Lesion Localization in Patients with Hyperparathyroidism Using Double-phase Tc-99m MIBI Parathyroid Scintigraphy (부갑상선기능항진증 환자에서 Tc-99m MIBI 부갑상선 신티그라피를 이용한 병소 국소화)

  • Purpose: This study was performed to evaluate the diagnostic usefulness of double-phase Tc-99m MIBI parathyroid scintigraphy with single photon emission computed tomography (SPECT) in patients with hyper-parathyroidism. We also evaluated the relationship between Tc-99m MIBI uptake and oxyphil cell contents in parathyroid glands. Materials and Methods: The subjects were 28 parathyroid glands of 10 patients who underwent Tc-99m MIBI parathyroid scintigraphy and parathyroidectomy for clinically suspected hyper-parathyroidism. Early and delayed pinhole images were obtained at 15 minutes and 2 hours after injection of Tc-99m MIBI, and SPECT images were followed. The weight and oxyphil cell contents of parathyroid tissue were obtained from pathologic specimen, and the scintigraphic findings were compared with histopathology. Results: In surgical histopathology, 6 parathyroid adenomas and 9 parathyroid hyperplasias were confirmed. The sensitivity, specificity, and positive predictive value of early and delayed images were 46.7% (7/15), 76.9% (10/13), 70% (7/10) and 66.7% (10/15), 92.3% (12/13), 90.9% (10/11), respectively. SPECT image detected an additional small hyperplasia.. The sensitivity, specificity, and positive predictive value of combined interpretation of early and delayed images with SPECT were 73.3% (11/15), 100% (13/13), 100% (11/11). The sensitivity was 100% (6/6) for adenoma, whereas that was 55.5% (5/9) for hyperplasia. Both adenomas and hyperplasias showed significantly increased oxyphil cell contents compared with normal parathyroid glands (p<0.0001), but the oxyphil cell content and weight were not significantly different between adenomas and hyperplasias. Conclusion: Double-phase Tc-99m MIBI parathyroid scintigraphy with SPECT is useful for lesion localization in patients with hyperparathyroidism. Although both adenoma and hyperplasia have increased oxyphil cell content, the sensitivity is high in adenoma, but low in hyperplasia.

  • Brown tumor is tumor like lesion resulted from hyperparathyroidism, and it has been rarely reported recently. We evaluated a 29-year-old woman who had brown tumor on proximal humerus caused by primary hyperparathyroidism. She first had presented pain and swelling over the upper arm, and was suspected as giant cell tumor in biopsy combined with hyperparathyroidism. Wide marginal excision and tumor prosthesis were performed. However, it was confirmed as brown tumor resulted from parathyroid adenoma according to laboratory findings and radioactive isotopes image, and surgical excision of adenoma relieved clinical symptoms. We report this case with a review of literatures.

  • [ $^{201}T1/^{99m}Tc$ ] pertechnetate subtraction scintigraphy of the parathyroid gland was performed in a study of 8 patients with clinical and biochemical evidence of hyperparathyroidism for diagnosis and Localization of the lesion prior to sugery. 6 cases of parathyroid adenomas were accurately localized and 2 cases of diffuse hyperplasia associated with chronic renal failure(CRF) and multiple endocrine neoplasia (MEN) type I. $^{201}T1/^{99m}Tc$ pertechnetate scan was useful for patients with hyperparathyroidism, especially in definite localization preoperatively.

  • In thyroid and parathyroid surgery, damage to the recurrent laryngeal nerve(RLN) is the most common iatrogenic cause of vocal cord paralysis. Identification and preservation of the BLNs and meticulous technique can siginificantly decrease the incidence of this complication. We experienced one case of NRRLN in a patient with the parathyroid adenoma. During the dissection, there was no branch to be considered as RLN in tracheoesophageal groove. While searching for the RLN, We found a white structure coursing horizontally at the level of cricoid cartilage directly arising from the vagus nerve in the carotid sheath. That structure was nonrecurrent recurrent laryngeal nerve(NRRLN) and NRRLNs are exceedingly rare. Awareness of the possibility of NRRLN will prevent the surgeon from accidentally severing one if it is encountered during surgery.

  • Three cases of hyperparathyroidism are presented from our experience at Presbyterian Medical Center in Chonju in the hope that this will alert us all to the variegated patterns of clinical presentation. In the first case the principal symptom was muscular weakness. In the second a bone cyst (which was part of the syndrome of osteitis fibrosa cystica); and in the third case rib cage tenderness, backache, and persistent epigastric pain. All three had adenomas, but in Case 2 the adenomas were multiple. All three responded to surgical resection and remain well.

  • Thymoma is the most common anterior mediastinal mass in adult, and arises from thymic epithelial cells which includes consists various proportions of epithelial and lymphocytic aspect. Thymic epithelial cell arises from the ventral wings of the third and fourth branchial pouches in the embryo, and these cells are considered tumorous condition of thymoma. Surgical resection is main treatment of thymoma. And adjuvant chemotherapy or radiation is considered due to postoperative pathologic diagnosis. We experienced a tumor which located from left anterior neck along superior mediastinum on chest radiograph incidentally and diagnosed parathyroid adenoma clinically. After surgical removal, final pathologic report was thymoma. Here, we present the case with a review of the related literatures.
