Dysfunction of Autonomic Nervous System in Patients with Chronic Obstructive Pulmonary Diseases (만성 폐쇄성 폐질환 환자의 자율신경 장애)

  • Background: Neural control of airway function is through parasympathetic, sympathetic and non-adrenergic, non-cholinergic mechanisms. The autonomic nervous system controls the airway smooth muscle tone, mucociliary system, permeability and blood flow in the bronchial circulation and release of mediators from the mast cells and other inflammatory cells. The cardiovascular and respiratory autonomic efferent fibers have a common central origin, so altered cardiovascular autonomic reflexes could reflect the altered respiratory autonomic status. Therefore, we performed this study to assess the autonomic abnormality and determine the correlating factors of severity of autonomic neuropathy in patients with chronic obstructive pulmonary disease(COPD) using easily reproducible cardiovascular autonomic reflex function test. Method: The study included 20 patients with COPD and 20 healthy persons obtained on Health Promotion Center in Yeungnam university hospital. All the patients had history and clinical features of COPD as defined by the American Thoracic Society. Any patients with myocardial ischemia, cardiac arrythmia, hypertension, central or peripheral nervous system disease, diabetes mellitus, or any other diseases known to produce autonomic neuropathy, has excluded. The autonomic nervous system function tests included three tests evaluating the parasympathetic system and two tests evaluating the sympathetic system. And also all subjects were subjected to pulmonary function test and arterial blood gas analysis. Results: Autonomic dysfunction was more commonly associated with patients with COPD than healthy person The parasympathetic dysfunction was frequent in patient with COPD, but sympathetic dysfunction seemed preserved. The severity of parasympathetic dysfunction in patients with COPD was correlated with the degree of duration of disease, smoking, reductions in the value of $FEV_1$ and FVC, and arterial hypoxemia but no such correlation existed for age, type of COPD, $FEV_1$/FVC, or $PaCO_s$. Conclusion: There is high frequency of parasympathetic dysfunction associated with COPD and the parasympathetic abnormality in COPD is increased in proportion to severity of airway disease. In COPD, parasympathetic dysfunction probably does not the cause of disease, but it may be an effect of disease progression.

  • 지난 25여년간 우리 사회가 이룩한 경제 . 사회분야의 발전에 의한 생활수준의 향상과 의료서비스의 접근성 및 이용가능성의 개선으로 우리나라의 사망력과 출산력은 급격히 하강하여 인구구조를 변모시키고 있다. 그리고 사망과 이환의 유형 역시 크게 달라져서 과거의 전염병 및 급성질환 위주의 보건문제에서 비전염성 또는 만성병 위주로 바뀌어 가고있다. 이런 추세로 가면, 우리나라에서도 가까운 장래에 선진국가의 유형대로 정신병이나 만성퇴행성질병이 주요한 보건문제로 대두될 것이 예상된다.

Factors Influencing on Chronic Renal Disease Patients' Use of Alternative Therapies (만성콩팥병 환자의 대체요법 이용에 영향을 미치는 요인)

  • 본 연구는 말기콩팥질환자의 대체요법 이용에 영향을 미치는 요인을 파악하기 위해 시도하였다. 자료수집은 2010년 4월 13일부터 5월 13일까지 서울의 한 종합병원 신장내과에 내원한 201명을 대상으로 설문 및 면접조사를 실시하였다. 연구결과 첫째, 이용기간과 연령과는 양의 상관관계를 보였다(r=0.159, p<0.05). 둘째, 만성콩팥병의 대체요법 이용에 영향요인은 연령이 많을수록(${\beta}$=1.64), 대체이용기간이 길수록(${\beta}$=0.059), 월비용이 높을수록(${\beta}$=0.003), 질병이환기간(${\beta}$=2.147)이 길수록 대체요법 이용은 증가하였다. 따라서 초기에는 말기 콩팥질환자들에게 대체요법의 효과와 한계를 지도해야 하며 환자들에게 영향을 주는 가족들에게도 올바른 정보를 제공해야 할 것이다.

  • 현대인들의 건강관리의 인식의 변화와 노령화 인구 증가는 현대 사회의 큰 이슈이다. 몸이 불편하지만 거동이 힘든 환자, 보호자가 항상 곁에 없는 환자, 스스로의 질병관리의 방법을 모르는 환자들이 스마트 기기만 있으면 이용 가능한 만성질환관리 하이브리드 앱서비스를 제안한다. 제안한 하이브리드 앱서비스는 웹페이지를 이용하여 운영체제에 상관없는 서비스 이용이 가능하도록 연구하였다.

  • Hepatitis B virus (HBV)는 전 세계적으로 약 3억 5천만 명이 만성적으로 보균하고 있으며, 한국에서도 보건의학적으로 가장 중요하여 높은 간암의 발병율과도 밀접한 관련이 있다. 최근에 와서 HBV가 유도하는 임상적 위험질환의 병태생리에 대해서 면역학적 지식의 발전을 바탕으로 이해가 깊어졌는데, 바이러스 자체보다는 인체의 다양한 면역반응이 질병의 경과에 더욱 중요하다. 이러한 사실로 한의학적 치료법의 과학적 연구는 만성 HBV 보균자들에게 효과적인 치료방법으로 적용될 것으로 기대될 수 있는데, 우선 여기서는 전형적으로 HBV와 관련해서 의학적 문제를 모두 가지고 있는 한 가족을 통해서 HBV에 대한 폭넓은 이해에 도움을 얻고자한다.

Differences in Chronic Diseases and Physical Activity of Elderly Women According to Body Mass Index (재가여성노인의 비만도에 따른 만성질병과 신체활동정도의 차이)

  • Purpose: The study sought to identify the differences in chronic diseases and physical activity in elderly women by BMI. Method: The subjects of this study were 644 elderly women 60-80-years-of-age living at home. The research instruments were physical activity levels and chronic diseases. Subjects were given a self-report questionnaire. Data were analyzed using the SPSS win program. Result: Hypertension, prevalence of diabetes mellitus and hyperlipemia were significantly different in the subjects according to body mass index, being higher in obese subjects than in non-obese subjects. Physical activity in each of the body mass index groups did not differ significantly. Conclusion: Obesity increases the risk of chronic diseases. This knowledge could help elderly women control their weight, reduce chronic diseases, and ultimately, gain better health.

  • 최근에 우리 사회는 급격히 증가하는 만성질환으로 위기에 처해있다. 진보된 의료기술도 여전히 만성 질환을 완벽히 진료해 주지 못하고 있다. 현대 사회의 많은 사람들은 긴 수명을 보장받는 대신에 만성질환 장애를 가지게 되었고 비싼 의료 비용은 심각한사회 문제로 대두되고 있다. IT가 발달함에 따라 병원 중심의 치료서비스에서 개인과 주치의가 상호 긴밀하게 협력하여 질병의 예방, 관리, 건강증진 등을 제공하는 u-Healthcare 시스템이 빠르게 발전하고 있다. u-Healthcare 시스템은 개인 건강 기기로부터 측정된 생체 정보 데이터가 병원과 주치의에게 전달되어 언제 어디서나 검사 및 피드백이 가능하도록 하는 것이 목적이다. 본 논문에서는 유헬스케어 시스템에 발 맞추어 스마트 폰 기반의 만성질환자를 위한 u-DailyCare 시스템의 설계한다. 스마트폰 사용자로부터 얻어진 행위 데이터 및 생체 정보 데이터를 수집하여 서버에 보내면, 이 정보들을 바탕으로 주치의 혹은 병원에서 실시간으로 데이터를 분석하여 피드백을 준다. 본 논문의 특징으로는 키넥트 카메라와 신체 활동을 측정하는 MET 단위를 사용하여 정확한 행위 데이터 측정과 분석에 도움을 주었다. 이로써 주치의는 환자에게 좀더 정확하고 정교한 피드백을 전달할 수 있다.

The Meaning of Illness among Korean Americans with Chronic Hepatitis B (미주 한인 만성 B형 간염 환자의 질병의 의미)

  • Purpose: This ethnography was done to explore the meaning of illness in Korean Americans with chronic hepatitis B. Methods: The participants were 6 patients with chronic hepatitis B and 6 general informants who could provide relevant data. Data were collected from iterative fieldwork with ethnographic interviews within Korean communities in two cities in the United States. Data were analyzed using causal chain analysis developed by Wolcott. Results: The analyses revealed three meanings for the illness: hidden disease, intentionally hidden disease, and inevitably hidden disease. The contexts of meaning of illness included characteristics of the illness, social stigma, structure of health care system and communication patterns and discourse between health care providers and clients. Conclusion: The meaning of illness was based on folk illness concepts and constructed in the sociocultural context. Folk etiology, pathology and interpretation of one's symptoms were factors influencing illness behavior. These findings could be a cornerstone for culture specific care for Korean Americans with chronic hepatitis B.

  • Sleep is not only an essential physiological function, but also serves important roles in promoting growth, maturation, and overall health of humans. There is increasing interest regarding the impact of sleep and its disorders on the regulation of inflammatory processes and end-organ morbidities, particularly in the context of metabolic and cardiovascular diseases (CVD) and their complications. Obstructive sleep apnea syndrome (OSAS) is an increasingly common health problem in children. In the last decade, the emergence of increasing obesity rates has further led to remarkable increases in the prevalence of OSAS, along with more prominent neurocognitive, behavioral, cardiovascular and metabolic morbidities. Although the underlying mechanisms leading to OSAS-induced morbidities are likely multifactorial and remain to be fully elucidated, activation of inflammatory pathways by OSAS has emerged as an important pathophysiological component of the end-organ injury associated with this disorder. To this effect, it would appear that OSAS could be viewed as a chronic, low-grade inflammatory disorder. Furthermore, the concurrent presence of obesity and OSAS poses a theoretically increased risk of OSAS-related complications. In this study, we will critically review the current state of research regarding the impact of insufficient and disrupted sleep and OSAS on the immune processes and inflammatory pathways that underlie childhood OSAS as a distinctive systemic inflammatory condition in children, and will explore potential interactions between OSAS and obesity.

Impact of Depression, Comorbid Chronic Diseases, and ADL on Health-Related Quality of Life Among Community-Dwelling Elderly People in Korea (한국의 지역사회 거주노인들에게 있어서 우울증, 만성질환, 일상생활활동이 삶의 질에 미치는 영향)

  • The purpose of study was evaluated associations among risk factors to have influence on HRQOL of community dwelling Elderly People. Total 314 community dwelling elderly persons(age:$75.80{\pm}5.64$[$mean{\pm}SD$]) who had no disease(n=81) and more than two chronic diseases (n=233) were interviewed based on the QOL questionnaires. During two months. The activities of daily living were evaluated using BI and FAI. Neuropsychological status was evaluated using GDS. Health related QOL was evaluated using the SF-36. The results emphasize the importance of preventing and controlling depression, chronic diseases, and low basic activities of daily living in order to reduce their impact on HRQOL among community dwelling elderly people.
