Primary Neuroendocrine Carcinoma of the Breast: A Case Report and Literature Review (유방에서 발생한 원발성 신경내분비암종: 증례 보고와 문헌고찰)

  • In general, neuroendocrine cancer develops in the digestive or respiratory tract, and when it is found in other organs, it is often due to metastasis. Primary neuroendocrine carcinoma of the breast occurs very rarely, and the exact clinical picture, radiological findings, treatment and prognosis are not well known. Furthermore, only a small number of literature reports have been published. Here, we report the imaging findings of primary neuroendocrine carcinoma in the breast of a 51-year-old female, along with a literature review.

  • Thoracic foreign bodies (FBs) are serious and relatively frequent in emergency departments. Thoracic FBs may occur in association with aspiration, ingestion, trauma, or iatrogenic causes. Imaging plays an important role in the identification of FBs and their dimensions, structures, and locations, before the initiation of interventional treatment. To guide proper clinical management, radiologists should be aware of the radiologic presentations and the consequences of thoracic FBs. In this pictorial essay, we reviewed the optimal imaging settings to identify FBs in the thorax, classified thoracic FBs into four types according to their etiology, and reviewed the characteristic imaging features and the possible complications.

  • Objectives The purpose of this study was to investigate the possibility of spinal mobilization with leg movement (SMWLM) using Korean Medicine treatment on acute lower limb muscle weakness through four case studies. Methods SMWLM, with other Korean Medicine treatments, was performed on four patients who underwent treatment at OOOO Korean Medicine Hospital from August 2017 to July 2018. Additionally, results of the Numerical Rate Scale(NRS), EuroQol-FiveDimensions(EQ-5D),Oswestry lowback pain disability questionnaire(ODI), Straight Leg Raise(SLR) test, and Manual Muscle Test(MMT) were evaluated. Results There was a significant increase in NRS, ODI, and EQ-5D scales with im-rovements of SLR angle and MMT figures to a normal range. Conclusions This study suggested that Korean Medicine treatment combined with SMWLM may affect treating acute lower limb muscle weakness. Further clinical studies are needed to establish a definite conclusion.

The Effect of Intensive Integrative Korean Medicine Treatment of Patients with Adrenoleukodystrophy: A Case Report (부신백질이영양증 환자에 대한 한의 복합 치료의 효과: 증례보고)

  • The aim of this case report was to present the effects of intensive integrative Korean medicine treatment in a patient with lower back pain, both under extremity numbness, neck pain induced by adrenoleukodytrophy. A 49-year-old male patient was treated with Cheongpajeon-H, acupuncture, pharmacoacupuncture (Shinbaro2), chuna therapy for total 50 days of hospitalization. Lower back pain, both under extremity numbness, neck pain were assessed with a Oswestry disability index (ODI), Vernon-Mior neck disability index (NDI), numeric rating scale (NRS) and EuroQoL-5 dimension (EQ-5D). With 50 days of treatment, the pain was relieved from NRS 6 to 2. EQ-5D score increased from 0.75 to 0.779, ODI decreased from 34 to 15.56, NDI decreased from 30 to 15.56. These results show that lower back pain, both under extremity numbness, neck pain can be relieved with intensive integrative Korean medicine treatment. However, further well-designed studies are required to confirm these findings.

Symptomatic Hypocalcemia Associated with Dioscorea tokoro Toxicity (도코로(Tokoro)마 중독과 관련한 저 칼슘혈증)

  • Dioscorea tokoro has long been used in Korean traditional medicine as a pain killer and anti-inflammatory agent. A 53-year-old male who consumed water that had been boiled with raw tubers of D. tokoro as tea presented with numbness and spasm of both hands and feet. Laboratory results showed hypocalcemia, hypoparathyroidism, and vitamin D insufficiency. During his hospital stay, colitis, acute kidney injury, and toxic encephalopathy developed. The patient received calcium gluconate intravenous infusion and oral calcium carbonate with alfacalcidol. His symptoms improved gradually, but hypocalcemia persisted despite the calcium supplementation. We suggest that ingestion of inappropriately prepared D. tokoro can cause symptomatic hypocalcemia in patients with unbalanced calcium homeostasis.

  • Cerebellar Hippocampal and Basal Nuclei Transient Edema with Restricted diffusion (CHANTER) syndrome is characterized by an altered mental status. The acute MRI lesions show abnormal restricted diffusion imaging bilaterally and symmetrically in the cerebellum, hippocampus, and basal nuclei. This syndrome is an unknown syndrome and is presumed to be mainly an opioidinduced toxidrome. Here, we present a case study wherein we show that it can also be caused by an antidepressant overdose.

The Effect of Oriental Medicine Treatments for Supraspinatus Tendinopathy: Systematic Review and Meta-Analysis (극상근 건병증의 한의학적 치료에 대한 연구 동향: 체계적 문헌 고찰과 메타 분석)

  • Objectives The aim of this study was to analyze the trends of researches on oriental medicine treatments for supraspinatus tendinopathy. Methods We used five databases for searching researches; Korean studies Information Service System, Oriental Medicine Advanced Searching Integrated System, Research Information Sharing Service, PubMed, and China National Knowledge Infrastructure. Only randomized controlled trials suitable for the subject were selected. The methodological quality of included randomized controlled trials (RCTs) was assessed by the Cochrane risk of bias tool. Results Twenty randomized controlled trials were analyzed. There were 9 types of treatment interventions; acupuncture, acupotomy, pharmacopuncture, electroacupuncture, fire needling, warming needle, catgut-embedding therapy, herbal medicine, cupping. The most frequently used treatment intervention was acupuncture and acupotomy. There were 9 outcome measurements including visual analog scale (VAS), Constant-Murley Score (CMS), and range of motion. The most used measurement was VAS. As a result of meta-analysis, acupuncture was more effective than control group in VAS. Additionally, acupotomy was clinically significant compared to control groups in VAS and CMS. Conclusions In this review, we analyzed researches on effectiveness of oriental medicine for supraspinatus tendinopathy. A provisional conclusion can be produced that acupuncture and acupotomy showed beneficial effect to supraspinatus tendinopathy. Although there were some RCT studies, many of them had a high risk of bias, so it is hard to conclude that our study can include overall clinical status. Further well-designed trials are needed.

A Case Report of Golf-Swing-Induced T2~T3 Clay-Shoveler's Fractures (골프 초보자에서 나타난 흉추 2~3번 극돌기의 견열 골절)

  • Clay-shoveler's fractures are rare stress-type avulsion fractures of the spinous processes especially in sports. There have been two case reports that discussed clay-shoveler's fractures in golf. A 36-year-old beginner golfer presented with a pain in the back after practicing golf swing. No fractures were detected using cervical radiography; however, computed tomography (CT) and magnetic resonance imaging (MRI) revealed T2~T3 spinous process fractures. The patient was treated conservatively and his pain subsided. The mechanism of injury is speculated to that of clay-shoveler's fractures. Therefore, if a golfer suffers persistent pain in the cervicothoracic region, clay-shoveler's fracture is one possibility to consider.

Pain and Weakness on Unilateral Upper Extremity Diagnosed as Brachial Plexopathy after Herpes Zoster Infection (대상포진후 상완신경총병증으로 진단된 편측 상지의 통증과 위약)

  • Motor paralysis is a less common neurologic complication of herpes zoster. Until now, a few cases have been reported, and most of these cases showed brachial plexopathy involving one or two segments. We report a patient with pain and weakness on upper extremity diagnosed as brachial plexopathy after herpes zoster infection. An 88-year-old female patient complained not only tingling sense, pain, and swelling on right whole arm, but also weakness on this right upper extremity. On physical examination, weakness is seen in right shoulder abduction·shoulder flexion·elbow flexion·elbow extension· wrist extension (grade 4), finger flexion·finger abduction·finger extension·finger DIP flexion (grade 3). In electrodiagnostic study and magnetic resonance imaging study, she was diagnosed as the brachial plexopathy, whole branch involved. This is the only case of post-herpetic brachial plexopathy involving whole branch in domestic.

  • Peripheral facial palsy generally results from damage to the facial nerve. As facial asymmetry is observed, it accompanies not only functional problems but also psychological and social difficulties. Medical institutions treat most patients, however, there are still patients who rely on invasive methods by unqualified practitioners for fear of sequelae. This case describes a 61-year-old woman who experienced facial palsy twice. She visited our hospital with skin damage after folk patch therapy for her facial palsy. Combined Korean medicine treatment was administered during treatment period. After treatments, the symptoms of facial palsy and skin lesions improved. This case showed that Korean Medicine was efficacious in improving symptoms of facial palsy with damaged skin caused by folk patch therapy. Education and awareness of uncontrolled invasive treatments for facial palsy are needed.
