Increasing Work Efficiency with Prevention of Reinspection for Bone Mineral Density (BMD) Exams (골밀도 검사의 재검사 방지에 따른 업무효율 향상)

  • Recently hospitals are implementing a One Stop Service as part of patient-care service. With the One Stop Service, medical treatment including consultation, inspection, and results are changing to be made available in one visit. Therefore most examinations are reserved for the same day; however, there are cases in which additional visits are necessary because of certain properties related to exams. This study compares and analyzes the number of reinspections before and after reforms. By designating the order of priority for BMD examinations and implementing education from information obtained in the OCS E-manual update, the number of reinspections for wards was reduced from 58 to 21, Outpatient departments were reduced from 51 to 12, and errors in reservations made by employees in the department of radiology were reduced from 98 to 11. Reinspections can be reduced with full understanding the factors related to reinspection and a background check prior to inspection in order to determine the order of priority for inspections. This will also reduce workers' stress and increase their efficiency and at the same time decease patient dissatisfaction and improve hospital reliability.

Nutrition Education Effect in Obesity Treatment for Children (소아비만 치료의 임상영양 교육 효과)

  • The aim of this study was to investigate nutrition education effect in the long term and short term nutrition education intervention for childhood obesity. Seventy-seven obese children aged seven to twelve who visiting obesity clinic in a university hospital were given nutrition education therapy. Long-term as well as short-term education effect had been measured based on reducing level of Body Mass Index (BMI), Relative Body Weight (RBW) and lipid profile as well as dietary behavior. Identification of factors influencing the effect of nutrition education had been tested using multiple regression analysis as well as Main Whitney test. The result showed that nutrition education intervention had a significant impact on reducing the level of BMI, RBW and lipid profile during long term and short term period. Duration of clinic visit and total number of clinic visit were the most significant factors for the short term effect for obesity treatment. However, frequent clinic visit per month was not significant factor on short term effect. Therefore sustained participation for nutrition education by the obese individual seems more important factors than other factors especially for short term effect. Although, there was the tendency of reducing weight with longer duration and frequent clinic visit, and changing health behavior, none of the factors showed statistically significant effect on the long term effect for the obesity treatment. The results may imply that more complex factors involved for the long term treatment of childhood obesity than short term treatment.

  • Every anesthetic begins with preoperative evaluation. This meeting of patient and anesthesiologist allows for an exchange of information, an assessment of physical status, and the formulation of an anesthetic plan. The aim is to have the patient in the best possible condition, both mental and physical, prior to surgery. The preoperative visit is as important as any aspect of introperative management in providing a safe, effective anesthetic.

A Multi-level Analysis of Injection Requests and Associated Patient Characteristics in the Korean Acute-care Outpatient Setting (국내 병의원 이용 환자들의 주사제 요청과 관련된 특성에 대한 다수준 분석)

  • 서론: 주사제 사용을 줄이기 위한 정책의 일환으로 건강보험심사평가원은 의료기관별 주사제 처방률을 통보하고 있으나, 여전히 주사제 처방은 높은 실정이다. 따라서 본 연구에서는 주사제에 대한 환자의 인식수준을 살펴보고, 환자의 주사제 요청에 영향을 미치는 요인을 파악하고자 하였다. 연구방법: 본 연구는 최근 6개월 이내에 병의원을 방문한 전국의 20세 이상의 남녀를 대상으로 전화조사한 단면설계 연구다. 환자의 일반적 특성과 주사제에 대한 태도, 인식을 조사하였고, 환자가 주사제를 요청하는데 영향을 미치는 특성을 규명하기 위해 일반적 특성(성, 연령, 결혼여부, 보험형태, 지역규모, 질환, 교육, 소득 등), 주사제에 대한 인식, 태도를 독립변수로 하고, 주사제 처방 요청여부를 종속변수로 하며, 16개 행정지역을 무작위 효과로 층화한 다수 준 분석을 실시하였다.결과: 연구대상에 포함된 응답자는 997명이었고(응답률 82.2%), 응답자 중 24%가 병의원 방문 당시 주사제를 요구했다고 응답했고, 58%가 한번 이상 주사제를 맞은 경험이 있다고 보고했다. 92%가 주사제에 대해 잘못된 인식을 갖고 있었고, 15%는 의사가 부적절하게 주사제를 처방한다고 응답했다. 다수준 로지스틱 회귀분석 결과, 남성의 경우(Odds ratio(OR) 0.71, 95% confidence interval(CI) 0.52-0.99), 고졸이상자(OR 0.63, 95% CI 0.41-0.96), 기혼자(OR 1.72, 95% CI 1.01-2.92)가 주사제를 더 요구하는 것으로 나타났고, 대도시에 비해 농촌지역 환자가(OR 2.12, 95% CI 1.24-3.63), 호흡기계 질환으로 방문한 경우(OR 1.48, 95% CI 1.03-2.12), 주사제를 처방하면 경구제에 비해 신뢰감이 생긴다는 응답자의 경우(OR 1.91, 95% CI 1.33-2.73) 주사제를 더 요구하는 것으로 나타났다. 결론: 본 연구결과 여성, 기혼자, 농촌 거주자, 호흡기계 질환으로 방문한 환자의 경우와 주사를 맞으면 신뢰감이 생긴다는 잘못된 태도를 가진 환자가 주사제를 요구하는 것으로 나타났고, 이러한 환자특성을 고려하여 주사제 사용을 감소시키기 위한 정책을 실시하는 것이 필요하겠다.

  • Places exposed to mass media induce behavior by forming curiosity and expectations for potential travelers. The places reported through mass media influence viewers. Among TV programs, the most influential genre is drama, and reality programs that provide immersion with different characteristics from dramas influence viewers' choice of destination. CVM is mainly used for estimating the value of objects that cannot be evaluated in the market, such as tourist destinations. This study conducted an economic valuation of filming locations for dramas and travel entertainment programs using CVM, and then compared and analyzed the research results of the two filming locations. Linear and log logit analysis were performed to measure the willingness to pay for the filming location of the drama/travel entertainment program, and the payment amount was derived. The conclusion of the study is that as the travel cost required to visit the filming location of the drama/travel entertainment program increased, the intention to visit decreased. The amount payable when visiting the filming location of the drama/travel entertainment program was higher than the average consumption amount for a day trip, and the amount payable for the drama was higher than that of visiting the filming location of the travel entertainment program.

Development and evaluation of physical activity promotion program in the resident elderly (재가노인을 위한 신체활동증진 프로그램의 개발 및 효과)

  • The purpose of this study was to identify the effects of a visiting exercise program for physical activity promotion on physical (grip strength, static balance) and psychological (depression, perceived health status) function of frail elderly patients receiving home visiting service care. Subjects included a total of 49 patients; 24 in the control group and 25 in the experimental group. The visiting exercise program was carried out 16 times for 8 weeks. The collected data were analyzed by ${\chi}^2-test$ and t-test using SPSS/WIN 21.0 program. There was a significant improvement in the left grip strength (t=2.19, p<.000), right grip strength (t=5.07, p=.032), static balance (t=3.98, p=.050), and depression (t=-8.83, p=.009) in the experimental group compared with the control group. The visiting exercise program for physical activity promotion was effective on physical and psychological functions for frail elderly patients receiving home visiting service care. Further research is needed in a longitudinal study incorporating various centers to examine the maintenance of such effects.

The Effect of Traditional Market Attributes and Service Quality on Visiting Intention: Focusing on Hygiene Factor Moderating Effect (전통시장 속성 및 서비스품질이 방문의도에 미치는 영향: 위생요인조절효과를 중심으로)

  • Recently, In traditional markets, visitors are declining. The reason is the growth of large stores and Internet shopping malls. The government continues to support and policy to revitalize traditional markets. Government support has been focused on the selective attributes of traditional markets. However, the purchase intention of users in traditional markets is lowered. The reason is that it is in the hygiene of the traditional market. This study analyzed whether the optional attributes of traditional markets and service quality increase the intention of visit, In addition, the users of the traditional market analyzed the hygiene factor as an important factor in the intention of the visit. The results of the analysis is First, convenience, accessibility, transparency, attractiveness, and economic feasibility of selective attributes of traditional markets were analyzed to affect the intention to visit. Second, the merchant efficiency, the display efficiency, the product efficiency, and the transaction efficiency of the service quality of the traditional market influence on the visit intention. However, facility efficiency was not found to have any effect. Third, merchant hygiene factors, facility hygiene factors, and commodity hygiene factors were found to affect the intention to visit. These traditional market hygiene factors were analyzed to control the intention to visit. Therefore, it can be said that the hygiene factor of the traditional market plays a role in raising the intention of visiting the traditional market in activating the traditional market. The conclusion is that merchants and support groups should be prioritized in order to revitalize traditional markets. The importance of environmental hygiene is introduced and implications for research results are suggested.

A Comparative Study on the Pattern of Outpatient Department Utilization at a Tertiary Level Hospital before and after Implementation of the Patient Referral System (의료전달체계 실시 전후의 3차 진료기관 외래환자 이용양상 비교)

  • This study was conducted to assess the effects of implementation of the patient referral system started July 1st, 1989. A comparison on the pattern of outpatient services of the Departments of Internal Medicine, General Surgery, and Pediatrics at the Yeungnam University Hospital was conducted for each one year period before and after implementation of the patient referral system. The pre-implementation period was from July 1, 1988 to June 30, 1989 and the post-implementation period was from July 1, 1989 to June 30, 1990. The information used for this study was obtained from official forms, prepared by the Yeungnam University Hospital, and submitted to the Korean Medical Insurance Cooperatives. After implementation of the patient referral system, the number of outpatient cases in the Department of Internal Medicine decreased 36.1% from 9,669 cases to 6,181 cases a year. Cases in the Department of General Surgery decreased 23.7% from 1,864 cases to 1,422 cases a year. The number of cases in the Department of Pediatrics decreased 36.9% from 3,372 cases to 2,128 cases a year. After implementation of the patient referral system, the average age of cases in the Departments of Internal Medicine and General Surgery was 52.5 and 49.7 years old, respectively. This was a significant increase in comparison with the pre-implementation period. After implementation of patient referral system, the proportion of new outpatients in the Department of Internal Medicine decreased from 24.1% to 14.6%, the Department of General Surgery from 36.0% to 23.4%, and the Department of Pediatrics from 15.5% to 8.3%. The number of visits per case decreased significantly in the Department of Internal Medicine(from 1.74 to 1.61), but there was no significant change in the Departments of General Surgery and Pediatrics. The length of treatment per case increased significantly in all three departments(from 16.1 days to 19.3 days in the Department of Internal Medicine, from 12.0 days to 15.2 days in the Department General Surgery, and 8.9 days to 11.2 days in the Department of Pediatrics). The number of clinical tests per case increased significantly in the Department of Internal Medicine (from 2.2 to 2.5), in the Department of Pediatrics(from 0.8 to 1.1) and increased in the Department of General Surgery(from 6.4 to 6.6). The average medical cost per case decreased from 43,900 Won to 42,500 Won in the Department of Internal Medicine, while the cost increased from 75,900 Won to 78,500 Won in the Department of General Surgery and from 12,700 Won to 13,500 Won in the Department of Pediatrics. In case-mix, the chronic degenerative disease(i. e. hypertension, diabetes mellitus, angina pectoris, malignant neoplasm, and pulmonary tuberculosis) ranked higher and acute infectious diseases and simple cases(i. e. gastritis and duodenitis, haemorrhoids, anal fissure, carbuncle, acute URI, and bronchitis) ranked lower after implementation of the patient referral system compared to before implementation.

A Mobility Management Scheme based on the Mobility Pattern of Mobile Networks (이동 네트워크의 이동 패턴에 기반을 둔 이동성 관리 기법)

  • Recently, small-scale mobile network which is composed of many mobile devices in a man becomes popular. Also, Examples of large-scale mobile network can be thought access networks deployed on public transportation such as ships, trains and buses. To provide seamless mobility for mobile nodes in this mobile network, binding update messages must be exchanged frequently. However, it incurs network overhead increasingly and decreases energy efficiency of mobile router. If we try to reduce the number of the messages to cope with the problem, it may happen the security -related problems conversely Thus, mobile router needs a effective algorithm to update location information with low cost and to cover security problems. In this paper, mobility management scheme based on mobile router's mobility pattern is proposed. Whenever each mobile router leaves a visiting network, it records related information as moving log. And then it periodically computes mean resident time for all visited network, and saves them in the profile. If each mobile router moves into the visited network hereafter, the number of binding update messages can be reduced since current resident time may be expected based on the profile. At this time, of course, security problems can happen. The problems, however, are solved using key credit, which just sends some keys once. Through extensive experiments, bandwidth usages are measured to compare binding update messages in proposed scheme with that in existing scheme. From the results, we can reduce about 65% of mobility-management-related messages especially when mobile router stays more than 50 minutes in a network. Namely, the proposed scheme improves network usage and energy usage of mobile router by decreasing the number of messages and authorization procedure.
