  • While only the experts could create the contents in the existing VR contents business, in the proposed "Untact Social VR Video-Based Tour Scheduling", the GUI (Graphic User Interface) allows anyone to easily and quickly share the information taken through VR camera after creating. It is intended to serve as a variety of SNS-based media by developing a structure that can be shared in html format after creating with the GUI menus. In particular, it is applied to companies that operate the SNS-type publicity projects including experience facilities and viewing facilities, and the various SNS-type publicity businesses that create the VR contents. Communication is clear compared to the existing homepage-based information delivery method, and It aims to provide universality that anyone can easily edit in an environment where only experts can edit VR video contents of.

Economic Effects of Welfare Policy: An Analysis of 2003 Korean Social Accounting Matrix (정부의 복지지출이 경제부문별 소득분배에 미치는 효과 분석)

  • I constructed the 2003 Korean 'social accounting matrix'(SAM) to analyze the multipliers of total demand for each economic activity. I find that the relative magnitude of the influence of the welfare policy to the national economy measured by input-output production multipliers tends to be underestimated compared to SAM multipliers. This is because the total demand multipliers of SAM include the private sector effects, which is not considered in the input-output model. The result also support that income inflows in public service areas including education, health and social work, generate gains in the relative income of households.

Relationship between Relevance Index and Hospital Management Performance (지역 의료이용 친화도(RI)와 병원 경영성과의 관계)

  • The purpose of this study is to present the effective management strategy of RI based on the results of research on the causal relationship between the regional medical care capacity and the hospital management performance by calculating the RI of the general hospitals in Korea, This research has significance. The results of this study are as follows: First, statistically significant differences were found in Number of beds and recurring profits in urban areas by the general characteristics. Second, the correlations between the RI and the variables of the regional medical use are as follows: Recurring profit ratio, New Outpatient Visits, Operating Margin, Daily Outpatient Visits per 100 Beds, Daily Inpatient Days per 100 Beds, and Average Charge per Inpatient Day. Based on the results of this study, the significance of this study is as follows. First, we calculated the affinity for local medical use, which is the index of local medical utilization. Secondly, it is analyzed according to internal and external environmental factors such as city size, hospital size, etc. It can be said that the hospital provided basic data for establishment of hospital management strategy to increase the utilization rate of local medical care.

Analysis of Factors Affecting Length Of Stay for A Serious Patients Using Medical Records (의무기록자료를 이용한 중증질환자의 재원일수에 미치는 요인 분석)

  • Objectives: In this study, we tried to analyze the factors affecting Length Of Stay for serious patients in Republic of Korea. Methods: The study included 139,172 serious patients in the 2012-2016 discharge details. Using the SPSS 23.0 program, we conducted a rank regression analysis with social and social demographic characteristics as control variables, medical institution characteristics and medical use characteristics as independent variables, and Average Length Of Stay as a dependent variable. Results: Average Length Of Stay for participants was found to be 9.92days. And the location and bed size of medical institutions were not statistically significant, the hospitalization path was more urgent(B=0.43) than the outpatient (p<0.001), and there was no secondary diagnosis(B=0.35). However, Average Length Of Stay was higher (p<0.001) than there was no main surgery(B=0.80). After discharge, Average Length Of Stay for funding(B=0.43) and death(B=0.72) was long (p<0.001). Average Length Of Stay for participants was found to be 9.92days. And the location and the bed size of the medical institution were not statistically significant, and the hospitalization pass had longer Length Of Stay for emergency patients(B=0.43) than for outpatients(p<0.001). There was a longer Length Of Stay(B=0.35) than none was diagnosed. There were longer Length Of Stay(p<0.001) than there was no major surgery(B=0.80). After discharge, the outpatients had longer Average Length Of Stay(B=0.43) and deaths(B=0.72) than those who returned home(p<0.001). Conclusion: As a result of analyzing the factors affecting Average Length Of Stay of the participants, it was confirmed that regardless of the location and bed size of medical institutions, hospitalization route, department diagnosis, main surgery, and whereabouts after discharge. Therefore, appropriate interventions and necessary support must be provided so that efficient Length Of Stay can be managed according to the medical use characteristics of serious patient.

Effectiveness of the Trauma Team-Staffed Helicopter Emergency Medical Service (헬리콥터 응급의료서비스의 외상팀 탑승 여부와 외상환자의 생존율)

  • Background: Whether there is a difference in outcomes for trauma patients transferring to the helicopter emergency medical service (HEMS) according to their previous team composition is controversial. The purpose of this study is to evaluate the effectiveness of trauma team-staffed-HEMS (TTS-HEMS) when transferring to a trauma center. Methods: A retrospective comparison was conducted on patients transported to a trauma center over a 6-year period by the TTS-HEMS and paramedic-staffed-HEMS (119-HEMS). Inclusion criteria were blunt trauma with age ${\geq}15years$. Patient outcomes were compared with the Trauma and Injury Severity Score (TRISS) (30-day mortality) and the Cox proportional hazard ratio of mortality (in hospital). Results: There were 321 patients of TTS-HEMS and 92 patients of 119-HEMS. The TTS-HEMS group had a higher Injury Severity Score and longer transport time but a significantly shorter time to emergency surgery. The prehospital data showed that the trauma team performed more aggressive interventions during transport. An additional 7.6 lives were saved per 100 TTS-HEMS deployments. However, the TRISS results in the 119-HEMS group were not significant. In addition, after adjusting for confounders, the hazard ratio of mortality in the 119-HEMS group was 2.83 times higher than that in the TTS-HEMS group. Conclusion: HEMS was likely to improve the survival rate of injured patients when physicians were involved in TTS-HEMS. Survival benefits in the TTS-HEMS group appeared to be related to the fact that the trauma team performed both more aggressive prehospital resuscitation and clinical decision making during transportation.

Analysis of the Relationship between the Convergence Factors Related to Organizational Citizenship Behavior of Some Hospital Administrative Staffs (일부 병원행정직의 조직시민행동과 관련된 융복합 요인의 관련성 분석)

  • This paper attempted to find the convergence factors related to the organizational citizenship behavior of hospital administration staff(HAS). The survey was conducted on 274 administrative staff at 22 hospitals in J regions randomly selected using self-administered questionnaires. The survey period took about a month from July 1 to July 31, 2019. The hierarchical multiple regression analysis shows the following results. The organizational citizenship behavior of respondents turned out to be significantly higher in following groups: The higher the procedure justice, sub-areas of organization justice, the higher the intragroup trust, and the higher the organizational commitment. Their explanatory power was 41.9%. The results of the study indicate that the efforts, to increase organization justice, intragroup trust and organizational commitment, are required to improve organizational citizenship behavior among HAS. The above results are expected to be utilized in human resource management and industrial health education of medical institutions that enhance organizational citizenship behavior of HAS. Later research requires the establishment of a structural equation model that affects organizational citizenship behavior of HAS.

  • Recently the interest in secondary use of medical information has emerged. But the domestic legislation or guidelines, such as being able to say that already specialize in healthcare information, can be seen a 'national medical privacy guidelines'. However the guidelines have suggested that only a violation of privacy laws in the medical information, it does not defined clearly with respect to protected health information(PHI) for secondary use. In this paper, we learn the HIPAA(Health Insurance Portability and Accountability Act) Privacy Rule of the US legislation which provides a non-identifiable screen instructions for secondary utilization of medical information, domestic guidelines and other country's guidelines. comparing with the HIPAA, national medical privacy guidelines and the domestic studies, we propose a new domestic target non-identifying information suitable for the domestic field and present future research direction.

A Study on the Medical Use of Total Replacement Arthroplasty Patients for Life Care (라이프케어를 위한 관절 치환술 환자의 의료이용에 관한 연구)

  • The purpose of this study was to analyze influential factors for blood transfusion for patients undergoing total knee arthroplasty and total hip arthroplasty, which are chronic degenerative arthritis, using the 2017 sample data of Health Insurance Review & Assessment Service, to research the impact of blood transfusion on the usage of medical services among arthroplasty patients, and ultimately to provide some information on how to offer quality medical services. The findings of the study were as follows: First, whether there were any significant differences in the use or nonuse of transfusion during total knee arthroplasty according to hospital characteristics and patient characteristics was compared. As for significant variables, the type of health care institution, the level of sickbed, gender and anemia were found to have been statistically significantly related. Second. whether there were any significant differences in the use or nonuse of transfusion during total hip arthroplasty according to hospital characteristics and patient characteristics was compared. As for significant variables, the type of health care institution and the level of sickbed were found to have been statistically significantly related. Third, whether there were any significant differences in the presence or absence of diabetes among the total knee arthroplasty patients according to hospital characteristics and patient characteristics was compared. As for significant variables, the type of health care institution, the number of sickbed and anemia were found to have been statistically significantly related. In the case of the total hip arthroplasty patients, there were no variables that were significantly related.

Comparative Analysis of Health Administration and Policy through Inaugural Address of Minister of Health and Welfare (역대 정권별 보건복지부 장관의 취임사를 통한 보건행정 및 정책 비교분석)

  • Objectives: The purpose of this study is to comprehensively compare the trends of health administration and health policy in the field of health care using the semantic network analysis in the inaugural address of the Ministry of Health and Welfare of each regime in Korea. Methods: This study used a language network analysis method that uses Korean Key Words In Context (KrKwic) program and NetMiner program in sequence. The analysis was conducted by Minister Hwa-joong Kim during the Moo-hyun Roh government, Minister Jae-hee Jeon during the Myung-bak Lee government, Minister Young Jin of Geun-hye Park government and Government Jae-in Moon's inaugural address of Neung-Hoo Park Minister, respectively. Results: The key words differentiated by each regime are that the Moo-hyun Roh Government's Minister Hwa-joong Kim had high connection centrality values in the words 'balanced development', 'comprehensive' and 'reform'. Minister Jae-Hee Jeon of Myung-bak Lee Government had high connection centrality values in the words 'poverty' and 'return'. In the case of Minister Young Jin of Geun-hye Park Government had high connection centrality values in the words 'demand', 'Customized' and 'Life cycle'. In the case of Minister Neung-Hoo Park of Jae In Moon Government had high connection centrality values in the words 'Welfare state', 'Embracing' and 'Soundness'. Conclusions: If the role of health administration in the health care field and the health care policies are constantly changed according to the policies of each regime, it is inconsistent and it is difficult to approach from the long term perspective for public health promotion. In the future, health policy should be developed and implemented with a long-term perspective and consistency based on the consensus and participation of the people with less influence on the change and direction of each government's policies.

Association between alcohol and mental health in menopause and andropause middle-aged : Focusing on gender difference (갱년기 중년의 알코올과 정신건강의 연관성 : 젠더 차이를 중심으로)

  • Objectives: The purpose of this study was to provide baseline data for management policies by exploring the association between alcohol and mental health in menopause and andropause middle-aged Methods: This study used the data from from the 8th National Health and Nutrition Examination Survey (2019-2021). Adults between the ages of 40 and 64 were considered menopause and andropause middle-aged and 7,704 of the total number of 8,549 subjects were selected as the final study subjects without missing data. Mental health was defined as depression diagnosis and suicidal thoughts, and drinking was defined as current drinking and high-risk drinking. SAS 9.4 was used for all statistical analyses. Results: There were significant differences between menopause middle-aged men and women in current drinking (83.4% for men and 66.6% for women) and in high-risk drinking (24.2% for men and 4.9% for women). The significant factors affecting the depression diagnosis were identified as economic activity, income level, and subjective health status for men and economic activity, income level, subjective health status, and smoking for women (p < .05). The significant factors affecting suicidal ideation included economic activity, income level, and subjective health status for men and economic activity, high-risk drinking, current smoking, and subjective health status for women (p < .05). Conclusion: As a result of analyzing the association between alcohol and mental health in menopause and andropause middle-aged adults, Drinking alcohol and high-risk drinking in women were associated with mental health, but insignificantly in men. Based on this study, it is necessary to program development and health education for the physical and mental health of menopausal middle-aged women exposed to alcohol, and to develop menopausal mental health management policies that take into account the gender differences.
