Localized nodular synovitis of the knee joint is a rare benign tumorous condition, and should be differentially diagnosed with pigmented villonodular synovitis. We report a case of localized nodular synovitis in the knee that was noted to have a pedicle and characteristic findings on MR image.

In this study, the femur, the tibia, the articular cartilage and the menisci are three dimensionally reconstructed using MR images of healthy knee joint in full extension of 26-year-old male. Three dimensional finite element model of the knee joint is fabricated on the reconstructed model. Also, the FE models of ligaments and tendons are attached on the biologically suitable position of the FE model. Bones, articular cartilages and menisci are considered as homogeneous, isotropic and linear elastic materials, and ligaments and tendons are modeled as truss element and nonlinear elastic springs. The numerical results show the contact pressure and the von Mises stress distribution in the soft tissues such as articular cartilages and menisci which can be regarded as important parameters to estimate the failure of the tissues and the pain of the patients.

In this paper, we propose an automated segmentation method of femoral cartilage in knee MR images using multi-atlas-based locally-weighted voting. The proposed method involves two steps. First, to utilize the shape information to show that the femoral cartilage is attached to a femur, the femur is segmented via volume and object-based locally-weighted voting and narrow-band region growing. Second, the object-based affine transformation of the femur is applied to the registration of femoral cartilage, and the femoral cartilage is segmented via multi-atlas shape-based locally-weighted voting. To evaluate the performance of the proposed method, we compared the segmentation results of majority voting method, intensity-based locally-weighted voting method, and the proposed method with manual segmentation results defined by expert. In our experimental results, the newly proposed method avoids a leakage into the neighboring regions having similar intensity of femoral cartilage, and shows improved segmentation accuracy.

In this paper, we propose an automatic segmentation method of meniscus in knee MR images by automatic meniscus localization, multi-atlas-based locally-weighted voting, and patch-based edge feature classification. First, after segmenting the bone and knee articular cartilage, the volume of interest of the meniscus is automatically localized. Second, the meniscus is segmented by multi-atlas-based locally-weighted voting taking into account the weights of shape and intensity distribution in the volume of interest of the meniscus. Finally, to remove leakage to the collateral ligaments with similar intensity, meniscus is refined using patch-based edge feature classification considering shape and distance weights. Dice similarity coefficient between proposed method and manual segmentation were 80.13% of medial meniscus and 80.81 % for lateral meniscus, and showed better results of 7.25% for medial meniscus and 1.31% for lateral meniscus compared to the multi-atlas-based locally-weighted voting.
Purpose : To investigate the feasibility and accuracy of Proton Resonance Frequency (PRF) shift based magnetic resonance (MR) temperature mapping utilizing the self-developed center array-sequencing phase unwrapping (PU) method for non-invasive temperature monitoring. Materials and Methods : The computer simulation was done on the PU algorithm for performance evaluation before further application to MR thermometry. The MR experiments were conducted in two approaches namely PU experiment, and temperature mapping experiment based on the PU technique with all the image postprocessing implemented in MATLAB. A 1.5T MR scanner employing a knee coil with $T2^*$ GRE (Gradient Recalled Echo) pulse sequence were used throughout the experiments. Various subjects such as water phantom, orange, and agarose gel phantom were used for the assessment of the self-developed PU algorithm. The MR temperature mapping experiment was initially attempted on the agarose gel phantom only with the application of a custom-made thermoregulating water pump as the heating source. Heat was generated to the phantom via hot water circulation whilst temperature variation was observed with T-type thermocouple. The PU program was implemented on the reconstructed wrapped phase images prior to map the temperature distribution of subjects. As the temperature change is directly proportional to the phase difference map, the absolute temperature could be estimated from the summation of the computed temperature difference with the measured ambient temperature of subjects. Results : The PU technique successfully recovered and removed the phase wrapping artifacts on MR phase images with various subjects by producing a smooth and continuous phase map thus producing a more reliable temperature map. Conclusion : This work presented a rapid, and robust self-developed center array-sequencing PU algorithm feasible for the application of MR temperature mapping according to the PRF phase shift property.

Purpose : To determine the incidence of truncated triangle appearance of anterior horn (AH) to body of medial meniscus (MM) and determine its clinical significance. Materials and Methods: IRB approval was obtained, and informed consent waived for this study. The criteria of "pseudoradial tear" was truncated triangle appearance of the tip of AH to body of MM on one or more coronal images with adjacent fluid signal intensity at the blunted tip. Two musculoskeletal radiologists retrospectively evaluated 485 knee MR images independently for the presence and number of sections with "pseudoradial tear" of AH to body of MM using proton density-weighted coronal MR images. Inter-and intraobserver agreement was calculated using kappa coefficients. Medical records were reviewed for arthroscopic correlation. Results: A pseudoradial tear in the AH to body of MM was present in 381 (78.6%) patients. Locations were 112 in AH (29.4%), 143 in AH to body (37.5%), and 126 in body (33.1%). Number of consecutive sections of pseudoradial tear were 1 in 100 (26.2%), 2 in 164 (43.0%), 3 in 94 (24.7%), 4 in 21 (5.5%), and 5 in 2 (0.5%). Interobserver agreement was 0.99 for presence and 0.43 for number of sections of pseudoradial tear. Arthroscopies were performed in 96 patients and none of the pseudoradial tears were proven as true radial tears on arthroscopy. Conclusion: Pseudoradial tears are frequently seen in AH to body of MM on coronal MR images and may be another pitfall that a radiologist needs to be aware of and be able to differentiate from true radial tear.

In this study, we intended to design the optimal Fermi filter to apply the k-space date that is knee image of the rheumatoid arthritis patient acquired from the MRI (magnetic resonance imaging) instrument. After deciding the suitable coefficient for the Fermi filter, the results were compared with modified Fermi filter and inverse Chebyshev filter, Chebyshev filter, Elliptic filter and Butterworth filter. Firstly, in comparison to the results, the radiologist confirmed that modified Fermi filter was best decision for boundary of the rheumatoid arthritis images. The number of the black voxels of the histogram showed the quantity of the results. At the proposed filter images, numbers of the blacks voxels were statistically decreased. That meant voxels only appeared the black color were changed to others voxels color. Because the number of the total voxels was fixed, the area appeared block color could be effected to the other areas. If the modified Fermi filter were used for rheumatoid arthritis patient, the result will be better than other filters.

Purpose : Within a clinically acceptable time frame, we obtained the high resolution MR images of the human brain, knee, foot and wrist from 3T whole-body MRI system which was equipped with the world first 37 active shield magnet. Materials and Methods : Spin echo (SE) and Fast Spin Echo (FSE) images were obtained from the human brain, knee, foot and wrist of normal subjects using a homemade birdcage and transverse electromagnetic (TEM) resonators operating in quadrature and tuned to 128 MHz. For acquisition of MR images of knee, foot and wrist, we employed a homemade saddle shaped RF coil. Topical common acquisition parameters were as follows: matrix=$512{\times}512$, field of view (FOV) =20 cm, slice thickness = 3 mm, number of excitations (NEX)=1. For T1-weighted MR images, we used TR = 500 ms, TE = 10 or 17.4 ms. For T2-weighted MR images, we used TR=4000 ms, TE = 108 ms. Results : Signal to noise ratio (SNR) of 3T system was measured 2.7 times greater than that of prevalent 1.5T system. MR images obtained from 3T system revealed numerous small venous structures throughout the image plane and provided reasonable delineation between gray and white matter. Conclusion The present results demonstrate that the MR images from 3T system could provide better diagnostic quali\ulcorner of resolution and sensitivity than those of 1.5T system. The elevated SNR observed in the 3T high field magnetic resonance imaging can be utilized to acquire images with a level of resolution approaching the microscopic structural level under in vivo conditions. These images represent a significant advance in our ability to examine small anatomical features with noninvasive imaging methods.
Magnetization Transfer (MT) imaging generates contrast dependent on the phenomenon of magnetization exchange between free water proton and restricted proton in macromolecules. In biological materials in knee, MT or cross-relaxation is commonly modeled using two spin pools identified by their different T2 relaxation times. Two models for cross-relaxation emphasize the role of proton chemical exchange between protons of water and exchangeable protons on macromolecules, as well as through dipole-dipole interaction between the water and macromolecule protons. The most essential tool in medical image manipulation is the ability to adjust the contrast and intensity. Thus, it is desirable to adjust the contrast and intensity of an image interactively in the real time. The proton density (PD) and T2-weighted SE MR images allow the depiction of knee structures and can demonstrate defects and gross morphologic changes. The PD- and T2-weighted images also show the cartilage internal pathology due to the more intermediate signal of the knee joint in these sequences. Suppression of fat extends the dynamic range of tissue contrast, removes chemical shift artifacts, and decreases motion-related ghost artifacts. Like fat saturation, phase sensitive methods are also based on the difference in precession frequencies of water and fat. In this study, phase sensitive methods look at the phase difference that is accumulated in time as a result of Larmor frequency differences rather than using this difference directly. Although how MT work was given with clinical evidence that leads to quantitative model for MT in tissues, the mathematical formalism used to describe the MT effect applies to explaining to evaluate knee disorder, such as anterior cruciate ligament (ACL) tear and meniscal tear. Calculation of the effect of the effect of the MT saturation is given in the magnetization transfer ratio (MTR) which is a quantitative measure of the relative decrease in signal intensity due to the MT pulse.

Recently, medical equipments are developed and used for diagnosis or studies. In addition, demand of techniques which automatically deal with three dimensional medical images obtained from the medical equipments is growing. One of the techniques is automatic bone segmentation which is expected to enhance the diagnosis efficiency of osteoporosis, fracture, and other bone diseases. Although various researches have been proposed to solve it, they are unable to be used in practice since a size of the medical data is large and there are many low contrast boundaries with other tissues. In this paper, we present a fast and accurate automatic framework for bone segmentation based on multi-resolutions. On a low resolution step, a position of the bone is roughly detected using constrained branch and mincut which find the optimal template from the training set. Then, the segmentation and the registration are iteratively conducted on the multiple resolutions. To evaluate the performance of the proposed method, we make an experiment with femur and tibia from 50 test knee magnetic resonance images using 100 training set. The proposed method outperformed the constrained branch and mincut in aspect of segmentation accuracy and implementation time.
