Frailty assessed by the electronic frailty index and its impact on health outcomes in older adults with chronic diseases: a systematic review (전자허약지수(electronic frailty index)를 활용한 만성질환 노인의 허약평가와 건강결과 간의 관계: 체계적 고찰)

  • Purpose: The electronic frailty index (eFI), which is derived from electronic health records, has been recommended as screening tool for frailty due to its accessibility and ease of use. The objective of this systematic review was to identify the prevalence of frailty assessed by the eFI and its influence on health outcomes in older adults with chronic diseases. Methods: We searched PubMed, Embase, Web of Science, CINAHL, SCOPUS, Cochrane, Google search, and nursing journals in Korean from January 2016 to December 2022. Results: Twelve studies were analyzed. The eFI score, based on routine clinical data, was associated with adverse health outcomes. The most frequent outcome studied was mortality, and the eFI was associated with increased mortality in nine studies. Other outcomes studied included hospitalization, length of stay, readmission, and institutionalization in relation to hospital care usage, and cardiovascular events, stroke, GI bleeding, falls, and instrumental activities of daily life as health conditions. Conclusion: Early identification of frailty in older adults with chronic diseases can decrease the burden of disease and adverse health outcomes. The eFI has a good discriminative capacity to identify frail older adults with chronic diseases.

  • 고혈압과 당뇨와 같은 만성질환은 장기적인 관리가 필요하다. 최근 주목받고 있는 폐쇄형 SNS 시스템 설계를 통해서 동일한 질병을 가지고 있는 그룹간의 커뮤니케이션을 활성화하여 자유로운 게시글을 통해서 환자 맞춤형 건강정보를 발굴하고 공유하는 시스템을 안드로이드 어플리케이션을 통해서 구현하였다. 본 서비스를 통해서 건강정보 공유 서비스 연구가 활성화될 것으로 기대한다.

Factors associated with Experience of Diagnosis and Utilization of Chronic Diseases among Korean Elderly : Focus on Comparing between Urban and Rural Elderly (한국 노인의 만성질환 진단경험 및 의료이용에 관련된 요인 : 도시와 농촌 간 비교를 중심으로)

  • Objectives: The purpose of this study was to identify and compare the difference and related factors with general characteristic and health behaviors, a experience of diagnosis and treatment of chronic diseases between rural and urban among elderly in Korea. Methods: We used the data of Community Health Survey 2017 which were collected by the Korean Center for Disease Control and Prevention. The study population comprised 67,835 elderly peopled aged 65 years or older who participated in the survey. The chi-square test, univariate and multivariate logistic regression analysis were used to analyze data. Results: We identified many significant difference of health behaviors, an experience of diagnosis and treatment with chronic diseases between rural and urban. Compared to urban elderly, the odds ratios (ORs) (95% confidence interval) of rural elderly were 1.136 (1.092-1.183) for diagnosis of diabetes, 1.278 (1.278-1.386) for diagnosis of dyslipidemia, 0.940 (0.904-0.977) for diagnosis of arthritis, 0.785(0.736-0.837) for treatment of arthritis, 1.159 (1.116-1.203) for diagnosis of cataracts, and 1.285(1.200-1.375) for treatment of cataracts. In the experience of diagnosis and treatment of chronic diseases, various variables were derived as contributing factors for each disease. Especially, there were statistically significant difference in the experience of diabetes diagnosis, arthritis diagnosis, cataract diagnosis and dyslipidemia except for hypertension diagnosis (p <0.01) between urban and rural elderly. There were statistically significant differences in the experience of treatment for arthritis and cataract (p <0.01), but there was no significant difference in the experience of treatment for hypertension, diabetes, dyslipidemia between urban and rural elderly. Conclusion: Therefore, it would be necessary to implement a strategic health management project for diseases that showed significant experience of chronic diseases with diagnosis and treatment, reflecting the related factors of the elderly chronic diseases among the urban and rural areas.

Study on the Efficacy of Paeonia Japonica, Cucurbita Moschata and Prunus Cerasus Complex Extract for Alleviating Stress Associated with Chronic Skin Conditions (만성 피부 질환으로 발생하는 스트레스 개선을 위한 호박, 작약, 타트체리 복합물의 효능 연구)

  • In modern society, where tension and stress are ubiquitous, individuals often experience psychological imbalances. These stressors not only affect mental well-being but also manifest physically, through the skin. Consequently, a new term psychodermatology combining psychiatry and dermatology, has emerged, garnerning attention and research focus. In this study, we aimed to develop materials improving chronic skin conditions caused by stress by utilizing a compound of Cucurbita moschata, Paeonia japonica, and Prunus cerasus known to alleviate skin disorders. We sought to develop and validate the efficacy of materials alleviating chronic skin conditions induced by stress in keratinocytes..Therefore, in this study we analyzed the effects of a complex extract using Cucurbita moschata, Paeonia japonica, and Prunus cerasus on HaCaT keratinocyte cells to understand how it influences them. The complex extract on HaCaT keratinocyte cells showed a concentration-dependent decrease in the expression levels of TNF-α, IL-1β, IL-6, MDC, and TARC at concentrations of 12.5, 25, 50 and 100 ㎍/mL. Particularly noteworthy was the efficacy observed in inhibiting IL-1β, with a reduction of over 40% at a concentration of 100 ㎍/mL. Additionally, the production levels of AQP-3, HA, and filaggrin exhibited a significant concentration-dependent increase. The protein expression of p-ERK, p-JNK, and p-p38, which were elevated by TNF-α/IFN-γ, was significantly decreased with the treatment of the complex extract. These findings suggest that the compound extract may be utilized as a material for treating and preventing skin conditions, potentially mitigating the adverse effects of the mutual relationship between skin disorders and stress.

Comparison of MicroRNA Expression in Placenta-derived Mesenchymal Stem Cells and Bone Marrow-derived Stem Cells (태반유래 줄기세포와 골수유래 줄기세포에서의 마이크로RNA 발현비교)

  • Mesenchymal stem cells (MSCs) have been widely used as cellular therapeutic agents. They have their own characteristic stemness, and thus, they can be used in the treatment of many chronic diseases and in anticancer therapy. MSC therapy has many advantages over chemical therapy. MSC therapy is based on self or homogeneous origin; as such, it is expected to be effective in the treatment of various diseases. In addition, microRNAs in particular have been studied for their structure and function, and they are also expected to prove effective for use as therapeutic agents in cancer or chronic diseases. MicroRNAs are largely associated with metabolism and homeostasis. Therefore, over- or under-expression of microRNAs leads to chronic diseases. Conversely, effective control of the expression of specific microRNAs reduces the risk of many chronic diseases. However, there have been no reports thus far on the synergistic effects of MSCs and microRNAs. Therefore, in this study, we examined the relationship between MSCs and microRNAs using placenta-derived MSCs (PDSCs), bone marrow-derived MSCs (BM-MSCs), and fibroblast (WI-38) cells. We studied the expression of some microRNAs in MSCs and compared the expression in each cell line and cell passage. As a result, we found that the expression of microRNA-34a was higher in PDSCs than in BM-MSCs and that the expression of microRNA-27a, 33a, 33b, and 211 was higher in BM-MSCs than in PDSCs. Therefore, we expect that each MSC line will be used as cell therapy, considering its expressed functional microRNA.

Association of Health Perception and Health Promoting Behavior to Attitudes about Child with Asthma in Pre-service Elementary Teachers (예비초등교사의 건강지각, 건강증진행위 및 천식아동에 대한 태도간의 관계)

  • This study was conducted to identify the preservice teacher's health perception and health promoting behavior, and attitudes towards children with asthma, as well as to examine the relationship between these variables and the influencing factors. The participants were 329 third and fourth grade students from the national university of education in G city. The data were analyzed using Pearson correlation coefficients, t-test, ANOVA, Scheffe test, and multiple regression with SPSS/WIN 23.0. The score for the participants' health perception was 2.85 out of a possible 4, health promoting behavior about 2.49 out of a possible 4, and their score for attitude towards asthma was 3.67 points of a possible 5. The health perception and health promoting behavior showed a positive correlation with the attitudes about child with asthma. Interpersonal relations and health perception and grade had a significant influence on the attitudes about child with asthma. These variables accounted for 12.2% of the attitudes about child with asthma. The results highlight the need for intervention strategies to enhance the health promoting behavior and health perception.

Self-Rated Health Status of Korean Older People: An Introduction for International Comparative Studies (우리나라 중.노년 인구의 건강상태: 주관적 건강상태의 국가간 비교연구 시론)

  • This study analyzes differences of self-rated health status between Korea and three European countries. Self-rated health status is highly correlated with objective health status such as chronic diseases and ADL(Activities of Daily Living)/IADL(Instrumental Activities of Daily Living), but it is also influenced by individual attitude or belief about health. Therefore, differences of self-rated health status among countries are determined by the combination of (1) differences of objective health status and (2) socio-cultural characteristics affecting individuals' attitude and belief. Using 'Korean Longitudinal Study of Ageing(KLoSA 2006)' and 'Survey of Health, Ageing, and Retirement in Europe (SHARE 2004)', we found that Korean older people are more likely to feel negatively on their health status than their European counterpart. The findings are explained in two different ways. First, how strongly the objective health status affects on the subjective health status varies among countries. Korean older people with chronic diseases are more likely to evaluate their health status negatively because of the diseases than their European counterparts do. Second, after controlling the effects of the objective health condition, the subjective health status of Korean older people is still lower than that of the European elderly.

Intrafamilial Transmission of HCV: Comparison with HBV (HCV의 가족내 감염양상: HBV의 가족내 감염양상과 비교)

  • Among 85 patients with anti-HCV positive chronic liver disease, only 21.2% have past history of blood transfusion and over half the cases, they do not have any suspicious risk factors for HCV infection, 3 of 85 families show anti-HCV positive family members. On the other hand, 40 of 60 patients with HBsAg positive chronic liver disease show HBsAg positive family members. In Korea, HBV is transmitted mainly through vertical and intrafamilial infection but HCV disease might be rather horizontal and sporadic than vertical. To define the evident source of infection in sporadic hepatitis C, first of all, simple test with high sensitivity and specificity for diagnosis of HCV infection would be needed.

  • This study is a qualitative study in which nursing students analyzed the contents of nursing skills recorded on a structured video observation record sheet while observing a colleague team's real-time video of chronic obstructive pulmonary disease scenario implementation during simulation practice. As a result of the analysis using the content analysis method, categories and topics of effective and ineffective aspects were derived in the areas of observation record: accuracy of procedures, adherence to aseptic technique, consideration of safety and safety, explanation and education, and purpose explanation and method education. This study is meaningful in that it presents factors that can increase the efficiency of nursing education through simulation-based practice.

Application of Complementary Alternative Therapies(CAT) and Influence of Health Locus of Control on CAT in Patients with Chronic Liver Disease (만성 간 환자의 보완대체요법 시행 실태와 건강 통제위의 영향)

  • Purpose: This study was done to investigate the application of complementary and alternative therapies (CAT) in patients with chronic liver disease. and to analyze the Influence of health locus of control (HLOC) on CAT. Method: Questionnaires on CAT and a multidimensional health locus of control (MHLOC) scale developed by Wallston and Wallston were completed by 141 patients. Result: Analysis showed that 44.7% of the patients had experience with CAT and 49.2% of the patients reported being willing to use CAT. Reliability scores for the scale were HLOC-I 0.7376, HLOC-P 0.6383, HLOC-C 0.7351. The mean scores for the HLOC were HLOC-I 24.86, HLOC-P 22.86, HLOC-C 16.00. There were no significant differences in mean scores for HLOC between the patients who had used CAT and those who had not. Eight response Patterns for HLOC were identified. Among them, 'yea sayer' was the largest group (27.0%). A significant difference was found in the 'yea sayer' pattern between the group who had used CAT and the group who had not. Conclusion: CAT relates to various cognitive factors such as HLOC. Further study is needed to examine the influence of HLOC on CAT and its response patterns.
