Tuberculous otitis media is a very rare disease in extrapulmonary tuberculosis and in chronic otitis media nowadays. The characteristic signs seem to have altered from the past ones. And the suspicion index is very low that there is often a considerable delay prior to diagnosis. This might result in irreversible complications such as hearing loss, facial nerve paralysis and labyrinthitis. It is difficult to be defined by culture, so many patients with tuberculous otitis media are subjected to have surgery without a correct etiologic diagnosis. Therefore bacteriologic study of secretion and pathologic examinations of biopsied tissue are mandatory. We report one year and 11 months old boy with tuberculous otitis media having painful otorrhea and perforated tympanic membrane diagnosed by histologic finding and polymerase chain reaction(PCR). And we would like to emphasize the fact that tuberculous otitis media must be considered in the differential diagnosis of persistent suppurative otitis media despite appropriate antibiotic therapy considering the increasing tendency in incidence of Tuberculosis in Korea these days.
Pathogenesis of otitis media was studied in an animal model of rats from a histopathological and biochemical point of view. Basic anatomical outline, and distribution and type of normal epithelial cells of the rat bulla were described as a background study. Pseudomonas otitis media was developed in rats by inoculating $10^{9}$ bacteria into the tympanic bulla. Histopathologic change of the mucoperiosteal layer showed acute stage of infection from 3 days to 3 weeks, and it became chronic after 4 weeks animals through 12 weeks. Enzyme profile in the extracts of the inflammatory middle ear tissue was studied. The levels of three enzymes, PZ-peptidase, LDH, and lysozyme were much higher in the middle ear tissue than in the corresponding sera as might be expected. Tissue/serum ratios of the enzyme activities were 13-38 for PZ-peptidase, 63-177 for LDH, and 18-94 for lysozyme. Possible role of the PZ-peptidase and possible origins of the three enzymes detected in the tissue were discussed.
A Clinical and bacteriologcal observation was performed 128 cases of chronic otitis media who had taken middle ear surgery at Dept. of otolaryngology of St. Benedict Hospital during the period of Feb. 1981 to Feb. 1983. The following results were obtained. 1) Among total 128 cases, male were 60 cases (46.9%) and female were 68 cases (53.1 %) and age distribution showed 48 cases (37.5 %) in 3rd decade, 37 cases (28.9 %) in 2nd decade and 27 cases (21.1 %) in 4th decade. 2) Site distribution were 71 cases (55.5%) in right, 57 cases (44.5 %) in left. 3) Degree of preoperative hearing loss were 64 cases (50%) in moderate, 32 cases (25% ) in mild, 27 cases (21.1 %) in severe and 5 cases (3.9%) were normal. 4) Central perforation were observed in 65 cases (50.8 %), total perforation in 44 cases (34.4 %), attic perforation in 12 cases (9.4 %) and marginal perforation in 7 cases (5.4 %). 5) Pathologic changes of middle ear and mastoid antrum showed granulation in 81 cases (63.3 %), cholesteatoma in 47 cases (36.7%). 6) The route of approach were 123 cases (96.1%) in postauricular, 3 cases (2.3%) in transmeatal and 2 cases (1.6 %) in endaural. 7) Type of operation were 53 cases (41.4 %) in intact canal wall tympanoplasty with mastoidectomy, 42 cases (32.8 %) in tympanoplasty without mastoidectomy, 23 cases (18%) in modified radical mastoidectomy and 10 cases (7.8%) in radical mastoidectomy. 8) Type of anesthesia were 95 cases (74.2 %) by local anesthesia, 33 cases (25.8 %) were by general anesthesia. 9) Among 93 specimens of culture, proteus (31.2%), staphylococcus (28.7%), pseudomonas (23.7 %), streptococcus (7.5 %) and etc. (8.7%) in order of frequency.
The aim of ear operation is to cease otorrhea and to restore hearing. The ceasation of otorrhea and the graft of tympanic membrane are reached to successful stage, and yet hearing restoration process still remains further to need a matter of development. Authors carried out 67 cases hearing restoration operation among 154 patients who had been undertaken mastoid-tympanoplasty during period of Feb. 1978 through March 1979. 54 cases have been followed for this study among 67 cases hearing restoration operation. The used material of operation are almostly incus and some cases with TORP. This paper is dealing with its surgical method and postoperative audimetric study.
배경 및 목적: 전신마취를 위하여 시행한 기관내 삽관은 삽관튜브와 성대내면의 접촉에 의한 압력과 마찰로 후두 미세한 손상을 주게된다. 저자들은 단기간 기관내 삽관 전,후의 음성분석을 통하여 손상의 유무와 회복을 측정할 수 있는 객관적인 음성지표를 찾아보고자 하였다. 대상 및 방법: 만성 중이염 수술시 전신마취를 목적으로 경구기관 튜브를 거치한 성인 남자 10명과 여자 15명 환자를 대상으로 수술 1일전과 술후 24시간 후 각각 "a"음을 연장 발성시켜 CSL 4300B (KAY elemetrics Corp)의 MDVP(multidimensional voice program)을 이용하여 harmonic to noise ratio(NHR), Jitter, Shimmer, Fundamental frequency를 측정 비교하였다. 결 과: 남녀 모두에서 Jitter, Shimmer는 각각 평균 0.70%에서 1.06%, 1.92%에서2.28%로 증가되는 경향을 보였으나 통계학적 유의성은 없었다. Fundamental frequency는 여자에서 평균 220Hz에서 221Hz로 남자는 125Hz에서 128Hz로 변화를 보이지 않았고 harmonic to noise ratio(NHR)또한 평균 0.11로 수술 전, 후 변화를 관찰할 수 없었다. 결 론: 이상의 결과로 2내지 6시간의 단기간 삽관으로 인한 성대의 손상은 경미하여 24시간 이내에 회복되는 것으로 판단된다. 향후 6시간 이상의 기관내삽관이나 수일이상의 장기간 삽관후의 음성지표의 측정등의 연구가 필요할 것으로 사료된다.
Objective : The purpose of this study is to report the effect of Korean medicine on Bell's palsy with Chronic suppurative otitis media. Methods : We used herbal medication, acupuncture, pharmacopuncture and cupping therapy for treatment. To evaluate the results of the treatment, we observed the patient's condition. Results : The patient's objective and subjective symptoms showed noticeable improvement after treatment. Conclusions : We suggest that Bell's palsy with chronic suppurative otitis media can be treated by Korean medicine effectively.
In normal auditory systems, the difference between the bone conduction thresholds obtained with the test ear opened and occluded in low frequency signals below 1, 000Hz is about 10~25 dB. But no difference in these thresholds suggests the presence of a lesion in the conductive mechanism. Sullivan added the differences in these thresholds at 250Hz, 500Hz and 1,000Hz and called "Occlusion Index". In oder to study the audiological significance, we have measured the occlusion index in 40 ears of normal persons, 20 ears of chronic otitis media patients and 20 ears of sensori-neural hearing impairment patients. We also measured the static compliance in 40 normal ears and observed the correlation with the occlusion index. The results are as follows: 1. Occlusion index was 33. $10\pm10.63dB$ in normal group, $3.10\pm3.03dB$ in chronic otitis media; group, $28.10\pm15.17dB$ in sensori-neural group. 2. Static compliance in normal group was $0.61\pm0.31cc$ (0.22~1.75cc) 3. Occlusion index showed inverse proportion to static compliance.

Objectives : Otomycosis is superficial fungal infection of external auditory canal accompanied with itching, pain, discharge, hearing loss, tinnitus and ear fullness. And chronic inflammation like chronic suppurative otitis media exacerbates otomycosis. The purpose of this study is to report one case of Korean medical treatment for otomycosis with chronic suppurative otitis media. Methods : This study was conducted with one case of otomycosis with chronic suppurative otitis media patient hospitalized in Ophthalmology, Otolaryngology & Dermatology Clinic of Korean medical Hospital. This patient was treated with Korean medical treatment including herbal medicine, acupuncture, pharmacopuncture and venesection. After the treatment, we took picture of external auditory canal by otoscope and evaluated subjective symptoms. Results : After the treatment, this patient had improved condition of external auditory canal and subjective symptoms. Conclusions : This study suggests that Korean medical treatment may be effective for otomycosis with chronic suppurative otitis media.
Recently the efficacy of all ear drops in perforated tympanic membranes has been questioned and the safety of using the drugs intratympanically has been challanged. Many drugs not normally ototoxic when given systemically become ototoxic when administered via the middle ear. According to Stupp and others (1973) among neomycin, polymyxin-G, gentamycin, erythr omycin, tetracycline and penicillin, only penicillin was found to be free of toxic effects. The ototoxic antibiotics probably reach the inner ear by permeating the round window membrane, into the perilymph and then through Reissner's membrane to the endolymph. The main pathological changes are degeneration of stria vascularis, degeneration of sensory epithelia and degeneration of ganglion cells. We report with reference, a case of profound sensorineural deafness following the application of Terramycin powder intratympanically, in a 63 year old woman.

Objectives : This study is to report the treatment of chronic suppurative otitis media with Bojungikgi-tang and acupuncture without western medicine. Methods : A 57-year-old female patient visited my korean medical clinic on November 19, 2019 with complaints of ear fullness and yellowish effusion in the left ear. The patient had perforated eardrum with yellowish effusion. For the first month she took Bojungikgi-tang 3 times a day and received acupuncture 5-6 days a week without western treatment. The next month, only acupuncture treatment was performed twice a week. The condition of the eardrum and otorrhea was observed through an otoscope, and the degree of ear fullness and head heaviness were evaluated by visual analogue scale. Results : During the treatment, the eardrum was reperforated once, but it was regenerated, the otorrhea disappeared, and all other symptoms improved. Afterwards, it was confirmed through follow-up that the eardrum was maintained in a regenerated state. Conclusions : These results show that the combined treatment of herbal medicine and acupuncture is effective for chronic suppurative otitis media.
