The Clinical Usefulness of Serum Aminotransferase Activities for Predicting Fatty Liver in Obese Children (비만아에서 지방간의 예측 인자로서 혈청 Aminotransferase의 임상적 유용성)

  • Purpose: Non-alcoholic fatty liver disease (NAFLD) has been recognized as an important childhood liver disease, especially where the prevalence of childhood obesity is increasing. The purpose of this study is to clarify the usefulness of elevated serum aminotransferase activities and their ratio for predicting the presence of fatty liver and its severity in obese children. Methods: Forty-four children (M/F 29/15, age 4 to 16 years) with obesity (weight excess>20%) were analyzed retrospectively with medical records based on degree of obesity, bioelectrical impedence, serum aminotransferase activities, lipid profiles and ultrasonography. Results: 1) Ultrasonography was carried out in 34 cases. Elevated serum ALT was found in 89.7% (26/29) of the patients diagnosed as fatty liver by ultrasonography and decreased AST/ALT ratio (<1) was found in 96.6% (28/29). There was a strong correlation between elevated serum ALT (>45 IU/L) or decreased AST/ALT ratio (<1) and the fatty liver in obese children (p<0.05). 2) There was no significant correlation between the serum ALT or AST/ALT ratio and the degree of fatty liver (p>0.05). 3) There was a significant correlation between total cholesterol, triglyceride and fatty liver (p<0.05). Conclusion: Serum ALT activity and AST/ALT ratio were useful to predict the presence of fatty liver diagnosed by ultrasonography in obese children, whereas they were not useful to predict the degree of fatty liver. Therefore, to prevent fatty liver progressing to advanced liver disease, it is necessary to manage and monitor the obese children continuously, especially those who have predicting factors of fatty liver.

Study on Consequent Body Fat and Serum Lipid Metabolism after Cocoon Hydrolysate, Green Tea Leaves and Dietary Fiber Supplementation (누에고치 유래 실크 펩타이드와 녹차잎 및 식이 섬유소 보충이 체지방 및 혈청 지질 대사에 미치는 영향에 관한 연구)

  • This study is to investigate consequent nutrient intake status, Influences of body mass index (BMI) and serum lipid composition, and fat distribution on the cocoon hydrolysate, green tea leaves and dietary fiber supplementation. During 2 months of this research (April to May, 2002), 47 women aged 20 yr-30 yr (average age 26.2 yr) were selected as subjects. Nutrient intake was investigated by questionnaire and 24-hr recall method. Antropometric assessments of the subjects were investigated by SBIA method (Segmental bioimpedance assay, Inbody 3.0). The results were as follows: mean body weight was 60.7 kg, mean body height 161.7 cm and mean BMI 23.4. Status of energy intakes significantly decreased (p<0.01) and dietary fiber intakes significantly increased (p<0.001) after supplementation. BMI and WHR (waist-hip ratio) significantly decreased (p<0.01) and body fat significantly decreased after supplementation (p<0.001). Total cholesterol and LDL-cholesterol significantly decreased after supplementation (p<0.05). HDL-cholesterol was negatively correlated with BMI and WHR (p<0.01). LDL/HDL ratio was positively correlated with BMI (p<0.01) and WHR (p<0.05). Above results of this study show that low-molecule peptide, green tea leaves and dietary fiber supplementation-added routine diet improves lessening body fat distribution, total cholesterol, LDL-cholesterol. Especially, decrease of abdominal fat and WHR were notable. That meant decrease of risk factors.

Relationships of Obesity, Total-Cholesterol, Hypertension and Hyperglycemia in Health Examinees with Disabilities (장애인 건강검진 수검자들의 비만, 콜레스테롤, 고혈압, 고혈당의 관련성)

  • Among the employer-supported subscribers to the National Health Insurance Service, 6,797 people with mild disabilities with western ages of 20 and up and who received health checkups were investigated. Of these 6,797 people, 3,186 and 3,611 received health checkups in 2009 and 2013, respectively. Those people who were diagnosed with physical handicaps, brain lesions, visual impairment, hearing impairment, intellectual disabilities, mental disorders, kidney disorders or other disorders according to the classification standard for people with disabilities were classified into disability groups of the 3rd through 6th degrees. The purpose of this study was to examine the dangerous influence of obesity of people with mild disabilities on their hyperglycemia, hypertension and high cholesterol. The items measured in this study were abdominal obesity, body mass index, fasting glucose, total cholesterol, systolic blood pressure and diastolic blood pressure. To look for connections between the obesity level and at-risk groups for each disease, cross tabulation and multinomial logistic regression analyses were utilized. Higher levels of abdominal obesity and BMI were found among those who were male, were younger and had higher incomes. The risks of abdominal obesity and BMI were higher in the abnormal groups for each disease. In 2009, the obesity group whose BMI was higher had a 1.51-fold higher risk of hypertension than the normal group. The abdominal obesity group had a 1.59-fold higher risk of high cholesterol, a 1.26-fold higher risk of hypertension and a 1.54-fold higher risk of hyperglycemia than the normal group. In 2013, the obesity group whose BMI was higher had a 1.72-fold higher risk of high cholesterol and a 1.43-fold higher risk of hypertension than the normal group. Those with abdominal obesity had a 1.59-fold higher risk of hyperglycemia than the normal subjects. As the risk of obesity was higher in those with disabilities than in those without disabilities, the former should be encouraged to undergo health checkups on a regular basis, and the coverage of the health checkups should be extended to keep track of their illness. In addition, appropriate education and concern are both required to prevent obesity.

Development of Suit-tops Size-system for Abdomen-obese Adult Males (복부비만 성인 남성의 상의류 치수체계 제안)

  • This helps plus-size consumers purchase ready-made clothes and improves fitness by analyzing the upper body shapes of abdomen-obese adult males; in addition, it creates a size system based on each size interval of obesity shapes. The criteria for the 572 male subjects surveyed were over 25 kg/㎡ of the BMI, over 90 cm of waist, and over 0.85 waist hip ratio. The results were as follows. First, the higher the age group, the higher the degree of abdominal obesity in each age group. The degree of obesity decreases age increase; however, was the risk of abdominal obesity increased. Second, Compared to standard body types, the average body size of Abdomen-obese males is significantly larger, especially in the waist area than in the chest area. Third, to propose a size system for abdominal obese adult males, 16 sections with a final 2% rate of appearance were finally adopted as suit-tops size system for abdominal obese adult males by applying a 3 cm dimension gap between each designation as specified in the KS standard. The coverage ratio for the 16 sections was 72.6% and the coverage effienciency was 4.5%. The results of this study can be used in a various clothes; in addition, the activation and segmentation of the plus-size market are also believed to be significant.

Prevalence of Metabolic Syndrome and Related Risk Factors of Elderly Residents in Andong Rural Area 1. Based on the Anthropometric Measurements and Health Behaviors (안동 농촌지역 중년 및 노인 주민의 대사증후군 유병율과 관련 위험요인 분석 1. 신체계측결과와 건강습관을 중심으로)

  • This study was performed to estimate the prevalence and the risk factors of metabolic syndrome in Andong rural area. A total of 1,431 people (533 males, 898 females) aged over 45 years participated in this study in 2003. The prevalence of metabolic syndrome was 38.2% (male 23.5%, female 46.9%, p<0.001). As age increased, the prevalence of the metabolic syndrome increased in female, but not in male. The major underlying components of metabolic syndrome were high blood pressure (67.1%), low HDL-cholesterol (60.6%), and abdominal obesity (39.9%). The distinctive component for male was high blood pressure (70.1%), and for female, low HDL-cholesterol (73.6%), high blood pressure (65.3%), and abdominal obesity (54.5%). Subjects having more than one component were 94.7%. The risk factors for metabolic syndrome were analyzed using the multiple logistic regression method according to gender and expressed as age-adjusted odds ratio (OR). The results of comparing female to male (OR=2.953), and of comparing obese by % body fat (M: OR=5.786, F: OR=13.498) or BMI (M: OR=3.782, F: OR=13.301) to normal body weight showed significantly higher risk for metabolic syndrome (p<0.001). Health related habits, such as smoking, alcohol drinking, and exercise, didn't show any effect on metabolic syndrome. This study revealed that the prevalence of metabolic syndrome was significantly higher in female subjects compared to both male and female, and high blood pressure was the main cause of metabolic syndrome. We suggest that the strategy for prevention or reducing the prevalence of metabolic syndrome in this area should be concentrated on reducing high blood pressure through lowering obesity and abdominal obesity.

Effect of Improve Obesity with Sprout Raw Grains and Vegetables (발아 생식을 이용한 비만개선 효과에 관한 연구)

  • This study was carried out to investigate the effect of weight loss and the change of serum lipids composition for normal and obese women after sprout raw grains and vegetables diet intake. Diet were given to the subjects which were composed of 11 normal women and 7 obese women for 4 weeks. After diet intake, 4.6% in normal group and 3.5% in obese group were shown weight loss. And the both groups of normal and obese women showed a significant reduction in the thickness of subcutaneous fat and the body girth. Particularly, the waist measure was shown to be remarkably reduced in the normal group (p< 0.0001) and to be reduced in the obese group (p<0.017) significantly. After diet intake. the levels of total cholesterol and triglycerides were reduced in both groups significantly. And in the obese women, HDL-cholesterol level increased 5.7%. As those results, it could be seen that weight and the thickness of subcutaneous fat were reduced and the component of blood serum were improved after diet intake for 4 weeks. Specially, abdomen, waist measures and hip girth were reduced remarkably, so it is considered that the diet is more effective in the diet therapy of obese women with upper obese status.

  • Background : Cardiopulmonary exercise test is a useful test for the evaluation of the cardiovascular and respiratory systems. Obese subjects have an increased resting metabolic rate ($VO_2$) compared to non~obese subjects and the increase is more marked during dynamic exercise, which results in the limitation of maximal exercise in obese subjects. In this study, the influence of the obesity and fat distribution on the maximal exercise capacity were evaluated. Methods : Maximal exercise capacity was represented by maximam maximum oxygen uptake and $VO_2$ max in the cardiopulmonary test. Obesity, total fat content and abdomina1 obesity(waist to hip ratio, WHR) were measured by bioelectrical impedence method. Total of 42 volunteers (male 22, fema1e 20) were evaluated. Results : 1) Weight to height ratio (mean$\pm$SD) was 110$\pm$14.9% in men and 100$\pm$11.1% in women. 2) Fat ratio (mean$\pm$SD) was 23.3$\pm$5.2% in men and 27.55$\pm$3.9% in woman. 3) Waist to hip ratio (mean$\pm$SD) was 0.85$\pm$0.04 in men and 0.8$\pm$0.03 in woman. 4) In men, $VO_2$ max/min/Kg was negatively correlated with obesity, fat ratio, and abdominal fat distribution. 5) In woman, $VO_2$ max/Kg was negatively correlated with obesity and fat ratio, but did not show significant relationship with abdominal fat distribution. Conclusion : Obesity was a limiting factor for maximal exercise in both men and women. Abdominal obesity was a limiting factor for maximal exercise in men but its implication to women needs further evaluation.

The Analysis of Relationship between Abdominal Adipose Tissue Distribution and Health Risk Factors in Adolescent Obese Man (청소년기 비만 남성의 복부지방조직 분포와 건강관련 위험지표의 관련성 분석)

  • In the current study, the relationships between abdominal adipose tissue depots and skinfold thickness, body circumference and composition, and blood lipids and pressure were investigated in adolescent obese man. Total abdominal adipose tissue (TAT) and subcutaneous adipose tissue (SAT) were correlated with skinfold thickness. Especially subscapular skinfold was highly correlated with TAT and SAT, and it was found that chest skinfold was the only predictor for estimating visceral adipose tissue (VAT). Body circumference and body composition were associated with TAT and SAT, not VAT, and correlation coefficient of waist circumference was highest. On the other hand, body fat mass was highly correlated with TAT and SAT than %body fat and fat free mass. It was also shown that blood triglyceride concentration and systolic blood pressure were the only predictor for estimating VAT and SAT respectively.

Anthropometric Measurements and Biochemical Nutritional Status of the Older Residents (50 years and over) in Andong Area (2) (안동주변 농촌지역 50세 이상 주민의 신체계측치 및 생화학적 영양상태에 관한 연구 (2))

  • This study aimed to assess the health status based on the anthropometric and biochemical measurements of middle-aged and elderly people living in Andong area. The subjects were 1,384 people (532 males, 852 females) aged 50 years and over (average 62.7 years). The mean anthropometric values for males and females were heights of 163.7 and 151.5 cm; weights 63.6 and 57.3 kg; body mass index (BMI) 23.6 and $24.9kg/m^2$; body fat 21.8 and 31.8%, respectively. Height and weight were lower, however, waist circumference (in female) and BMI were higher than those of the 2001 National Health and Nutrition Survey (NHNS). Obesity incidences of male and female subjects were 28.7% and 47.3% by BMI; 25.8% and 50.8% by % body fat; and 15.6% and 80.9% by waist circumference, respectively. Also, abdominal adiposity was very severe in female subjects of 50s. The mean biochemical measurements of male and female were as follows: systolic and diastolic blood pressure 136.9, 83.8 mmHg and 133.6, 82.5 mmHg; hemoglobin (Hb) 14.3 and 13.0 g/dL; hematocrit (Ht) 44.7 and 39.8%; blood albumin 4.15 and 4.04 g/dL; total-cholesterol 170.0 and 183.1 mg/dL; HDL-cholesterol 43.6 and 42.7 mg/dL; fasting blood glucose 96.7 and 93.0 mg/dL, respectively. Also, the prevalence of biochemically abnormal subjects according to each cut-off point of biochemical measurements were analyzed. The results for male and female were; hypertension 58.0% and 47.2%; iron deficient anemia 19.3% and 20.6% by Hb, 7.2% and 11.9% by Ht; hypoalbuminemia 9.8% and 11.7%; diabetes 12.0% and 10.2%; hypercholesterolemia 19.5% and 30.5%, respectively. From those results we found that hypoalbuminemia, hypertension and hypercholesterolemia were prevalent, and obesity in females of 50s, iron-deficient anemia and diabetes in males of 70 years and over were significant health problems in this area. Therefore, it seems to be necessary to examine their health status periodically and provide the appropriate health and nutrition education program, which includes low sodium intake, balanced diet, exercise and weight control, to prevent the occurrence of chronic diseases.

  • 최근 들어 국내에서는 체중조절이 사회적인 문제로 다루어질 만큼 커다란 관심의 대상이 되고 있으며, 이에 체중조절을 시도하는 사람들의 급속한 증가와 아름다워지려는 여성들의 요구에 의해 무분별한 다이어트가 시행되어지고 있다. 이와 함께 이와 관련된 상업성 다이어트 업체들도 현저히 늘어나고 있는 실정이다. 그러나 특정 다이어트제품의 비만도(body mass index; BMI (kg/$m^2$)의 감소에 따른 효능평가에 관한 연구는 보고 되고 있으나 건강 위험인자의 개선에 대한 효과 평가는 미흡한 실정이다.(중략)
