초기 자궁경부암의 수술 후 방사선치료의 효과 (Postoperative Radiation Therapy in the Management of Early Cervical Cancer)

  • 목 적: 초기 자궁경부암 환자에서 수술 후 방사선치료의 결과 및 생존율에 영향을 미치는 예후인자를 알아보고자 하였다. 대상 및 방법: 1985년 6월부터 2002년 11월까지 본원에서 수술 후 방사선치료를 받은 133예의 자궁경부암 환자를 후향적으로 분석하였다 병기는 IB 113예, IIA 20예였고, 조직학적 분류는 상피세포암 118예, 선암 15예였다. 자궁경부 기질 내 침범 10 mm 초과 67예, 10 mm 이하 45예였다. 골반 림프절 양성 39예, 림프혈관강 침범 24예, 수술 절제연 양성 8예가 관찰되었다. 모든 환자를 $45{\sim}50.4\;Gy$의 외부방사선으로 치료하였으며, 수술 절제연이 양성이거나 불충분한 경우에는 $15{\sim}39\;Gy$의 강내치료를 추가하였다. 추적기간의 중간값은 48개월 이었다. 결 과: 대상 환자 전체의 5년 생존율 및 무병생존율은 88% 및 84%였다. 자궁경부 기질 내 침범 10 mm 초과한 환자에서는 5년 무병생존율이 감소하는 양상을 보였다(p<0.05). 골반 림프절 양성인 환자에서도 5년 무병생존율이 감소하는 양상을 보였다(p<0.05). 조직학적 분류, 림프혈관강 침범, 수술 절제연 양성은 5년 무병생존율의 감소와 무관하였다. 수술의 방법, 항암제 추가 또한 5년 무병생존율의 감소와 무관하였다. 재발의 양상은 골반 내 재발 4예, 원격전이 11예, 골반 및 원격전이 1예 등으로 관찰되었다. 치료에 따른 부작용은 수술을 요한 소장폐색 2예, 내과적 치료를 요한 혈뇨 2예가 관찰되었다. 결 론: 고위험인자를 가진 자궁경부암 환자에서 수술 후 방사선치료가 국소제어에 도움이 되지만, 원격전이를 막을 수 있는 보다 효과적인 치료가 필요하다고 생각된다. 전체 5년 생존율과 무재발 생존율은 각각 51.5%와 58.7%였다. N 병기에 따른 생존율은 N0, N1, N2에서 각각 100%, 53.7%, 0%였고(p=0.012), 무재발 생존율은 각각 100%, 47.6%, 41.2%였다(p=0.009). 종양의 위치에 따른 무재발 생존율은 상부, 중간부, 하부에서 각각 55.0%, 78.5%, 31.2%였다(p=0.006). 다변량 분석에서 5년 전체 생존율에 영향을 미치는 유의한 예후인자는 N 병기였고(p=0.012), 무재발 생존율에 영향을 미치는 예후인자는 N 병기와(p=0.001) 종양의 위치였다(p=0.006). 수술을 요하는 장 관련 후유증은 3명(6.5%)에서 발생하였다. 결 론: 직장암의 치료에서 근치적 수술 후 보조 화학방사선요법은 국소영역제어 측면에서는 효과적이었으나 원격제어에 대한 효과는 부족하다고 생각된다. 향후 생존율을 더욱 향상시키기 위해서는 원격실패를 감소시키려는 노력이 시도되어야 될 것으로 생각한다.만주의로 표면화된 하나의 일본근대 유화의 형식 그대로가 한국아카데미즘의 성격을 결정하고 있다고 할 수 있다. 게다가 1922년에 시작된 선전은 심사위원 대부분이 동경미술학교 교수이면서 구로다의 제자이거나 동료였다는 것은 이러한 사실을 더욱 확고하게 뒷받침하고 있는 것이다. 초기유학생들이 남긴 작품들은 완전한 인상파에 대한 지식이나 깊은 자아의식을 가지고 제작된 것은 아니라, 일본 동경미술학교에서 가르치고 있었던 것들을 그대로 수용했다고 할 수 있다. 따라서 이들의 그림은 구로다의 외광파라고 부르는 것들의 영향보다는, 인상파를 보고 배웠던 동경미술학교 교수들의 영향을 더 많이 받았다고 할 수 있다. 그리고 이들의 영향은 그대로 한국 근대미술의 아카데미즘을 형성하는데 커다란 영향을 미치고

  • 본 연구는 예술심리치료 및 심상유도의 접근을 통해 영상치료로서의 시각적, 청각적 표현 범위와 치료요소를 찾는데 목적을 두고 있다. 현재 인간의 사회 활동 중에 생기는 정신적 육체적 스트레스를 예방 및 해소하기 위해 이루어지는 치료 활동 중 영상을 이용한 심리치료의 노력이 다방면으로 행해지고 있다. 치료의 형태는 주로 예술심리치료에 속하는 이미지와 기능음악, 클래식 등을 결합한 영상과 음향의 복합적 활용 방식이 주를 이루고 있다. 그러나 이는 치료적 요소에 초점을 두고 체계적인 제작이 되지 앉아 심리치료의 한계를 드러내고 있다. 따라서 본 연구에서는 먼저 기존의 병원, 테라피 공간 및 공공장소에서 일반인을 대상으로 행해지고 있는 영상심리치료의 현황을 파악하고, 행해지는 치료 형태 분류를 통해 색채.미술심리치료에서 사용되어지는 이미지 활용과 음악치료 기법 중 GIM(Guided Imagery and music: 음악과 심상유도) 활용을 중심으로 일반인을 대상으로 하여 실험연구를 진행하였다. 실험 대상물은 영상이미지와 음향의 두 가지 자극이 복합되어 혹은 단일의 자극으로 주어지는 경우를 구분하여 정서적 반응을 조사하고, 전반적인 치료영상에 대한 선호도 조사를 통해 긍정적 정서를 불러일으키는 이미지 및 음향 요소를 도출한다. 이는 향후 영상심리치료의 체계적인 가이드라인 제작과 평가 척도 개발을 위한 기초 자료로 활용할 수 있다.

부모교육이론을 적용한 집단미술치료가 초등학생 어머니의 양육스트레스와 부모효능감에 미치는 효과 (The Effects of Group Art Therapy on Maternal Parenting Stress and Self-Efficacy)

  • The purpose of this study was to examine the effects of group art therapy on maternal parenting stress and self-efficacy. The subjects of the study included 16 parents of elementary school students who participated in a group art therapy program offered at B Education Center, Pusan. Eight parents who wanted to receive parental education were allocated into an experimental group, While the other eight parents were regarded as the control group. Mothers in the experimental group participated in the group art therapy program which discussed parent education theories. This Program was composed of 17 sessions, performed once a week for one and a half hours. Research tools employed included parent stress test and parent self-efficacy test. The following results were obtained: Group art therapy was found to bring about a statistically significant decrease in parenting stress. Group art therapy was also found to bring about a statistically significant increase in parent self-efficacy. These results suggest that a program which integrates various parents education theories, within a group art therapy setting, can become an effective parent education tool which decreases maternal parenting stress and increases parent self efficacy.

가출여중생의 자아존중감과 부모-자녀 의사소통 및 가족관계 향상을 위한 가족미술치료 사례연구 (A Case Study of Family Art Therapy to Improve a Runaway Middle School Girl's Self Esteem, Child-Parent Communication, and Family Relationship)

  • The purpose of this study is to investigate if and how much a family art therapy can influence a runaway middle school girl's self esteem, communication with parents, and family relations. Study subjects are a middle school girl (called S) and her parents, receiving a total of 13 sessions of art therapy from January 3 to March 28 of 2007, held at P-kindergarten located in Kyungbuk province, with each session of 90-120 minutes. Research instruments adopted for the study are a comparison between pre-test and post-test regarding S's self-esteem and a measure of dynamic relationship between S and her parents determined by KFD (Kinetic Family Drawing) in pre- and post-tests. The results are as follows. First, family art therapy shows its effect on the runaway middle school girls' self-esteem. Second, family art therapy proves to be effective on child-parent communication. Third, family art therapy displays positive influence on family relations.

통합미술치료가 발달장애 아동의 모-아 상호작용 증진에 미치는 효과 (The Effects of Integrative Art Therapy on Interaction Promotion of Mother and Child with Developmental Disorder)

  • The purpose of this study is to examine how integrative art therapy affected the interaction of children with developmental disorder with their mothers from the viewpoint of object-relation theory that accentuated mother role for the promotion of mother-child interaction, and based on earlier studies on mother-child interaction and the interaction of children with developmental disorder with their mothers. The subjects in this study are five selected children and their mothers who used B and G social welfare agencies in Gyeonggi province. They are at the age of five and six in Western age, and have difficulties interacting with their parents. When a social maturity test is conducted, their social age ranged from 2.1 to 3.9, and as Portage Test is implemented, their developmental levels of sociability, cognition and language were identical to those of younger children who were at the age of one to 3.5. The major findings of the study are as follows. First, the children who suffered from developmental disorder and participated in this study showed a positive change in their interaction with their mothers. Second, as for connections between each session and mother-child interaction, most of them came to interact with their mothers better as the program came to an end.

가정폭력 피해자녀들의 심리적 복지감을 위한 집단미술치료 프로그램의 효과 (The Impact of Group Art Therapy on the Psychological Well-Being of Children Who have Experienced Domestic Violence)

  • The purpose of this study was to evaluate the effectiveness of a group art therapy program on the psychological well-being of children who have experienced domestic violence. The therapy program was designed to enhance self-esteem and reduce anxiety for these children. The participants of this study included seven elementary school students and five junior high school students. The therapy program consisted of thirteen sessions that were held between September 25, 2002 and December 17, 2002. Each session lasted approximately 100 minutes. The effectiveness of this program was evaluated by a pretest and a post-test. The instruments used in the tests were the Anxiety Scale and Self-Esteem Scale. In addition, HND (Here and Now Drawing) was adopted for qualitative evaluation. The data were analyzed by paired t-tests. The results show the following: First, the group art therapy program was effective in enhancing the children's self-esteem and in reducing their anxiety. Second, there was a qualitative change in children's HND (Here and Now Drawing) after the therapy program, which proves that the group art therapy was effective.

  • Purpose: The purpose of this study was to evaluate the effects of a group art therapy on decreasing job stress and burnout of clinical oncology nurses. Methods: The study was conducted using a non-equivalent control group, non-synchronized design with repeated measures. A total of 29 clinical oncology nurses participated in the study; 15 were assigned in the experimental group, and 14 in the control group. Data were collected from May to July, 2014. The study questionnaires were administered in three different time points (pretest, 8th week, and 12th week). Data were analyzed using $x^2-test$, Fisher's exact test, t-test, paired t-test, and repeated measures ANOVA. Results: The first hypothesis "the experimental group with group art therapy will have lower job stress than that of the control group" was support (F=7.88, p=.003). The second hypothesis, "the experimental group will have lower burnout than that of the control group" was not statistically supported (F=1.80, p=.188). Conclusion: The findings of the study indicated that the group art therapy for oncology nurses may positively influence in decreasing job stress, which open a venue for developing a nursing intervention with group art therapy for clinical nurses in future.

집단미술치료가 결손가정아동의 정서에 미치는 영향 -우울, 불안, 충동성 중심으로- (Group Art Therapy Program of Low-income Broken; Effects, on Depression, Anxiety and Impulsivity)

  • This is to study the effects of the program of group art therapeuties for the low-income bracket children's melancholy and uneasiness. The experimental group is 19 children at their option of all 38 children in P child center at J city, Jeonbuk. The program has been going on after-school hours program at the facilities at 16times from May. 4 to August. 17, 2007. I have made the before-medium-after examination the COD, RCMAS and K-MFF through Repeated Measures ANOVA. The result is as follows, The melancholy of experimental group in comparison with that of control group is reduced by control group 0.42 and experimental group 2.67. And in lower rank group, the melancholy of experimental group for control group is dropped in the same way. In uneasiness, by the decrease of control group 0.10, experimental group 0.33, the uneasiness of experimental group for that of control group is decreased. In all drive, the drive of experimental group for that of control group is decreased by control group 0.12 and experimental group 0.33. And similarly, in all lower field, the drive of experimental group is reduced for that of control group. It is through our help to learn the proper way that they express their experience before the group members, listen to another person's story with attention, recognize their own and other's emotion, and are aware of feeling and action.

미술치료가 ADHD 아동의 문제행동과 또래관계에 미치는 효과 (The effect of Art Therapy apply to ADHD child's problem behavior and child relationship)

  • This study shows that Art therapy has positive effects on ADHD child's problem behavior and child relationship. Also we want to provide more data to education field. We work with 3 children. They are recommended by their class teacher and they are attending J elementary school which is located in K city in Chunbuk. The experiment period was 7 weeks - 2 times a week, total is 14th- and each time required was 40-60 minutes when they finish their school. Since we want to see each ADHD child's results, we use ADHD teacher evaluating measure and Child relationship measure to compare the results after the fact. The results are Art therapy has positive effects on the ADHD child's problem behavior specially concentration, excess behavior, social skill, hostile behavior and aggregation with same ages, sociality, lack of sociality, exclusive, unstable, and hypersensitiveness.

위축 아동을 위한 집단 미술치료프로그램의 효과 (The Effect of Group Art Therapy in Children with Withdrawn Behaviors)

  • Purpose: The purpose of this study was to examine the effects of group art therapy on the withdrawn behavior, social behavior, social problems, anxiousness/depression, internalizing problems, total behavior problems and self-esteem in children who are withdrawn. Method: A randomized controlled pre-post test design was used. The participants were 31 children who were withdrawn attending J. P. Elementary School in Pohang City. Sixteen were assigned to the experimental group and 15 to the control group. The program consisted of 20 sessions of 80 minutes per session, 2 days a week, for 10 weeks. Instruments used for this study were the Children's Behavior Check List -Korean version (K-CBCL) and a self-esteem inventory. The data were analyzed using fisher exact test and Mann-Whitney U test. Results: Thescores for withdrawn behavior, social problems, anxiousness/depression, internalizing problems and total behavior problems decreased significantly in the experimental group as compare to the control group. Social behavior and self-esteem increased significantly in the experimental group as compared to the control group. Conclusions: This study provides evidence for the pontential and beneficial effects of group art therapy in children who are withdrawn. The program could be adequately used to improve problem behavior in withdrawn children.
