  • 지속성 복막투석환자의 경우 오랫동안의 만성신부전으로 인해 영양소 섭취가 저하되어 근육의 손모, 체중감소 등으로 인해 골밀도에 부정적인 영향을 미치게 된다. 본 연구에서는 요추(L2-L4)와 대퇴부(femoral neck, trochanter)의 평균 골밀도의 T값이 모두 -1 미만인 군 86명과(골감소증군), 두 부위중 1군데 이상의 T값이 -1 이상인 군 32명 (정상군)을 대상으로 하여 신체계측치 및 생화학적 표지자, 생활습관의 차이를 조사하였다.(중략)

The Effects of Cool Dialysis on Pruritus of Chronic Renal Failure Patients (저온투석이 만성신부전 환자의 소양증에 미치는 영향)

  • Purpose: The purpose of the study is to identify the effects of cool dialysis on pruritus of chronic renal failure patients. Methods: The degree of pruritus in 30 patients treated at Hemodialysis Unit in D Medical Center was examined by the itching NRS tool. The data were collected from February 1 to February 28, 2009 and analyzed by the student t-test. The 30 patients were divided into two groups, 15 of experimental group and the same number of control group. Results: The first Hypothesis, that after two hours of hemodialysis the degree of pruritus for the experimental group would be lower than that of the control group is rejected (t=0.726, p=.474). The second Hypothesis, that right after receiving hemodialysis the degree of pruritus would be lower for the experimental group than the control group, is also rejected (t=1.321, p=.199). Conclusion: The research on searching for the effective method of nursing intervention to relieve pruitus for the chronic renal failure patients who receive hemodialysis should be continued.

Experience of Patients Living with Chronic Renal Failure (만성신부전 환자의 체험)

  • The Purpose of the study was to understand the experience of chronic renal failure patients for the qualified individual care for them. The purpose of this study was to explore the experience of patients living with chronic renal failure and to identify the meaning and structure of their experience. The subjects were four patients, two females and two males. The age range was from 21 to 54. Data was collected with a few in-depth interviews by the authors until the data was fully saturated. The framework and methodology of this study was based on Parse′s "Human Becoming methodology," an existential phenomenological research methodology. The findings of this study were as follows. Three experience structures of chronic renal failure patients were : 1. Sufferings and conflicts originated in the frustration caused by uncurable disease. 2. Dependence upon God and significant others with complex emotions. 3. Acceptance of sufferings, emerging hope for serving people, and gratitude for living. In conclusion the experience of chronic renal failure patients could be described from the findings (three structures) as "Experiencing the sufferings, conflicts originated in the frustration caused by uncurable disease, dependence upon God and significant others with complex emotion, acceptance of the suffering and hope for serving people, and gratitude for living." The three structures of the lived experience of patients with chronic renal failure, the findings of this study, could be explained by the three concepts of "Theory of Human Becoming," the first structure could be explained with values, the second with revealing-concealing, and the third with transforming.

Experience of Hemodialysis in the Chronic Renal Failure Clients (만성신부전 환자들의 혈액투석 경험)

  • The purpose of this study was to discover the experience of hemodialysis which clients confronted hemodialysis at the same time as CRF diagnosis and to understand the nature and meaning of their crisis experience. The research subjects were 6 clients receiving hemodialysis in Seoul from Jan. 1996 to Jul. 1996. Data were collected by informal indepth interview and participant observation. Content Analysis(by Seaman & Verhonick, 1982; Woods & Catanzaro, 1988) was applied to collect similar contents and common experience in order to derivate concepts and categories for better understanding of their hemodialysis experience. As a result, 6 categories derivated to indentify their hemodialysis experience of the CRF clients-confronted hemodialysis at the same time as CRF diagnosis-were as follows: 1) The category of shocking crisis composed the concepts of shock, amagement and suffocation. 2) The category of denial composed the concepts of disease refusal and hemodialysis refusal. They repeatedly visited hospitals or didn't visit hospital in order to refuse disease, then depended on folk remedy or shamanistic method. 3) The category of severe anxiety composed the concepts of abandoned feeling, shame, resentment, neurosis and anger. 4) The category of depression composed the concepts of grief, suffering of unfairness, tearing, desire to death. 5) The category of powerlessness composed the concept of hopelessness. 6) The category of resigned acceptance composed the concepts of resignation and acceptance. In this study, the CRF clients who confronted hemodialysis at the same time as CRF diagnosis experienced six stages in accepting hemodialysis but these stages were mingled simultaneously and went on.

Lipoprotein and Lipid Abnormalities in Uremic Children with Maintenance Dialysis (투석 치료중인 만성 신부전 소아에서의 지질 및 지질단백 이상)

  • Purpose : Chronic renal failure is often accompanied by severe dyslipidemia, a known risk factor for cardiovascular disease. Lipoprotein(a) [Lp(a)] has recently been characterized as a risk factor for atherosclerosis and thrombosis. Cardiovascular disease is the leading cause of death in adult patients on dialysis. However, there are only limited data available concerning risk factors for atherosclerosis in uremic children. We have measured serum levels of lipids, lipoproteins, apolipoproteins and Lp(a) in uremic children with maintenance dialysis. Methods : Ten uremic children with hemodialysis (HD) and 14 with peritoneal dialysis (PD) in our dialysis unit were included in this study. The mean age of HD patients was $162{\pm}59$ months and the male to female ratio was 7:3. The mean age and sex ratio of PD patients were $123{\pm}63$ months and 6:8, respectively. The levels of cholesterol, triglyceride, lipoproteins, apolipoproteins and Lp(a) were measured from serum sampled after 14 hours of fasting. The normal control levels were cited from 2 articles presenting the normal blood lipid and lipoprotein levels of primary school and middle school children in Korea. Results : There was no difference in age, sex ratio, body mass index and duration of dialysis between the HD and the PD group. The serum concentration of the cholesterol, triglyceride and apolipoprotein B were significantly elevated in dialysis patients compared with normal subjects. The serum level of Lp(a) was significantly elevated in only PD group. The serum Lp(a) level was below 30 mg/dl in 13 and above 30 mg/dl in 11 patients. The serum albumin level was significantly decreased in high Lp(a) group than in low Lp(a) group. Conclusion : The uremic children receiving dialysis reveal abnormal serum lipid and lipoprotein profiles. These results suggest that they have a higher risk for coronary heart disease, although there has been no clinical evidence of coronary heart disease at present. A long-term follow-up study of these children to clarify the suggestion should be started now.

The Lived Experience of Caregivers of Chronic Renal Failure Patient (만성신부전환자의 주보호자로 살기)

  • Purpose: The research aims to understand the lived experience of the caregivers of chronic renal failure(CRF) patients and its essential meaning. The results of the study can be used as basic materials for developing comprehensive intervention methods of care givers of CRF patients. Method: The research used van Manen's hermeneutic and phenomenological research methods in order to describe the lived experience and to understand its meaning. It concentrates on the understanding the essence of experience and consists of existential survey, hermeneutic and phenomenological reflection and hermeneutic writings. Participants in this research were five women care givers of CRF patients who had hemolysis at C university hospital in a metropolitan city, the period of data collection was from July 27 to Sep. 4, 2004 and major data of results in the following 5 essential themes. "sole responsibility for the patient enduring everything", "creating their own field", "heavy and painful life without hope of their private life", "wishing not to be inherited and consoling each other". Conclusion: The above findings point out that the experience of care givers of CRF patient affected and changed all parts of an individual life and his or her family life. Therefore, it suggests that total family nursing care must be considered in order to provide the holistic caring for CRF patients and their care givers.

  • The quantitative analysis of various kinds of free amino acids contained in blood serum of patients with chronic mandible ostities, epidermic hemorrhagic fever, chronic renal failure and liver cirrhosis were measured with the gas-liquid chromatography (G. L. C.). The results compared with the quantity of free amino acids of healthy persons. It was found that the quantity of free amino acids were differently contained in blood serum in accordance with kinds of patients.

  • Recent advances in the managements of chronic renal failure have increased the number of the candidates for cardiac operation in patients with chronic renal disease. There have been reports that the operative mortality of the open cardiac surgery in patients with end stage renal diseases was equal to that of the patients with normal renal function. Aortic valve replacement and mitral annuloplasty was successfully performed in a patient with chronic renal failure, and the pre-and postoperative managements are presented.

Serum Lipid Peroxide Level in Chronic Renal Failure (만성 신부전 환자의 혈청과산화 지질에 관한 연구)

  • Serum lipid peroxide levels in 25 chronic renal failure patients undergoing hemodialysis were examined by determining TBA reaction with spectrofluorometry. The lipid peroxide levels, 208.9$\pm$88.4nmol/ml, in the patient group was significantly higher than 152.4$\pm$43.9nmol/ml of 48 control healthy subjects. It is likely that the elevated serum lipid peroxide levels can play a role in increasing tendency of hemorrhage and incidence of atherosclerosis in chronic renal failure patients.

  • Heart and kidney transplantation has made great progress in the modern era. Coupled with the growing successes in individual solid organ transplantation, there has also been an increase in the number of multiple organ transplants, such as heart-kidney transplantation. This trend has been in part due to a better understanding of immunobiology, advances in surgical technique and postoperative care, and an often-common pathologic association between dual-organ failure. This pathologic course is representative for end-stage heart failure leading to secondary renal dysfunction or failure, or for end-stage renal failure as a cause for (uremic) cardiomyopathy. However, refractory cardiac failure has long been considered a contraindication to kidney transplantation. Additionally, cardiac transplantation has been denied for patients with end-stage renal disease. Over recent years, combined heart-kidney transplantation has been offered to select patients who were once denied transplantation. We report the first experience of combined heart-kidney transplantation with one year follow-up results.
