Quantitative Micro-CT Evaluation of Microleakage in Composite Resin Restorations (Micro-CT를 이용한 복합 레진 수복물 미세 누출도의 정량 분석)

  • One of the most important and basic test of dental restorative materials is the evaluation of microleakage into the tooth-restorative interface. There are many techniques to test microleakage, but most of them have several disadvantages. Recently developed microtomography(micro-CT) can provide the three dimensional image and information about the internal component in non-destructive way, therefore using micro-CT, it is possible to evaluate microleakage exactly in quantitative manner. The purpose of this study is to find a new method for quantitative and non-destructive evaluation of microleakage in composite resin restorations using micro-CT and to compare the new method with conventional dye penetration method. Thus, microleakages of two kinds of dentin bonding systems were evaluated with above two methods. 40 extracted sound human premolars were randomly divided into two groups consisting of 20 samples and restored accordingly. Group 1 : Class V resin restorations with $Adper^{TM}$ Singe Bond Group, 2 : Class V resin restorations with $Adper^{TM}\;Promp^{TM}$ L-pop. The $Filtek^{TM}$ Supreme was applied to the Class V cavities of all teeth. After that, 10 teeth from each group were applied to evaluation of microleakage using micro-CT, and other 10 teeth from each group were using conventional dye penetration method. The conclusions of this study were as follow : 1 Using micro-CT, Group 1 showed significantly less microleakage than Group 2 and there was statistically significant difference(p<0.01) between two groups. 2. Using conventional dye penetration method, Group 1 leaked less than Group 2 and there was statistically significant difference(p<0.01) between two groups 3. The difference between two groups is more evident in the method using micro-CT. 4. In all two methods, microleakage appeared more into the cavities to dentinal margins than enamel margins.

THE EFFECTS OF PHOTOPOLYMERIZATION OF ADHESIVE ON SHEAR BOND STRENGTH AND MICROLEAKAGE OF FLOWABLE RESIN RESTORATION (유동성 레진 수복술에서 접착제 중합 여부에 따른 미세누출과 전단결합강도에 관한 연구)

  • This study was performed to evaluate possibility to reduce treatment time for child patient who have limited control activity during flowable composite resin restoration according to comparing microleakage and shear bond strength. Group I, II with Single $Bond^{TM}$, group III, IV with AQ Bond $Plus^{TM}$, group V, VI with Prompt L-$Pop^{TM}$, and group VII as control group without adhesive system. 12 premolars and 10 molars were assigned into each group. Restorative material was used the Filtek $Z350^{TM}$ flowable. The results were as follows; 1. The result of microleakage evaluation, in the group I, II with Single $Bond^{TM}$ showed low dye penetration score. Dye penetration between group I and group II were similar. 2. Results for shear bond strength at group I and group II showed values higher than other groups. (p<0.05) There were no statistical differences between group I and group II. (p>0.05) 3. There was no statistical differences among I and II, III and IV, V and VI groups. 4. Group VII showed significantly low shear bond strength than group I, II, V, VI(p<0.05). Group VII showed low value than group with III and IV, but no statistical significances. In conclusion, adhesive bonding resin was not affect significantly to microleakage and shear bond strength of flowable resin restoration. Therefore, to reduce the operation time, adhesive bonding resin and flowable composite resin can be cured at once. But contamination of saliva, location of cavities can affect to bond strength in clinical field.

MOLAR RESTORATION WITH AN ORTHODONTIC BAND (교정용 밴드를 이용한 구치부 수복)

  • The use of stainless steel crowns are indicated for restoration of primary or permanent molars with proximal dental caries, extensive dental caries, or previous pulp treatment with increased danger of tooth fracture. Stainless steel crowns were introduced by Humphrey in 1950. For their improved durability, longevity, and success rate, they have been strongly considered for restoring extensive and multi-surfaced dental caries of molars in pediatric dentistry. However, they also have shortcomings, such as possibility of pulpal exposure or damaging proximal surface of adjacent teeth. In addition, when oversized stainless steel crowns are used, eruption of the adjacent permanent teeth may be disturbed by their prominent margin. As a means to compensate the shortcomings of stainless steel crowns, use of orthodontics bands may be considered. It is an alternative restoration method, where an orthodontic band is placed on a tooth first and cavity is restored with filling material, such as composite resin, glass ionomer, or amalgam. The use of an orthodontic band is indicated for molar restoration with cervical dental caries, extensive dental caries, enamel hypoplasia, or previous pulp treatment. Because it requires shorter chair time compared to stainless steel crown, its application is very useful for children with poor behavior. However, restoration using an orthodontic band requires good oral hygiene after its application. This case report illustrates the conservative restoration of primary molars and permanent molars with extensive dental caries using orthodontic bands.

EFFECT OF CAVITY DISINFECTANT ON THE BOND STRENGTH AND MICROLEAKAGE OF DENTIN BONDING AGENTS (와동 세척제가 상아질 결합제의 결합에 미치는 영향)

  • Incomplete removal of bacteria contaminated dentin or enamel associated with caries is a potential problem in restorative dentistry Secondary or residual caries, pulpal inflammation and hypersensitivity may result from bacteria left after the initial preparation, especially if an adequate seal against microleakage is not obtained. A possible solution to eliminate residual bacteria left in a cavity preparation would be to treat the cavity with cavity disinfectant wash. But a potential problem with using a cavity disinfectant with dentin bonding agents could be their interference with the ability of the resin to bond to the tooth micromechanically. The purpose of this study was to evaluate the effect of 2% chlorhexidine containing cavity disinfectant ($Consepsis^{(R)}$) on shear bond strength and microleakage of dentin bonding agents, $Adper ^{TM}$ $Scotchbond^{TM}$ Multi-Purpose, $Adper^{TM}$ Single Bond and $Adper^{TM}\;Prompt^{TM}\; L-Pop^{TM}$ Sixty and sixty sound human third molar teeth, respectively, were used for shear bond strength and microleakage test. For experimental group, cavity disinfectant was applied before dentin bonding agents, and was not applied for the control group. The result from the this study can be summarized as follows ; 1. Use of 2% chlorhexidine containing cavity disinfectant($Consepsis^{(R)}$) does not significantly affect the shear bond strength of dentin bonding agents. 2. Use of 2% chlorhexidine containing cavity disinfectant($Consepsis^{(R)}$) does not significantly affect the microleakage of dentin bonding agents.
