간호사의 병원 내 성희롱 피해 경험: 현상학적 연구 (Experience Sexual Harassment of Nurses: Phenomenological Study)

  • 본 연구는 간호사가 경험한 성희롱의 본질적 구조와 의미를 파악하여, 성희롱을 예방하고, 체계적인 중재방안을 마련하고자 수행되었다. Giorgi의 현상학적 연구방법과 심층면담에 의한 연구로서 자료수집은 2022년 8월 6일부터 2023년 3월 25일까지 종합병원 간호사 7명의 연구참여자로부터 수집하였다. 면담에 사용된 주질문은 "귀하에게 성희롱이란 무엇을 의미합니까?"이었다. 필사된 자료는 Giorgi의 질적 분석방법에 따라 전체의 인식, 의미단위의 구분, 의미단위의 심리학적 표현으로의 변형, 일반적 구조로의 통합의 단계를 거쳐 분석하였다. 그 결과 총 159개의 의미단위, 37개의 본질적 심리의미, 13개의 하위 구성요소, 5개의 구성요소가 도출되었다. 5개의 구성요소는 '성희롱의 최전방에 노출됨', '혼자 감당하며 고군분투함', '조직으로부터 내팽개쳐진 심정', '함께 헤쳐나갈 방법을 찾음', '변화하고 있음을 느낌'이 포함되어 있다. 이상의 결과를 토대로 간호사의 성희롱을 예방하고 이들을 위한 체계적인 중재방안 마련과 제도 개선에 유용한 기초자료를 제공할 것으로 기대된다.

간호단위의 팀 효과성에 대한 고찰 (A Literature Review of Team Effectiveness on Hospital Nursing Units)

  • Purpose: The purpose of this study was to analyze literature related to nursing team effectiveness and to summarize the definition variables included, measurement tools, and findings. Basic data on operation and research for team effectiveness in nursing units was sought. Methods: A literature search was performed to identify all studies published between 2003 and 2016 from periodicals indexed in PUBMED, CINAHL, SCIENCE DIRECT, RISS, KISS, and NAL databases. The following keywords were used in the search: 'team effectiveness', 'nurse', 'nursing', and 'hospital'. Ten studies were analyzed. Results: The analysis included domestic and international literature on nursing team effectiveness. The foreign literature included studies of various organizations including nurses, use of various research tools, leadership programs and/or new nursing delivery systems. In the Korean studies, most of the research on team effectiveness surveyed nurses on team satisfaction, team commitment, and team performance in general nursing units, operating rooms, and intensive care units. Conclusion: The findings show the necessity to develop a definition of team effectiveness that can be accepted comprehensively in nursing organizations in Korea. The definition should reflect team effectiveness that includes all cooperating units not only nurses of the nursing unit but also all other related health care teams.

관리자 리더십이 조직문화 및 조직몰입에 미치는 영향;병원 수간호사를 중심으로 (he Effects of Managerial Leadership on Organizational Culture and Organizational Commitment;The Case of Hospital Head Nurses)

  • Purpose : The purpose of the present study was to examine the causal relationships among head nurses' leadership (consideration oriented leadership, structure initiated leadership), nursing unit culture (human development culture, task development culture), and nurses' organizational commitment. Method : Four large general hospitals located in and around Seoul were selected to participate. The total sample of 286 registered nurses represents a response rate of 90 percent. Data for this study was collected from August to September in 2000 by questionnaire. Path analyses with LISREL program were used to test the fit of the proposed model to the data and to examine the causal relationships among variables. Result : Both the proposed model and the modified model fit the data adequately. The model revealed relatively high explanatory power. 52% of human development culture was explained by predicted variables and 44% of task development was explained by predicted variables; 47% of the organizational commitment was explained by predicted variables. In predicting developmental (both human and task) nursing unit culture, the findings of this study clearly demonstrated that head nurses' consideration oriented leadership might be the most important variable. The result also showed that head nurses' consideration oriented leadership might be the most important variable among the predicted variables in predicting organizational commitment. Conclusion : Based on the findings of this study, it was suggested that various studies, for example, matching leadership and nursing unit culture, were needed.

간호사의 팀워크가 직무만족에 미치는 영향: 자기효능감의 매개효과 (Effects of Nurses' Teamwork on Job Satisfaction at Hospital: Mediating Effect of Self-efficacy)

  • 본 연구는 환자를 간호하면서 발휘하는 간호사간의 팀워크가 간호사의 직무만족에 미치는 효과를 확인하고, 팀워크와 직무만족 인과관계에서 자기효능감의 매개효과를 규명하기 위해 시도한 서술적 연구이다. Y시 소재 600병상이상 대학병원 한 곳의 최소 2명 이상의 간호사들이 함께 근무하는 간호단위조직에서 직접 및 간접간호를 담당하고 있는 근무경력 3개월 이상의 간호사 304명을 대상으로, 구조화된 설문지를 사용하여 자료를 수집하였으며, 다중회귀분석과 Sobel Test(1982)등을 이용하여 분석하였다. 간호부서의 팀워크는 5점 만점 중 평균 3.78(${\pm}0.594$)점, 직무만족은 3.30(${\pm}0.542$)점, 그리고 자기효능감은 2.80(${\pm}0.271$)점 이었다. 팀워크가 간호사의 직무만족에 유의한 영향을 미치고 있었다(F=33.324, p<.001). Baron과 Kenny(1986)의 분석한 결과 자기효능감이 팀워크와 직무만족 관계상에서 부분매개효과가 있었으며, Sobel test 결과 역시 자기효능감 매개효과가 유의한 것으로 나타났다(Z=5.25, p<.001).

임상간호사의 업무소진, 대인관계 능력, 전문직 자아개념과 우울간의 관련성 (Relations of Burnout, Interpersonal Relations, Professional Self-concept and Depression in Clinical Nurses)

  • 본 연구의 목적은 임상간호사의 업무소진, 대인관계 능력, 전문직 자아개념 및 우울에 대해 파악하고, 이들 변수와 우울간의 관련성을 파악하는 것이다. 본 연구는 충청도 소재의 A종합병원 8개의 간호단위에서 근무하는 임상간호사 150명을 대상으로, 구조화된 설문지를 이용한 자가보고식 설문조사로 2012년 12월 10일부터 2013년 1월 30일까지 시행되었다. 연구결과, 업무소진은 대인관계능력, 전문직 자아개념과 음의 상관관계를, 우울과는 양의 상관관계를 나타냈다. 대인관계 능력은 전문직 자아개념과 양의 상관관계를, 우울과는 음의 상관관계를 나타냈고, 전문직 자아개념은 우울과 음의 상관관계를 나타냈다. 본 연구에서 일반적 직업관련 특성 중 우울에 유의한 차이를 보였던, 연령, 결혼상태, 근무경력, 근무부서와 주요변수인 업무소진, 대인관계 능력, 전문직 자아개념 중 업무소진 만이 우울에 유의한 영향을 미치는 변수로 파악되었고, 이 변수의 우울에 대한 설명력은 43%로 나타났다.

병원 간호단위관리자의 다면평가($360^{\circ}$ feedback)에 관한 연구 (A Study on $360^{\circ}$ Feedback of Nursing Unit Manager in a Hospital)

  • Purpose: The purpose of this study was to develop 360 feedback for nursing unit manager based on the current personnel evaluation system and to evaluate the new tool according to evaluation subjects. Total of 277 subjects of nurse unit managers and staff nurses were participated in this study. Method: The study was conducted in three phases each for development, application, and analysis of 360 feedback. SAS program was utilized for data analysis with descriptive statistics, t-test, and analysis of variance. Result: The evaluation criteria of the developed 360 feedback tool consisted of 13 subscales such as professional knowledge, apprehension & judgement, job performance, applicability, creativity, leadership, responsibility, promptness & accuracy, administrative ability & sense of mission, activeness, cooperation, communicability, and general attitude. The internal consistency of the tool was Cronbach's alpha .939. The evaluation score by! peers(M=4.30) was the highest one, followed by self-evaluation(M=4.23), evaluation by supervisor(M=4.17), and evaluation by subordinate(M=4.10). The differences in the total evaluation scores among the subjects supervisor, self, peer, and subordinate were not statistically significant, but significant differences were found in some subscales scores. Conclusion: Further research is required to test the reliability and validity of the $360^{\circ}$ feedback tool, and to test the outcome and the process of $360^{\circ}$ feedback system.

일개대학 간호학과 졸업 후 5년 이내 간호사의 교육목표 달성도와 영향요인 -비판적 사고 성향과 임상수행능력을 중심으로 (Evaluation of Nursing Educational Objectives Achievement & Related Factors in Nurses Within Five Years After Graduation: Focusing on Critical Thinking Disposition and Clinical Competence)

  • 본 연구는 졸업생의 교육목표 달성도, 비판적 사고 성향과 임상수행능력 정도를 파악하고, 교육목표 달성도와 비판적 사고 성향, 임상수행능력 간의 상관관계와 교육목표 달성도에 영향하는 요인을 확인하고자 시행하였다. 연구대상자는 간호학과를 졸업 한 후 병원에서 근무하는 간호사로서 대학 졸업 후 5년 이내 졸업생 82명과 그 졸업생을 지도하는 간호관리자 68명이였다. 자료는 2014년 1월부터 3월까지 수집하였다. 수집한 자료는 SPSS/WIN 23.0 프로그램을 이용하여 기술통계, t-test, ANOVA, pearson correlation과 multiple regression으로 분석하였다. 연구 결과 졸업생의 교육목표 달성도는 평균 $3.60{\pm}0.60$점/6점, 비판적 사고 성향은 평균 $3.46{\pm}0.28$점/5점, 임상수행능력은 평균 $4.17{\pm}0.56$점/6점이었다. 교육목표 달성도는 비판적 사고 성향(r=0.52, p<.001), 임상수행능력(r=0.52, p<.001)과 양의 상관관계를 보였다. 교육목표 달성도에 영향을 미치는 요인은 비판적 사고 성향(${\beta}=.30$, p=.018)과 임상수행능력(${\beta}=.26$, p=.029)이었고, 회귀분석 모형의 설명력은 34.25%이었다(F=6.65, p<.001). 간호단위 관리자가 평가한 졸업생들의 교육목표 달성도와 임상수행능력이 졸업생들이 자가 평가한 것보다 높았다(p<.001). 이러한 결과는 앞으로 교육목표 재설정과 교육과정 개선에 반영될 것이며, 교육목표 달성도를 높이기 위한 전략을 수립하는 데 기초자료로 활용할 수 있을 것이다.

전산 시스템이 도입된 일개 종합병원 간호단위의 간호시간 산정에 관한 연구 (A Study on the Nursing Time in Nursing Units in Hospital to Applied Computer System)

  • This study was done to determine direct and in-direct nursing time in nursing units in hospital to ap-plied computer system and expect that those results contribute to measure efficiency of nursing practice and planning of nursing manpower. The design for study was a descriptive study. The study results are as follows. 1. Patients admitted to the Internal Medicine and General Surgery units were grouped into four. Group Ⅰ, 32.9% of the total patients, consisted patients whose condition was considered minor Group Ⅱ, 26.1%, was of those whose condition was considered moderate, Group Ⅲ, 41. 8%, moderate severe and Group Ⅳ, 29.2% the most severe. 2. Nursing intervention times by care type were as follows four minutes spent for suction eight minutes, for simple position change, ten minutes, for sheet change, seven minutes for a hot or cool compress, six minutes for dressing change, four minutes for Ⅰ.M. injection, six minutes for patient health education and five minutes for body temperature check. 3. Direct care time by patient group revealed the following : Group Ⅰ rquired 191.4 minutes, Group Ⅱ required 331.1 minutes, Group Ⅲ rquired 499.4 minutes, and Group Ⅳ rquired 1328.0 minutes. 4. The ratio of time for adequate nursing care and direct care time in the Internal Medicine and Goneral Surgery units was 67.4%-83.4% and 94.7%-99.3% in the Intensie Care Unit. 5. Average daily direct care time per patient was 5.5 hours in the Internal Medicine unit and 11.5 hours in the Intensive Care Unit. 6. Time spent in indirect care was 48.3 minutes for computer recording,34.8 minutes for giving and receiving patient information for shift duty, 28.0 minutes for eating and resting time, 26.6 minutes for transfering and identifying patients, 25.6 minutes for identifying Doctor's order, 23.9 minutes for recording vital signs. 7. Time spent in indirect care was 282.2 minutes by head nurses (charge nurses), 258.7 minutes by nurses and 261.6 minutes by nurse aids. 8. The average nurse's workload was 9.3 hours and daily indirect nursing time required 46.3%-50. 5% of above mentioned workload time. .9. The average daily indirect care time per patient was expected to be 57.7 minutes in the Internal Medicine unit and 3.3 hours in the Intensive Care Unit.

일개 대학병원의 소아혈액종양 간호단위의 간호업무량 측정 (Calculation of nursing care hours in a pediatric oncology nursing unit)

  • The shortage of nursing personnel was become one of the most serious problems in operating pediatric oncology nursing unit which was the first pediatric oncology nursing unit in Korea. The purpose of this study was to estimate the optimal number of nursing personnel by calculating nursing care hours. The subjects were 13 staff nurses and inpatients of pediatric oncology nursing unit at Seoul National University Hospital during the period of May 20, 1996, to June 2, 1996. The number of nurses' duty was 132, the number of patients treated was 1288 for these 2 weeks. The tools used for this study were pediatric patient classification indexes and direct & indirect care indexes. Each nurse measured the time that they spent for their activities by self record under the supervision of their nurse manager. The method used to calculate the number of nursing personnel was multiplication of the average number of nursing care hours per patient per day with the number of patients. Percentage, average, t-test, F-test were used for data analysis. The results of this study were as follows : 1) The distribution of patient class : Class I & II none, Class III 86.8%. Class IV 12.9% 2) Direct nursing care hours for a patient per shift according to patient classification: Class III : 27.64 minutes, Class IV : 54.64 minutes The average direct nursing service hours for a patient per shift(3 shift) was 31.54 minutes(94.62 m/day). The average indirect nursing service hours for each patient per duty(3 shift) is 21.3 minutes (63. 91 m/day). 3) The average nursing hours for a patient per duty was 52.80 minutes(2.64h/day). 4) The group of administering medications in direct care activities showed the highest percentage (38.9%). Checking vital signs among observation took the most time am.ong each direct care activity (6.88 minutes for a patient per duty). 5) Charting took the most time of each indirect care activity(52.53 minutes/ duty/nurse). 6) The average personal time per duty is 29.40 minutes, which 'was below 30 minutes of this hospital regulations. 7) The average nursing hours that a nurse provided for a duty was 8.60 hours, which meant that a nurse worked 1.10 hours overtime. 8) Standardizing to a 33 bed to a unit, 17 nurses were needed at the present nursing level.

중환자실 전담간호사 역할에 대한 중환자실 의료인의 중요도와 만족도 (The Importance and Satisfaction of Intensive Care Unit Health Care Providers with Intensive Care Unit Clinical Nurse Expert's Role)

  • 본 연구는 중환자실 전담간호사 역할에 대한 중환자실 의료인의 중요도와 만족도를 파악하기 위하여 시행되었다. 일개 기관의 4개 단위 내·외과계 중환자실 소속 의사, 간호사 총 138명을 대상으로 2021년 5월 3일부터 29일까지 설문을 시행하였다. 수집한 자료는 기술통계, t-test, Importance-Performance Analysis(IPA) 방법을 사용하여 분석하였다. 중환자실 전담간호사의 역할에 대한 중요도는 5점 만점에 의사 4.18점, 간호사 3.95점, 만족도는 5점 만점에 의사 4.39점, 간호사 3.85점으로 중요도(t=2.33, p=.024)와 만족도(t=5.43, p<.001)가 두 군에서 모두 통계적으로 유의한 차이가 있었다. 두 직종 모두 전문간호실무, 전문직 활동, 조정협동, 교육상담, 연구 순으로 중요도와 만족도 순위가 일치하였다. 집중개선 영역으로는 의사는 실무 표준 및 가이드라인 개발, 전문가 역할 모델과 자기 개발과 환자/가족 교육으로 나타났고, 간호사는 의료진 교육으로 평가하였다. 중환자실 전담간호사 역할에 대하여 전문성을 강화하고, 개선이 필요한 부분의 역량을 증진시키는 전략이 필요하다고 생각된다.
