Correlation of Unmet Healthcare Needs and Employment Status for a Population over 65 Years of Age (65세 이상 인구의 고용형태와 의료요구 미충족 경험률의 관련성)

  • The present study aimed to investigate the rate of unmet healthcare needs for elderly over the age of 65 years, as well as analyze the relevance between employment status and unmet healthcare needs due to financial reasons. With regard to the study method, a logistic regression analysis was performed to investigate the correlation between employment status and unmet healthcare needs due to financial reasons, targeting 5,528 subjects over the age of 65 years. The results showed that the rate of unmet healthcare needs was 18.9%, in which the rate of unmet healthcare needs due to financial reason was 8.1%. The rate of unmet health needs was higher for temporary workers(ORs=1.75) than for retirement workers. However, the rate of unmet healthcare needs caused by financial reasons was higher among day workers(ORs=1.92). In conclusion, in order to prevent unmet healthcare needs for senior Korean patients, it is necessary to not only improve the income security system for the elderly, but also improve the occupational form and level of income of these economically active citizens, considering the increase in average life expectancy. Moreover, it is also necessary to reinforce health insurance coverage systems for settling medical expenses.

Unmet Care Needs Among Community-dwelling Middle-aged and Older People in Korea (지역사회 거주 중고령자의 미충족 돌봄요구와 관련요인)

  • This study examined the prevalence of and factors associated with unmet care need among community-dwelling middle-aged and older people in Korea. Data were from the 2006 Korean Longitudinal Study on Ageing (KLoSA), a national survey of 10,254 non-institutionalized adults aged 45 or older. Having unmet care needs was defined as needing personal assistance with activities of daily living (ADL) or instrumental activities of daily living (IADL) but having no available helper. Weighted logistic regressions were fitted to examine factors associated with unmet care needs. Overall, 7.3%, 14.5%, and 41.8% of subjects among the middle-aged, younger old, and older old, respectively, reported care needs of these, 34%, 33%,and 24% had unmet needs. Factors associated with unmet needs differed among the three groups: Education and income level were negatively associated with unmet needs among the middle-aged, but living alone was the only factor positively associated with unmet needs in both the younger and older old. The prevalence of and factors associated with unmet care needs differ by life-stage. Needed are home- and community-based care and services to meet the need for personal assistance among the elderly living alone in a community.

Comparison of unmet need and quality of life between permanent ostomates and temporary ostomates (영구적 장루보유자와 일시적 장루보유자의 미충족 요구와 삶의 질 비교)

  • This study was conducted to compare the unmet needs and quality of life of permanent and temporary ostomates. The research subjects were 128 ostomates with colorectal cancer or other advanced cancers. Data were collected by self-reported completion of the Comprehensive Needs Assessment Tool in Cancer (CNAT) and the City of Hope Quality of Life (COH-QOL) Ostomy Questionnaire from February 1 to April 5, 2018. Collected data were analyzed by chi-squared and independent t-tests using IBM SPSS 21.0. The results showed that the unmet need for temporary ostomates was significantly higher than that of permanent ostomates (t=-2.284, P=0.024). Moreover, temporary ostomates showed a higher unmet need for information and education (t=-2.747, p=0.007), psychological problems (t=-2.578, p=0.011), and physicians (t=-2.599, p=0.010). Although quality of life of temporary ostomates seems lower than that of permanent ostomates, there was no significant difference between groups (t=-1.364, P=0.0175). Therefore, it is necessary to develop a self-management program to provide appropriate information and psychological support for temporary ostomates.

Convergence Study on Unmet Demand and Influencing Factors in Advanced Cancer Patients Receiving Conventional Chemotherapy (고식적 항암화학요법 받는 진행성 암환자의 미충족 요구와 영향요인에 대한 융합적 연구)

  • This study was attempted to use as basic data for developing a nursing intervention program suitable for terminal cancer patients by analyzing the unmet needs of advanced cancer patients receiving palliative chemotherapy and identifying the influencing factors. The subject of the study consisted of 71 patients with advanced cancer who were admitted to a university hospital and received palliative chemotherapy, and the research tool used CNAT (Comprehensive Needs Assessment tool in Cancer) developed by Shin et al., and data analysis was an independent sample. It was analyzed by t-test, ANOVA and multiple regression analysis. As a result of the study, the medical staff's area was the highest at 2.34 ± 0.63 out of 4 points, followed by psychological problems (2.09 ± 0.53) and information education (1.69 ± 0.45). Factors influencing unmet needs include educational background (β=.207, p=.047), thoughts on death (β=-.269, p=.018), diagnosis (β=-.283, p=. 013). Based on the results of this study, it is necessary to consider the needs of the medical staff in advanced cancer patients, provide psychological support, and consider ways to meet the needs according to academic background and diagnosis.

A Literature Review on Unmet Needs of High-Prevalence Cancer Survivors: Focus on Breast Cancer, Thyroid Cancer, Colorectal Cancer, and Lung Cancer (호발암 생존자의 미충족 수요에 대한 문헌 고찰: 유방암, 갑상선암, 대장암, 폐암을 중심으로)

  • Objectives : This study aimed to identify unmet needs and influencing factors for patients who have breast cancer, colorectal cancer, lung cancer, and thyroid cancer. Methods : We reviewed the SCIE publications on unmet need of four prevalent cancer patients published after 2010 through a web search. Results : The measurement tools primarily used were Cancer Survivors' Unmet Needs and Supportive Care Needs Survey questionnaire. Lung cancer patients reported a relatively higher rate of unmet needs. Breast cancer patients frequently reported unmet needs in the healthcare system and information, while thyroid cancer patients in post-treatment management and psychological issues. Colorectal cancer patients reported unmet needs in psychological and comprehensive care domain, and lung cancer patients reported unmet needs in physical and daily life management. Younger age, a shorter time since diagnosis or treatment, and higher levels of anxiety, depression, distress, and reduced quality of life were associated with more significant unmet needs. Conclusions : Unmet needs and influencing factors vary by cancer type. Considering the characteristics of each patient group and unmet needs can help in development of more effective treatment and support programs.

Analysis of Unmet needs by Elapsed Time in Kidney Transplantation Patients (신장이식 환자의 수술 후 경과기간에 따른 미충족요구 분석)

  • This study was conducted to for the development of nursing education programs suitable for the period by identifying the unmet needs of kidney transplant surgery patients who need continuous self-care according to the elapsed period. The subjects of the study consisted of 84 inpatients and outpatients for up to 1 year after kidney transplant surgery. The data collection period was from February 17 to May 31, 2020, and the collected data were analyzed by Chi-square, independent t-test, one-way ANOVA, and post-test by Scheffé using the IBM SPSS 21.0 program. As a measurement tool, the Comprehensive Needs Assessment Tool in Cancer(CNAT), a tool to assess unmet needs of cancer patients, was modified and supplemented to receive expert validity, and then self-report through a questionnaire consisting of a total of 57 questions. As a result of the study, the average of the total score out of 171points for the unmet needs of kidney transplant patients was 68.28(±38.88), and information and education accounted for the highest score of 17.92(±8.36) out of the eight sub-areas. The unmet needs according to the elapsed period were significantly higher in the 6 months ~1 year group(F=7.57, p=.001) compared to the 3-6 months group. Based on this, it is necessary to develop a nursing intervention program that can provide appropriate information and support at a time when the unmet needs of patients up to increase from 6 months to one year after kidney transplantation increase.

Unmet Medical Service Needs in Family Caregivers of Terminal Cancer Patients (말기암환자 가족간병인의 미충족 의료 분석)

  • Purpose: The unmet medical service needs of caregivers critically influence their caring for terminal cancer patients, but not much research has been done in this regard. Thus, the purpose of this study is to investigate the association between caregivers' characteristics and their unmet medical service needs. Methods: The survey was conducted with 109 family caregivers of terminal cancer patients admitted to four hospice units. The data were collected from March 2014 through December 2014 using a structured questionnaire. The unmet medical service needs were measured using 14 items which were adopted and modified by authors. Results: Seven areas of unmet medical service needs were shown to be significant. A well-educated group showed stronger needs for counsel about cancer screening and complementary-alternative medicine and health supplement food. A never-smoked group was identified with less need for sexual dysfunction counsel. Counsel about family and personal relations was more necessary for current drinkers and current workers, and less necessary for the married. Insurance counsel was more needed for a no-religion group. Occupation counsel was less necessary for healthy patients. Financial support was less necessary for the married group. Conclusion: Based on the results, it is highly recommended to further investigate the unmet medical service needs of family caregivers for terminal cancer patients and causes of the unmet needs.

Survey of Unmet Dental Treatment Needs among Adult Disabled (장애인의 미충족 치과치료 실태조사)

  • This study analyzed the rate of unmet dental care service for the disabled who were judged to require the dentistry due to oral health-related problems and those reasons, and also examined the oral health status according to the unmet dentistry. 375 subjects, who were 19 years or older and completed oral health examination and a survey, among 444 responded positive of disability registration after participation in National Health and Nutrition Examination Survey(NHANES), were analyzed. Disability type was classified into 6 kinds(physical disability, brain lesions, sensory defect, developmental disability, mental disorder, endocrine disorder). Participants had 128 of their dental care needs unmet when it was required. 'Financial burden' was the main reason, with the subject of 49.2%, followed by 'lower on the priority scale' were 18.0%. The high frequency and progression permanent teeth caries experiencs and periodontal status of their dental disease were confirmed. In availability aspect of dentistry resources, it is required for using dentistry service smoothly through providing sufficient proper dental care service comparing with the number of the disabled.

  • 미 육군은 21세기 미국의 국가 안보와 관련된다면 FCS에 대한 도박외는 다른 선택이 없다는 것을 알고 있다. 만일 성공한다면, FCS는 역사상 미 육군에 의해 취해진 가장 혁명적인 변형(transformation)의 산물이 될 것이다. FCS가 프로그램상 요구의 절반만을 충족시킨다고 할지라도, 빠르게 기동시키고, 결정적으로 싸울 수 있도록 미 육군의 능력을 현저히 향상시킬 것이다. 결론적으로 미 육군 지휘부 구조조정과 더불어 대대적인 Army Transformation을 추진하는데 핵심적인 요소 중의 하나인 미래전투체계(Future Combat System)의 성공적인 시행을 위한 시험 및 평가계획이 완성된 것으로 생각된다.
