Diagnosis of Lateral Ankle Ligament Injury in the Evaluation of Chronic Lateral Ankle Instability (만성 외측 발목 불안정과 발목 인대 손상의 진단)

  • Chronic lateral ankle instability (CLAI) is a major complication of acute ankle sprain that can cause discomfort in both daily and sports activities and may lead to degenerative changes in the ankle joint as the long-term sequelae. The precise cause of CLAI remains debatable and is most probably multifactorial. However, identifying chronic lateral ankle ligament injury is critical in determining the primary etiology and formulating an appropriate treatment plan. This review discusses the features and effectiveness of various imaging techniques in diagnosing chronic lateral ankle ligament injury.

Broden photographing method for Calcaneus Subtentaculum tali observation of Ankle plaster patient (발목부상을 당한 석고환자의 발꿈치뼈·목말받침돌기 관찰을 위한 Broden 촬영법 연구)

  • There is an Ankle Oblique(Broden' low) as an examination for the patient whose ankle was encased in orthopedic plaster. Some types of Ankle Joint coalition - Calcaneonavicular coalition, Talocalcaneal coalition and Naviculocuneiform coalition. This study is focused on the relation between Ankle Joints and the structural change of soft tissues, also finding the most proper angle to obtain good images of Ankle Joint from the patient who wore a plaster on his ankle, when we x-ray with Harris-Beath View($30^{\circ}{\sim}55^{\circ}$) - for observing Subtalar joint, Calcaneus Fracture, Subtentaculum, Tali Fracture and Talocalcaneal coalition. We intend to get the angle which makes us achieve the good image that shows Calcaneus Fracture, Subtentaculum, and Tali Fracture by changing internal angles of the patient's ankle. We evaluated the images obtained from 51 patients with PACS monitor. The result of the evaluation, subtalar joint was not seperated but opened, and Subtentaculum Tali Fracture was seen overlaid. at the angle 30, we could observe Calcaneus Fracture, Subtentaculum Tali Frature and the front part of behind side of subtalar joint well. And Calcaneo Navicularcoalition, Talocal Canealcoalition, Naviculo Cuneiform coalition condition were clearly seen at that angle. At the angle 35, we could achieve the clear images of subtalar comminuted fracture, talus, the behind joint of heel bone and get the high definition image on the degree of talocalcaneal joint separation. In addition to, We could obtain the good wide image of Sinus Tarsi. At the area of 45, We can distinguish the soft tissues from gyps separation. The outer-talus and density of the bone were definitely seen and Calcaneus is more separated than that of at the angle of 35, but this image is distorted. Calcaneus, Subtentaculum Tali show $1.20{\pm}0.414$ at the angle 25, $2.47{\pm}0.516$ at the angle 30, $2.27{\pm}0.458$ at the angle 45. This difference is statistically meaningful. (p<0.05). Including the degree of distortion, The distortion appears less at the area of $30^{\circ}$ but at the area of 40, there is heavy distortion. So, We could get the best image for making a diagnosis. At the $30{\sim}35^{\circ}$ degree for X-raying ankle. and at the $30{\sim}40^{\circ}$ for Calcaneus Fracture, Subtentaculum Tali Fracture.

  • Gait analysis has been conducted in various environments, but the biomechanics during the transition from uphill walking to downhill walking have not been reported. The purpose of this study is to investigate the knee and ankle joint kinematics and kinetics during walking on a triangle-shaped slope compared with those during level walking. Kinematic and kinetic data of eighteen participants were obtained using a force plate and motion capture system. The greater peak ankle dorsiflexion angle and moment and the peak knee extension moment were observed (p<0.05) during both uphill and downhill walking on the triangle-shaped slope. In summary, uphill walking on a triangle-shaped slope, which showed a peak knee flexion of more than $50^{\circ}$ with greater peak knee extension moment, could increase the risk of patellofemoral pain syndrome. Downhill walking on a triangle-shaped slope, which involved greater ankle dorsiflexion excursion and peak ankle dorsiflexion, could cause gastrocnemius muscle strain and Achilles tendon overuse injury.

  • This study was performed to evaluate the effects of Visual feed-back ankle training combined with Ankle joint Functional electrical stimulation on balance, gait ability on patient with Chronic Stroke. A total of 22 chronic stroke patients were divided into VFAF Group, CON group. Each group performed 60 minutes a day 5 times a week for 8 weeks. VFAF group revealed significant differences in balance and gait ability as compared to the CON groups(p<.05). The exercises were conducted for 60 min per day, five, per week for eight weeks. Balance and gait ability were examined at 0 week and after 8 weeks of intervention. Our results showed that VFAF was more effective on balance ability and gait ability in chronic stroke patients. We suggest that this study will be able to be used as an clinical intervention data for recovering balance and gait ability in chronic stroke patients.

Convergence Comparison of the Angle Change of Ankle Joint while Walking according to the Heel height of Insole Shoes in 20s males with Flatfoot (평발을 가진 20대 남성이 인솔구두의 굽 높이에 따른 보행 시 발목관절의 각도변화에 대한 융복합적 비교)

  • The purpose of this study is to convergence comparison the ankle joint angle change during walking of college students in their 20s with flat foot according to the heel height of insole shoes. Qualisys Track Manager Software ver. 2.8 (Qalisys Track Manager) was used for 15 college students. Functional shoes with insoles were manufactured, and the heel heights of the shoes were set to 3cm and 7cm. The subjects wore shoes with two high heels and gaited by attaching a reflex marker to the side of the ankle joint. The angle change of the ankle joint was measured in the gait stance phase. The angle of the ankle joint significantly decreased both heel strike, foot flat, midstance, and toe off to the heel height increased when the subjects with flat feet wore insole shoes. Therefore, it is thought that flat feet should wear low shoes when wearing insoles to reduce the fatigue of the soles and to walk comfortably.

The Diagnostic Usefulness of Stress Radiography in Chronic Lateral Ankle Instability (만성 발목 관절 외측 불안정성의 진단에서 스트레스 방사선검사의 유용성)

  • Purpose: This study was performed to evaluate the diagnostic usefulness of ankle stress radiograph for evaluation of chronic lateral ankle instability. Materials and Methods: Among patients undergoing the modified-Brostrom procedure, 42 cases with complete rupture of the anterior talofibular ligament were enrolled in this study. Sixty Korean adults (120 cases) were recruited as the control group. Radiologic measurement of talar tilt and anterior talar translation was performed through stress radiographs using Telos device. We obtained the normal range of Korean adults, and used as a standard value for judgment of mechanical instability. We analyzed the sensitivity, specificity, positive and negative prediction value of ankle stress radiograph. Results: On ankle stress radiograph, normal range of talar tilt angle and anterior talar translation was below $8.3^{\circ}$, below 7.6mm. Talar tilt angle on varus stress radiograph showed 57% of sensitivity, 97% of specificity, 89% of positive and 86% of negative prediction value. Anterior talar translation on anterior drawer stress radiograph showed 69% of sensitivity, 97% of specificity, 91% of positive and 90% of negative prediction value. Conclusion: Ankle stress radiograph had a good specificity, positive and negative prediction value for the evaluation of mechanical instability. However it underestimated the mechanical instability of ankle joint. It must be remembered that normal stress radiograph does not exclude ankle instability.

  • 등산은 누구나 손쉽게 즐기는 국민 스포츠이다. 그러나 생각만큼 간단한 운동은 아니다. 의욕만 앞세우다 자칫 발목이나 무릎 관절에 무리를 줄 수 있으며, 심한 경우 관절이나 관절 주변 조직이 찢어져 수술을 받아야 한다. 관절이나 주변 조직의 부상은 만성 관절염으로 연결되기 쉬우며, 만성 관절염은 노후 인공관절 수술을 받게 되는 가장 큰 원인이 된다. 이번 호에서는 등산으로 인한 근육∙골격계 질환에 대해서는 알아보기로 한다. 이 내용은 동아일보에 게재된 내용을 발췌 정리한 것이다.

Influence on Amputee Gait by the Ankle Joint Alignment (발목관절 조절각도가 절단환자의 보행에 미치는 영향)

  • Socket pressure distributions with gait analysis of a trnsfemoral and a transtibial prostheses were measured in order to assess an optimal socket fitting and function. Ankle joint was aligned by the neutral and the dorsi/plantar flexed positions. Compared to dorsi and plantar flexed positions of ankle joint, cadence and walking speed increased with the neutral ankle joint alignment. Other gait parameters were close to the normative data with the neutral ankle joint alignment. For the transfemoral amputee, dorsiflexed alignment of the ankle joint created high pressure on the lateral aspect of the socket, on the other hand, plantarflexed alignment resulted in increased pressure on the medial aspect of the socket. For the transtibial amputee, dorsiflexed alignment of the ankle resulted in high pressure on the antero-lateral aspect of the socket during mid-stance, but apltarflexion of the ankle joint showed slight increases in pressure at the same location in the socket. The present study clearly demonstarted that malalignment of a prosthesis results in localized increasesing pressure within the socket. Proper slignment of the prosthesis is required in order to acquire an appropriate socket-limb interface as well as the proper gait.
