心), spleen (脾), and kidney (腎); and 3 disease natures, including Yin Deficiency (陰虛), Qi Stagnation (氣滯), and Blood Deficiency (血虛). In addition, we added 3 supplemental disease natures, including Yang Deficiency (陽虛), Qi Deficiency (氣虛), and Heat (火熱), in consideration of syndrome differentiation categories that may possibly be added in a follow-up clinical questionnaire. Conclusions: This resulted in a total of 7 differentiated syndromes, 4 disease locations, and 6 disease natures. We translated the clinical symptoms of these 17 categories into Korean Hangeul. After consulting with 5 Oriental medicine experts and a psychology expert, we produced a questionnaire for use in diagnosing menopause based on syndrome differentiation. The calculation of scores for the syndrome differentiation screening tool will be confirmed through clinical research based on the results of a review of existing literature.

Interventions of Korean Medicine for Chronic Pelvic Pain: a Literature Review of Randomized Controlled Trials (만성 골반통의 한의약 치료 : 무작위 대조군 연구에 대한 문헌 고찰)

  • Objectives: This study aimed to review randomized controlled trials (RCTs) on the effects of interventions of Korean medicine for chronic pelvic pain (CPP). Methods: We searched eleven electronic databases from inception up to Apr 2018. RCTs evaluating the effects of Korean medicine interventions for CPP were retrieved. Results: 11 RCTs were included and total number of experimental group was 390 cases. A total of 4 types of interventions were used, of which acupuncture (45.5%), electro-acupuncture (18.2%), electro-acupuncture + auricular acupuncture + moxibustion (18.2%) were the most frequently utilized. 關元 (CV4) (100.0%), 會陰 (CV1) (80.0%), 三陰交 (SP6) (80.0%), 陰陵泉 (SP9) (80.0%) were most frequently used acupoint in acupuncture treatment. Conclusions: For evidence-based treatment of korean medicine intervention for CPP, high quality RCTs must be conducted.

Clinical Comparative Report of Separation of Symphysis Pubis and Symptom-Giving Pelvic Girdle Relaxation after Delivery (자연분만 후 발생한 치골결합분리 1례와 증상유발 골반이완증 1례 임상 비교 보고)

  • Objectives: This study is designed to compare clinical symptoms between Symptom-giving pelvic girdle relaxation and Separation of the pubic symphysis. And report the clinical effectiveness of Traditional Korean Treatment on two case. Methods: Two patients who delivery in one week ago had pain that located symphysis and around. Treatment was acupuncture, Placenta Herbal Acupuncture and herbal medicine. The progress of symptom and aggressiveness of pain and disability were measured by Visual Analogue Scale, McGill pain Questionnaire-short form, Oswestry Disability Index and Active Straight Leg Raise test. Results: After treatment most two patients' pain and disabilty were improved. At first case, width of symphysis pubis were increased in normal range. Conclusions: These cases shows that Traditional Korean therapy might be effective in decreasing symptoms on separation of symphysis pubis during delivery and Symptom-giving pelvic girdle relaxation.

A Study of Improvement on Low Back Pain Caused by Lumbar Disc Herniation Depending on Female Inpatient's Bone Mineral Density(BMD), Body Mass Index(BMI), Age, Pain Duration (여성의 골밀도가 요추 추간판 탈출증으로 인한 요통 호전도에 미치는 영향 및 골밀도와 비만도, 연령, 병력 기간의 관계에 관한 연구)

  • Objectives: This study explored the relationships between Bone Mineral Density (BMD), Body Mass Index(BMI), Age, pain duration outcomes of treatment for Low Back Pain(LBP) in 3 groups(normal,osteopenia,osteoporosis) of female patients. Methods: We evaluated 32 cases of normal, 30 cases of osteopenia and 38 cases of osteoporosis based on BMD, BMI, Age, pain duration among the female inpatients with LBP caused by lumbar disc herniation, admitted at Jaseng Hospital of Oriental Medicine from January 2010 to December 2011. And we measured VNRS(Verbal Numeric Rating Scale) to evaluate the conservative treatment effects. Results: In improvement of female LBP caused by lumbar disc herniation, BMD(T-score), BMI had not proved correlation(P-value>0.05). Female patients' age was negatively correlated with T-score. In pain improvement, BMD normal group showed more reduction of NRS(numeric rating scale) than osteoporosis group, but that had not proved correlation(p-value=0.893). Conclusions: Statically, among the female inpatients with LBP caused by lumbar disc herniation, had no significant differences between the normal group, osteopenia group and osteoporosis group's improvement rate, depending on treating period.

A Study on the Impacts of Women with Age-related Obesity on Skin Resistance Variability(SRV) (여성의 연령에 따른 비만도가 피부저항변이도에 미치는 영향)

  • Purpose: Obesity affects dysfunction of Hypothalamus-Pituitary-Ovary(H-P-O) axis. Dysfunction of H-P-O axis could be related to Skin Resistance Variability (SRV). This study investigated the impacts of obesity which related to female age on SRV. Methods: We randomly selected 400 women who had taken CP-6000A test and InBody test on the same day at Oriental Hospital of Sangji University from Jan. 2008 to Jun. 2011. In this study, the Body Mass Index(BMI) range of normal group is 18.5~24.9, obesity group is over 25, the age range of childbearing age group is 24~34 years, post-menopause group is 50~65 years. 139 women that met the criteria of this study were assorted by their BMI and age. After detection of SRV, SPSS 19.0 has conjugated for data analysis and the independent samples t-test and paired samples t-test were used to verify the results. Results: At 1st and 2nd trial, the height of 3 area of childbearing age obesity group was significantly higher than that of childbearing age normal group. At 1st and 2nd trial, the height of 1, 2 area of childbearing age obesity group was higher than that of childbearing age normal group, but there were no significant difference. At 1st and 2nd trial, the height of 1, 2, 3 area of post-menopause obesity group was mostly higher than that of post-menopause normal group, but there were no significant difference. Conclusions: This study showed the SRV of obesity group was higher than that of normal group in most of 1, 2, 3 area. Further study will be needed.

Correlations of Deficiency and Excess Patterns between Menstrual Symptoms and Whole Body Symptoms (월경통(月經痛) 증후(證候)와 전신 증후(全身 證候)의 허실(虛實) 상호 관련성 연구)

  • Purpose: Dysmenorrhea mostly depends on the causative factor, which usually falls under the categories of Deficiency and Excess pattern in traditional Oriental medical theories and diagnosis. Thus, we investigated menstruation symptom and sign related to dysmenorrhea and verified the validity of Deficiency and Excess pattern identification. Methods: We investigated menstruation symptom and sign related to dysmenorrhea in total 14 gynecology-medical books including the book ${\ll}$Exemplar Of Korean Medicine (Dongui Bogam)${\gg}$ and whole body symptom and sign identifying Deficiency and Excess pattern at the same time. A survey based on this investigation was carried out targeting women of childbearing age. Results: Total of 14 gynecology-medical books have mostly narrated pre-menstrual and mid & post-menstrual pelvic pain depending on the time of its manifestation for identifying Deficiency and Excess pattern. Dysmenorrhea in pre-menstrual period belonged to Excess pattern and dysmenorrhea in mid & post-menstrual period belonged to Deficiency pattern. Among a total of 343 women, 196 subjects suffered from dysmenorrhea. The number of dysmenorrhea in pre-menstrual period (Excess pattern) was 116 people and in mid & post-menstrual period (Deficiency pattern) was 80 people. Deficiency and Excess pattern of dysmenorrhea in menstrual period significantly correlated to Deficiency and Excess pattern of whole body symptom and sign in the statistics(P-value < 0.05). Conclusion: The results suggest that pre-menstrual and mid & post-menstrual pelvic pain depending on the time of its manifestation is preferentially utilized as symptom and sign related to dysmenorrhea identifying Deficiency and Excess pattern.

In Vitro Antibacterial Effects of Wandae-tang Aqueous Extracts and Their Combination Effects with Clindamycin against Gardnerella Vaginalis (완대탕(完帶湯)의 Gardnerella vaginalis에 대한 시험관내 항균력 및 Clindamycin과 병용효과)

  • Objectives: The object of this study was to observe the in vitro antibacterial effects of Wandae-tang extracts and combination of Wandae-tang extracts and Clindamycin against Gardnerella vaginalis ATCC14018. Methods: Antibacterial activities against Gardnerella vaginalis ATCC14018 of Wandae-tang extracts were detected using standard agar microdilution methods. In addition, the effects on the bacterial growth curve were also monitored at minimal inhibitory concentration(MIC) and $MIC{\times}2$ levels. The combination effects of Wandae-tang extracts with Clindamycin were observed by Checkerboard microtiter assay, and the effects of bacterial growth curve was treated with Wandae-tang extracts MIC+Clindamycin MIC, 1/2 MIC and 1/4 MIC, respectively. Results: MIC of Wandae-tang extracts and Clindamycin against Gardnerella vaginalis ATCC14018 were detected as $1.719{\pm}0.856$(0.782~3.125) $mg/m{\ell}$ and $0.010{\pm}0.006$ (0.004~0.016) ${\mu}g/m{\ell}$. In addition, Clindamycin and Wandae-tang extracts were also showed marked dosage-dependent inhibition of bacterial growth, and more dramatical inhibitions were detected in Clindamycin+Wandae-tang extracts MIC treatment. Fractional inhibitory concentration index in combination of Wandae-tang extracts and Clindamycin were detected as $0.294{\pm}0.052$(0.250~0.375) at Checkerboard microtiter assay. Conclusions: The results obtained in this study suggest that Wandae-tang extracts showed antibacterial effects against Gardnerella vaginalis ATCC14018, and they also showed dosage-dependent inhibitory effects on the bacterial growth. In addition, combination treatment of Wandae-tang extracts with Clindamycin showed more synergistically potent inhibitory effects on the growth of Gardnerella vaginalis.

Investigation of Clinical Research of Acupuncture Treatment on Hyperemesis Gravidarum in Medline (Medline 검색을 통한 임신오저에 대한 국외 침치료의 연구 동향 분석)

  • Purpose: The purpose of this study is to examine acupuncture treatment suggested on the papers of hyperemesis gravidarum(HG) in medicine journals and to establish the future direction. Methods: The papers which were associated with acupuncture and HG published between 2000 and 2012 in Medline were searched. Results: 14 studies met the condition. They were 13 randomized controlled trial (RCT)s and 1 clinical study. 13 studies revealed that acupuncture has efficacies in HG. 1 study was shown to have no statistically significant efficacy. 2 studies revealed that acupuncture was safe treatment, and 1 study revealed that acupuncture was reliable treatment. Conclusions: From the paper research, acupuncture treatment seems to be effective on HG. However, due to the lack sufficient research of HG, it would be necessary to fulfill further study about the period of acupuncture treatment on HG and the selection of acupoint through well-designed study and establish standard of evidence of them.

A Study on Heart Rate Variability(HRV) of Women with Fibroadenoma (유방 섬유선종이 있는 2, 30대 여성의 HRV 특성 연구)

  • Purpose: This study aims to analyze Heart Rate Variability(HRV) in women with fibroadenoma compared with non-fibroadenoma women. Methods: We studied 82 patients visiting Kang-Nam Kyung-Hee Korean Hospital Medical Examination Center from January, 2010 to November, 2011. The subjects were categorized in two groups, fibroadenoma group(n=16) and non-fibroadenoma group (n=66). We investigated the difference of HRV between two groups. Results: The square root of the mean squared difference of successive NNs (RMSSD) in fibroadenoma group is significantly lower than non-fibroadenoma group. LF/HF ratio in fibroadenoma group is significantly higher than non-fibroadenoma group. There was no significant difference in low frequency(LF) and high frequency (HF). However, normalized LF in fibroadenoma group is significantly higher than non-fibroadenoma group. In addition, normalized HF in fibroadenoma group is significantly lower than non-fibroadenoma group. Conclusions: Fibroadenoma group is related to autonomic nerve imbalances that is assumed by psychological problems.
