특집 - 당뇨병환자가 감염되었을 때

  • 당뇨병환자에서의 감염증은 인슐린과 항균제의 사용으로 그 이환율과 사망률이 현저히 감소하였으나, 현재까지 당뇨병환자의 진료에 있어서 해결해야 할 중요한 문제로 남아있다. 당뇨병환자에서 감염증의 발생빈도는 대부분 일반인과 차이가 없다고 보고되고 있으나 일부 특정 감염증의 빈도는 현저하게 증가하고 감염증의 정도가 훨씬 심하며, 임상양상도 정상인과 다른 것으로 알려져 있다. 특히 인플루엔자, 세균뇨, 악성 외이도염, 비뇌모균증, 기종성 담낭염, 기종성 신우신염 또는 방광염, 급성 신유두괴사, 신주위 농양, 진균성 요로 감염증, 그람 음성균에 의한 폐렴, 괴사성 연조직 감염과 족부 궤양 관련 감염증 등은 당뇨병과의 관련성이 이미 입증된 감염질환들이다. 당뇨병 환자에서 감염증의 발생빈도는 약 14.4%에서 39.5%까지 보고되고 있으며, 국내에서는 폐결핵 20%, 요로 감염 20%, 폐렴 16%, 진균감염 7.6%, 균혈증 6.2%, 족부 궤양 관련 감염증 5.5%의 순으로 보고되고 있다. 당뇨병환자가 감염증에 대한 원인에 대해서는 확실히 밝혀지지는 않았지만 탈수, 영양실조, 다형 백혈구의 기능 장애, 혈액 순환장애 및 신경병증 등으로 설명되고 있다. 특히 당뇨병환자에서는 미세 혈관병증과 죽상경화증이 흔히 발생하기 때문에 각 조직으로의 혈액 공급이 감소되어 각 장기 조직에서는 정상적인 영양 및 산소 공급과 적절한 면역 반응을 유지하는 것이 어려워 결과적으로 감염증에 걸리기 쉽다. 이 글에서는 여러 가지 감염증 중 족부 궤양 감염증, 기종성 신우신염, 비대뇌모균증, 간농양에 대해 사례를 중심으로 알아보겠다.

Bladder Preservation by Combined Modality Therapy for Invasive Bladder Cancer : A Five-Year Follow-up (근침윤성 방광암에서 화학방사선 병용을 통한 방광보존치료)

  • Purpose : To determine the long-term results of bladder-preserving approach by transurethral resection of the bladder (TURB), systemic chemotherapy, and radiation therapy for muscle-invasive bladder cancer Methods and materiaals : From 1991 Jan. through 1994 Dec., 25 patients with muscle invading clinical stage T2 to T4NxM0 bladder cancer were treated with induction by maximal TURB and (arm 1, n=4) three cycles of chemotherapy [MVAC(methotrexate, vincristine, adriamycin, ciplatin)] followed by 64.8 Gy of radiation with concomitant cisplatin, or two cycles of chemotherapy [MCV (methotrexate, ciplatin, vincristine)] after irradiation with concomitant cisplatin (arm 2, n=14), or concurrent chemoradiation only (arm 3, n=7). Tumor response was scored as a clinical complete response (CR) when the cystoscopic tumor-site biopsy and urine cytology results were negative. Those with less than a CR underwent cystectomy. The median follow-up of all patients was 70 months. Resulst : Most treatment toxicities were mild to moderate. Grade 3 acute hematologic toxicity and chronic cystitis were observed in only 1 and 2 patients, respectively. Overall 5 year survival was $67.3\%$. Complete remission rate was $80\%$ (20/25). Sixty-three percent of all survivors retained their bladders. In multivariate analysis, prognostic factors that significantly affect survival were T-stage (p=0.013) and Complete remission (p=0.002). Conclusion : Combined modality therapy with TURB, chemotherapy, and radiation has a $67.3\%$ overall 5 year survival rate. This result is similar to cystectomy-based studies for patients of similar clinical stages.

An Improved Case Report of Interstitial Cystitis/Bladder Pain Syndrome Treated by Korean Medicine Treatment (한의 복합 치료로 호전된 간질성 방광염/방광통증증후군 1례 : 증례보고)

  • Objectives: The purpose of this study is to report the clinical effectiveness of Korean medicine treatment on interstitial cystitis. Methods: The patient diagnosed with Interstitial cystitis/Bladder pain syndrome (IC/BPS) was treated with Korean medicine treatment during 22 days of hospitalization. The patient received Korean medicine treatment including herbal medicine, acupuncture, pharmacopuncture, moxibustion, cupping and extracorporeal magnetic stimulation therapy. The effect of treatment on IC/BPS was evaluated through the Numeric Rating Scale (NRS), voiding diary, and Interstitial Cystitis Symptom Index/Problem Index (ICSI/ICPI). Results: After the treatment, the clinical symptoms of IC/BPS such as pelvic pain, pelvic pressure, frequent urination and urinary urgency were improved. Also, the patient had better scores in ICSI/ICPI. Conclusions: This case report proved that the Korean medicine treatment may be effective for improving clinical symptoms of IC/BPS. However, a large-scale clinical study is needed in the future to prove the effectiveness of Korean medicine treatment for IC/BPS.

A Case Report of Interstitial Cystitis Treated with $Sa-am$ Acupuncture Treatments (간질성 방광염의 사암침법(舍巖鍼法) 자침 1례)

  • Objectives : The purpose of this study was to evaluate the effects of $Sa-am$ acupuncture treatment on an interstitial cystitis patient. Methods : A 51 year-old woman who diagnosed as interstitial cystitis, was enrolled in this study. The clinical data was analyzed on a patient with interstitial cystitis due to $Shinheo$(腎虛). The patient had visited Korean medicine department of Yangsan public health care center, and was treated with a $Sa-am$ acupuncture($Sinjeongkyeok$) for 4 weeks. Results : After 4 weeks of $Sa-am$ acupunture treatment($Sinjeongkyeok$), improvement was seen in the subsection of the international prostate symptom score and the part of qualiveen questionnaire score. Conclusions : This clinical case indicates that acupuncture treatment is significantly effective in treatment of a interstitial cystitis.

  • Purpose : The purpose of this report is to record the effects of acupuncture and moxibustion treatment on Chronic Relapsing Cystitis. Methods : This is a case report of female patient suffering from urinary disturbance based on the bladder stimulated. 28 years old woman diagnosed with chronic relapsing cystitis was enrolled in this study. She was treated with acupunctures at CV6(Qihai, 氣海), CV4(Guanyuan, 關元) and CV2(Qugu, 曲骨) for two weeks, seven times. During the treatments, we checked changes of symptoms. Results : After acupuncture treatments, clinical symptoms of chronic relapsing cystitis and general conditions were improved. Conclusion : This clinical case shows that acupuncture is effective in treatment of the chronic relapsing cystitis. More clinical data and studies are required for the treatment of chronic relapsing cystitis.

  • Objective : The purpose of this study was to report the improvement in a patient with cystitis by a herbal medication based on Shanghanlun disease pattern identification diagnostic system (DPIDS). Methods : A patient was diagnosed with cystitis according to DPIDS. The patient was treated with Galgunhwangryunhwanggum-tang. Results : The symptoms of cystitis disappeared after treatment and did not recur after 4 months. Conclusions : The herbal medication Galgunhwangryunhwanggum-tang is effective in this case of cystitis.

  • Interstitial Cystitis/Painful Bladder Syndrome: Prevalence Estimates, Quality of Life and Depression among Older Adult Korean Women. What is already known about the topic? 1) Interstitial Cystitis/Painful bladder syndrome (IC/PBS) is a chronic, painful, inflammatory condition of the bladder wall. 2) Previous studies examining the prevalence and impact were focused on middle life women and not elderly women. 3) Epidemiologic studies of IC/PBS have been predominantly conducted in Western countries and little research reported in Asian countries.

Three Cases of Chronic Relapsing Cystitis with Herb-medicine and Sweet Bee Venom Pharmacopuncture (한약과 봉약침(Sweet BV)으로 병행치료한 만성 재발성 방광염 환자 치험 3례)

  • Objectives: This study aims to report the effects of Korean medicine treatment by pattern identification and Sweet Bee Venom Pharmacopuncture on Chronic Relapsing Cystitis Methods: The patients was treated with Korean medicine by pattern identification and Sweet Bee Venom Pharmacopuncture at Qugu (CV2), Guanyuan (CV4). We evaluated treatment effects by changes of symptoms and urine analysis (UA) finding. Results: After treatments, the clinical symptoms such as painful urination, dysuria, frequent urination were improved and the state of urinalysis was improved. Conclusions: This clinical study suggests that Korean medicine treatment by pattern identification and Sweet Bee Venom Pharmacopuncture are significantly effective in treatment of a Chronic Relapsing Cystitis

  • Objective: This case report describes the improvements of a patient with recurrent cystitis who was treated using an herbal medication according to the Shanghanlun disease pattern identification diagnostic system (DPIDS). Methods: 'DPIDS'and 'Interpretation based on Etymological chinese characters,'the patient was diagnosed with Shaoyin-bing, provision number 315, and administered Baektong-tang herb medication for 30 days. The changes in the symptoms of recurrent cystitis were estimated using the Interstitial Cystitis Symptom Index and Problem Index-Korean version (ICSI/ICPI-K) and Visual Analogue Scale (VAS). Results : The ICSI/ICPI-K score changed from 33 to 0 and VAS score changed from 7 to 0. Conclusions: The main causes of the disease were 微 and 利,and the symptoms of recurrent cystitis

  • Kawasaki disease (KD) is a systemic vasculitis that can affect many organ systems. Renal manifestations include pyuria, hematuria, proteinuria, tubulointerstitial nephritis, acute renal failure, hemolytic uremic syndrome, or renal scarring. Although its precise pathogenesis remains unknown, it is considered an autoimmune disease. In the literature, it has been reported that KD may develop in conjunction with urinary tract infections. However, many of these previous studies did not use imaging methods such as renal sonograms, dimercaptosuccinic acid renal scans, and voiding urethrocystograms. We report a case of an 8-month old male infant with high grade vesicoureteral reflux, who developed incomplete KD after recurrent pyelonephritis. Acute pyelonephritis can be an early manifestation of KD. Such cases require the evaluation of urinary tract anomalies according to the guidelines for the management of urinary tract infections.
