The Development and Effects of Timeout Protocol for Safety in Cataract Surgery Patients (백내장 수술 환자 안전을 위한 타임아웃 프로토콜 개발 및 효과)

  • Purpose: This study was conducted to develop effective timeout protocol in coordination with current practice and test its clinical effectiveness in pursuit of safety management for patients undergoing cataract surgery. Methods: A total of subjects were 60 women, 50~65 years old, who visit C ophthalmology clinic in D city. They were assigned to 30 experimental group and 30 control group, respectively. Based on the comprehensive literature review, timeout protocol that was suitable for patients undergoing cataract surgery was developed, and then test its effectiveness by measuring blood pressure, pulse, anxiety and sense of well-being among surgical patients. Results: The timeout protocol was found to reduce blood pressure, pulse, and anxiety and increase well-being among surgical patients. Conclusion: As a results, it is necessary to introduce an effective timeout protocol giving positive responses to surgical patients, hence it should be develop a timeout protocol and explore the effectiveness of the protocol.

Surgical Management of Coexisting Glaucoma and Cataract (녹내장과 백내장이 동반된 환자의 수술적 치료)

  • The management of coincident glaucoma and cataract is not only a common clinical challenge but also an important research topic in the ophthalmic surgical field. The purpose of this article is to compare the different surgical options on the basis of their achievable postoperative intraocular pressure (IOP) control, success rates, and complication rates reported in the related literature, and to give advice on how to manage typical situations of patients with both glaucoma and cataract. Main topics were focused on indications and rationale of 3 surgical options (only cataract surgery first and later trabeculectomy, only trabeculectomy first and later cataract surgery, or simultaneous combined surgery). Modern clear corneal cataract extraction techniques resulted in a modest intermediate-term reduction of IOP and has considerably improved the success rates of combined glaucoma and cataract surgery. It also enabled future trabeculectomy to be successfully performed at a later date if necessary. Trabeculectomy alone achieved better IOP regulation than phacotrabeculectomy (combined surgery), but subsequent cataract surgery may compromise preexisting filtering bleb. Combined surgery augmented with mitomycin C achieved a lower IOP than combined surgery alone but had a higher complication rate. In conclusion, the choice of the preferred surgical method should be determined according to the target pressure, the amount of glaucomatous damage, and the grade of visual disturbance caused by the cataract. Phacotrabeculectomy with adjunctive mitomycin C offers visual improvement and achieves the best IOP lowering of all types of combined glaucoma and cataract surgery currently used but is associated with potentially sight-threatening complications.

Determinants of Multidimensional Outcomes of Patient Satisfaction in Operated Cataract Patients (백내장 환자의 수술후 진료만족도의 다면적 평가와 결정요인)

  • This study was to compare multidimensional outcomes of patient's satisfaction after cataract surgery and to identify factors influencing satisfaction after operation. Patient's satisfaction was measured with three dimensions : interpersonal care, physician explanation and hospital care. Overall satisfaction was measured as means of three dimensional scores. For the study, a prospective study was performed with 389 patients who had undergone cataract surgery for either one eye or both eyes. The surgery was performed by 20 ophthalmologists who were practicing at university hospitals and general hospitals. Patients were interviewed and clinical data (the visual acuity of operated eye, visual function, symptom score and satisfaction with vision) were obtained. The doctors were questioned with self-reported questionnaire forms. Medical records were also examined to understand surgery Process. The survey was conducted before(389) and after operation(327). Alter excluding cases with incomplete data, 3n cases were enrolled In this study. Both the overall satisfaction and the satisfaction with physician explanation increased after the operation whereas the satisfaction with interpersonal care and hospital care did not change significantly. Multiple regression analysis showed that the level of education, baseline satisfaction scores and the degree of vision improvement were statistically significant variables. The preoperative lower level of education, higher level of overall satisfaction (interpersonal care, physician explanation, hospital care scores) and the more the satisfaction with vision improvement were associated with the improvement of postoperative satisfaction scores.

The Effects of the Hand-holding Program as an Independent Nursing Intervention for the Patients with Anxiety in Local Anesthesia (국소마취 백내장 수술환자의 불안에 대한 직접간호중재로서의 손잡아주기 프로그램 효과)

  • Operation usually brings about psychological stress as well as physical stress to the patient and it greatly influences the maintenance of the homeostasis. Until now, most of the nursing research concerning the anxiety of the patient undergoing an operation has been done while the patient was under general anesthesia. Under local anesthesia, the anxiety of the patient who undergone operation is short. However, there is much demand of the operational nurses to mediate between the patient who is undergoing operation when fully conscious, as they are aware of the patient's anxiety and to decrease the patient's anxiety of this local anesthesia. Therefore, this research tries to verify the effects of the Hand-holding program on the anxiety of the operational patient undergoing local anesthesia and affirms the role of the nursing intervention. As an unequivalent control group non-synchronized designs, this research selects out 20 control groups and 21 experimental groups at random who have received cataract operation while under local anesthesia and after implementing the Hand-holding program to the experimental groups while undergoing the operation, the effect of the experiment was observed. By observing the Epinephrine, Cortisol, blood pressure, and pulse for the physical indexes before and after the enforcement of the hand-holding, and using the measurement of Speilberger(1972) for the psychological indicators for the anxiety of the patient before and after hand-holding, the results were analyzed through SAS program. As a results, intervention of hand-holding had a remarkable effect to decrease the anxiety felt by the patient and the diastolic blood pressure also was effectively decreased. On the other hand, through the decrease of the systolic blood pressure was also observed, it was not statistically significant. In the Epinephrine and Cortisol indicators, the intervention of hand-holding was revealed to have an insignificant effect. The Hand-holding was revealed to have an indicators, the intervention of Hand-holding was revealed to have an insignificant effect. The Hand-holding program may be said to have an effect which decreases the psychological anxiety of the patient undergoing local anesthesia.

Photochromic lens for patients with photophobia and estimation of clinical trial efficacy (광시증 환자를 위해 개발된 광변색렌즈와 임상 효과 평가)

  • Photochromic lens is the color changing lens from colorlessness to colorness when exposed to UV light. It can protect eyes from UV-B light which cause the cataract and can be used as sunglass in the summer since it can cut off 70~80% of sun light. Surface coating technique was used to develop lens which has 70% light transmittance and of which fading time of color change is within 5 min. Various color lenses were developed so that these had various color such as blue, green, brown, violet, yellow and red etc. Lens has an excellent physical properties, 100% adhesion and 4~5H hardness. The chemical and the scratch resistance of this lens were the greatest and the thermal stability was also higher. The clinical trial of developed lens were applied to 65 patients who had photophobia by various reasons at ophthalmology of Soonchunhyang University Hospital. Results showed that glaring was significantly reduced and the visual health were remarkably improved. Especially, anti-glaring effect in the night was great for the patient who had the LASIK operation. Protection of UV-B and blue light also can prevent the patient from cataract and the yellowish crystalline lens at old ages. Through the result of clinical. trial, we know that photochromic lens could be a new technique for both cure and precaution of photophobia.

A Case of Suprachoroidal Hemorrhage after Pars Plana Vitrectomy for Rhegmatogenous Retinal Detachment (열공망막박리 환자에서 유리체절제술 후 발생한 맥락막상강출혈 1예)

  • Purpose: To report a case of a 60-year-old female with rhegmatogenous retinal detachment, presenting with suprachoroidal hemorrhage after vitrectomy. Case summary: A 60-year-old woman visited our clinic complaining of floaters, flashing, and blurred vision. Best-corrected visual acuity was 0.1 in her left eye, and fundus examination of her left eye revealed macula-involved retinal detachment with a retinal break at the superotemporal quadrant. She underwent cataract surgery, 23-gauge transconjunctival sutureless vitrectomy, and 14% C3F8 gas tamponade under general anesthesia. One hour after anesthesia recovery, she suddenly complained of severe pain in her left eye. The intraocular pressure measured after removal of the pressure patch from her left eye was as high as 58 mmHg. Her ocular pain improved spontaneously within 10 minutes, and the intraocular pressure decreased to 8 mmHg. Fundus examination of her left eye revealed a reddish-brown raised lesion, suggesting suprachoroidal hemorrhage. She was placed in a prone position with a pressure patch over her left eye. Bleeding through the sclerotomy site was observed 1 day after surgery. Subsequently, hemorrhagic choroidal detachment of her left eye continued to decrease without deterioration. Three weeks after surgery, the patient received an intravitreal injection of 100% C3F8 gas into her left eye. At 3 months after surgery, best-corrected visual acuity had improved to 0.8 in her left eye, and the retina was stable. Conclusions: Suprachoroidal hemorrhage may be suspected in a patient complaining of severe ocular pain after vitrectomy.

Clinical Effects of an Improved Pump Reaction Rate and Automatic Occlusion Sensing System in Phacoemulsification (수정체유화장치의 초음파 출력속도 및 자동막힘감지 기능 향상의 술 후 임상결과 비교)

  • Purpose: A recently introduced phacoemulsification system, the WhiteStar $Signature^{(R)}$ PRO, has demonstrated improved nucleus followability and cutting efficiency via an improved pump regulator with a higher reaction response and an automatic occlusion sensing system. In this study, we compared various phacoemulsification parameters between the new system and an older version of the device. Methods: A total of 80 eyes of 68 patients with cataracts who had undergone phacoemulsification by a single surgeon were included in this study. Forty eyes of patients underwent phacoemulsification using the older $Signature^{(R)}$ system (WhiteStar); these patients were classified as the control group. Another 40 eyes of patients underwent phacoemulsification with the newer enhanced system, the WhiteStar $Signature^{(R)}$ PRO; these patients were assigned to the experimental group. During the operation, operative parameters, including the effective phaco time (parameter of effective phaco time with a specific coefficient for the transversal movement expressed in seconds, EFX), ultrasound time (seconds [s]), effective phacoemulsification time (EPT, s), average phacoemulsification power (AVG, %), and balanced salt solution usage, were measured to determine the performance enhancement offered by the updated system. Central corneal thickness was measured before and after surgery to compare corneal edema. The relationships between the two groups were analyzed using an independent t-test. Results: The Signature $PRO^{(R)}$ system showed a lower EFX (p < 0.001), a shorter EPT (p < 0.001), and a smaller AVG (p < 0.001). Postoperative corneal thickness did not differ significantly between the two groups. Conclusions: Comparing the efficacy of the improved reaction speed of the device and automatic occlusion sensing system in performing phacoemulsification, the updated Signature $PRO^{(R)}$ system demonstrated superior followability and cutting efficiency regardless of nuclear cataract hardness.

A Case of Urologic Manifestation of IARS2-associated Leigh Syndrome (IARS2 유전자 연관 리 증후군(Leigh syndrome) 여아에서 방광기능장애 증례)

  • Leigh syndrome is a rare progressive neurodegenerative mitochondrial disorder with clinical and genetic heterogeneity. Recently, balletic IARS2 variants have been identified in a number of patients presenting broad clinical phenotypes from Leigh and West syndrome to a rare syndrome CAGSSS characterized by cataracts, growth hormone deficiency, sensory neuropathy, sensorineural hearing loss, and skeletal dysplasia syndrome (OMIM#616007). We describe a child with Korean Leigh syndrome with urologic manifestations resulting from a compound heterozygote mutation in IARS2. A 5-year-old girl visited the emergency room with a complaint of abdominal pain accompanied by abdominal distension. Abdominal-pelvic CT showed a markedly distended urinary bladder without definite obstructive lesions. She was diagnosed with neurogenic bladder dysfunction based on a urodynamic study. She had global delayed development due to neurologic regression after 6 months of age and a history of bilateral cataract surgery at the age of 2 years. Her brain magnetic resonance imaging showed symmetrically increased signal intensities in the bilateral putamen and caudate nuclei with diffuse cerebral atrophy. No gene variants were identified through whole-mitochondrial genome analysis. Whole exome sequencing was performed for diagnosis, and compound heterozygous pathogenic variants were identified in IARS2: c.2446C>T (p. Arg816Ter) and c.2450G>A (p. Arg817His). To the best of our knowledge, this is the first case report of bladder dysfunction manifestation in a patient with IARS2-related Leigh syndrome. Thus, it broadens the clinical and genetic spectrum of IARS2-associated diseases.

Factors Affecting Outcomes in Cataract Surgical Patient - Pilot study - (백내장 수술환자의 진료결과에 미치는 요인 - 사전 연구를 중심으로 -)

  • Background : This study is to identify preoperative patient characteristics associated with a lack of improvement on one or more measures peri operatively, postoperative 3-4 months, and postoperative 12 months. Methods : For the assessment, prospective study was performed with 92 patients who had undergone either one eye or both eye cataract surgery by 3 ophthalmologists practicing at a university hospital. The criteria of improvement were (1) Snellen visual acuity, (2) a cataract-related symptom score(possible range: 0, 0 of 6 symptoms present or bothersome, to 18, all 6 symptoms very bothersome), and (3) VF-14 score - a measure of functional impairment in patient with cataract - (possible range: 0, inability to perform any of the applicable activities, to 100, no difficulty in performing any of the applicable activities). Results : Although 14 patients (15.2%) failed to improve on one or more of the outcome measures assessed, no one failed to improve on all three measures. Both eyes of surgery than one eye, preoperative cataract symptom score of 1-4, 5 or higher than 0 were associated independently with the increased likelihood of improvement (odds ratio 8.95, 7.16, 8.87 respectively). And the preoperative level of Snellen visual acuity was not associated with the likelihood of improvement Conclusion : We conclude that specific preoperative characteristics (both eyes, cataract symptom score) are independent predictors of patient outcome after cataract surgery.

Scotopic Pupil Size in Myopes (근시안에서 암순응상태의 동공크기)

  • Purpose: This research was performed to measure and analyze scotopic pupil size in myopes and to figure out the factors that influence it. Methods: The pupil size of 191 healthy myopic subjects were measured with the pupillometer (Colvard pupillometer, OASIS medical, USA) in scotopic and analyzed with the age, corneal size, spherical equivalent refractive error, corneal curvature. In addition, it was compared with the measurements of intra-examiner and inter-examiner to verify reproducibility of pupillometer. Results: The mean (${\pm}$SD) scotopic pupil size was $6.64{\pm}0.68$ mm (range, 5.00~8.00 mm), the lower age and the larger corneal size, The bigger the pupil size. The lower spherical equivalent refractive error and steepper corneal curvature tends to be smaller. The reproducibility of intra-examiner and inter-examiner in pupillometer showed the reliability highly (Guttman splithalf point > 0.91). Conclusions: The pupil size associated with age, corneal size, spherical equivalent refractive error and corneal curvature in scotopic condition. It can refer to prevent inconvenience that may occur RGP contact lenses, cataract surgery and refractive surgery.
