Factors Influencing Care Burdens of Caregivers of Elders with Dementia who Request Dementia Domiciliary Welfare Services (재가복지서비스 신청 치매노인 주부양자의 부양부담감 영향 요인)

  • Purpose. The purpose of the study was to identify the factors, which predict care burden perceived by caregivers of elders with dementia. Methods: The participants in this descriptive survey were 92 caregivers who used one of six daycare centers located in Incheon. The data were collected by questionnaires composed of items on general characteristics of the elders and caregivers, care burden, caregiver fatigue, and functional status of the elders. Results: General characteristics of the elders associated with care burden were age, gender, religion, and the presence of a spouse. Features of caregivers related to care burden were education, relationship with elderly, amount of rest, intention to care, monthly family income, and perceived family economic status. There were significant correlations among care burden, fatigue of caregiver and functional status of the elders. In stepwised multiple regression analysis, significant influencing factors were identified as caregiver fatigue, functional status of the elders, intention to care, relationship with the elders, amount of rest and elder's gender. Those variables explained 46% of variance of care burden. Conclusion: Functional status of elders and amount of rest were significant predictors and are supported by other previous studies. Future interventions for caregivers need to be focused on the slowing down of functional status of elders and increasing of number and length of caregiver's rest periods.

Effects of Family Support Programs for Caregivers of People with Dementia - Caregiving Burden, Depression, and Stress: Systematic Review and Meta-analysis (치매 가족 지지프로그램이 가족 부양자의 부양부담감, 우울, 스트레스에 미치는 효과: 체계적 고찰 및 메타분석)

  • Purpose: The objective of this systematic review was to assess the effects of family support programs on caregiving burden, depression, and stress in family caregivers of people with dementia. Methods: A literature search was conducted of electronic databases to identify randomized controlled studies with family support programs done between 2000 and 2014. Studies published in English and/or Korean were included for the analysis with search strategies adapted from the Cochrane Dementia and Cognitive Improvement Group. Studies were rated for quality assessment by two independent reviewers using the appraisal checklist developed by Cochrane Reviews and Dissemination. Of 8,334 articles identified in the literature search, full texts of 76 articles that met the inclusion criteria were reviewed and 38 were found to include relevant outcomes. Results: Results from selected studies were pooled in statistical meta-analysis using Review Manager Software and heterogeneity between combined studies was assessed using the Chi-square test. Meta-analysis showed that the effect sizes of family caregiver support programs were small to medium for categories of caregiving burden (Hedge's g= - 0.17, 95% CI= - 0.30~ - 0.04), depression (Hedge's g= - 0.30, 95% CI= - 0.40~ - 0.20), and stress (Hedge's g= - 0.39, 95% CI= - 0.52~ - 0.25). Conclusion: The review results indicate that a support programs can assist family caregivers in reducing their psycho-emotional distress.

Effects of Dementia Caregiver Program on Caregiver Burden (치매가족교실 프로그램이 치매 가족의 부양부담감에 미치는 효과)

  • Objectives : Dementia is one of the most distressing mental health problems in the older population. Caregivers also experienced physical, psychological, and emotional stress from taking care of dementia patients. So, we developed program for supporting dementia caregiver and evaluated its efficacy on reducing caregiver burden. Methods : We provided 5 sessions of dementia caregiver supporting program to 30 caregivers who were taking care of dementia patient in their home. Program was held in Cheonan Center for Alzheimer's disease and other dementia that was established by Cheonan city government for supporting dementia patients and their caregivers. We evaluated caregiver burden using short Zarit burden inventory consisted of 12 items scoring 0 (no burden) to 4 (everyday burden) before and after program. We evaluated satisfaction of caregiver about program using satisfaction survey consisted of 10 items scoring 0 (very dissatisfy) to 4 (very satisfy) after program. Results : Mean age of caregiver was 61.9. 40.0% (n=12) of caregivers were spouse. 53.3% (n=16) of caregivers were son or daughter. Caregiver burden that was estimated by short Zarit burden inventory were significantly decreased after program (p<0.001). When each item was compared, 4 items (7, 10, 11 and 12) were significantly decreased after program (p=0.036, p=0.018, p=0.01, p=0.024). All mean scores of 10 items about satisfaction were over 3 meaning that participants generally satisfied to program. Conclusions : Our study suggested that dementia caregiver supporting program could reduce caregiver burden and provide satisfaction. Therefore, programs for supporting dementia caregivers might be important as well as treating dementia patients. So, we should be interested in developing and providing efficiently this kind of program to reduce caregiver burden.

A Comparative Study of the Factors Influencing Burden of Primary Family Caregivers according to the Degree of Serious Illness of Elderly Patients Admitted in an Intensive Care Unit (중환자실 입원 노인 중증도별 주 부양가족의 부담감 영향요인 비교)

  • Purpose: This study was to compare and examine the factors influencing burden of primary family caregivers according to the severity of illness of elderly patients admitted in an intensive care unit. Methods: Subjects were the families of elderly patients in intensive care units of K, S and Y hospitals in Seoul. Data were collected from March to October 2007. Subjects were 108 persons over age 65. Data were analyzed by SAS statistics. Results: First, groups 5 and 3 showed higher burden than that of group 4. Second, high correlation was found between stress and burden, stress and anxiety, and burden and anxiety. Third, factors influencing family burden were found to be stress for group 5, stress, anxiety, and monthly income for group 4, and stress and patient age for group 3. Conclusion: Specific nursing interventions to decrease the stress of primary family caregivers of serious ill elderly patients in an intensive care unit are needed. Additionally, more effective and systematic activation of a long-term medical insurance system for seriously ill seniors is considered necessary to mediate the burden of primary family caregivers.

Relationship between Family Burden, Family Support Depression, and Satisfaction of the Elderly Caregiver (장기요양서비스 노인 주 부양가족의 가족부담감, 가족지지, 우울, 만족도 평가)

  • Purpose: This study was aimed to explain the relationship of family burden, family support, depression and satisfaction among caregivers of the elderly. Methods: The study population were 126 caregivers of 5 elderly residing in institutionalized nursing homes in Seoul and Kyunggi. The research tool of this study was a structured questionnaire on family burden, family support, depression and satisfaction. The data were collected from March 13 to April 30, 2008, and analyzed by Cronbach's alpha, descriptive statistics, t-test, ANOVA and Pearson's correlation coefficient and multiple regression by using the SPSS/WIN 14.0 program. Results: The major findings of this study were as follows: a significant positive correlation between family support and depression (r=.65, p=.001) was found. A significant positive correlation between family support and satisfaction (r=.68, p=.001) was perceived. Significant factors influencing Satisfaction were Family Support, which explained 38.2% of the variance of Satisfaction of Elderly Caregiver. Conclusion: This study suggested that we should develop programs and policies to increase the satisfaction of caregivers for their family support. In conclusion, the study was done to give suggestions to improve caregiver satisfaction of the aged and to serve as a basis for policy strategies by examining the current conditions of the nursing facilities.

A Prediction Model for Unmet Needs of Elders with Dementia and Caregiving Experiences of Family Caregivers (재가치매 환자의 미충족요구와 가족부양자의 돌봄경험 예측모형)

  • Purpose: The purposes of this study were to develop and test a prediction model for caregiving experiences including caregiving satisfaction and burden in dementia family caregivers. Methods: The stress process model and a two factor model were used as the conceptual frameworks. Secondary data analysis was done with 320 family caregivers who were selected from the Seoul Dementia Management Survey (2014) data set. In the hypothesis model, the exogenous variable was patient symptomatology which included cognitive impairment, behavioral problems, dependency in activity of daily living and in instrumental activity of daily living. Endogenous variables were caregiver's perception of dementia patient's unmet needs, caregiving satisfaction and caregiving burden. Data were analysed using SPSS/WINdows and AMOS program. Results: Caregiving burden was explained by patient symptomatology and caregiving satisfaction indicating significant direct effects and significant indirect effect from unmet needs. The proposed model explained 37.8% of the variance. Caregiving satisfaction was explained by patient symptomatology and unmet needs. Mediating effect of unmet needs was significant in the relationship between patient symptomatology and caregiving satisfaction. Conclusion: Results indicate that interventions focusing on relieving caregiving burden and enhancing caregiver satisfaction should be provided to caregivers with high levels of dementia patients' unmet needs and low level of caregiving satisfaction.

A Critical Review and Visim of Family Caregiving Research on the Demented Elderly간s in Republic of Korea (우리나라 치매노인 가족간호제공에 관한 연구의 현황과 전망)

  • Little is known about the impact of family caregiving for the demented elderly in Korea. The purpose of this study was to identify the current state of development of family caregiving research for the demented elderly in Korea and to identify correlates of caregiver or health problems and burdens within the socio-political contexts of Korea. A critical review of 17 family caregivers was carried out. The review revealed that various caregiving impacts and correlates of caregiver burden or health problems have been studied in relation to demented elderly family caregiving. Family caregiving for the demented elderly is a very complex phenomenon and various factors were related to caregiver burden, or their emotional and physical health. Findings from studies reviewed have shown inconsistent, inconclusive, and contradictory results. Furthermore, several conceptual and methodological problems were identified in the studies reviewed: restricted conceptualization, unrepresentative study samples, inadequate sample size, inappropriate study design, absence of comparison groups, inappropriate psychometric properties, and uncontrolled confounding factors. More research, as well as directions for further research, is recommended to identify family caregiving the impact of for the demented elderly, and to clarify the factors that explain results.

Caregiver Burden and Rewards in New Generation and Middle-aged Couples (신세대와 중년기 부부의 노부모 부양에 따른 부담감 및 보상감 분석)

  • The purpose of this study was to compare the caregiver burden and rewards in new generation and middle-aged couples. Data were collected from 207 new generation couples and 196 middle-aged couples who had either of older parents and lived in Seoul. Major findings are as fallows; 1. The burden were constituted with restriction of privacy physical burden strain psychological burden economic cost and guilt. The rewards were constituted with recognition pride maturity improving relationship and assistance of household and childcare. 2. The caregiver burden of new generation and middle-aged wives were hgiher than husbands. Hwever the caregiver rewards of new generation and middle-aged husbands were higher than wives. 3. The caregiver burden of middle-aged husbands were higher than new generation husbands bur there was no significant difference between new generation and middle-aged wives. 4. The caregiver rewards of middle-aged wives were higher than new generation wives but there was no significant difference between new generation and middle-aged husbands.

  • This research was conducted to identify the following: the home care needs of patients with dementia and the burden on the primary family care giver: to provide basic data required to develop nursing intervention for the care giver: and to suggest recommendations for medical institutions and social services that could reduce the burden on the families of people suffering from dementia. subjects of this research were 53 patients of the two Public Health Centers of Cheju Province who are suffering from dementia and their families. The instrument used in the research was Kuen. Jung Don (1994)' s assessment tool of burden in the primary family care giver who has parents with senile dementia and Yoo. Young Mi(1998)'s assessment tool of home care need. modified by the researcher in the questionnaire by a Likert rating scale. The period of data collection was from February 8. 2000 to March 10. 2000. Collected data was analyzed by SPSS, using mean, standard deviation. ANOVA, t-test and Pearson correlation coefficient. The result of this research was that there was not a significant correlation between the burden on the care giver and the level of dementia, its duration, the patient's ability to perform daily tasks, the period of care giving. and the use of social services, although the lower the patient's ability to perform daily tasks. and the worse the care giver's own health situation, the higher the burden on the primary family care giver. The following suggestions are made based on the results of this research. 1. More than half of the subjects don't use social facilities and services. More publicity and referral efforts are needed about medical institutions. nursing institutions and other facilities that specialize in services for dementia sufferers and their families. 2. Nursing services should include intensive education for the primary care giver in the most important aspects of home care. 3. Further research should be done, and should include data from all parts of Cheju Province.

Factors Related to Family Caregivers' Burden with the Community-Dwelling Disabled Elderly under the Long-Term Care Insurance System (장기요양 재가서비스 이용자를 돌보는 가족주부양자의 부양부담감에 영향을 미치는 요인)

  • Background: Informal care is increasingly recognized as placing a significant burden on the lives of family caregivers. The purpose of this study is to investigate factors related to family caregivers' burden with the community-dwelling disabled elderly under the long-term care insurance system, using the Stress Process Model developed by Pearlin (1990). Methods: Total 1,233 family caregivers with the disabled elderly, using the long-term care services in their home from May to June 2009, completed questionnaires finally. The questionnaire of this study consists of a total of 32 questions, including 11 questions related to background and context, 17 questions related to objective stressors, and 4 questions related to coping resourses. Family caregivers' burden is measured by the Korean Revised Caregiving Appraisal Scale (K-RCAS, Cronbach's alpha=0.86). To investigate factors related to family caregivers' burden, multiple regression analysis was conducted. Results: The average score of caregivers' burden was 22.0 (${\pm}6.12$). In multiple regression analysis, there were statistically significant factors affecting on the family caregivers' burden, that are related to background and context (region, living status, education level, relationship with beneficiary), objective stressors (duration of caregiving), coping resourses (caregiver's health status). Conclusion: This study found that family caregivers experience a considerable burden. The findings suggest that policies must be taken to relieve family caregivers of their duties temporarily, and to support them with counselling and education.
