The Research Relating to QA of the Absorbed Dose in the 10 MeV E-beam Facility in Accordance with the International Standards (국제표준에 따른 10 MeV급 전자빔 조사시설의 흡수선량 품질보증에 관한 연구)

  • In the field of healthcare, the conventional sterilization treatments have been replaced by irradiation methods which are in accordance with internationally well established quality standards. The quality control in radiation sterilization assures that the absorbed dose of the irradiated material is in agreement with its requirements and standards. The electron beam irradiation requires technical assessments of more process parameters than gamma irradiation does. Korea has witnessed wide uses of electron accelerators since early 2000 but there hasn't been research experiences relating to quality system in accordance with international standards. The new large scale e-beam irradiation system with the specification of 10 MeV, 8 kW was installed and operated in 2008 by Seoul Radiology Services Co. It consists of the electron accelerator, product handling system, safety, documentation and control subsystems into an integrated system to meet the requirement of the Good Manufacturing Practice such as process quality assurance and management of product tracking records. To implement the international standard such as EN ISO11137, it is necessary to understand the purposes aimed in the standard and carry out the tests following the procedures required. This study presented the specification of the e-beam facility and showed what its design requirements and features are. The test results on a variety of process parameters were presented and validated it they are within the required limits.

Comparison of ESD and Major Organ Absorbed Doses of 5-Year-Old Standard Guidelines and Clinical Exposure Conditions (소아 5세 표준촬영 가이드라인과 임상 촬영조건의 입사표면선량과 주요 장기흡수선량 비교)

  • Pediatrics are more sensibility to radiation than adults and because they are organs that are not completely grown, they have a life expectancy that can be adversely affected by exposure. Therefore, the management of exposure dose is more important than the case of adult. The purpose of this study was to determine the suitability of the 10 year old phantom for the 5 year old pediatric's recommendation and the incident surface dose, and to measure the organ absorbed dose. This study is compared the organ absorbed dose and the entrance surface dose in the clinical conditions at 5 and 10 years old pediatric. Clinical 5 year old condition was slightly higher than recommendation condition and 10 year old condition was very high. In addition, recommendation condition ESD was found to be 43% higher than the ESD of the 5 year old group and the ESD of the 10 year old group was 126% higher than that of the 5 year old group. The recommended ESD at 5 years old and the ESD according to clinical imaging conditions were 31.6%. There was no significant difference between the 5 year old recommended exposure conditions and the organ absorbed dose due to clinical exposure conditions, but there was a large difference between the Chest and Pelvic. However, it was found that there was a remarkable difference when comparing the organ absorbed dose by 10 year clinical exposure conditions. Therefore, more detailed standard exposure dose for the recommended dose of pediatric should be studied.

Evaluation of Skin Dose of Intensity Modulated Radiation Therapy in Breast Cancer Patients (유방암환자의 세기조절방사선치료에서 피부선량 평가)

  • In the case of radiotherapy following breast conservation therapy for breast cancer patients, the characteristic of skin dose was investigated in the treatment of intensity modulated radiation therapy (IMRT) for breast cancer patients by comparing and analysing entrance skin dose irradiated during radiotherapy using tangential technique radiotherpy, and IMRT. The calculation dose irradiated to breast skin was compared with TLD measurement dose in treatment planning by performing the two methods of radiotherapy using tangential technique, and IMRT in treatment planning equipment. The skin absorbed dose was measured to pass a nipple by spacing of 1 cm distance from center to edge of body. In the radiotherapy of tangential technique, for the irradiation of 180 cGy to PTV, the calculation dose was ranged from 103.5 cGy to 155.2 cGy, measurement dose was ranged from 107.5 cGy to 156.2 cGy, and skin dose in the center was maximum 1.45 times more irradiated than that in the edge. In the IMRT, for the irradiation of 180 cGy to PTV, the calculation dose was ranged 9.8 cGy at 80.2 cGy, measurement dose was ranged 8.9 cGy at 77.2 cGy, and skin dose in the center was maximum 0.23 times less irradiated than that in the edge. IMRT was more effective for skin radiation risks because radiation dose irradiated to skin in IMRT was much less than that in radiotherapy of tangential field technique.

The Measurement of Ho-166 Absorbed Dose for the Endovascular Irradiation with a Balloon Angio Catheter Using a GafChromic Film (GafChromic 필름을 이용한 Ho-166 의 혈관내 방사선조사를 위한 선량분포 측정)

  • The GafChromic film was used for the dosimetry of a balloon angio catheter filled with the radioisotope HO-166 for endovascular irradiation. The balloon angio catheter was 2 cm long and 3 mm in diameter when inflated. The isotope, Ho-166, was produced by the neutron bombardment using the research reactor in Korea Atomic Energy Research Insititute. Co-60 teletherapy beam was used for making H-D curve for the Gaf-Chromic film. The film dosimetry was measured with a videodensitometer. The radial dose distribution indicated that the absorbed dose dropped to about 20% of the surface dose at the 1 mm away from the balloon surface and at 5 mm position the dose decreased to below 1% of the surface dose. The result also shows that with the specific activity of Ho-l66, 250 mCi/ml it takes 230 seconds to deliver 1200 cGy to the region where is 1mm away from the balloon surface. The concentric isodose curves were also presented. The Ho-166 is an another alternative for endovascualr irradiation to prevent restenosis after PTCA (Percutaneous Trans Coronary Angioplasty)

Evaluating the Reduction of Spatial Scattering based on Lead-free Radiation Shielding Sheet using MCNPX Simulation (MCNPX 시뮬레이션을 이용한 무납 방사선 차폐 시트 기반의 공간산란 저감화 평가)

  • Most of the spatial scattered dose caused by the scattered rays generated by the collision between the object and X-rays is relatively easily absorbed by the human body as electromagnetic waves in the low energy region, thereby increasing the degree of radiation exposure. Such spatial scattering dose is also used as an indicator of the degree of radiation exposure of radiation workers and patients, and there is a need for a method to reduce exposure by reducing the spatial scattered dose that occurs indirectly. Therefore, in this study, a lead-free radiation shielding sheet was proposed as a way to reduce the spatial scattering dose, and a Monte Carlo (MC) simulation was performed based on a chest X-ray examination. The absorbed dose was calculated and the measured value and the shielding rate were compared and evaluated.

Development of Quality Assurance Software for $PRESAGE^{REU}$ Gel Dosimetry ($PRESAGE^{REU}$ 겔 선량계의 분석 및 정도 관리 도구 개발)

  • The aim of this study is to develop a new software tool for 3D dose verification using $PRESAGE^{REU}$ Gel dosimeter. The tool included following functions: importing 3D doses from treatment planning systems (TPS), importing 3D optical density (OD), converting ODs to doses, 3D registration between two volumetric data by translational and rotational transformations, and evaluation with 3D gamma index. To acquire correlation between ODs and doses, CT images of a $PRESAGE^{REU}$ Gel with cylindrical shape was acquired, and a volumetric modulated arc therapy (VMAT) plan was designed to give radiation doses from 1 Gy to 6 Gy to six disk-shaped virtual targets along z-axis. After the VMAT plan was delivered to the targets, 3D OD data were reconstructed from 512 projection data from $Vista^{TM}$ optical CT scanner (Modus Medical Devices Inc, Canada) per every 2 hours after irradiation. A curve for converting ODs to doses was derived by comparing TPS dose profile to OD profile along z-axis, and the 3D OD data were converted to the absorbed doses using the curve. Supra-linearity was observed between doses and ODs, and the ODs were decayed about 60% per 24 hours depending on their magnitudes. Measured doses from the $PRESAGE^{REU}$ Gel were well agreed with the TPS doses at central region, but large under-doses were observed at peripheral region at the cylindrical geometry. Gamma passing rate for 3D doses was 70.36% under the gamma criteria of 3% of dose difference and 3 mm of distance to agreement. The low passing rate was resulted from the mismatching of the refractive index between the PRESAGE gel and oil bath in the optical CT scanner. In conclusion, the developed software was useful for 3D dose verification from PRESAGE gel dosimetry, but further improvement of the Gel dosimetry system were required.

  • This study determines a range of CT parameter values in Brain CT which are minimizing patient absorption dose without compromising the image quality and optimal exposure condition. We measured dose and image noise using conventional CT parameters in Brain CT. In additon, we evaluated dose, SNR and PSNR of head phantom images while changing kVp and rotation time. In this study, effectiveness of dose that was achieved from dose reproducible experiments in conventional head CT condition is determined by changing kVp and rotation time. Dose and PSNR is related to low dose-high resolution condition. In conclusion, we suggest that using proposed conditions is effective for imaging to compare with conditions proposed by the manufacturer.

A Comparison between Three Dimensional Radiation Therapy and Intensity Modulated Radiation Therapy on Prostate Cancer (전립샘암의 방사선 치료 시 입체조형치료법와 세기조절방사선 치료법의 비교)

  • In this study, we evaluated to the superiority of treatment techniques on prostate cancer, apply to each other treatment techniques-3D conformal therapy versus IMRT-using dose distribution and dose coverages. Obtained 10 patients CT simulation, divided tumor volume and critical organs. Prescription dose was 80 Gy on tumor volume and Each of plans was set by two different plans. As a result, Dose coverage was superior to IMRT. The IMRT's tumor absorbed dose(100.2%) was close to prescription doses. Normal tissue(bladder, rectal, bowel Lt Rt fumoral head) absorbed dose rate was superior. In other words, the radiation therapy of prostate cancer with intensity modulated radiation therapy was better than conformal radiation therapy on dose.
