Comparison and Evaluation of radiotherapy plans by multi leaf collimator types of Linear accelerator (선형가속기의 다엽콜리메이터 형태에 따른 치료계획 비교 평가)

  • Purpose : An aim of this study was to compare the effect of multi leaf collimator(MLC) types for high dimension radiotherapy in treatment sites used clinically. Material and Method : 70 patients with lung cancer, spine cancer, prostate cancer, whole pelvis, head and neck, breast cancer were included in this study. High definition(HD) MLC of TrueBeam STx (Varian Medical system, Palo Alto, CA) and millenium(M) MLC of VitalBeam (Varian Medical system, Palo Alto, CA) were used. Radiotherapy plans were performed for each patient under same treatment goals with Eclipse (Version 13.7, Varian Palo Alto USA, CA). To compare the indicators of the radiotherapy plans, planning target volume(PTV) coverage, conformity index(CI), homogeneity index(HI), and clinical indicators for each treatment sites in normal tissues were evaluated. To evaluate low dose distribution, $V_{30%}$ values were compared according to MLC types. Additionally, length and volume of targets for each treatment sites were investigated. Result : In stereotatictic body radiotherapy(SBRT) plan for lung, the average value of PTV coverage was reduced by 0.52 % with HD MLC. With SBRT plan using HD MLC for spine, the average value of PTV coverage decreased by 0.63 % and maximum dose decreased by 1.13 %. In the test of CI and HI, the values in SBRT plan with HD MLC for spine were 1.144, 1.079 and the values using M MLC were 1.160, 1.092 in SBRT plan for lung, The dose evaluation of critical organ was reduced by 1.48 % in the ipsilateral lung mean dose with HD MLC. In prostate cancer volumetric modulated arc therapy(VMAT) with HD MLC, the mean dose and the $V_{30}$ of bladder and the mean dose and the $V_{25}$ of rectum were reduced by 0.53 %, 1.42 %, 0.97 %, and 0.69 %, respectively (p<0.05). The average value of heart mean dose was reduced by 0.83 % in breast cancer VMAT with M MLC. Other assessment indices for treatment sites showed no significant difference between treatment plans with two types of MLC. Conclusion : Using HD MLC had a positive impact on the PTV coverage and normal tissue sparing in usually short or small targets such as lung and spine SBRT and prostate VMAT. But, there was no significant difference in targets with long and large such as lung, head and neck, and whole pelvis for VMAT.

A study of usefulness for the plan based on only MRI using ViewRay MRIdian system (ViewRay MRIdian System을 이용한 MRI only based plan의 유용성 고찰)

  • Purpose : By comparing a CT fusion plan based on MRI with a plan based on only MRI without CT, we intended to study usefulness of a plan based on only MRI. And furthermore, we intended to realize a realtime MR-IGRT by MRI image without CT scan during the course of simulation, treatment planning, and radiation treatment. Materials and Methods : BBB CT (Brilliance Big Bore CT, 16slice, Philips), Viewray MRIdian system (Viewray, USA) were used for CT & MR simulation and Treatment plan of 11 patients (1 Head and Neck, 5 Breast, 1 Lung, 3 Liver, 1 Prostate). When scanning for treatment, Free Breathing was enacted for Head&Neck, Breast, Prostate and Inhalation Breathing Holding for Lung and Liver. Considering the difference of size between CT and Viewray, the patient's position and devices were in the same condition. Using Viewray MRIdian system, two treatment plans were established. The one was CT fusion treatment plan based on MR image. Another was MR treatment plan including electron density that [ICRU 46] recommend for Lung, Air and Bone. For Head&Neck, Breast and Prostate, IMRT was established and for Lung and Liver, Gating treatment plan was established. PTV's Homogeneity Index(HI) and Conformity Index(CI) were use to estimate the treatment plan. And DVH and dose difference of each PTV and OAR were compared to estimate the treatment plan. Results : Between the two treatment plan, each difference of PTV's HI value is 0.089% (Head&Neck), 0.26% (Breast), 0.67% (Lung), 0.2% (Liver), 0.4% (Prostate) and in case of CI, 0.043% (Head&Neck), 0.84% (Breast), 0.68% (Lung), 0.46% (Liver), 0.3% (Prostate). As showed above, it is on Head&Neck that HI and CI's difference value is smallest. Each difference of average dose on PTV is 0.07 Gy (Head&Neck), 0.29 Gy (Breast), 0.18 Gy (Lung), 0.3 Gy (Liver), 0.18 Gy (Prostate). And by percentage, it is 0.06% (Head&Neck), 0.7% (Breast), 0.29% (Lung), 0.69% (Liver), 0.44% (Prostate). Likewise, All is under 1%. In Head&Neck, average dose difference of each OAR is 0.01~0.12 Gy, 0.04~0.06 Gy in Breast, 0.01~0.21 Gy in Lung, 0.06~0.27 Gy in Liver and 0.02~0.23 Gy in Prostate. Conclusion : PTV's HI, CI dose difference on the Treatment plan using MR image is under 1% and OAR's dose difference is maximum 0.89 Gy as heterogeneous tissue increases when comparing with that fused CT image. Besides, It characterizes excellent contrast in soft tissue. So, radiation therapy using only MR image without CT scan is useful in the part like Head&Neck, partial breast and prostate cancer which has a little difference of heterogeneity.

Evaluation of Combine IGRT using ExacTrac and CBCT In SBRT (정위적체부방사선치료시 ExacTrac과 CBCT를 이용한 Combine IGRT의 유용성 평가)

  • Purpose : The purpose of this study is to compare and analyze the set-up errors using the Combine IGRT with ExacTrac and CBCT phased in the treatment of Stereotatic Body Radiotherapy. Methods and materials : Patient who were treated Stereotatic Body Radiotherapy in the ulsan university hospital from May 2014 to november 2017 were classified as treatment area three brain, nine spine, three pelvis. First using ExacTrac Set-up error calibrated direction of Lateral(Lat), Longitudinal(Lng), Vertical(Vrt), Roll, Pitch, Yaw, after applied ExacTrac moving data in addition to use CBCT and set-up error calibrated direction of Lat, Lng, Vrt, Rotation(Rtn). Results : When using ExacTrac, the error in the brain region is Lat $0.18{\pm}0.25cm$, Lng $0.23{\pm}0.04cm$, Vrt $0.30{\pm}0.36cm$, Roll $0.36{\pm}0.21^{\circ}$, Pitch $1.72{\pm}0.62^{\circ}$, Yaw $1.80{\pm}1.21^{\circ}$, spine Lat $0.21{\pm}0.24cm$, Lng $0.27{\pm}0.36cm$, Vrt $0.26{\pm}0.42cm$, Roll $1.01{\pm}1.17^{\circ}$, Pitch $0.66{\pm}0.45^{\circ}$, Yaw $0.71{\pm}0.58^{\circ}$, pelvis Lat $0.20{\pm}0.16cm$, Lng $0.24{\pm}0.29cm$, Vrt $0.28{\pm}0.29cm$, Roll $0.83{\pm}0.21^{\circ}$, Pitch $0.57{\pm}0.45^{\circ}$, Yaw $0.52{\pm}0.27^{\circ}$ When CBCT is performed after the couch movement, the error in brain region is Lat $0.06{\pm}0.05cm$, Lng $0.07{\pm}0.06cm$, Vrt $0.00{\pm}0.00cm$, Rtn $0.0{\pm}0.0^{\circ}$, spine Lat $0.06{\pm}0.04cm$, Lng $0.16{\pm}0.30cm$, Vrt $0.08{\pm}0.08cm$, Rtn $0.00{\pm}0.00^{\circ}$, pelvis Lat $0.06{\pm}0.07cm$, Lng $0.04{\pm}0.05cm$, Vrt $0.06{\pm}0.04cm$, Rtn $0.0{\pm}0.0^{\circ}$. Conclusion : Combine IGRT with ExacTrac in addition to CBCT during Stereotatic Body Radiotherapy showed that it was possible to reduce the set-up error of patients compared to single ExacTrac. However, the application of Combine IGRT increases patient set-up verification time and absorption dose in the body for image acquisition. Therefore, depending on the patient's situation that using Combine IGRT to reduce the patient's set-up error can increase the radiation treatment effectiveness.

Improvement of Fire Resistance for Timber Framed Walls by Reinforcement of Heavy Timber Frame

  • Fire resistance of new hybrid timber framed wall systems was evaluated in this study. These wall systems are composed of two major structural parts. One part is a heavy timber frame part designed to take charge of whole vertical load using heavy timber post and beam, and the other is an infill wall structure, designed to take charge of whole horizontal load and to provide an established level of fire resistance. A basic concept of this hybrid wall is adopted from a typical furniture structure with frame. A timber post and beam frame is constructed with Japanese Larch solid timber post(180mm by 180mm) and beam(180mm by 240mm). As infill wall systems, two types of walls are applied. One is a typical light timber framed wall with solid blocking and another is a structural insulated panel wall, in which polystyrene insulation is filled between two structural panels to make single structure. For all tested walls, two layers of 12.5mm thick type-X gypsum boards are used on fire exposed side. Prior to tests for hybrid walls, only infill walls are tested without heavy timber frame. All fire resistance tests are carried out in accordance with KS F 2257, and temperatures on several points within wall structure and unexposed wall surface are measured during fire tests. It is considered that the reinforcement of heavy timber frame is significantly efficient for improving the fire resistance of timber framed walls.

Marrow Conversion Pattern of the Femora and Acetabulum Around Hip Joints According to Age Group : MR Evaluation (연령에 따른 고관절주위 대퇴골 및 절구뼈 골수전환 양상 : MR 평가)

  • Purpose : Marrow edema and fatty degeneration of the hip joint bones could be initial and late signs in hip joint and its bone diseases respectively, which might be differentiated from age-related marrow conversion pattern. So authors have investigated normal marrow conversion pattern of the femur and acetabulum around the hip joints. Materials and methods : Three coronal MR images of 288 hip joint bones in 144 subjects, aged 2 days to 76 years divided to 8 groups every 10 years, were retrospectively analysed for the location and appearance of the converted yellow marrow. The converted yellow marrow were divided to head, neck, and trochanter in the proximal femur and below and above 50% in acetabulum of the hip. Results : The most common type of marrow conversion is the converted yellow marrow in the entire proximal femur and below 50% of acetabulum of the hip. We observed the start of marrow conversion in just before and after 2 years old and stopped at just before and after 20 years old. Conclusion : The understanding of age-related marrow conversion pattern of the hip joint bones would provide some information for differentiation from pathologic condition of them such as edema, fatty degeneration, metastasis, or leukemia.

Study on the Centipede Bite Cases Who Visited a Medical Center (일개 의료원을 방문한 지네 교상 환례에 대한 조사)

  • Thirty six patients, female 21(58.4%), male 15(58.4%), with centipede bite visited emergency room of Gimcheon Medical Center, from September 1998 to August 1999. The number of patients accompanying the seasons were increased once in June and were high in August and September; peaked in September, which conforms to the habitude that centipede likes the warmer climate. Frequency accompanying the time was high after sunset(from 6 p.m. to 6 a.m. next morning) than before sunset, which conforms to the habitude that centipede is nocturnal. Distribution of the bite sites was finger(30%), neck(28%), foot(25%) and hand(14%) as same as the previous researches showed. In case of finger and hand, patients got bitten while they were wearing gloves or doing works; foot, wearing shoes. They got bitten the neck while sleeping. Among the localized symptoms were localized pain: 36 patients(100%), swelling: 36 patients(100%), erythema: 26 patients(72.2%), paresthesia: 5 patients (13.9%). Systemic symptoms were dizziness 2 patients(5.6%) and nausea 1 patient(2.8%). Mostly the localized pain was improved within 48 hours, swelling in 48 hours and within 72 hours in case of paresthesia. Systemic symptoms such as dizziness and nausea were improved within 24 hours. Treatment and prognosis of scolopendra have similarity in many countries. We suggest that Korean standard treatment manual is needed so that the localized symptom, systemic symptom and external wound can be cured soon.

Estimation of Merchantable Volume and Yield Using A Stem Shape Simulation (수간부위 형상 시뮬레이션을 이용한 임목 이용재적 및 목재수율 추정)

  • This study was conducted to estimate merchantable volume and yield per diameter (DBH) class of Pinus koraiensis and Larix kaempferi. Stem volume of trees in each diameter class was calculated using the existing equations for taper and height curve. Saw logs and pulpwood volume were then estimated from the stem volume in each diameter class. The final step was to estimate merchantable volume and yield from saw logs, assuming saw logs were used for lumber (boards or dimension products) production only. The results showed that the stem volume of Larix kaempferi was 23~38% higher than that of Pinus koraiensis at the same diameter classes. Both species were able to produce saw logs from the diameter class of 18 cm. Saw logs and pulpwood yield for both species rapidly increased due to the size limitation on small end diameter. This yield reached a maximum of 90% and 88% at 26 cm and 38cm diameter class, respectively, for Pinus koraiensis and Larix kaempferi. Lumber yield estimated for board products ranged from 23% to 56%. In the case of dimension products, Lumber yield became significantly smaller as saw logs were used for larger dimension products. These results can be used for calculating merchantable volume and economic value of trees, as well as determining final lumber products.

Effects of Stand Age Classes on Biomass Expansion Factors and Stem Densities in Chamaecyparis obtusa Plantations (편백 조림지에서 영급이 바이오매스 확장계수와 줄기밀도에 미치는 영향)

  • Biomass expansion factors and stem density values were commonly used in converting stand volumes into total carbon stocks for the purpose of national inventories of greenhouse gas emissions and carbon sequestration. The objective of this study was to examine the influence of stand age classes on aboveground and total biomass expansion factors, and stem density values in Chamaecyparis obtusa species. A total of 25 representative sample trees based on the three different stand age classes were destructively sampled to measure green weights and dry weights of the major four(root, stem, branch and foliage) portions of C. obtusa species grown in Jangseung-gun of southern Korea. According to the results of this study, as stand age classes increase, total biomass expansion factors tended to be decreased with the ranges from 3.64 to 1.44, while the stem density values tended to be slightly increased with the ranges from $0.35(g/cm^3)$ to $0.44(g/cm^3)$. There were statistically significant differences in biomass expansion factors and stem density values between stand age classes, but became nearly constant after 30 years old for C. obtusa species. This information could be very useful to improve a national-scaled inventory of greenhouse gas emissions and carbon sequestration for the C. obtusa species by applying different biomass expansion factors and stem density values.

Congenital Dropsy in Korean-native Calf (한우 송아지의 선천성 전신수종)

  • A 4-year old Korean-native cow of third parturition delivered an deformed calf with congenital anasarca and hypoplasia of the extremity at Muan County in Chonnam Province. At parturition, the dam showed amniotic fluid approximately two times more than that at the normal and also occurred severe dystocia. The anomaly died before parturition was 82 cm long and weighed 25 kg. It was difficult to recognize the neck and face of the anomaly due to severe edema and the stretched tongue through mouth was found. The shape of the extremity was recognizable and the systemic hypoplasia of the body with marked edema was found. At necropsy, there was a large amount of serous fluid both in pleural and peritoneal cavity. Also the fluid was contained some of yellowish granular materials and diffusely was fulfilled in all subcutaneous tissue. There was a partial opening in diaphragm which was resulted in the protrusion of approximately half of liver toward peritoneal cavity. There were two kidneys (15${\times}$21 and 13${\times}$18 cm) on the left which were enlarged and relatively larger than the right one (13${\times}$9 cm). In addition, there were found slight to moderate hepatomegaly and splenomegaly with slight congestion, and diffuse edema of intestinal serosa. Lung (32${\times}$49 mm) with marked hypoplasia was smaller than the size of heart (56${\times}$45mm). Serological test to the dam showed relatively high antibody titer (>64) to Akabane virus which might be involved in the outbreak of the deformed Korean-native calf as one of many causative agents.

Strength Properties of Wooden Model Retaining Wall Using Preservative Treated Square Timber of Domestic Pinus rigida Miller (리기다소나무 방부 정각재를 이용한 목재 옹벽의 강도 성능 평가)

  • The strength properties of wooden model retaining wall made of pitch pine (Pinus rigida Miller) was evaluated. Three different types of wooden model retaining wall were made of the 11cm square timber treated with CUAZ-2 (Copper Azole). The retaining wall was made into the 4 layers of crossbar and the 3 layers of vertical-bar, of which the size was 86 cm high, 200 cm long and 96 cm wide. Type I was control and in Type II 20 cm vertical-bars and 93 cm vertical-bars were arranged alternately to decrease wood usage. TypeIII was similar to TypeII except that the connection between crossbars was reinforced with the wooden armature. In each type, the strength properties of retaining wall were investigated by horizontal loading test and the deformation of structure by image processing (AICON 3D DPA-PRO system). In horizontal loading test of Type I, Type II and Type III was 63.17, 57.80, and 60.97 kN/m, respectively. The deformation of the top layer in Type II was 1.5 times larger than in Type I and Type III. Consequently, the economic efficiency and strength performance were better in Type III than in Type I and Type II.
