미세혈관문합 후 혈관내벽의 치유과정에 관한 실험적 연구 (EXPERIMENTAL STUDIES ABOUT HEALING PROCESS OF BLOOD VESSELS FOLLOWING MICROVASCULAR ANNASTOMOSES)

  • Microvascular surgery has been widely used in the clinical field of replantation and reconstructive surgery. Since the last 20 years, microsurgical techniques and instruments have been rapidly developed and the success rate is remarkably increased. But thrombotic occlusion of vessels remains the major reason for clinical failure. The change of vessel wall is the most important factor in thrombus formation. If we can reduce the traumatic changes in the vessel walls during surgery, the success rate can be markedly increased. For this study, femoral arteries and veins of 36 Sprague-Dawley rats with average weights of 300gm were used. The author observed the histological changes and healing process in the anastomostic site after 1 hour, 24 hours, 1, 2, 3 and 4 weeks under light microscopy and scanning electron microscopy. The results were as follows : 1. The patency rate was 100% in femoral arteries and 85% in femoral vein. 2. At the early stages after microvascular anastomosis, the loss of endothelial cell in the vessel walls was observed in the wide area including anastomotic site. In scanning electron microscopic finding the anastomotic site was covered with much fibrin, many red blood cells and some platelets. 3. At 1st week, new endothelial cells were formed toward anastomotic site and at 3rd week, the anastomotic site was completely covered by new endothelial cells. At 4th week, the complete endothelialization over the threads was observed. 4. The media extended from the anastomotic site toward the end of the specimen. At later stages, the extent of media necrosis was markedly decreased. But the media necrosis of anastomotic site was not regenerated till 4th week. 5. Intimal hyperplasia appeared at 1st week and increased till 4th week. The layer consisted of endothelialization the most luminal layers and smooth muscle in the deeper layers. But in veins, the response was less pronounced than in arteries. 6. Foreign body granuloma remained during 4 weeks and aneurysm was observed at 3rd week in artery. In aneurismal wall, media necrosis, loss of elastic lamina and intimal hyperplasia were seen.

모야모야 환자에게서의 인공심폐기를 이용한 개심술 -1예 보고- (Cardiac Surgery Using CPB in Moyamoya Disease - A Case Report-)

  • 모야모야 질환은 원위부 내경 동맥과 근위부 뇌동맥들의 폐쇄성 내피 이형증으로 특징지워는 뇌혈관 질환으로, 아직까지 정확한 병인이 밝혀지지 않은 질환이다. 상기 질환은 양측 내경 동맥의 원위부에 부분적인 협착이나 폐쇄를 야기하게 되고, 이러한 뇌혈관의 해부학적인 변이는 뇌의 기저부에 매우 미세한 혈관들의 문합을 형성하게 하는데, 이러한 신생 혈관들이 뇌혈관 조영검사상 구름처럼 보인다 하여 ‘Moyamoya’라 불리운다. 모야모야 환자군들에게서 관상 동맥 질환 등으로 인한 개심술의 시행은 매우 드물다고 알려져 있으며, 이러한 환자군들에서의 인공심폐기를 이용한 개심술은 수술 중, 그리고 수술 후 허혈성 뇌질환 등의 위험성이 매우 높다고 보고되고 있다. 본 교실은 모야모야 환자에서 심방중격결손 폐쇄와 관상동맥우회술을 인공심폐기를 이용한 개심술을 시행하여 성공적으로 치료하였기에 간략한 문헌 고찰과 함께 보고하는 바이다.

유리 피판을 이용한 인두식도 결손의 재건 (Reconstruction of Pharyngoesophageal Defects Using free Flaps)

  • 저자들은 1990년 12월부터 1999년 2월까지 48례의 경우에서 하인두에 발생한 악성종양을 광범위 절제한 후 유리피판 또는 근피판을 이용하여 재건하였다. 39례에서 유리공장 피판, 5례에서 유리전박 피판을 tubing 형태로 사용했으며, 2례에서는 유리전박 피판을 patch 형태로 사용하였고, 2례에서는 대흉근 근피판을 이용하여 경부식도를 재건하여 다음과 같은 결과를 얻을 수 있었다. 1. 하인두의 악성종양이 고령의 나이에 발생한다는 사실을 감안했을 때 유리전박 피판에 비해 급양공장루를 통해 조기에 영양섭취가 손쉬운 유리공장 피판이 환자의 상태를 정상으로 회복시키는데 장점이 있었다. 2. 술후 가장 흔한 합병증인 누공의 발생은 문합을 제대로 시행했을 경우 우려할 필요 없으며, 따라서 술후에 시행하는 식도조영검사는 누공의 증상이 있는 경우에만 선별적으로 실시해야 할 것이다. 3. 문합부 내경의 협착이 우려될 때는 직경이 큰 비강영양튜부(nasogastric tube)를 조기에 삽입하여 극단적인 협착을 감소시키고, 영양섭취 경로를 확보해야 한다. 4. 문합부 협착을 예방하기 위해 상하부 문합부 모두를 파형으로 도안하여 피판을 문합 봉합해야 하며, 협착이 의심스러울 때는 내시경검사를 시행하여 확진해야 한다. 5. 혈관문합은 유리전박 피판을 시행하는 경우에 있어서 수월하였으며, 유리공장 피판을 시행할 때는 술전에 정맥이식을 고려해야 한다.

전완부 유리피판술을 이용한 연부조직 결손의 치료 (Treatment of the Soft Tissue Defect in Extremities by Forearm Free Falp)

  • 사지의 연부조직 결손으로 고려대학교 의과대학 정형외과학 교실에서 입원했던 12명의 환자 13례에 대하여 전완부 유리 피판술을 시행 후 최저 3개월에서 최고 37개월간 평균 14.3개월간 추시하여 다음과 같은 결론을 얻었다. 1. 13례 전례에서 연부조직 결손의 치료로 만족할만한 결과를 얻었다. 2. 이식피판의 크기는 평균 $54cm^2$였으며 유리피판의 평균 혈류 차단시간은 74분 이었다. 3. 술후 3례에서 합병증이 발생하였으며 3례중 2례는 동맥 문합부위에 혈전이 발생하였으나 술후 제 2일에 혈전 재거술을 시행하여 해결하였으며, 1례는 이식 정막내의 혈전으로 이식피판의 표층괴사가 발생하여 고식적인 피부이식술로 치료하였다. 4. 체중 부하를 받는 족저부의 재건에는 감각신경을 포함한 전완부 유리피판술이 좋은 방법의 하나이다. 5. 전완부 피판술은 비교적 넓은 연부조직 결손을 치료할 수 있고 이식 혈관의 내경이 커서 문합이 비교적 쉬워 숙련된 미세수술 수기를 익힌 외과의사에게는 성공률이 높은 유리피판술 중의 하나이다.

스트렙토조토신으로 유도된 당뇨백서에서 미세혈관문합술후 혈관의 조직병리학적 연구 (Histopathologic study on the microvascular anastomosis of streptozotocin induced diabetic rats.)

  • Purpose : The purpose of this study is to estabilish clinical guidance of microvascular anastomosis in diabetic patients. This study was performed with experimental microvascular anastomosis in streptozotocin induced diabetic rats and observed histopathologic change and endohelial healing process. Materials and Methods : 70 Sprague-Dawley rats, weighting 200 to 250grams, were used for the experiment. 35 induced diabetic rats with streptozotocin and 35 control group were selected. After end-to-end carotid artery microvascular anastomosis was done, the experimental rats were sacrificed at different time interval (1st day, 3rd day, 1st week, 2nd, 4th, 6th and 8th week) for histologic examination. Light microscope observation was used in this study. Results : 1. Histopathologic changes are nearly the same healing process in two groups. But period of tissue reaction was faster in the control than diabetic group. 2. In endotheliall healing, control group started at 1 week after and completed at 4 weeks after, but diabetic group was observed partially at 4 weeks after and complete healing was not observed still at 8 weeks after. 3. In subintimal hyperplasia, control group was observed at 6 weeks after but diabetic group was observed at 6 weeks after and partially at 8 weeks after. 4. All groups showed severe inflammatory response in the early period. This respond is decreased at 2 weeks after in control group but still remained at 8 weeks after in the diabetic group. 5. In media, inflammatory response and degeneration were observed in early period. Regeneration of smooth muscle cell was observed at 1 week after in control group but 4 weeks after in the diabetic group. Conclusions : As the results of study, it could be thought that vascular regeneration process was not failured but delayed in the diabetes. It was considered that diabetes mellitus was not absolute contraindication of microvascular anastomosis.

Laser를 이용한 새로운 미세혈관 문합술의 실험적 연구 (Experimental Study of Laser Assisted Microvascular Anastomosis(LAMA) Using the Nd:YAG Contact Laser)

  • A comparative study was undertaken to evaluate the contact Neo-dymium : yttrium aluminum garnet(Nd:YAG) laser system for vascular anastomosis of small caliber blood vessels(diameter 0.5-1.2 mm) in the animal model. In this study 40 femoral arteries and 40 femoral veins of Sprague-Dawley rats were anastomosed by contact laser assisted microvascular anastomosis(LAMA) utilizing 3 stay sutures which were placed 120 degrees apart and the intervals welded with contact Nd:YAG laser unit, conventionally sutured anastomosis(CSA) served as controls. The time needed for vascular anastomosis, patency rate(immediate postoperative, postoperative 2nd day, postoperative 1 week, postoperative 4 weeks), gross and microscopic evaluations were compared to conventional microsurgical suture technique. The results are as follows: 1. Postoperative patency rate was 82.5% for femoral artery and 75% for femoral vein by contact LAMA technique compared to 90% and 75% by CSA technique at postoperative 4 weeks. 2. Less time-consumed for arterial anastomosis by 6 minutes 23 seconds and venous anastomosis by 8 minutes 55 seconds with contact LAMA technique compared to CSA technique. 3. Grossly almost complete healing had taken place by postoperative 1 week by contact LAMA technique. 4. Aneurysm formation was 5% for femoral artery and 15% for femoral vein by contact LAMA technique compared to 5% and 10% respectively by CSA technique. 5. Microscopically, re-endothelization was complete by postoperative 7th day by contact LAMA technique. There was less medial hypertrophy and hyperplasia and also less inflammatory response compared to CSA.

당뇨백서(糖尿白鼠)의 대퇴동맥(大腿動脈) 미세혈관문합(微細血管吻合) 후(後) 내피세포(內皮細胞)의 재생(再生)에 관한 연구(硏究) (RE-ENDOTHELIZATION OF MICROVASCULAR ANASTOMOSIS IN DIABETIC RAT FEMORAL ARTERY ; A SCANNING ELECTRON MICROSCOPIC STUDY)

  • Recently, diabetic patients are increasing in the field of microvascular surgery. Diabetes melltius is known to be related to arterial damage, platelet malfunction and thrombus formation. After microvascular anastomosis, delayed repair and vascular occlusion occurred more frequently in diabetic state. This study was performed to investigate the patency rate and process of endothelial healing after microvascular anastomosis of femoral artery in diabetic rat by scanning electron microscope. The animals were divided into two groups, 20 diabetic-induced and 20 non-diabetic groups. Diabetes was induced with a injection of Streptozotocin(50mg/kg b.w., Sigma Chemical Co.) to tail vein. The results obtained were as follows: 1. Macroscopically, anastomotic site was intact except a few cases showed minimal inflammatory sign around the wound site. But the inflammatory change was frequently occurred in diabetic-induced group. 2. The patency rate was 95% (19/20) in non-diabetic group and 65% (13/20) in diabetic-induced group. 3. In the non-diabetic group, anstomotic region was mostly endothelized by the alignment along the long axis of vessel but stitchs were not covered with endothelial cells. The thichkening of vessel wall was not observed. 4. In the diabetic-induced group, anastomotic region was not endothelized but covered with blood cellular components and connective tissue instead of endothelial cells. The thickening of the vessel wall was prominent in some diabetic-induced rats. These results suggest that diabetes was related to delayed regeneration of endothelium of vessels after microsurgical anastomosis.

항혈전성 소구경 인조 혈관 및 봉합편에 대한 동물 실험 (Animal Experiments on an Antithrombogenic Small-Caliber Vascular Prostheses and Vascualr Patch : Observation in Canine Models)

  • 최근 증가 추세에 있는 동맥 경화성 혈관 질환에 대한 가장 중요한 외과적 치료 도구로서 생체 적합성이 우수한 인조 혈관의 필요성이 더욱 강조되고 있는 실정이다. 새로운 항혈전성 중합체 혈액 접촉 표면으로서 폴리비닐알코올-폴리우레탄 융합체와 폴리우레탄-흑연 융합체를 개발하여 실험견의 대퇴동맥에 장단기 삽입 실험을 시행함으로써 그 혈액 적합성 및 조직 적합성을 분석, 연구하는데 본 연구의 목적이 있다. 대상 및 방법 : 폴리비닐알코올-폴리우레탄 융합체로 제작된 소구경 인조 혈관 및 폴리우레탄-흑연 융합체로 제작된 봉합편과 폴리우레탄만으로 제작된 소구경 인조 혈관 및 봉합편을 6마리의 실험견에 삽입 실험(1, 2, 4, 6, 8, 16주)을 시행하였고, 4마리의 실험견에 폴리우레탄-흑연 융합체 봉합편 및 폴리우레탄 봉합편 삽입실험(1, 2, 4, 6주)을 시행하여 혈액 적합성 및 조직 적합성을 비교, 분석하였다. 결과 : 폴리비닐알코올-폴리우레탄 융합체 인조 혈관 및 폴리우레탄 인조 혈관 삽입 실험에서 1, 4, 6, 8주 실험견에서는 실험군 및 대조군 모두에서 개방성이 관찰되지 않고 혈전으로 폐쇄되어 있었고, 2주 실험견에서는 모두 개방성이 관찰되었다. 그러나 16주 실험견에서는 폴리비닐알코올-폴리우레탄 융합체 인조 혈관만이 개방성을 유지하고 있는 것이 관찰되었는데 혈액 접촉 표면은 혈전 부착이 거의 없는 깨끗한 상태를 유지하고 있었고 병리 조직 검사에서 인조 혈관의 유공성 사이로 주위 조직의 침투가 관찰되었다. 반면 폴리우레탄 인조 혈관에서는 문합부를 중심으로 혈전 형성으로 인한 폐쇄가 관찰되었다. 폴리우레탄-흑연 융합체 봉합편 및 폴리우레탄 봉합편 삽입 실험에서 2주 실험견을 제외하고는 실험군 및 대조군 모두에서 개방성을 유지하고 있는 것이 관찰되었다. 개방성을 유지하였던 봉합편에서는 봉합편 내면 위로 혈전 형성은 관찰되지 않았으며 4, 6주 실험견에서는 봉합편 내면 위로 신내막이 형성되어 있는 것이 관찰되었다. 주사형 전자 현미경 관찰 결과 폴리우레탄-흑연 봉합편의 경우 1주 실험견에서는 봉합편 내면에 섬유소가 피복되어 있었으며 간헐적으로 세포가 혼합되어 있는 것이 관찰되었다. 이와 대조적으로 4주 실험견에서는 뚜렷한 초기 형태의 내피세포가 섬유소층을 덮어서 자라고 있는 것이 관찰되었다. 이 시기에 관찰한 내피세포의 확대 사진에는 내피세포의 활성을 나타내는 미세융모가 잘 관찰되었다. 그리고 6주 실험견에서는 내피세포가 보다 성숙하여 내피세포간 단층으로 밀접한 연결을 이루면서 상대적으로 납작한 모양을 보여 마치 자갈 모양의 전형적 형태를 취하고 있었다. 폴리우레탄 봉합편의 경우 전체적으로는 폴리우레탄-흑연 봉합편과 유사한 소견을 보였지만 4, 6주 실험견에서 상대적으로 내피세포 피복 정도가 적은 것이 관찰되었다. 그러나 전자 현미경 분석의 제한상 두 군간에 내피세포 피복 정도를 정량화하기는 어려웠다. 이상의 결과로 폴리비닐알코올-폴리우레탄 융합체와 폴리우레탄-흑연 융합체는 대조군인 폴리우레탄 인조 혈관 및 봉합편에 비해 일부 우수한 양상을 보였지만 본 실험의 범위내에서는 통계적 정량적 차이를 제시할 수는 없었다. 향후 보다 광범위한 동물 실험이 필요할 것으로 사료된다.

무지 피부 결손에 신경 분리-문합을 이용한 이종 수지 신경혈관 도서형 유경 피판술의 결과 (Results of Thumb-pulp Loss Reconstruction with a Heterodigital Neurovascular Island Flap Using a Nerve Disconnecting-Reconnecting Technique)

  • Purpose : The problems of late deterioration of sensation and a double sensibility phenomenon are a known deficiency of the original neurovascular island flap. To eliminate this problems, the authors was performed to assess the results of Thumb-pulp loss reconstruction with a heterodigital neurovascular island Flap Using a Nerve Disconnecting-Reconnecting Technique Material and Methods : Between July. 1987 and November. 2000, we have used a disconnecting-reconnecting technique for heterodigital island flaps in which divided nerve of the transferred island flap is sutured to original nerve of the thumb in 9 patients. 9 patients with a mean follow-up period of 3.6 years were examined, while 7 patients were follow-up for 2years or more. Evaluation of results was based on flap quality, donor site quality, classic two-point sensory discrimination(2PD), and location of sensation(double sensibility), donor site cold intolerance, neuroma formation, scar contractures, total active movement(TAM) of the thumb and donor digit and subjective results and objective results. Results : The two-point discrimination was average 6.7 mm. Donor site paresthesia was observed more frequently in a short postoperative period. Sensation was referred to the thumb in all cases and the recovery occurred over a period of a two months. There were no flap complication or failures and the length of the thumb tip was preserved in all cases. Conclusion : This technique was found to provide well-vascularized pulp of the thumb with proper cortical representation in one operative stage.

수지접합술의 생존율과 문합혈관수의 상관관계 (Correlation Between the Number of Anastomosed Vessels and Survival Rate in Digit Replantation)

  • From January 1990 to December 1998, 449 consecutive single-digital replantations were reviewed retrospectively in order to determine the essential number of vascular anastomoses for successful finger replantation. The correlations between the number of anastomosed vessels and survival rate were examined according to the amputated digital levels and all of the correlated results were compared with each other statistically. In zone I, The survival rate of the digits with a repaired vein was higher than that of digits treated with external bleeding method. In zone II, the equal number(s) between the arterial and venous repair was an important factor in successful replantation. And the repaired arteries more than repaired veins in number led to venous congestion and resulted in a failure of replantation, which was maybe due to the large amount of arterial input relative to small volume of amputated stump with small sized vein. In zone III, the equal number(s) between the arterial and venous repair was also an important factor in successful replantation. But unlike in zone II, venous congestion was scarcely happened if the venous drainage was sufficient with a repaired large vein alone. In zone IV, two or more arteries and veins were required for successful replantation. In conclusion, it is desirable that the repair of vessels as many as possible to increase the possibility of a good result. But digital amputations and their condition for replantation were variable, therefore, the numbers in vascular repair should to be modified and straightforward as the case may be.
