Adrenal Vein Sampling in Primary Aldosteronism: A Pictorial Essay for Optimal Left-Side Sampling (일차성고알도스테론혈증에서의 부신정맥채혈술: 최적의 좌측채혈을 위한 임상화보)

  • Primary aldosteronism (PA) is a curable cause of hypertension. Recent studies have revealed that the actual prevalence of PA is higher than previously recognized. Adrenal vein sampling (AVS) is an essential diagnostic procedure for revealing the cause of PA and determining the treatment plan. The success of AVS is confirmed by comparing cortisol levels between the samples from each adrenal vein and peripheral vein. The failure rate of the procedure is reported to be high in the right adrenal vein, which is directly connected to the inferior vena cava, while that in the left adrenal vein is relatively low; however, this has rarely been reported. In this review, we introduce and analyze cases of failure in left adrenal vein sampling.

  • Isolated left-sided portal hypertension is a rare clinical syndrome caused by splenic veno-occlusive disease. Splenic vein thrombosis and extrinsic compression causes proximal splenic vein hypertension, and the splenic blood flows into the superior mesenteric or portal vein through the upper stomach's collateral vessels, such as the short gastric, coronary, and gastroepiploic veins. Open splenectomy is recommended to treat gastrointestinal bleeding caused by isolated left-sided portal hypertension. Interventional management could be a clinically useful option for selected patients who want to avoid surgical corrections. The report presents two cases of left-sided portal hypertension with gastric variceal bleeding.

  • An umbilical venous catheter (UVC) is commonly placed for central venous access in preterm or critically ill full-term neonates to provide total parenteral nutrition (TPN) and medication. However, UVCs can result in complications, including infection, portal vein thrombosis, and hepatic tissue injury. The inadvertent administration of hypertonic fluid through a malpositioned UVC can also cause hepatic parenchymal damage with mass-like fluid collection that simulates a tumorous condition during imaging. Ultrasonography and radiographic examinations play an essential role in detecting UVC-related complications. This pictorial essay aims to present the imaging findings of UVC-related hepatic complications in neonates.

  • This study aimed to report the improvement in a vascular oculomotor nerve palsy patient treated with Korean medicine. A 68-year-old man with vascular oculomotor nerve palsy was treated with acupuncture, pharmacopuncture, and an herbal medicine (Gangwhalyufung-tang). This case was measured and assessed by the numerical rating scale (NRS), EuroQol-5 Dimension Index (EQ-5D), score of diplopia, and ratio of MRD1. The NRS decreased, and the patient showed improvement in EQ-5D, score of diplopia, and ratio of MRD1. Korean medicine treatments are considered helpful in treating diplopia and ptosis due to vascular oculomotor nerve palsy.

  • Subependymomas are rare benign central nervous system tumor which account of 0.7% of all intracranial tumors. Subependymomas show characteristic MR findings according to their location. However, sometimes atypical findings could be found. In addition, subependymomas can occur very rarely in the spinal cord. We report two cases of pathologically confirmed subependymoma, one of which shows atypical appearances in spite of their intraventicular location and the other of which shows rare case of spinal intramedullary subependymoma. We review the clinical symptoms and radiologic findings of two cases.

Changes in the characteristics of patients transferred to the emergency room through private institutions during inter-hospital transport after the COVID-19 outbreak : A retrospective study (COVID-19 발현 이후 병원간 이송 시 민간 이송업체를 통해 응급실로 전원된 환자들의 특성 변화 : 후향적 연구)

  • Purpose: The purposes of this study were to determine the changes in the pattern of patients who were transferred to the emergency room through inter-hospital private institutions and to determine how long transport takes following the COVID-19 outbreak. Methods: This retrospective observational study analyzed the emergency medical services reports of private institutions following the COVID-19 outbreak in South Korea. The study was conducted in Busan between January 19, 2019 and January 18, 2020, and between January 19, 2020 and January 18, 2021. Results: Upon comparing the patient transport times during the "Pre-COVID-19 period" and "COVID-19 period," a significant delay was noticed in the preparation for transfer of patients during the "COVID-19 period" (p<.05). There were significantly more patients with respiratory infections and patients who complained of general symptoms during the "COVID-19 period." Moreover, there was a higher frequency of patients who were transferred to a 'Level I' emergency room during the "COVID-19 period" compared to during the "Pre-COVID-19 period" (p<.05). Conclusion: Following the COVID-19 outbreak, there is a delay in patient transport to the emergency room through private institution inter-hospital transport and an increase in the number of patients complaining of respiratory infection symptoms. Thus, emergency medical services need additional administrative and economic support to transport infected patients.

Hepatic Pseudolymphoma Mimicking a Hypervascular Tumor: A Case Report (과혈관성 종양으로 오인된 간의 가성림프종: 증례보고)

  • Hepatic pseudolymphoma is a rare benign liver mass that is characterized by proliferation of non-neoplastic lymphocytes extranodally. To the best of our knowledge, only 46 cases have been reported in the English literature. We described the case of a 75-year-old woman with hepatic pseudolymphoma mimicking a hypervascular tumor. After the histological confirmation of the rectal neuroendocrine tumor, CT scan revealed a 1.0 cm-sized, poorly-defined and low-density nodule in the liver. On MRI, the hepatic nodule showed an arterial enhancement and a low-signal intensity on the hepatobiliary phase. On diffusion-weighted imaging, the hepatic nodule showed a high signal intensity on a high b-value. On fluorodeoxyglucose (FDG) positron emission tomography (PET)/CT, it revealed a high standardized uptake value nodule. The US showed the hypoechoic nodule and the US-guided biopsy confirmed the hepatic pseudolymphoma.

A Study on Characteristic of Image Quality according to CT Table Height in Computed Tomography (컴퓨터단층촬영 검사 시 테이블 높이에 따른 화질 특성에 관한 연구)

  • In addition to protocol adjustments during CT examinations, the height of the CT table can also affect image quality. Therefore, this study aimed to investigate the change in image quality depending on the height of the table in brain CT, which accounts for a large proportion of CT examinations, by measuring signal to contrast to noise ratio (CNR) and noise power spectrum (NPS) using the head phantom and evaluating them. The head phantom images were acquired using Philips Brilliance iCT 256. When the image was acquired, the table height was adjusted to 815, 865, 915, 965, 1015, and 1030 mm, respectively, and each scan was performed 3 times for each height. The CNR result showed the highest value at 965 mm, which is the height adjacent to the center of the head phantom. NPS showed the lowest NPS at 915 mm, the center of the head phantom in the low frequency region. From these results, it can be seen that the height of the table in CT examination is closely related to the image quality, and it can be seen the characteristics of image quality according to CT table through quantitative evaluation methods such as CNR and NPS.

Trends in Domestic and International Clinical Research on Korean Medicine Treatments for Triangular Fibrocartilage Complex Injury (삼각섬유연골 복합체 손상의 한의학 치료에 대한 국내외 임상 연구 동향)

  • Objectives The purpose of this study is to analyze the trends in domestic and international clinical research on the Korean medicine for triangular fibrocartilage complex (TFCC) injuries and to contribute to future domestic research. Methods In this study, clinical studies on TFCC injuries treated with Korean medicine were searched through databases such as Research Information Sharing Service, ScienceON, DBpia, China National Knowledge Infrastructure, PubMed, Medline, and Embase. Fifteen clinical studies were selected, of which publication years, intervention methods, assessment tools, and various other factors were analyzed, including potential side effects. Results Among the selected 15 studies, total of 13 intervention methods were used. Electroacupuncture was the most frequently used method (40%), followed by herbal medicine (33%), acupuncture (26.7%), chuna manual therapy (26.7%), pharmacopuncture (20%), acupotomy (13%), herbal ointment (13%), hot medicinal compress therapy (13%) and etc. Most studies showed that Korean medicine treatments effectively alleviated TFCC injuries, with visual analogue scale being the most commonly used assessment tool. Conclusions Korean medicine treatments for TFCC injuries appears to be effective; however, further research is needed to enhance understanding. Large-scale clinical trials comparing the efficacy of Korean medicine treatments, along with investigations into side effects and recurrence rates are necessary. By doing so, the safety and efficacy of Korean medicine treatments for TFCC injuries can be further established.

Parosteal Lipoma Associated with a Growing Osteochondroma of the Right Ilium (우측 장골능선의 골연골종과 연관된 방골성 지방종의 증례 보고)

  • Parosteal lipoma is a rare type of lipoma, the incidence being approximately 0.3% of all lipomas. Moreover, parosteal lipoma coexisting with osteochondroma is extremely rare. A few cases with coexistence of osteochondroma and parosteal lipoma have been reported and they were thought to be reactive changes of adjacent bone by parosteal lipoma. However, temporal relationship of these tumors could not be explained. Here, we report a case of parosteal lipoma associated with osteochondroma of the right ilium developed over 6 years, with follow-up radiographs.
