Technical Note of Meniscal Allograft Transplantation using Minimal Incision (최소 절개술에 의한 반월상 연골 동종이식 수술기법)

  • The current treatment of extensive meniscal injuries has resulted in numerous investigations and clinical trials to restore normal meniscal functions. A cryopreserved meniscal allograft transplantation is one of the successful methods available to restore the meniscus. All the procedures of 26 cases were performed in an minimal open fashion, though initial four cases were done with the aid of arthroscope. In all of the grafts, we used a bone bridge which was attached to meniscus for better stability and healing. Anterior cruciate ligament reconstructions were also performed simultaneously with the meniscal procedures. We attempted to minimize articular cartilage by employing so called the "Key-hole technique" for the medial meniscus transplantation. First, the meniscal cartilage bone bridge was shaped into a cylinder and a bone tunnel was made just beside the medial border of the anterior criciate ligament insertion of the recipient knee joint, and the bone bridge of the meniscal cartilage was push to press-fit. The inserted meniscal cartilage was sutured by the usually employed technique under arthroscopic control. The lateral meniscus was shaped different to the medial meniscus in that the bone bridge was semicylindrical and the bone trough was made beside the lateral border of the anterior criciate ligament insertion of the recipient knee joint. The meniscus was put into the bone trough and the leading suture was extracted anterior to the tibia and tied the knot. The inserted meniscus was sutured in the same manner as the medial meniscus transplantation. By the above described method, the authors were able to minimize the articular cartilage invasion and transplant the meniscus with relative accuracy.

  • 동종 반월상 연골 이식술은 불가피하게 반월상 연골이 소실된 환자에서 증상을 호전을 위한 매우 효과적인 치료이다. 관절연골의 보호 효과 등 장기적인 결과에 대해서는 아직까지는 많은 연구가 필요한 상태이지만, 이러한 환자에서 반월상 연골의 기능을 회복시키기 대안이 많아 점차 널리 시행되는 추세이다. 이식된 반월상 연골이 제 기능을 하기 위해서는 적절한 수술방법을 통하여 반월상 연골 이식물을 정확한 위치에 이식을 하는 것은 필수적일 것이다. 여러 저자들 마다 서로 다른 다양한 반월상 연골 이식술의 방법들을 사용하고 있지만, 어떤 방법을 선택하던지 간에 이식된 반월상 연골의 전각 및 후각을 해부학적인 위치에 견고하게 고정을 하는 것이 매우 중요하다. 술자의 선호도에 따라 다양한 방법중의 하나를 선택을 할 수 있지만, 어떤 방법이든 수술 술기상의 어려움이 있으므로, 철저한 준비와 노력이 필요할 것이다. 본 종설에서는 반월상 연골 이식술 중 개방적 방법에 대한 간단한 소개와 함께 저자들이 사용하는 관절경을 이용한 수술방법을 정리하였다.

2 Separate Meniscal Cysts Arising from Tears of the Medial Meniscus - A case report - (내측 반월상 연골 파열에서 발생한 독립된 2개의 반월상 연골 낭종 - 1예 보고 -)

  • Meniscal cyst is an uncommon disease of knee joints, arising less frequently from medial meniscus than lateral. Most of they are accompanied with horizontal tear of meniscus. This is the case of 24 year old man who had 2 separate medial meniscal cysts with longitudinal tear on posterolateral side of medial meniscus without trauma. Treatment consisted of arthroscopic decompression of cysts and meniscal repair.

Diagnostic Value of Sonographic Medial Meniscal Extrusion (초음파적 내측 반월상 연골 탈출의 진단 가치)

  • Purpose: The purpose of this study was to correlate the degree of sonographic medial meniscal extrusion with MRI and arthroscopic findings. Materials and Methods: Out of these 32 patients, with medial meniscal tear who went on arthroscopic surgery, 14 patients had the medial meniscal tear and 18 patients had both lateral and medial meniscal tear. The extent of meniscal extrusion without meniscal tear was assesed in 24 patients who did not show meniscal tear on MRI. The extent of meniscal extrusion was measured between tibial medial joint line (excluded osteophyte) and the outer margin of the medial meniscus. Kellgren-Lawrence grading scale was assessed in plain X-ray image and the location of lesion was assessed during arthroscopic surgery. We also measured the extent of meniscal extrusion on MRI and sonography respectively, and compared each other. Results: There was significant difference between patients with medial meniscal tear and both lateral and medial meniscal tear, patients with medial meniscal tear, patients with both medial and lateral meniscal tear, patients without meniscal tear (P value<0.05). Conclusion: The Meniscus tear must be considered when sonography shows the meniscal extrusion more than 5 mm in length.

Appearance of Meniscus Tear Associated with ACL Rupture - Analysis of Location and Type of Meniscus Tear - (전방 십자인대 파열과 동반된 반월상 연골 파열 양상 - 파열부위 및 형태의 분석 -)

  • Purpose: We analyzed the location and type of meniscus tear associated with ACL rupture in order to estimate and prepare whether the meniscus tear is in a repairable location. Materials and Methods: We reviewed 78 cases who had ACL reconstructive surgery due to ACL rupture. We set the period of acute injury on the basis of under 12 months after trauma. The location and type of meniscus tear was analyzed in accordance with MRI findings and arthroscopic findings. Results: The 50 cases of meniscus tear were detected out of the 78 ACL rupture;32 cases were lateral meniscus tears, 24 were medial meniscus tears and 6 cases were both menisci tears. From a total of 56 meniscus tears, 30 cases were longitudinal tears, 22 cases were red-red zone tears and 35 cases(62%) were posterior horn tears. Conclusion: The most common type of meniscus tear associated with ACL rupture war longitudinal tear at the red-red zone or meniscosynovial junction. Majority of the tears located at that place can be healed with conservative treatment, arthroscopic meniscus suture.

  • Many types of meniscal anomalies have been reported. The authors encountered one case of anomalous insertion of the anterior horn of the medial meniscus to the anterior horn of the lateral meniscus, which ran up along the course of the transverse ligament. This anomalous band was detected in MRI of the knee as a horizontal tear of anterior horn of the lateral meniscus but found during the operation as a anomalous insertion of the anterior horn of the medial meniscus. We report the case with a review of the literature.

  • Treatment options are limited for young, meniscal-deficient patients with pain. This patient population is not age appropriate for total joint replacement, but the loss of the meniscus leaves them at significant risk for the development of osteoarthritis. One increasingly popular option is the use of allograft meniscal transplantation. However, many questions still surround allograft meniscus transplantation. Furthermore, most reports in the literature on the results of meniscal transplantation describe small case series using clinical outcome measures and/or incomplete direct evaluation of the meniscus. Therefore, the results of meniscal allograft transplantation have been difficult to interpret and compare due to many confounding variables. In this study, we reviewed the current research of concerns on the results of meniscal allograft transplantation.

Automatic Meniscus Segmentation from Knee MR Images using Multi-atlas-based Locally-weighted Voting and Patch-based Edge Feature Classification (무릎 MR 영상에서 다중 아틀라스 기반 지역적 가중 투표 및 패치 기반 윤곽선 특징 분류를 통한 반월상 연골 자동 분할)

  • In this paper, we propose an automatic segmentation method of meniscus in knee MR images by automatic meniscus localization, multi-atlas-based locally-weighted voting, and patch-based edge feature classification. First, after segmenting the bone and knee articular cartilage, the volume of interest of the meniscus is automatically localized. Second, the meniscus is segmented by multi-atlas-based locally-weighted voting taking into account the weights of shape and intensity distribution in the volume of interest of the meniscus. Finally, to remove leakage to the collateral ligaments with similar intensity, meniscus is refined using patch-based edge feature classification considering shape and distance weights. Dice similarity coefficient between proposed method and manual segmentation were 80.13% of medial meniscus and 80.81 % for lateral meniscus, and showed better results of 7.25% for medial meniscus and 1.31% for lateral meniscus compared to the multi-atlas-based locally-weighted voting.

A Bucket Handle Tear after Revisional Meniscal Allograft Transplantation in Lateral Meniscus - A Case Report - (외측 반월상 연골 재이식술 후 발생한 양동이형 파열 - 증례 보고 -)

  • Meniscal allograft transplantation (MAT) is an accepted treatment for the symptomatic postmeniscectomized knee in younger patients. However, the likelihood of a successful outcome is reduced by various complications. One of the complications after MAT is manifested as grafted meniscal tear. However, a bucket handle meniscal tear (BHMT) following MAT is an uncommon complication and furthermore a BHMT after revisional MAT (RMAT) is a extremely rare complication. We encountered a case of BHMT after RMAT in a lateral meniscus. Retrospectively reviewing the case, we could assume the cause, which would be helpful to prevent the complication in further MAT procedures.

  • Root tear of the posterior horn of the medial meniscus can occur from trauma or chronic degeneration, leading to meniscus extrusion, articular cartilage loss, osteophyte formation, and medial joint space narrowing. It is common on middle age with or without minor trauma. We experienced a case of medial meniscus posterior horn root tear in 13 years old boy during baseball game. We performed 1 direct suture anchor repair for medial meniscus posterior horn root tear in adolescent and report clinical result.
