  • 세기관지를 침범하는 많은 질환들이 세기관지에 유사한 병리소견을 일으킴으로 이들의 원인, 병인, 진단을 위하여서는 임상 및 병리소견을 연관시킴이 거의 필수적이다. Small airways disease를 포함한 세기관지 질환의 병리소견 특히 호흡성 세기관지염, bronchiolitis obliterans organizing pneumonia, constrictive bronchiolitis obliterans 등에 대하여 중점적으로 기술하였다. BOOP은 어떤 급성 폐손상에 대한 비특이성 소견이며 조직병리적인 기술이다. 반면 Idiopathic BOOP은 COP와 동의어로써 특정한 질환이며 이의 주된 병리소견이 BOOP인 것으로 이해하는 것이 좋겠다. Constrictive bronchiolitis obliterans는 임상적으로나 병리학적으로 BOOP과는 다르다고 하겠다.

  • 캡슐내시경은 일반적인 내시경의 접근이 어려운 소장을 관찰하는 데 특화되어 있다. 보통 캡슐내시경 영상을 판독하기 위해 의사는 해부학적 지식과 병리학적 지식과 같은 도메인 지식을 활용하여 영상에 내재된 정보를 획득한다. 이와 같은 내재된 정보는 도메인 지식을 활용하여 분석될 수 있는 의미적 정보라고 할 수 있다. 본 논문에서는 의미적 정보를 추출하는 과정을 의미 기반 분석이라 정의하고, 의미 기반 분석 과정에서 요구되는 도메인 지식 모델링 기법을 제안하고자 한다. 캡슐내시경의 의미 기반 분석 과정은 크게 이미지 특징 추출, 해부학적 분석, 병리학적 분석으로 구성되며, 이와 같은 분석 과정에 활용될 수 있는 해부학 모델과 병리학 모델을 제안한다. 제안하는 모델은 캡슐내시경 영상의 주요 대상 기관인 소장에 특화된 모델로 효과적인 영상 판독을 보조할 수 있음을 보인다.

  • 본 연구에서는 자궁종양 세포를 정상, 비정상으로 진단하기 위한 세포핵의 특성값 추출 방법으로 3차원 형태학적 분석 방법을 제안한다. 컨포컬 현미경을 이용하여 3차원 볼륭데이터를 획득하고 3차원 연결 성분 레이블링을 적용하였다 레이블링 후, 각각의 세포핵으로부터 3차원 형태학적 특성값을 추출하였으며 정상세포핵과 비정상세포핵의 3차원 형태계측에 대한 차이를 비교하였다. 이는 잘린 단면의 각도나 두께에 따라 서로 다른 분석 결과를 나타내는 2차원 영상분석방법의 한계를 극복할 수 있으며 실체에 가까운 계측으로 보다 객관적이고 정확한 병리진단을 위한 보조도구로써 활용될 수 있다.

  • 유방 질만 분석을 위한 병리진단에서는 암종세포의 세관형성 정도와 핵의 다형성과 유사분말정도를 기준으로 하여 나누는 방법이 용이하고 재현성이 높다. 그 중에서도 세포핵의 크기와 다형성, 핵과 세포질의 비율은 양성종양과 악성종양을 분류하는데 있어서 중요한 요소중의 하나이다. 그러므로 본 논문에서는 유방 질단 영상에서 세포핵을 추출하여, 핵의 형태학적 특징값인 핵의 연적 둘레, 가로·세로(장·단축)의 길이를 구하고 핵과 세포질의 비율을 계산한 후, 추출한 형태학적 특성 칸들이 양성종양과 악성종양으로 분류하는데 있어서 유의한 특성 값인지 비교·분석하였다.

  • Thymic carcinoid tumor is a rare mediastinal tumor, which was firstly described by Rosai and Higa in 1972. A carcinoid tumor of the thymus has recently been regarded as a distinct tumor from thymoma, and is probably Kultschizky cell origin. The pathologic diagnosis of thymic carcinoid is made from findings from light microscopy, immunohistochemical studies and electron microscopy. About 50% of thymic carcinoids were seen with endocrinopathies. Recurrences and extrathoracic metastasis are characteristics of thymic carcinoids. Surgical removal of the intial and tumor recurred are considered to be the most effective treatment today. However, the role of the adjuvant radiotherapy and the chemotherapy is still uncertain. Herein we report a case of thymic carcinoid tumor, which was confirmed by operation and pathologic study.

Investigation of Diseases of Thai koi, Anabas testudineus (BLOCH) from Farming Conditions in Winter (겨울철 양식장 환경에서 Tai koi, Anabas testudineus (BLOCH)의 임상병리학적 특징)

  • Investigation on health conditions of Thai koi (Anabas testudineus) were carried out through clinical and histopathological observations from different farms of Mymensingh district for seven months during August 2006 to February 2007. Fish sampling and water quality parameters (temperature, dissolved oxygen and pH) were monitored on a monthly basis. Clinical examination of fishes was also carried out for any kind of abnormalities at monthly intervals. Samples of skin, muscle, gill, liver and kidney were observed by histological technique. Among the water quality parameters the values of water temperature, dissolved oxygen were found to be at unfavorable level for fish during the colder months in the farms. Clinically it was observed that fishes were more affected from December to January and almost normal in appearance during August to September and February. Different clinical symptoms like scale loss, dermal lesion, ulcer and loss of caudal fin were noticed in December and January. In histopathological study, structures of fish organs were normal from August to September. In the months of October and November, minor pathologies were found to be started. Marked pathological changes like necrosis, pyknosis, inflammation, hemorrhage, hypertrophy, hyperplasia, missing of primary and secondary gill lamellae were observed in the months of December and January. Whereas, in the month of February the pathological condition of fish gradually reduced. Again when considered individual fish farm, fishes of Reliance Aqua Farm were more affected than Sotota Matshya Hatchery. The study showed that severity of clinical and pathological changes were increased in December and January. During the period EUS and protozoan diseases were noticed in A. testudineus of the investigated farms.

  • 목적: 피부조직과 같은 미세 인체구조 연구를 위해 고해상도 3T MRI 시스템에 적합한 고분해능의 RF surface coil을 개발하고 있다. In vivo 연구를 위한 여러 parameter를 최적화하여 기능영상에도 부합된다. 비침습적인 In vivo 검사에 의한 세포수준의 극 미세구조의 연구가 가능해짐으로써 과거 시행하던 침습적인 생검없이 각종질환의 진단적 접근이 병리학적 수준으로 향상되어 질병의 정확한 진단이 가능해지게 될 것이다.

Effect of Testicular Histopathology on Pregnancy Outcomes in Non-Obstructive Azoospermia (비폐쇄성 무정자증 환자에서 고환의 조직병리학적 진단에 따른 체외수정시술 결과의 비교)

  • Objective: To evaluate outcomes of patients with non-obstructive azoospermia (NOA) undergoing the testicular sperm extraction (TESE) combined with intracytoplasmic sperm injection (ICSI) with different histopathologic subgroups. Method: A total of 122 embryo-transferred TESE/ICSI cycles were compared among NOA subgroups; Germ-cell aplasia (GA, 40 cycles), Maturation arrest (MA, 32 cycles) and severe hypospermatogenesis (S-HS, 50 cycles). Obstructive azoospermia (OA, 667 cycles) patients were served as a control. TESE/ICSI outcomes such as fertilization rate (FR), clinical pregnancy rate (CPR) and live birth rate (LBR) were evaluated. Results: The 2PN FR of embryo-transferred TESE/ICSI cycle was 58.1% in GA, 42.2% in MA and 48.0% in S-HS, which was significantly lower than that of OA (72.9 %, p<0.001). For ICSI-spermatozoa cycles, there were no significant differences in CPR (22.6%, 29.4% and 26.1%) and LBR (16.1%, 29.4% and 19.6%) among NOA subgroups. The CPR of ICSI-spermatid cycles was 0.0%, 9.1% and 0.0% without a live birth. For ICSI-spermatocyte cycles, no clinical pregnancies occurred in any group. Conclusion: There was no significant difference in the FR of embryo-transferred TESE/ICSI cycles among NOA subgroups. The FR among all NOA subgroups was significantly lower than that of OA. Testicular histopathology in NOA did not affect successful pregnancy if spermatozoa extraction from the testis is successful and embryo transfer is possible.

Histologic Accuracy of Diagnostic Methods in Lung Cancer (폐암에서 진단수기의 조직학적 분류의 정확성에 관한 연구)

  • Background : Identification of the histologic cell type of lung cancer is important because it is related to the treatment modality and prognosis. Currently, many diagnostic methods are used to determine the cell type. We have studied to evaluate the histologic accuracy of each diagnostic methods in lung cancer. Method : 168 cases were analysed retrospectively, who underwent curative thoracotomy for lung cancer in Seoul National University Hospital from January, 1994 to February, 1997. Sputum cytology, percutaneous needle aspiration(PCNA), percutaneous needle biopsy(PCNB), bronchial washing cytology and flexible bronchoscopic biopsy were evaluated respectively. The k coefficient was applied to evaluate the degree of concordance between the histologic diagnosis obtained by each methods and the one derived from thoracotomy. Results : The k value in sputum cytology was 0.86. In the 34 cases with squamous cell carcinoma(SQ), 32 cases were finally diagnosed as having such by thoracotomy. 7 of the 8 cases with adenocarcinoma(AD) turned out to have the same. The k value in PCNA was 0.51. In the 31 cases with SQ, only 14 cases were finally diagnosed as having such. All of the 3 cases with small cell carcinoma(SC) turned out to have the same. The k value in PCNB was 0.77. The diagnosis was correct in 13 of the 16 patients with SQ and in 30 of the 32 cases with AD. The k value in bronchial washing cytology was 1.0. In all of the 29 cases with SQ and all of the 7 cases with AD, the diagnosis was correct. The k value in flexible bronchoscopic biopsy was 0.77. The diagnosis was correct in 51 of the 52 cases with SQ and in 1 of the 2 cases with SC. Conclusion : The concordance rate with the final histologic diagnosis in sputum cytology, PCNB, bronchial washing cytology and flexible bronchoscopic biopsy were excellent ($k{\geq}0.75$), while that in PCNA was fair (k=0.53). Because PCNA showed lower concordance rate than other diagnostic methods, PCNA is recommended to perform with PCNB.

Comparison of the Usefulness of Diagnostic Tests for Rheumatoid Arthritis (류마티스 관절염 진단을 위한 검사의 유용성 비교)

  • Rheumatoid arthritis (RA) is a chronic inflammatory disease, which is mainly characterized by disease of joints affected with synovial hyperplasia, pathological immune response, and progressive destruction; all of which represent an important social health problem. These provide new insights in its pathogenesis of rheumatoid arthritis diagnosis and disease progression in molecular changes. This review focuses on new serological and immunological markers which seem to be useful in early diagnosis and prognosis of rheumatoid arthritis. Therefore, such tests are widely conducted for serological biomarkers and the developments with such immunological factors to identify patients who are at risk for disease progression. This evidence of the disease based on laboratory medicine could provide the best outcome for patients. Finally, data from recent studies will help to refine the ultimate usefulness of this novel approach for early diagnosis, treatment, and helping clinicians to optimize therapy by using this approach.
