Thrombectomy of Superior Mesenteric Artery Occlusion - A case report - (상장간막 동맥 폐쇄증의 혈전 제거술- 1예 보고 -)

  • A 50-year old man was admitted to our hospital because he complained of sudden abdominal pain. Multidetector abdominal CT showed proximal occlusion of the superior mesenteric artery. Emergency open laparotomy and Fogarty thrombectomy were done on admission day and repeat Fogarty thrombectomy and partial resection of the small bowel were done the next day. We report here on a case of superior mesenteric artery occlusion.

  • A 7-year-old Shih Tzu cross dog was presented for severe abdominal pain, persistent vomiting and anorexia. Laboratory tests revealed mild anemia and hypoproteinemia. Abdominal survey radiography revealed marked gastric distension and large circular foreign body in the gastric pylorus. The foreign body was removed using a videoendoscope, fishing line and retrieval forceps. The dog's clinical signs resolved following foreign body removal.

  • Objectives This study for evaluating the effects of HyangsayukgunjatangGamibang on childhood with abdominal pain. Methods This clinical study has been carried out with 38 children, who visited to the Department of Pediatrics, $\bigcirc\;\bigcirc$oriental medical center from January 2005 to August 2007, who had been treated for abdominal pain with herbal medicine (HyangsayukgunjatangGamibang). This study had done by chart-review or by telephone research. Results 1. After treating with HyangsayukgunjatangGamibang, 34 children's abdominal pain was improved. (63.2%) 2. After treating with HyangsayukgunjatangGamibang, 13 children eat more than before. 3. After treatment with HyangsayukgunjatangGamibang, children's symptoms-constipation, nausea, headache, anorexia were improved. Conclusions HyangsayukgunjatangGamibang is effective in childhood abdominal pain.

  • Isolated small bowel intussusception accounts for 10% of all pediatric intussusception. It is more common in children older than 2 years of age. Presentation usually is with vomiting and abdominal pain. Currant jelly stool and palpable mass are less frequent than typical intussusception. There are few reported cases of children with transient small bowel intussusception. We describe 3-year-old boy presented with intermittent cyclic crampy abdominal pain for 6 months was diagnosed as having recurrent transient small bowel intussusception by abdominal ultrasonography and small bowel series.

  • Childhood tuberculous peritonitis is difficult to diagnose especially in cases without pulmonary involvement. It may present as mechanical ileus, perforation, simulating acute appendicitis, enterocolitis or intusussception. Early diagnosis in children may be difficult, largely because of variable vague symptoms and nonspecific signs. Surgery has often been required for pathologic confirmation. We have experienced a case of tuberculous peritonitis presenting with abdominal pain, abdominal distension and persistent high fever in a 10-year-old boy who was diagnosed by explo-laparotomy and pathologic confirmation from biopsy specimen from omentum. The patient was treated with antituberculous drugs and recovered uneventfully.

  • Childhood intestinal tuberculosis is difficult to diagnose for its protean clinical manifestations, especially in cases without pulmonary involvement. Differential diagnosis with Crohn's disease, inflammatory bowel disease and other malignancy is also important. Surgery has often been required for pathologic confirmation or therapy. Colonoscopy may be performed safely under consciousness sedation in children for bacteriologic and histopathologic confirmation of the biopsy specimen in addition to gross appearance of the lesion. We have experienced a case of intestinal tuberculosis presenting with chronic abdominal pain, diarrhea, weight loss and anemia in a 9 year old girl who was diagnosed by a colonoscopic examination and culture of the biopsy specimen from the ascending colon. The patient was managed with antituberculous drugs and recovered uneventfully.

  • Crohn disease is a chronic transmural inflammatory disease that may involve any portion of the gastrointestinal tract. An increased incidence of Crohn disease in the general population has been reported, along with a greater than threefold increase of Crohn disease in children under the age of 16 years noted in a recent study. Crohn disease may be seen as early as infancy, but the most common pediatric age of onset is during the teenage period. We experienced a case of Crohn disease in 6 year old male child complained abdominal pain, oral aphthous ulcers, arthralgia, anorexia, and growth failure. A brief review of related literature is also presented.

  • Hemophagocytic syndrome(HPS) has four subgroup, sporadic disorder, associated with acute infection, familial form seen in children, and associated with malignant disorders, immunodeficiencies, defective leukocyte function. Histologically, Hemopoietic cells are actively ingested by moncytes/macrophages in various organs, including lymph nodes, bone marrow, liver, and spleen. Epstein-Barr virus(EBV) is now thought to be one of the major causes for the virus-associated hemophagocytic syndrome(VAHS). Epstein-Barr(EB) virus infection is common, with up to 90% of individuals demonstrating positive titiers by age 20. Although elevated liver function tests commonly occur, severe hepatitis is rare. Only seven cases of ascites complicating Epstein-Barr infection are reported, but none clearly demonstrate the abscence of other causes of hepatic dysfunction. We are reporting a case of Epstein-Barr Virus-associated hemophagocytic syndrome with ascites.

  • Objective The aim of this study was to report significant improvement of primary insomnia in a Soyangin Cold-related diarrhea accompanied by abdominal pain Symptomatic pattern Patient. Methods The patients were diagnosed with Soyangin Cold-related diarrhea accompanied by abdominal pain Symptomatology(身寒腹痛亡陰證) and treated with Hyungbangjihwang-tang(荊防地黃湯). The primary outcome measures for this study were condition of sleep using a questionnaire with Pittsburgh Sleep Quality Index (PSQI) Secondary outcome assessment included change of original symptoms such as patient's digestion, sweating and feces. Result The symptoms of primary insomnia improved by the end of the a treatment period without side effect. original symptoms were also changed. Conclusions This result show Hyungbangjihwang-tang(荊防地黃湯) can be used to treat primary insomnia in a Soyangin Cold related Mangeum Symptomatic Pattern accompanied by abdominal pain Symptomatology(身寒腹痛亡陰證). Meaning and process of primary insomnia are different according to Sasang Constitutions.

  • Phytolacca americana poisoning is a benign plant intoxication that causes gastrointestinal symptoms, including abdominal cramps, vomiting, diarrhea, and gastrointestinal bleeding. Other signs and symptoms include diaphoresis, salivation, visual disturbance, and seizures or mental changes. We report two cases of patients who experienced confusion and abdominal pain, vomiting, and hematemesis after oral ingestion of pokeweed. A 60-year-old female with confusion and a 67-year-old female with abdominal pain, vomiting, and diarrhea were admitted to the emergency department after pokeweed poisoning. After supportive treatment of hydration and gastrointestinal medication, the two patients showed full recovery within 24 h and were discharged from the hospital.
