A Study on the Functional Efficiency according to Composition of Single Bedroom in Ward - Focused on Distance from NS to Bed - (종합병원 병동부 1인 병실 적용에 따른 기능효율성에 관한 연구 - 간호거점에서 병상간의 거리를 중심으로 -)

  • Purpose: This study examines distance from nurse station to bed according to composition of single bedroom. Methods: This study has two groups of target sample ward planes. The first group has diverse patients bedroom. The other group is 36 single bedrooms and 2 3-bedrooms. The guideline of distance measurement was instituted for consistency. This study has 3 main concepts of the guideline. 1) The distance is shortcut from NS to bed. 2) The traffic line is center line between walls and center of door. 3) The start point is center of NS and the end point is bed. Results: The result of this study can be summarized into two points. The first one is that single bedroom has good privacy and longer distance from NS to bed. The second one is that weak point of single bedroom was asked ward operating system and architectural planning like as patients room, bed per unit, location of NS and plan type. Implications: Consideration Should be taken into account for the effective bedroom composition and allocation in ward. This Stuy hopefully serve as a stepping stone for the standard design of space program in ward planning.

The Influencing Factors forming the Atmosphere of Ward (병실 분위기 조성에 영향을 주는 요인)

  • Working as nurses, authors found that some patients were not content with their hospital life. Some patients wanted to move to another ward, and others complained about their ward atmosphere. In spite of patients' discomfort, nurses didn't know what made some patients complain about their ward. So, authors tried to find factors that influence atmosphere of hospital wards of the neurosurgery patients. To know the atmosphere of wards, authors selected five neurosurgery ward of a university hospital in Seoul. Observation took a month. An author observed using molar approach, in the morning, in the day time, and in evening time. Authors used concealment /no intervention technique. An author checked condition of people in the wards, and observed their verbal and nonverbal communication behavior their activities and environmental characteristics, and interpreted their meaning through ethnographic research methodology by Spradly. Authors found there was an important factors that influence the atmosphere of ward. It was a human and his attitude. At least one person who was willing to help others made ward atmosphere better. Helping others physically whenever needed, sup-porting the depressed emotionally, offering foods, or talking to others friendly brought good atmosphere. On the contrary, if everyone was indiffernt to others, the atmosphere became cold. Self-centered or selfish behaviors such as occupying too much area, using the toilet too long, covering other's suction bottle without permission and seeing others delicate or urinate were hurtful. In addition to the attitude of patients and their families, unkindness of medical teams including nurses and doctors and tasteless meal caused bad ward atmosphere. Based on this research finding, authors suggest the followings. A For the better atmosphere of ward 1. Nurses should try to make the ward atmosphere better by introducing new patient to older ones. 2. Every ward should have dividing curtains to keep patient's privacy. 3. All hospital personnel should be kind enough to make patients feel that they are repected. 4. Hospital should serve high quality meals to patients. 5. Patients had better stay with those in the similar condition. B. For the future studies 1. Repeated researches are necessary to check reliability of this results. 2. Researches for patients in different area such as ICU, or hemodialysis unit are necessary.

A Study of Evacuation in Elderly Care Facilities Fire (요양시설 화재 시 피난에 관한 연구)

  • The development of science and technology has increased the average life expectancy and resulted in a significant increase in the elderly population. Due to the preponderance of the nuclear family in modern society and the weakening of the spirit of filial piety and respect for the elderly caused by the influence of Western civilization, elderly people are usually not living with their family, but have become dependent on elderly care facilities and, consequently, the number of elderly care facilities has increased significantly. On May 28, 2014, a fire occurred in the elderly care facilities in Jangseong, South Jeolla Province, and despite the rapid extinction of the fire by firefighters, 21 of the 28 casualties died due to smoke inhalation. This study was conducted for the purpose of providing measures to prevent smoke from entering into the rooms from which elderly people have difficulty evacuating in the case of fire. The use of the air curtain method to prevent smoke inflow was proposed and an experiment was conducted in which smoke was generated with a smoke generator. As a result of the experiment, it was confirmed that more than 90% of the smoke inflow was blocked if the door was closed in the room where the air curtain was installed.

Design and Implementation of Hospital Room Management System Based on IoT CareBots (IoT 케어봇 기반 병실 관리 시스템의 설계 및 구현)

  • Recent advances in network infrastructures, sensors, and IoT devices have accelerated the research and development of monitoring and interaction technologies designed for people, buildings, and environments. In particular, there has been an increasing demand for monitoring technologies for vulnerable people such as the infirm, disabled, and children. In this paper, we propose an efficient hospital room management system based on IoT care robots. The status of hospital rooms can be monitored and controlled more efficiently and intuitively by utilizing IoT devices and a cloud platform. We demonstrated the feasibility of the proposed system through the implementation of a prototype based on ARTIK IoT devices and the ARTIK Cloud platform. We found that the proposed system requires approximately 600 ms and 130 ms to collect sensor data and respond to alerts, respectively, which demonstrates it can operate in real-time.

The Methicillin - Resistant Rate of Staphylococcus Aureus Isolated from the Nares and Throat of Patients Admitted to Medical Intensive Care Unit (내과계 중환자실 입원환자의 비,인후 배양에서 메치실린내성 황색포도구균의 빈도)

  • Background : Methicillin-resistant Staphylococcus aureus (MRSA) is an important pathogen in hospital-acquired infection, and is prevalent in intensive care units (ICU). The MRSA colonization rates of the nares and throat were examined in both the ICU and general ward. This study was performed to investigate the MRSA rate and necessity for MRSA screening cultures in patients admitted to ICU. Methods : Between June and September 2004, those patients admitted to both the medical ICU and general ward participated in this study. Bacterial cultures were performed on swabs of the nares and throat taken within 24 hours of admission. Clinical data were also collected. Results : One hundred and twenty one patients and 84 patients, admitted to the medical ICU and medical general ward, respectively, were investigated. The numbers of nasal MRSA colonization in the ICU and general ward were 3 (2.5%) and 3 (3.6%), respectively. There were 2 (1.7%) cases of throat MRSA colonization in the ICU, but none in the general ward. The MRSA colonization rates of the nares and throat were no different between the ICU and general ward. There were no significant differences in the previous admission, operation history and admission route between the ICU and general ward groups. Conclusion : The MRSA colonization rates of the nares and throat were 3.3 and 3.6% in the ICU and the general ward, respectively. The MRSA screening test does not appear to be required in all patients admitted to the ICU, but further studies, including high-risk patients, are recommended.

Improvement Way for Mobile X-ray Examinations by Rule Revision about Safety Management of Diagnosis Radiation Occurrence System (진단용방사선발생장치의 안전관리에 관한 규칙 개정에 따른 이동형 방사선검사의 개선방안)

  • A safety management rule of the diagnosis radiation system which opened a court 2006 February 10th was promulgated for safety of the radiation worker, patients and patients' family members. The purpose of this study is to minimize injury by radiation that can happen to patients and people around a sick ward when managing mobile X-ray system. This study analyzed sickroom environment of mobile X-ray examination and the statistical data of the Konkuk medical Information System(KIS) and the Picture Archiving Communication System(PACS). This study also investigated patient conditions, infection, relation information and related data, when the sickroom mobile X-ray examination is used. Through data analysis, many problems were expected such as restriction of space side, manpower and expense of business side, satisfaction degree decline of patient and protector of operation side. Therefore, we tried to restrict examination of multi bed sickroom, and to use treatment room in each ward to solve problem mentioned. As a result, the whole sickroom mobile X-ray examination rate decreased to near 50%, and mobile X-ray examination rate for inpatients decreased to more than 85%. This study shows that several attempts we did should be helpful for manpower, patients satisfaction and expenses. Also, they should protect patients in sickroom from unnecessary radiation exposure and could minimize inconvenience of patients and their family members from x-ray examination.

Nurses' Perception, Knowledge, and it's Affecting Factors on Performance of Nursing Practice in Hospital with No Guardian (보호자 없는 병실 간호사의 간호업무에 대한 인식, 지식과 수행도에 영향을 미치는 요인)

  • The purpose of this study was to investigate on paid hospital-nurses' work perception, knowledge, performance and the performance-related factors of a nurse who works in a hospital with no guardian which is phased in its beginning as well as interactions between the factors. Data were collected from nurses working in wards without guardian at 9 hospitals among a total of 10 Korean hospitals carrying out a national pilot project which is designed for institutionalization of national caregiving services. Self reported questionnaires were used to collect data from 167 nurses, and 135 returned forms were analyzed(December, 2010). The score of perception was 4.24/5.00, knowledge 4.25/5.00, and performance 3.49/5.00. The performance was significantly different depending on the hospital types(p=.002). Significant correlations were found among perception(p<.001), knowledge(p=.004) and performance. The degree of perception(p<.001) and types of hospital(p<.001) attributed to 16.5% of variance in the practice. To enhance level of performance by taking account of perception and types of hospital, can be effective for hospital-nurses when there is no guardian.
