Comparison of End-of-Life Care Intensity between Cancer and Non-cancer Patients: a Single Center Experience (암환자와 비-암환자의 임종기 치료 강도 비교: 단일 연구기관 자료)

  • Purpose: The aging of society has rapidly progressed, especially in Korea. Therefore, the necessity of research on end-of-life (EOL) care in elderly patients has increased. However, there are few studies on EOL care intensity for non-cancer patients. This study was designed to compare the EOL care intensity between cancer and non-cancer patients. Methods: We retrospectively analyzed the EOL care intensity based on medical records of decedents of Kyung Hee University Medical Center, a tertiary referral hospital from December 2014 through March 2015. And we compared EOL care intensity between cancer patients and non-cancer patients using statistical analysis of the frequency of invasive procedures and logistic regression analysis for factors that affect the EOL care intensity. Results: Statistical analysis showed invasive procedures, such as intensive care unit admission, endotracheal intubation followed by mechanical ventilation and emergency dialysis, were performed more frequently in non-cancer patients than cancer patients (29.3% vs. 72.4%, P<0.001). And age (P=0.038) and morbidity of cancer (P<0.001) influenced the invasive procedural decision when analyzed by logistic regression analysis. Conclusion: The EOL care was more intensive in non-cancer patients, and age and morbidity of cancer were major factors for the treatment intensity.

Children with Cancer: Adjustment to Disease and Body Image (소아·청소년 암환자의 신체상과 질병적응에 관한 연구)

  • The survival rate of children with cancer has increased significantly from less than 30% in 1960s to 75% during the recent years with the development of modern medical technologies. As a result, the cancer of children today is no longer classified as incurable diseases. Rather, it is recognized as a chronic illness. However, children with cancer are still suffering from physical and psychosocial ailments caused by long-term hospitalization. In particular, teenagers are more likely to be affected by these problems because of interests on one's appearance and peer relationships This study investigated the relationship of the level of body distortion and disease adjustment of children with cancer, and how demographic factors, disease factors, and the level of body distortion affect disease adjustment. Data were collected between October 22, 2007 and November 16, 2007 and the total respondents consisted of 82 children, ages 10 to 18. SPSS 12.0 with descriptive statistics, t-test, correlation and multiple regression were used for data analysis. The results showed that the factors which influence the disease adjustment of children with cancer were age, school enrollment, and the level of body distortion. The result of this study has major implications for the government to provide support for children with cancer to stay in school. Additionally, programs that assist children to build positive body images need to be developed in accordance with their ages and psychosocial characteristics.

Petrochemical Study on the Cretaceous Volcanic Rocks in Kageo island, Korea (가거도(소흑산도)의 백악기 화산암류에 대한 암석화학적 연구)

  • This study reports the results about the petrography and geochemical characteristics of 10 representative volacanic rocks. The Cretaceous volcanic rocks distributed in the vicinity of the Kageo island composed of andesitic rocks, dacitic welded tuff, and rhyolitic rocks in ascending order. Sedimentary rock is the basement in the study area covered with volcanic rocks. Andesitic rocks composed of pyroclastic volcanic breccia, lithic lapilli tuff and cryptocrystallin lava-flow. Most dacitic rocks are lapilli ash-flow welded tuff. Rhyolitic rocks consists of rhyolite tuff and rhyolite lava flow. Rhyolite tuff are lithic crystal ash-flow tuff and crystal vitric ash-flow tuff with somewhat accidental fragments of andesitic rocks, but dacitic rocks. The variation of major and trace element of the volcanic rocks show that contents of $Al_2O_3$, FeO, CaO, MgO, $TiO_2$ decrease with increasing of $SiO_2$. On the basis of Variation diagrams such as $Al_2O_3$ vs. CaO, Th/Yb vs. Ta/Yb, and $Ce_N/YB_N$ vs. $Ce_N$, these rocks represent mainly differentiation trend of calc-alkaline rock series. On the discriminant diagrams such as Ba/La and La/Th ratio, Rb vs. Y + Nb, the volcanic rocks in study area belongs to high-K Orogenic suites, with abundances of trace element and ternary diagram of K, Na, Ca. According to the tectonic discriminant diagram by Wood, these rocks falls into the diestructructive continental margin. K-Ar ages of whole rocks are from andesite to rhyolite $97.0{\pm}6.8~94.5{\pm}6.6,\68.9{\pm}4.8,\61.5{\pm}4.9~60.7{\pm}4.2$ Ma, repectively. Volcanic rocks in study area show well correlation to the Yucheon Group in terms of rock age dating and geochemcial data, and derived from andesitic calc-alkaline magma that undergone low pressure fractional crystallization dominated plagioclase at <30km.

The Experience of Cancer Survivor's Return to Everyday Life (암 생존자의 일상생활 복귀 경험)

  • This study was undertaken to identify ways that enable a successful comeback to everyday life and improvement in the quality of life, by understanding the experiences of cancer survivors in returning to everyday life. Totally, 19 people diagnosed with complete cure after 5 years of cancer treatment, were recruited for the study. Data was collected through in-depth interviews from January 18 to February 25, 2017. One-time interviews took 90 to 120 minutes, and data analysis was achieved by applying the grounded theory. The central phenomenon of 'reality that cannot be escaped' and 'uncertain reality that cannot know tomorrow' was attributed to the causal conditions 'hard reality', 'physical exhaustion', 'psychological exhaustion' and 'economic exhaustion'. Depending on context conditions such as 'lack of family support', 'shift to a vulnerable working class', 'insufficient support system', 'hope for the societal support system', 'daunted gender', 'prejudice against cancer/cancer patients', 'information on life after rare full healing'. The strategy for a successful return was influenced by intervention conditions such as 'robust family fence' and 'effective cancer insurance', which resulted in 'building a new life' or 'enduring'. We conclude that for a successful return to daily lives, cancer survivors require comprehensive information, health and social-welfare interventions.

Petrology of the Volcanic Rocks in Geoje Island, South Korea (거제도 화산암의 암석학적 연구)

  • Andesitic pyroclastics and lava flows are deposited as a part of composite volcanoes by Cretaceous volcanic activity in Geojae Island, off the coast of Korea. The andesitic pyroclastics are composed of tuff breccia and lapilli tuff minor intercalated tuff. Lava flows are divided into dense and porphyritic andesite containing phenocrysts of plagioclase, pyroxene, and/or hornblende. The andesitic rocks represent charactersitcs of carc-alkaline BAR association with basalt, basaltic andesite, andesite, and dacite to rhyolite. Major element variations of the volcanic rocks show that $Al_2O_3$, total FeO, CaO, MgO and $TiO_2$ decrease with increasing $SiO_2$ but $K_2O$ and total alkalis increase, and represent differntiation trend of calc-alkaline rock series. In spider diagram, contents of Sr, K, Rb, Ba, and Th are relatively high, but contents of Nb, P, Ti and Cr are low. These petrochemcial characteristics are similar to those of rocks from island arc or continental margein related to plate subduction. Chondrite-normalized REE patterns of volcanic rocks are paralle to subparallel, with LREE enriched than HREE, and show gradual increase of negative Eu anomaly from basalt to dacite and rhyolite, suggesting comagmatic fractional crystallization with minor effects of assimilation and magma mixing. Andesitic rocks are assumed medium-K orogenic andesites that formed in the tectomagmatic environment of subduction zone under normal continental margin arc.

Rank-based Multiclass Gene Selection for Cancer Classification with Naive Bayes Classifiers based on Gene Expression Profiles (나이브 베이스 분류기를 이용한 유전발현 데이타기반 암 분류를 위한 순위기반 다중클래스 유전자 선택)

  • Multiclass cancer classification has been actively investigated based on gene expression profiles, where it determines the type of cancer by analyzing the large amount of gene expression data collected by the DNA microarray technology. Since gene expression data include many genes not related to a target cancer, it is required to select informative genes in order to obtain highly accurate classification. Conventional rank-based gene selection methods often use ideal marker genes basically devised for binary classification, so it is difficult to directly apply them to multiclass classification. In this paper, we propose a novel method for multiclass gene selection, which does not use ideal marker genes but directly analyzes the distribution of gene expression. It measures the class-discriminability by discretizing gene expression levels into several regions and analyzing the frequency of training samples for each region, and then classifies samples by using the naive Bayes classifier. We have demonstrated the usefulness of the proposed method for various representative benchmark datasets of multiclass cancer classification.

Patterns Analysis of Prefrontal Brain Waves of Cancer Patients using Brain-Computer-Interface (뇌-컴퓨터-인터페이스를 이용한 암환자들의 전전두엽 뇌파 분석)

  • Cancer patients have been suffered from the instability of mind/body and unbalanced homeostasis because of cancer progression and medical treatment such as chemotherapy, It is very important that appropriated actions can be promptly taken by monitoring cancer patients' mental conditions. For this reason, it is crucial to develop a monitoring method which is convenient and not harmful to their body. Brain-computer-interface(BCI) system is introduced for the purpose in this paper. Prefrontal brain waves of cancer patients and control groups have been measured by a portable neurofeedback(NF) system based on self-regulation of the human electroencephalogram(EEG). The NF system consists of the portable EEG amplifier and a headband with dry electrodes placed on Fp1 and Fp2 sites. Patterns of the prefrontal brain waves taken by computer are correlated to brain quotients by EEG-analysis program. Basic rhythm quotient, attention quotient, emotional quotient, anti-stress quotient and correlation quotient of control group have shown high significant level compared with the cancer patients group. On the other hand, the EEG patterns analysis is shown its possibility to be an important methodology of monitoring cancer patients' condition.

  • The study aims to suggests mobile application service that helps to reduce physical and mental suffering after surgery for breast cancer patients constantly increasing. At first, I analyze literature research to understand the breast cancer and similar services for achieving study purposes. After I finally design a mobile application that offers to customed one-to-one specialist coaching service focused on post-treatment management by using a double diamond design process model. In detail, first, I analyze the current program and comprehend user needs through interviews with the person such as breast patient, protector, nurse, salesperson in cancer center. Second, I specify the entire scenario, concept and keyword based on the persona and the customer journey map representing user. Third, I propose the mobile application for the ultimate goal by producing prototype to effectively communicate with specific functional descriptions. I hope this service will improve the quality of their lives.

Diagnosis of Location and Size of Lesions using Chest X-ray Image (X-선 영상을 이용한 암의 위치 및 크기 진단)

  • X-ray general radiography is the simplest and most important one to get a lot of information. Nevertheless, current x-ray general radiography does not observation in-depth observation. Information about the anatomy of the human body and changes in disease in x-ray general radiography can be obtained but it is difficult to determine the size and shape of the actual lesion due to the disadvantage of expanding the image. In this study, PA and LAT images were acquired and cancer magnification was calculated in the images by measuring the distance of cancer samples. By adjusting the magnification the actual cancer length and thickness were measured and compared with the CT image and the actual cancer sample size. After the PA and LAT images of the inserted 6.0 mm cancer sample were obtained and the magnification was corrected, the length was 5.9 mm and the thickness was 6.1 mm. This value was measured similarly to the actual. The problem of obtaining the magnification that needs to know the actual length from the detector to the cancer sample was secured by obtaining the magnification through PA and LAT images and it is possible to accurately measure the cancer sample size. X-ray general radiography may provide useful information in situations where CT imaging is difficult.

Rutin Suppresses Neoplastic Cell Transformation by Inhibiting ERK and JNK Signaling Pathways (Rutin의 ERK 및 JNK 신호전달체계 억제를 통한 암예방 효능)

  • Rutin is a well-known flavonoid found in buckwheat. Recent studies have demonstrated that the biological actions of rutin include anti-inflammatory and anti-cancer effects, but the underlying molecular mechanisms of these actions are not yet fully understood. Neoplastic cell transformation is considered a major event that contributes to carcinogenesis, and the present study aimed to determine whether rutin would exert anti-tumor effects via the results suggest that rutin exerted a potent inhibitory influence on the molecular activity of the MEK/ERK and MKK4/JNK pathways and strongly attenuated EGF-induced neoplastic cell transformation. These findings provide insight into the biological actions of rutin and the molecular basis for the development of new chemoprotective agents.
