The primary cause of missing teeth vary depending on the region. The loss of posterior teeth is mainly due to dental caries, whereas that of the anterior teeth occur primarily due to trauma at the age of standing and walking and secondarily from the rampant dental caries. Particularly, reduction of the arch length in the cases of premature loss of primary teeth may compromise the eruption of succedaneous permanent teeth. This may result in crowding and impaction of the permanent teeth, and asymmetry of arch, thus a careful consideration for space maintenance should be made in such cases. Space maintainer is required in the case of premature loss of primary posterior teeth, because space loss result from the approximate and centrifugal movement of the neighboring teeth. Generally, in the case of primary incisor, space loss occurs when 1) tooth contact is relevant, 2) crowding in primary dentition is present, and 3) a primary incisor is lost before the eruption of primary canine. Contrarily, in the case of primary dentition with interdental space, space loss will not be observed, mostly when a primary incisor is lost after the eruption of primary canine. Thus, using a space maintainer in cases of premature loss of primary incisor has been introduced primarily not for the purpose of space maintaining but for an aesthetic purpose, prevention of parafunctional oral habits such as tongue thrust, and of pronunciation. Additionally, few case studies have been reported of space loss in cases of premature loss of primary incisor. This study is to report cases of the space loss following the premature loss of primary incisors observed in children.

Extracting mandibular incisors for orthodontic treatment may adversely affect the occlusion. However, when properly used, extraction of mandibular inciors is a selection for the correction of the malocclusion. Generally, treatment for crowding needs to select between nonextraction and four premolar extraction. Approaches for crowded mandibular incisors include distal movement of posterior teeth, lateral movement of canines, labial movement of incisors, interproximal enamel reduction, removal of premolars, removal of one or two incisors, and various combinations of the above. Extraction of incisors is used in case of crowding, anterior tooth size discrepancy, absent of maxillary lateral incisors, and ectopic eruption. But severe overjet. overbite, and space are the contraindication of it. A patient had severe crowding on upper anterior teeth, impacted upper left lateral incisor, palatal ectopic eruption of upper right incisor and severe crowding on lower anterior teeth. Lower lateral incisors are extracted for space availability and facial esthetics. We report the case of orthodontic treatment of upper and lower anterior crowding through extraction of lateral incisor.
Lateral cephalograms or 251 males md 286 females were taken and pubertal growth pattern or cranial base, maxillary and mandible of 7 to 17 years old Korean children was evaluated. 10 landmarks and 16 analytical measurements were evaluated. Analytical measurement and annual difference for each age group was calculated and tested for statistical significance. Analytical measurements were classified into three groups which were cranial base, maxillary and mandibular measurements and also classified into make and female measurements. Following results were achieved. 1. The circumpuberal growth spurt was earlier in Korean females than in males. 2. Cranial base, maxilla and mandible showed circumpuberal growth. The cranial base showed a relatively smaller amount of growth than the facial complex. 3. Middle and posterior cranial base length showed a floater increase than anterior cranial base length and circumpuberal growth spurt was also more definite. 4. the forward and downward growth or maxilla results from maxillary growht itself and transposition or the maxilla due to circumsutural growth aroud the maxilla. Ar-ANS and Ar-Pr which represent maxillary position relative to the cranial base showed more growth than ANS-PNS which represents maxillary bone growth. 5. mandible showed more vertical growth than horizontal growth but without significance. 6. Alveolar gwoth of maxilla and mandible show maximum growth rate of the time of permanent teeth eruption following loss of deciduous teeth . After this period alveolar growth shows a decreasing tendency.

The optimal time for the removal of supernumerary teeth can be divided into two groups; early removal and late removal. While each group has its own advantages, the effects on maxillary central incisor can be significant. The purpose of this study was to determine the ideal time for the removal of the supernumerary teeth by evaluating 166 patients for three months after surgical removal of supernumerary teeth. Relatively young patients in early Hellman's dental stage with less developed or unerupted maxillary central incisor had less midline deviation. No statistical significance was found in diastema, rotation of the maxillary incisors and their changes during a follow-up period. Removal of supernumerary teeth should be considered as a preventative measure prior to eruption of the maxillary incisors when the midline deviation is observed in panoramic radiographic examination. This study will be useful in determining the optimal time for the removal of supernumerary teeth depending on the location of the maxillary incisors. This study will be informative on deciding the optimal time to remove the supernumerary teeth depending on the various positions of the maxillary central incisor. (this sentence is better to use)

Transposition has been described as an interchange of position of permanent tooth and is a relatively rare dental anomaly. Transposition of teeth may occur both in the maxillary and mandibular arches. but it appears more often in the maxilla of individual teeth, the maxillary canine is the most often involved. A canine transposes most often with a first premolar and less frequently transposes with a lateral incisor. Incomplete transposition is a condition describing an interchange in the position of the crowns of two permanent teeth, while the root apices remain in their relative position. Complete transposition is a situation in which both the crowns and entire root structure are transposed. The etiologic factors of transposition are tooth buds interchange, retained deciduous canines, migration of the erupting canine, trauma to deciduous teeth etc. This report describes a case of a transposition between a maxillary left canine and a lateral incisor and impaction of a maxillary left central incisor due to trauma to deciduous dentition.
Purpose : The aim of this study was to investigate the timing and sequence of eruption of permanent canine and premolars, and to evaluate tooth calcification stage on emergence in Korean children. Materials and Methods : The sample was comprised of 1,266 children (male 720, female 546) aged from 7-13 years. Tooth eruption and calcification stages were determined through oral and panoramic radiographic examination, respectively. Probit analysis was used to calculate the timing of tooth eruption and tooth calcification stage from these cross-sectional data. Results : In both males and females, eruption occurred around the time when one third of tooth root or more was formed. The sequence was as follows: first premolar, canine, and second premolar in maxilla, and canine, first premolar and second premolar in mandible. Tooth eruption occurred earlier in girls compared with boys, averaging 0.63 years. Conclusions : Eruption sequence is identical in males and females with a trend for females to erupt earlier than males. Tooth eruption becomes earlier over the past decades in Korean children.
In this study, we attempt to investigate the mechanisms by which PDL cells regulate osteoclast formation and also tc know whether PDL retained their characteristic phenotype during tooth eruption and interdental separation. Rats were prepared at developmental days 21 (pre-root formation), 27(toot development), 34(advanced root formation/eruption) and at later times(adult rats). To induce severe resorption state of alveolar bone and tooth root, interdental separation with brass wire was performed between the lower first and second molars for 2 weeks in adult rats. Rat mandibles were demineralized and embedded in paraffin, and horizontal and frontal section were prepared for immuno-histochemical analysis using PDL-specific protein 22 (PDLs22), receptor activator of NFKB ligand (RANKL) and osteoprotegerin (OPG) antibodies. 1. Root formation and eruption stage of tooth development. 1) PDLs22 immunolocalization was observed in tooth follicle/PDL cells and osteoblasts throught out the root formation and eruption stages of tooth development. 2) RANKL expression became stronger at eruption stage than root formation stage of tooth development. 3) Strong expression of OPG was detected in follice/PDL cells of toot formation stage but it was decreased with tooth eruption. 2. Interdental separation between lower first and second molar 1) Comparared to normal animal, multinucleated osteoclasts and odontoclasts were markedly induced in the alveolar bone and tooth root with PDL remodeling in hematoxylin-eosin section. 2) PDLs22 expression was decreased with interdental separation. 3) RANKL expression was Increased with interdental separation in PDL fibroblasts, osteoblasts, odontoclasts and it lacunae, resorting dentin, cementum and bone matrix. 4) OPG expression was slightly decreased in the PDL cells adjacent to the alveolar bone and root surface with interdental separation. These results suggested that during tooth eruption and tooth movement, RANKL and OPG in the periodontal tissues are important determinants regulating balanced alveolar bone and tooth root resorption. And it is also suggested that PDL cells retained their characteristic phenotype during tooth eruption and interdental separation except for the short period of PDL remodeling.

Maxillary canine impaction is a frequently encountered clinical problem, and it may cause the resorption of adjacent tooth or cystic change. Treatment plan for maxillary canine impaction should be decided among extraction, orthodontic traction and autotransplantation according to several factors such as direction and position of unerupted tooth, degree of developing root apex, eruption space, exsitance of supernumerary tooth, odontoma, or cyst. Autotransplantation is a valuable alternative to extraction of impacted teeth, where surgical exposure and subsequent orthodontic realignment are difficult or impossible due to unfavorable impaction position. And its prognosis is dependent on a number of factors such as preservation of periodontal ligament, degree of root development, surgical technique, patient's age, endodontic treatment, time and type of splinting and storage medium, etc. The patients in these cases visited our dental clinic in the late permanent dentition with the chief complaint of unerupted maxillary canines. And it was thought that the spontaneous eruption guidance or orthodontic traction and alignment were difficult because of its unfavorable impacted position. Therefore, autotransplantaion and endodontic treatment were done and have been checked periodically until now.

Most of impacted teeth reported are permanent teeth. Impaction of primary tooth caused by primary failure of eruption is rare and the most of impacted primary tooth are second primary molars. The etiology of a primary molar impaction would appear to be a physical barrier, early ankylosis of the tooth, abnormal development of the primary molar germ or malposition of the successor bud. Transposition has been described as an interchange in the position of two teeth within the same quadrant of the dental arch. It is most commonly seen with canine and lateral incisor teeth, but is rarely associated with the primary dentition. The two cases show the transposition of impacted primary molar and the tooth germ of second premolar in their clinical and radiographic examinations. In case 1, lower second primary molar was deeply impacted inferiorly to the tooth germ suspected to be a second premolar. In case 2, impacted all second primary molars were positioned inferiorly to the tooth germs suspected to be the successors. We extracted all second primary molars under general anesthesia and the extracted tooth germs appeared to be second premolars.

Cleidocranial dysplasia(CCD) is a congenital disorder of skeletal and dental abnormality, which is mesodermal dysfunction influencing many tissues and organs. Skeletal abnormalities in CCD are delayed closure of cranial suture and fontanelle, presence of wormian bone and clavicle aplasia. And CCD has an effect on the long bones, phalanges, spine, pelvis, muscles, and central nervous system. Dental manifestations include retention of deciduous teeth, multiple supernumerary teeth, delayed or absent eruption of permanent teeth and formation of cysts around nonerupted teeth. But as a result in common with a lack of medical and physical disability patient may have no substantive complaint, there are many masticatory and psychological problem by absent eruption of permanent teeth after exfoliation of deciduous teeth. For this reason CCD is necessary fo early diagnosis and must be improvement of the patient's appearance as well as provision of a functioning masticatory mechanism by treatment of surgical removal of supernumerary teeth, followed orthodontically eruption of the natural permanent teeth at adequate time.
