  • Authors evaluated 152 patients at the department of Pediatric Dentistry in Seoul National University Hospital through clinical records and radiographs. And the following features were studied ; age, sex distribution, number of mesiodens per patients, location, status of eruption, shape and orientation of crown, and complication. From the above results, the relationship between features of mesiodens and complications were evaluated using chi-square analysis. 1. Complications due to the presence of mesiodens did not occur in 31.6%, delayed eruption of adjacent teeth was observed in 33.6%, midline diastema in 22.4%, rotation in 8.6%, displacement in 3.3%, and crowding in 0.7% of all evaluated patients. 2. As compared with the above 8.5 year group, in the under 8.5 year group, the frequency of complications was significantly higher(P<0.05). As compared with those positioned lingually, in mesiodens labially or within the arch the frequency of complications was significantly higher(P<0.01). Also, the frequency of complications was significantly higher when the mesiodens was tuberculate in form(P<0.05). 3. Of the 104 patients with complications, the frequency of delayed eruption was significant higher in the under 8.5 year group, and in above 8.5 year group, the frequency of malocclusion was significantly higher(P<0.05). When mesiodens were located in the midline region, the frequency of malocclusion was significant higher, while in case with laterally positioned mesiodens the frequency of delayed eruption was significantly higher(P<0.01).

  • Supernumerary teeth are teeth added to the normal complement of teeth. They are most often found in the maxillary anterior region. Most supernumerary teeth in the maxillary anterior region have complications, such as impaction or malposition of permanent teeth, formation of diastema, cysts, and eruption into the nasal cavity or into the oral cavity. These complications have influence on deciding the treatment and its prognosis is depends on how these complications are treated. In clinical studies, it has been found that the removal of supernumerary teeth before the age of 5 years resulted in significant fewer eruption problems of the permanent tooth compared to removal at 7 years of age or later. However, such an early removal may leads to serious consequences of tooth development and behavioral management of patients. Therefore clinicians must consider complications of supernumerary tooth when deciding the time of removal. These cases report five-type of complication associated with supernumerary tooth such as diastema, delayed eruption, external root resorption, migration into nasal cavity and cyst formation.

  • Authors evaluated 1171 patients at the department of Pediatric Dentistry in Kyoungpook National University Hospital through clinical records and radiographs. And the following features were studied ; age, sex distribution, number of mesiodens per patients, cause of discovery of mesiodens, location, status of eruption, shape and orientation of crown, complication, dental age of adjacent upper incisor tooth at the time of surgical extraction of mesiodens and the relationship between the resolution rate of complication after extraction of mesiodens and the dental age of adjacent upper incisor tooth, length of diastema, the eruption status of lateral incisor and the crowding status of premaxilla. The followings are the results : 1. The cause of discovery of mesiodens were as follows ; delayed eruption of the permanent incisors in 13.8%, midline diastema in 11.6%, radiographs taken for other reasons in 23.4% and for caries treatment in 15.1%. 2. Complication due to the presence of mesiodens did not occur in 36.8%, delayed eruption of adjacent teeth was observed in 16.1%, midline diastema in 34.4%, rotation in 8.8%, displacement in 2.4%, and crowding in 1.0% of all evaluated patients. 3. As for the dental age of adjacent upper incisor tooth at the time of surgical extraction of mesiodens, below 1/3 of total root length were observed in 3.5%, $1/3{\sim}1/2$ of total root length in 19.9%, $1/2{\sim}2/3$ of total root length in 54.7% and over 2/3 in 21.9% of all evaluated patients. 4. Resolution rate of delayed eruption after the extraction of mesiodens was significant higher in the group with the root length below 1/2. Resolution rate of midline diastema was significant higher in the group with diastema width below 3mm and with non-crowding of adjacent upper incisor teeth.

  • The aim of this study was to analyze the three dimensional characteristics of the size, shape and location of mesiodens and their effect on permanent teeth arrangement using Cone-beam computed tomography(CBCT). 191 patients under 15 years old who had visited the hospital between June 2006 and December 2012 and taken CBCT were investigated. Among them, 43 patients with diastema were analyzed to investigate the correlation between the amount of diastema and the mesiodens. The results showed that mesiodens were found in males 3.9 times more than females and found most in 7 years old. The mean length of mesiodens was 13 mm and the width was 6.8 mm. The conical shape comprised of 52.9%. Many mesiodens were located in the palatal side rather than the labial side and located vertically near the crown. 63.0% of mesiodens were reversed and the common complications were delayed eruption followed by diastema, and teeth rotation. And the amount of diastema had correlation with size and mesiodistal position of the mesiodens. The current study used 3D analysis to show the size and position of mesiodens and occurrence distribution of complication. Among these, the amount of diastema had shown the correlation with the size and mesiodistal position of mesiodens.

  • Mesiodens is defined as a supernumerary tooth in the premaxillary region. It can cause several clinical manifestations in normal eruption and position of adjacent teeth, such as displacement and impaction. Although the mesiodens accompanied by a clinical complication is indicated for removal, the optimal time of mesiodens removal is still controversial. Some authors suggest immediate intervention defining the removal of mesiodens as soon as possible after the first detection. On the other hand, others recommend delayed intervention which denotes the removal of mesiodens after complete root development of adjacent teeth. This case report is presented with three cases of spontaneous correction and proper alignment of rotated maxillary central incisors by extraction of mesiodens when the crowns of rotated incisors were completely formed while the roots of them were at an early developmental stage.

  • The bulging of mandibular angle area, frequently, make some difficulties in social life due to its reckless appearence, especially in oriental nations. So, many authors had reported its causes and surgical techniques for correction since late 19th century with diagnosis of masseter hypertrophy. But in spite of its muscular origin, major surgical techniques, in general, should aim complete bony reduction or osteotomy and supplemental myotomy as in hemimandibular hypertrophy or mandibular elongation. We used ultrasonogram for soft tissue depth estimation and expearenced some complications such as incomplete lingual cortical bony reduction, condylar fracture in mandibular angle reduction via intraoral circumvestibular approach. So we reported our surgical technique for angle reduction with possible complications.

  • The etiology of mesiodens is unknown but the most widely accepted theory is the hyperactivity of the dental lamina. Complications of mesiodens are delayed or prevented eruption of maxillary central incisors, displacement or rotation of permanent teeth, crowding of affected region, abnormal diastema or permanent space closure, dilaceration or abnormal root development of permanent teeth, primordial or follicular cyst formation, root resorption of adjacent teeth, eruption into nasal cavity. If mesiodens rotate the maxillary central incisors, space deficiency is not common and relapse is very common. So overcorrection is needed. To prevent the rotational relapse, early treatment, overcorrection, long retention period, properly formed proximal surface, use of coupled force, and surgical techniques have been suggested. The authors present two cases, whose chief complain were severely rotated maxillary incisors by mesiodens, treated by orthodontic and surgical technique and showed good results.
